1.Analysis of the current status of health facility development in Shanghai

ObjectiveTo compare the development status of health facilities in different regions (non-agricultural and agricultural districts)and different types across four dimensions: organizational management, health environment, health activities, and health outcomes, to explore factors that may affect the current development status and summarize effective experiences, and to provide a foundation for the subsequent comprehensive, standardized, and effective promotion of health facility development and tiered and classified management. MethodsInvestigators conducted a health status survey based on the four dimensions for 50 health facilities in 16 districts of Shanghai, representing three types (including government agencies, enterprises, and communities). Evaluation forms were filled out through on-site observation and document reviews, which were developed in accordance with the Shanghai Health Settings Evaluation Standards (for trial implementation in 2019). ResultsThe average total score of health facilities in Shanghai was (88.42±11.93) points, with an overall excellence rate of 86.0%. The excellence rate of each dimension, from highest to lowest, were healthy environment (84.0%), organizational management (82.0%), health activities (78.0%), and health outcomes (44.0%). Health facilities in agricultural districts scored higher in both total score and average score on the organizational management dimension compared to non-agricultural districts. The excellence rate for the organizational management dimension was also higher in agricultural districts than in non-agricultural districts. There was no statistical significance in the total score, the score of each dimension, the overall excellence rate, and the excellence rate of each dimension among different types of health facilities (P>0.05). ConclusionThe development of health facilities among government agencies, enterprises, and communities in Shanghai has begun to yield positive results. Health facilities in non-agricultural districts should actively draw on the experience of those in agricultural districts, particularly in organizational management, adjust development ideas and planning based on their own realities, so as to continuously improve the levels of development.
2.Effect of the transcription factor SOX3 on ovarian granulosa cell proliferation and estradiol secretion

Objective To study the effect of sex-determining region Y-frame protein 3(SOX3)on proliferation and estradiol secretion in human ovarian granulosa cells(KGN cell line).Methods The gene sequence of human SOX3(NM_005634.3)was searched in Gene-Bank,an NCBI database,and the target gene SOX3 was amplified by PCR,which was cloned into lentiviral vector pLV-EF1a-GFP-2A-Puro to obtain the overexpression lentiviral re-combinant plasmid pLV-EF1a-GFP-2A-Puro-SOX3;the correctly sequenced overexpressed lentiviral recombinant plasmid as well as packaging plasmids(pGag/Pol,pRev,pVSV-G)were co-transfected into human embryonic kidney cell line(HEK 293T)cells(pLV-SOX3 group),and pLV-EF1a-GFP-2A-Puro and packaging plasmids(pGag/Pol,pRev,pVSV-G)were co-transfected into HEK 293T cells(pLV-NC group),the lentiviral particles of both groups were collected and the titers of the viruses were measured after 48 h of transfection,the lentiviruses of the two groups were infected into KGN cells,and the stably expressed cell lines were obtained after puromycin screening for 2 weeks;real-time fluorescence quantitative PCR(RT-qPCR)and Western blot were used to detect the SOX3 mRNA and protein levels in the two groups;CCK-8 assay was used to detect the proliferative ability of the cells in the two groups;ELISA was used to determine the concentration of estradiol in the two groups.Results The identification of PCR products and sequencing results showed that the SOX3 gene fragment was amplified successfully,and the enzyme digestion and sequencing results indicated that the construction of overexpression lentiviral recombinant plasmid was completed;green fluorescence could be detected after lentiviral infection of HEK 293T cells,which indicated that lentiviral packaging was successful;the lentivirus was screened by puromycin after lentiviral infection of KGN cells,and the cells were observed to express green fluorescence under the fluorescence microscope;RT-qPCR and Western blot assays both showed that the expression level of SOX3 in the pLV-SOX3 group was significantly higher than that in the pLV-NC group(P<0.05).CCK-8 assay results showed that the proliferation ability of the cells in the pLV-SOX3 group significantly increased compared with that in the pLV-NC group(P<0.01).ELISA results showed that estradiol concentration was elevated in the pLV-SOX3 group com-pared with the pLV-NC group(P<0.05).Conclusion Overexpression of the transcription factor SOX3 can pro-mote the proliferation and estradiol secretion of human ovarian granulosa cells KGN.
3.Visualization analysis of hemodialysis water utilization at domestic and international levels based on CiteSpace

Objective The present study conducted a comprehensive literature review and visualization analysis of both domestic and international research on the utilization of hemodialysis water over the past two decades,aiming to gain insights into the current research status,identify prominent areas of interest,and highlight future development trends in this field,thereby of-fering valuable references for subsequent studies.Methods By employing bibliometric analysis,the relevant literature on hemo-dialysis water usage was retrieved from the Web of Science Core Collection(WoSCC)database and China National Knowledge Network(CNKI)for the period between 2004 and 2024.Subsequently,an in-depth examination of countries,research institu-tions,authors,and keywords associated with these publications was conducted.The visualization map was generated using CiteSpace 6.2.R4 software.Results A total of 3 304 papers were included,with 147 in Chinese and 3 157 in English.Over the past two decades,there has been a consistent upward trend in the number of publications both domestically and international-ly,although the growth rate of domestic literature lags behind that of foreign countries.The United States,China,and Japan rank as the top three countries in terms of publication volume,with the United States exhibiting the highest centrality.Foreign coun-tries tend to form small research groups with close institutional collaborations,while domestic research teams and institutions are relatively dispersed.Currently,foreign research primarily focuses on Fabrication,Ultrafiltration Membranes and Performance;meanwhile,domestic research emphasizes infection control,quality control,and daily maintenance.Conclusion From 2004 to 2024,both domestic and international researchers have consistently focused on water research for hemodialysis.However,China lags behind foreign countries in this field,necessitating enhanced collaboration among nations,institutions,and regions to broad-en the scope and depth of domestic research.
4.Association between fasting blood glucose level and the risk of early vascular aging

Objective:To investigate the association between fasting blood glucose levels and the risk of early vascular aging(EVA).Methods:The basic information, medical history, and laboratory results of 695 individuals who underwent health check-up at the Physical Examination Center of the First People′s Hospital of Changzhou from January 2020 to March 2021 were retrospectively analyzed.Results:A total of 695 healthy individuals were included in the study, among whom there were 249 cases of EVA and 446 cases of non-EVA. Compared to the non-EVA group, the EVA group showed significant differences in age, gender, triglycerides, high density lipoprotein-cholesterol(HDL-C), fasting blood glucose, left brachial-ankle pulse wave velocity(L-baPWV), right brachial-ankle pulse wave velocity(R-baPWV), systolic blood pressure, and diastolic blood pressure(all P<0.01). All participants were divided into three groups( T1, T2, T3) based on fasting blood glucose levels. As fasting blood glucose levels increased, body mass index, total cholesterol, triglycerides, HDL-C, low density lipoprotein-cholesterol, L-baPWV, R-baPWV, systolic blood pressure, diastolic blood pressure, and EVA indicators showed significant differences among the three groups(all P<0.05). Univariate logistic regression analysis showed that age, gender, triglycerides, HDL-C, systolic blood pressure, diastolic blood pressure, and fasting blood glucose levels were significantly associated with the occurrence of EVA(all P<0.05). For the continuous variable of fasting blood glucose, in the regression equations without correction, with preliminary correction, and with full correction of covariates, higher fasting blood glucose levels significantly increased the risk of EVA, with odds ratios( OR) of 2.249, 2.580, and 2.413, respectively(all P<0.001). Compared to the T1 group, the risk of EVA in the T2 group, after no correction, preliminary correction, and full correction of covariates, was 1.881, 2.040, and 1.972, respectively(all P<0.01). The risk of EVA in the T3 group, unadjusted, preliminary adjustment, and full adjustment of covariates, was 3.234, 3.733, and 3.410, respectively(all P<0.001). After adjusting for confounding factors, a linear relationship between fasting blood glucose levels and the occurrence of EVA was observed through smooth curve fitting. Conclusion:Elevated fasting blood glucose may increase the risk of developing EVA.
5.Inhibitory effects of ursolic acid on IL-6-mediated invasion and migration of breast cancer cells

OBJECTIVE To investigate the inhibitory effects of ursolic acid on interleukin-6 (IL-6)-mediated invasion and migration of breast cancer MDA-MB-231 cells (hereinafter referred to as “231 cells”). METHODS The effects of 20, 40, 80, 160 and 320 µmol/L ursolic acid on the proliferation rate of 231 cells were measured by CCK-8 method. The breast cancer 231 cells were divided into control group, model group and administration group. The migration and invasion abilities of cells were detected by scratch assay and Transwell assay. Real-time quantitative polymerase chain reaction (q-PCR) assay and Western blot assay were used to detect the mRNA and protein expressions of epithelial-mesenchymal transition-related makers such as E cadherin (E-cad), matrix metalloprotein 2 (MMP2), MMP9, vimentin (Vim), CD44 molecule (CD44) and aldehyde dehydrogenase 1 family member A1 (ALDH1A1). The phosphorylation levels of JAK2 and STAT3 in the Janus kinase 2/signal transducer and activator of transcription 3 (JAK2/STAT3) signaling pathway (in terms of p-JAK2/JAK2 ratio and p-STAT3/STAT3 ratio) were detected by Western blot assay. RESULTS A low concentration of ursolic acid of 20 µmol/L (no significant inhibitory effect on cell proliferation ability) was selected as the subsequent administration concentration. Compared with the control group, the migration and invasion abilities of cells in the model group were significantly enhanced (P<0.05); compared with the model group, the migration and invasion abilities of cells in the administered group were significantly reduced (P<0.05). Compared with the control group, the relative mRNA and protein expressions of epithelial-mesenchymal transition-related markers MMP9, MMP2, Vim, ALDH1A1 and CD44 were all elevated to different extents, and the mRNA and protein expressions of E-cad were all decreased to different extents in the model group cells, and part of the differences had statistical significance (P<0.05), the p-JAK2/JAK2 ratio and p-STAT3/STAT3 ratio were significantly increased in the model group (P<0.05); compared with the model group, the expressions of the above indicators were reversed to some extent in the administration group. CONCLUSIONS Ursolic acid blocks the activation of JAK2/STAT3 signaling pathwby the inflammatory factor IL-6, which ultimately interrupts the invasion and metastasis of breast cancer cells.
6.Surveillance of iodine deficiency disorders in coastal areas of Guangxi Zhuang Autonomous Region from 2017 to 2020

Objective:To learn about the status of iodine deficiency disorders in children and pregnant women in coastal areas of Guangxi Zhuang Autonomous Region (Guangxi for short).Methods:From January 2017 to December 2020, 12 counties (cities, districts) in Beihai City, Qinzhou City and Fangchenggang City in coastal areas of Guangxi were selected as the survey sites to carry out iodine deficiency disorders monitoring. Each county (city, district) was divided into five areas according to administrative regions: East, West, South, North and Middle. One township (town) was selected from each area, and 40 non-boarding children aged 8 to 10 (age balanced, half male and half female) and 20 pregnant women were selected from each township (town) as the survey subjects. Edible salt samples and urine samples were collected from children and pregnant women to detect salt iodine and urinary iodine levels; thyroid volume of children was determined and the rate of goiter was calculated.Results:From 2017 to 2020, a total of 8 905 children were monitored, and the median salt iodine of children was 23.4 mg/kg, and the medians salt iodine in each year were 23.7, 22.8, 23.5, 23.6 mg/kg, respectively; the median urinary iodine of children was 164.7 μg/L, and the medians urinary iodine in each year were 161.2, 169.7, 156.0, 171.1 μg/L, respectively; 30 of them had goiter, the rate of goiter of children was 0.34% (30/8 905). A total of 6 626 pregnant women were monitored, and the median salt iodine of pregnant women was 23.5 mg/kg, and the medians salt iodine in each year were 23.7, 22.5, 24.3, 23.8 mg/kg, respectively; the median urinary iodine of pregnant women was 139.6 μg/L, and the medians urinary iodine in each year were 129.6, 131.6, 134.4, 175.0 μg/L, respectively.Conclusions:The iodine nutrition of children in coastal areas of Guangxi is at an appropriate level (100 - 199 μg/L), and the rate of goiter has reached the national iodine deficiency disorders elimination standard (< 5%). But pregnant women are at risk of iodine deficiency (urinary iodine < 150 μg/L).
7.Expression and the relative mechanisms of circular RNA hsa_circ_0001445 in patients with postmenopausal osteoporosis

Objective:To investigate the clinical significance and regulatory molecular mechanism of the circular RNA(circRNA)hsa_circ_0001445 in postmenopausal osteoporosis(PMOP).Methods:RNA was extracted from clinically collected blood samples, and the expression of circRNA hsa_circ_0001445 was detected by quantitative real-time PCR(qRT-PCR). Luciferase reporter gene analysis and RNA pull-down experiments were performed to investigate the interaction between two genes.Results:Compared with healthy controls, the plasma circRNA hsa_circ_0001445 in PMOP patients was down regulated( P<0.001). The circRNA hsa_circ_0001445 distinguished PMOP patients from healthy controls with high sensitivity and specificity.The area under the ROC curve(AUC)was 0.9654(95% CI: 0.9361-0.9947, P<0.001), and the sensitivity was 94.0%, while the specificity was 88.0%.The circRNA hsa_circ_0001445 promoted the osteogenic differentiation and inhibited the adipogenic differentiation of hBMSCs.The circRNA hsa_circ_0001445 can directly bind to miR-127-5p. Conclusions:Plasma level of the circRNA hsa_circ_0001445 is a potential diagnostic biomarker of PMOP and regulates the balance between osteogenesis and adipogenesis in hBMSCs by sponging miR-127-5p.
8.Improvement of inflammation and glucolipid metabolism in adipose tissues of obese mice by Mushroom

Objective:
To investigate the ameliorative effects of Mushroom on adipose tissue inflammation and glucolipid metabolism in mice fed a high-fat diet, and to provide a theoretical basis for the mechanisms of Mushroom regulating glucolipid metabolism and inflammatory responses.
Methods:
C57 BL/6 J mice were fed with normal diet(LF) group, high-fat diet(HF)group and high-fat diet + Mushroom(HF+Mushroom) group for 15 weeks.Then, body weight subcutaneous and epididymal white adipose tissue weight were measured. The morphological changes of adipose tissues were compared by HE staining, and the expression of genes related to inflamation, glycolysis and fatty acid oxidation pathways were detected by RT-PCR and Western blot.
Results:
Compared with the LF group, the HF group had increased body weight, increased subcutaneous and epididymal white fat weight and adipocyte size, and upregulated expression of tumor necrosis factor-α(TNF-α), interleukin-6(IL-6), monocyte chemoattractant protein-1(MCP-1), CD68, inducible nitric oxide synthase(iNOS), pyruvate kinase(PK), phosphofructokinase(PFK), hypoxia inducible factor-1α(HIF-1α) and peroxisome proliferator activated receptor alpha(PPARα) in adipose tissues, while the expression of carnitine palmitoyl transferase-1 A(CPT-1 A), cytochrome P450 4 a10(CYP4 a10) and medium-chain acyl-coenzyme a dehydrogenase(MCAD) were downregulated(P<0.05). Compared with the HF group, Mushroom supplementation reduced body weight, adipose tissue weight and adipocyte size, and downregulated the expression of pro-inflammatory factors and glycolytic pathway-related factors in adipose tissues, while the expression of fatty acid oxidation pathway-related factors were upregulated(P<0.05).
Conclusion
Mushroom can ameliorate inflammation and disorders of glycolipid metabolism in adipose tissues of obese mice.
9.Clinical feature and variant analysis of a case with hereditary hypophosphatemic rickets with hypercalciuria.

OBJECTIVE:
To explore the clinical features and genetic basis for a patient with hereditary hypophosphatemic rickets with hypercalciuria(HHRH).
METHODS:
Clinical data of the patient was collected. The patient was subjected to whole exome capture and next generation sequencing (NGS). Suspected variants were verified by Sanger sequencing.
RESULTS:
The patient presented with hypophosphatemic rickets, short stature, hypercalciuria, and renal stones. NGS showed that he has carried compound heterozygous variants of the SLC34A3 gene, namely c.532_533delCA(p.Q178Vfs*6) and c.894_925+69del(splicing). His parents were asymptomatic heterozygous carriers of one of the variants. Based on ACMG guidelines, both variants were classified as pathogenic.
CONCLUSION
The compound heterozygous variants c.532_533delCA (p.Q178Vfs*6) and c.894_925+69del(splicing) of the SLC34A3 gene probably underlie the disease in this child. Above finding has enriched the variant spectrum for HHRH. Based on the results, prenatal diagnosis may be provided for the family.
