1.Marriage intention among HIV-infected men who have sex with men

Objective:
To investigate the marriage intention and its influencing factors among HIV-infected men who have sex with men (MSM), so as to provide the reference for formulating high-risk behavior intervention strategies and reducing the risk of HIV transmission among this population.
Methods:
Unmarried HIV-infected MSM were sampled from newly reported HIV/AIDS patients in Gongshu District of Hangzhou City from 2020 to 2022. Participants' sociodemographic information, family status and marriage intention were collected using questionnaire surveys. Factors affecting marriage intention were identified using a multivariable logistic regression model.
Results:
A total of 430 questionnaires were allocated and 400 valid questionnaires were recovered, with an effective recovery rate of 93.02%. The respondents had a mean age of (28.90±4.67) years, and included 271 cases with registered residence outside Hangzhou City (67.75%). There were 82 cases indicating that they would probably or definitely get married, accounting for 20.50%, including 46 cases (56.10%) aged 30 years and older, 67 cases (81.71%) engaged in commercial services/freelance work/worker and 53 cases (64.63%) being the only children. Multivariable logistic regression analysis showed that unmarried HIV-infected MSM who were aged 30 years and older (OR=2.751, 95%CI: 1.230-6.154), had a monthly income of <10 000 Yuan (5 000 to <10 000 Yuan, OR=7.010, 95%CI: 2.049-23.981; <5 000 Yuan, OR=5.229, 95%CI: 1.803-15.165), had a high school education and below (OR=5.935, 95%CI: 1.775-19.844), were the only children (OR=3.407, 95%CI: 1.468-7.910), came from families with economic conditions above the average level (OR=8.906, 95%CI: 2.070-38.308), did not disclose their sexual orientation (OR=3.298, 95%CI: 1.118-9.727), held a positive attitude towards government policies (OR=70.544, 95%CI: 14.583-341.237), had no homosexual intercourse in the past six months (OR=3.583, 95%CI: 1.278-10.044), and had not informed their sexual partners of their HIV status (OR=2.910, 95%CI: 1.228-6.897) were more willing to get married.
Conclusion
A certain proportion of unmarried HIV-infected MSM in Gongshu District are willing to get married, and their marriage intention is affected by age, educational level, family economic conditions, the only children or not, disclosing sexual orientation or not, having homosexual intercourse in the past six months or not, informing sexual partners of HIV status or not, and the attitude towards government policies.
2.Nanomaterial-based Therapeutics for Biofilm-generated Bacterial Infections

Bacterial biofilms gave rise to persistent infections and multi-organ failure, thereby posing a serious threat to human health. Biofilms were formed by cross-linking of hydrophobic extracellular polymeric substances (EPS), such as proteins, polysaccharides, and eDNA, which were synthesized by bacteria themselves after adhesion and colonization on biological surfaces. They had the characteristics of dense structure, high adhesiveness and low drug permeability, and had been found in many human organs or tissues, such as the brain, heart, liver, spleen, lungs, kidneys, gastrointestinal tract, and skeleton. By releasing pro-inflammatory bacterial metabolites including endotoxins, exotoxins and interleukin, biofilms stimulated the body’s immune system to secrete inflammatory factors. These factors triggered local inflammation and chronic infections. Those were the key reason for the failure of traditional clinical drug therapy for infectious diseases.In order to cope with the increasingly severe drug-resistant infections, it was urgent to develop new therapeutic strategies for bacterial-biofilm eradication and anti-bacterial infections. Based on the nanoscale structure and biocompatible activity, nanobiomaterials had the advantages of specific targeting, intelligent delivery, high drug loading and low toxicity, which could realize efficient intervention and precise treatment of drug-resistant bacterial biofilms. This paper highlighted multiple strategies of biofilms eradication based on nanobiomaterials. For example, nanobiomaterials combined with EPS degrading enzymes could be used for targeted hydrolysis of bacterial biofilms, and effectively increased the drug enrichment within biofilms. By loading quorum sensing inhibitors, nanotechnology was also an effective strategy for eradicating bacterial biofilms and recovering the infectious symptoms. Nanobiomaterials could intervene the bacterial metabolism and break the bacterial survival homeostasis by blocking the uptake of nutrients. Moreover, energy-driven micro-nano robotics had shown excellent performance in active delivery and biofilm eradication. Micro-nano robots could penetrate physiological barriers by exogenous or endogenous driving modes such as by biological or chemical methods, ultrasound, and magnetic field, and deliver drugs to the infection sites accurately. Achieving this using conventional drugs was difficult. Overall, the paper described the biological properties and drug-resistant molecular mechanisms of bacterial biofilms, and highlighted therapeutic strategies from different perspectives by nanobiomaterials, such as dispersing bacterial mature biofilms, blocking quorum sensing, inhibiting bacterial metabolism, and energy driving penetration. In addition, we presented the key challenges still faced by nanobiomaterials in combating bacterial biofilm infections. Firstly, the dense structure of EPS caused biofilms spatial heterogeneity and metabolic heterogeneity, which created exacting requirements for the design, construction and preparation process of nanobiomaterials. Secondly, biofilm disruption carried the risk of spread and infection the pathogenic bacteria, which might lead to other infections. Finally, we emphasized the role of nanobiomaterials in the development trends and translational prospects in biofilm treatment.
3.Study of honey-processed Hedysari Radix on the protection of intestinal mucosal barrier in rats with spleen deficiency

Objective To explore the protective mechanism of honey-processed Hedysari Radix in regulating intestinal mucosal injury in rats with spleen qi deficiency.Methods The three-factor composite modeling method of bitter cold diarrhea,overwork and hunger and satiety disorder was used to construct a spleen qi deficiency model rats.After the model was successfully made,they were randomly divided into model group,honey-processed Hedysari Radix group and probiotic group,with 15 animals in each group.Another 15 normal rats were taken as the blank group.The honey-processed Hedysari Radix group was given 12.6 g·kg-1 water decoction of honey-processed Hedysari Radix by gavage,the probiotics group was given Bifidobacterium Lactobacillus triple viable tablets suspension at a dose of 0.625 g·kg-1,and the blank group and the model group were given the same dose of distilled water.The rats in the four groups were administered once a day for 15 days.Enzyme-linked immunosorbent assay was used to detect diamine oxidase(DAO)in serum,D-lactic acid(D-LA),secretory immunoglobulin A factor,and Western blotting was used to detect the expression levels of AMP-activated protein kinase(AMPK),zonula occludens-1(ZO-1)and occludin in colon tissues.Results The serum levels of DAO in the blank group,model group,honey-processed Hedysari Radix group and probiotic group were(138.93±9.78),(187.95±12.90),(147.21±6.92)and(166.47±3.37)pg·mL-1;the contents of D-LA were(892.23±49.17),(1 099.84±137.64),(956.56±86.04)and(989.61±51.75)μg·L-1;the contents of SIgA in colon tissues were(14.04±1.42),(11.47±2.39),(11.84±1.49)and(12.93±1.65)μg·mL-1;the relative expression levels of ZO-1 protein in colon tissues were 1.18±0.11,0.42±0.04,0.77±0.05 and 0.95±0.07;the relative expression levels of occludin protein were 1.35±0.31,0.61±0.17,1.19±0.19 and 0.88±0.13;the relative expression levels of AMPK protein were 0.91±0.02,0.35±0.09,0.74±0.08 and 0.59±0.11.Compared with the model group,there were significant differences in the serum content of DAO and D-LA,SIgA content in colon,and the content of ZO-1,occludin and AMPK protein in the honey-processed Hedysari Radix group(P<0.01,P<0.05).Conclusion Honey-processed Hedysari Radix can enhance the protective effect on the intestinal mucosa of rats with spleen qi deficiency by regulating the expression of related inflammatory cytokines,intestinal mucosal upper cell enzymes and tight junction proteins in rats with spleen qi deficiency.
4.Correlation between expressions of serum COL1A1/2 and intracranial aneurysm rupture

Objective To detect the levels of serum collagen type Ⅰ alpha 1 chain(COL1A1)and collagen type Ⅰ alpha 2 chain(COL1A2)in patients with intracranial aneurysm(IA),and explore their correlations with aneurysm rupture.Methods A total of 110 IA patients admitted to our hospital were regarded as the IA group and another 100 volunteers who underwent physical examination in our hospital were regarded as the control group.The expression levels of serum COL1A1 and COL1A2 were detected by ELISA.The IA patients were divided into the ruptured group(n=66)and unruptured group(n=44)according to the presence or absence of aneurysm rupture,and the clinical data and expression levels of serum COL1A1 and COL1A2 were compared between the two groups.The expression levels of serum COL1A1 and COL1A2 in patients with different Hunt-Hess grades were compared.The risk factors of aneurysm rupture in patients with IA were analyzed by multivariate Logistic regression analysis.The predictive value of serum COL1A1 and COL1A2 for aneurysm rupture in patients with IA were evaluated by receiver operating characteristic(ROC)curve.The correlation of serum COL1A1 and COL1A2 with Hunt-Hess grade for patients in rupture group was analyzed by Spearman correlation analysis.Results The expression levels of serum COL1A1 and COL1A2 for patients in the IA group were significantly higher than those in the control group(P<0.05).The number of patients with hypertension,diabetes mellitus,hyperlipidemia,aneurysm diameter>10 mm,and the expression levels of serum COL1A1 and COL1A2 in the rupture group were significantly more/higher than those in the unruptured group(P<0.05).The expression levels of serum COL1A1 and COL1A2 in patients with Hunt-Hess grades from Ⅲ to Ⅳ were significantly higher than those in patients with grades from Ⅰ to Ⅱ(P<0.05).The expression levels of serum COL1A1 and COL1A2 for patients in the rupture group were positively correlated with Hunt-Hess grade(r=0.562,0.414,P<0.05).Multivariate Logistic regression analysis showed that hypertension,diabetes mellitus,aneurysm diameter>10 mm,and increased expression levels of COL1A1 and COL1A2 were risk factors for aneurysm rupture in IA patients(P<0.05).The area under the curve(AUC)of aneurysm rupture predicted by serum COL1A1 and COL1A2 together was significantly higher than that predicted by COL1A1 alone(Z=1.905,P=0.028)and COL1A2 alone(Z=1.754,P=0.040).Conclusion The increased expression levels of serum COL1A1 and COL1A2 are risk factors for aneurysm rupture in patients with IA,and their combined prediction of aneurysm rupture in IA patients has certain clinical value.
5.Correlation of CD200-CD200R axis and diseases and its research progress

CD200 and its receptor CD200R constitute an endogenous inhibitory signal. The binding of CD200 and CD200R can regulate the immune response to pathogenic stimuli, which has received much attention in recent years. It has been found that CD200-CD200R is involved in the regulation of many kinds of pathological inflammation, including autoimmune diseases, cardiac cerebrovascular disease, infection and tumor. This paper reviews the protein structure, distribution, expression, biological function of CD200-CD200R and the correlation with diseases, and analyses the current status and development ideas of CD200-CD200R as drug targets. It aims to provide theoretical support for new drug research and development based on this target.
6.Comparative study on cleansing effect of microbubble toothbrush and conventional pulsed oral irrigator

Objective To compare the oral cleansing effects of the microbubble toothbrush and the conventional pulsed oral irrigator to provide references for users.Methods Ninety identical 3D-printed resin tooth models were grouped and subjected to repeated experiments,which were divided randomly into five groups including a microbubble toothbrush high-speed gear(GN-H)group,a microbubble toothbrush medium-speed gear(GN-M)group,a microbubble low-speed gear(GN-L)group,a conventional pulsed oral irrigator high-speed gear(W-H)group and a conventional pulsed oral irrigator low-speed gear(W-L)group,with 18 teeth in each group.The cleansing effects of the microbubble toothbrush and the conventional pulsed oral irrigator were evaluated in terms of irrigating strength and abilities for eliminating plaque and debris.Results Both the two types of water flossers were provided with high irrigating strength and effectively reduced plaque and debris on tooth surfaces,and the GN-H,GN-M and GN-L groups behaved better significantly than the remained groups.The order of the five groups was GN-H group>GN-M group>W-H group>GN-L group>W-L group for irrigating strength,GN-H group>GN-M group>GN-L group>W-H group>W-L group for plaque removal,GN-H group>GN-M group>W-H group>GN-L group>W-L group for debris removal,with all the differences being statistically significant(P<0.05).Conclusion Both the two types of water flossers remove plaque and debris effectively,while the microbubble toothbrush gains advantages over the conventional pulsed oral irrigator.[Chinese Medical Equipment Journal,2024,45(9):67-72]
7.Study on TCM Syndromes of Retinal Vein Occlusion Based on Optical Coherence Tomography Angiography

Objective To explore the distribution of traditional Chinese medicine(TCM)syndromes in retinal vein occlusion(RVO)and to study the characteristics of optical coherence tomography angiography(OCTA)in RVO patients with various TCM syndrome types.Methods A total of 63 patients(involving 126 eyes)with RVO were selected for retrospective analysis.The Ward hierarchical cluster method was used for the cluster analysis of the patients'syndromes,and then the syndrome types were defined.The clinical information of the patients were summarized and analyzed to explore the distribution of syndrome types.And then the OCTA characteristics of each syndrome type were explored by the OCTA technology and through statistical analysis.Results(1)The analysis of general data showed that among the 63 patients,there were 33 cases of central retinal vein occlusion(CRVO)and 30 cases of branch retinal vein occlusion(BRVO).There were no significant differences in the distribution of gender and the affected side between the two groups(P>0.05),but there was significant difference in the distribution of onset seasons between the two groups(P<0.05).(2)The results of Ward hierarchical clustering showed that the clustering of 4 categories of syndromes was accorded with the clinical practice,and the 4 categories corresponded to qi stagnation and blood stasis type,yin deficiency and yang hyperactivity type,phlegm and blood stasis blocking collaterals type and heart-spleen deficiency type.(3)The analysis of the distribution of TCM syndromes showed that among the 63 patients,qi stagnation and blood stasis type accounted for the highest proportion,being 42.86%(27/63),and the other syndrome types in descending order were phlegm and blood stasis blocking collaterals type[31.75%(20/63)],yin deficiency and yang hyperactivity type[14.29%(9/63)],heart-spleen deficiency type[11.10%(7/63)].(4)The analysis of OCTA characteristics of each syndrome type showed that compared with the healthy eyes,the blood flow density of each retinal layer of in RVO patients with qi stagnation and blood stasis type and phlegm and blood stasis blocking collaterals type was significantly decreased(P<0.05),and the retinal thickness was significantly increased(P<0.05).There was no significant difference in retinal blood flow density and retinal thickness between the affected eyes and the healthy eyes of the other syndromes(P>0.05).The comparison of affected eyes among various syndrome types showed that the blood flow density of each retinal layer of the affected eyes in RVO patients with qi stagnation and blood stasis type was lower than that of the other types(P<0.05),and the blood flow density of phlegm and blood stasis blocking collaterals type was lower than that of yin deficiency and yang hyperactivity type and heart and spleen deficiency type(P<0.05),while there was no significant difference between yin deficiency and yang hyperactivity type and heart-spleen deficiency type(P>0.05).The retinal thickness of each retinal layer of the affected eyes in RVO patients with phlegm and blood stasis blocking collaterals was higher than that of the other types(P<0.05),and the retinal thickness of qi stagnation and blood stasis type was higher than that of yin deficiency and yang hyperactivity type and heart-spleen deficiency type(P<0.05),while there was no significant difference between yin deficiency and yang hyperactivity type and heart-spleen deficiency type(P>0.05).There were no significant differences in macular foveal avascular zone area(FAZ),perimeter(PERIM),blood flow(FLOW)and acircularity index(AI)among RVO patients with various syndrome types(P>0.05).Conclusion The decreased retinal blood flow density is presented in RVO patients with qi stagnation and blood stasis type and phlegm and blood stasis blocking collaterals type,particularly in RVO patients with qi stagnation and blood stasis type.The increased retinal thickness can be found in RVO patients with phlegm and blood stasis blocking collaterals type and qi stagnation and blood stasis type,particularly in RVO patients with phlegm and blood stasis blocking collaterals type.No specific OCTA characteristics are presented in RVO patients with other syndrome types.There are no significant differences in FAZ,PERIM,FLOW and AI among the RVO patients with various syndrome types,either.
8.Multivariate analysis of influencing functional recovery after ankle fracture of type Ⅱ and above supination-external rotation

Objective To explore influencing factors of functional recovery after ankle fracture of type Ⅱ degree and above supination-external rotation.Methods Clinical data of 120 patients with ankle fractures of type Ⅱ degree and above supination-external rotation admitted from February 2019 to April 2021 were retrospectively analyzed.According to American Orthopaedic Foot and Ankle Society(AOFAS),patients were divided into excellent group with 73 patients(90 to 100 points),good group with 35 patients(75 to 89 points),and fair group with 12 patients(<50 points).The differences of ankle active range of motion(ROM)and AOFAS score were compared among three groups at the latest follow-up.Multivariate Logistic regression analy-sis was performed to analyze the factors related to functional recovery after ankle fracture of supination-external rotation.Results There were significant differences in postoperative ROM(dorsoextension,plantar flexion,varus and valgus)and com-plications between excellent group and good and acceptable group(P<0.05).Univariate analysis showed there were differences in age above 50 years old,Ⅳ degree of supination-external rotation fracture,lower tibiofibular ligament injury,posterior ankle fracture,no drainage tube placed,infection,antibiotic use time above 7 days(P<0.05).Multivariate Logistic regression analy-sis showed age above 50 years old[OR=2.829,95%CI(1.049,7.628),P=0.040],Ⅳ degree fracture of supination-external ro-tation[OR=6.13,95%CI(1.153,32.593),P=0.033],lower tibiofibular ligament injury[OR=10.785,95%CI(3.338,34.894),P=0.000],and posterior ankle fracture[OR=6.349,95%CI(1.869,21.560),P=0.003]were independent risk factors for func-tional recovery after ankle fracture of supination-external rotation(P<0.05).Conclusion The postoperative excellent outcome of ankle fracture was good,and the recovery of joint motion was better.The older age of patient,Ⅳ degree of supination-exter-nal rotation fracture,the lower tibiofibular ligament injury,and posterior ankle fracture are all adverse factors affecting func-tional recovery after supination-external rotation ankle fracture.In clinical,effective measures should be taken to deal with these influencing factors,and strive to improve the functional recovery after the operation of this type of fracture and reduce the occurrence of related complications.
9.Study on methods of induced second-trimester abortion in women with scarred uterus

Objective To observe the effects of cervical dilation balloon combined with ethacridine lactate injection on termination of second-trimester pregnancy in women with scarred uterus and its impact on postpartum psychology of patients.Methods A total of 128 patients with scarred uterus in second-trimester pregnancy who underwent induced abortion in our hospital from January 2022 to December 2023 were selected and randomly divided into the observation group and the control group,with 64 cases in each group.Patients in the control group received traditional 150 mg oral mifepristone+ethacridine lactate injection 100 mg intraamniotic injection to induce labor;patients in the observation group received COOK cervical dilation balloon for promoting cervical maturation+150 mg oral mifepristone+esacridine lactate injection 50 mg intraamniotic injection to induce labor.The time of labor induction,the time of labor,the amount of postpartum hemorrhage,the total effective rate of labor induction,the incidence of soft birth canal injury and threatened uterine rupture,and the postpartum psychology of patients were compared between the two groups.Results The total effective rate of labor induction of patients in the observation group was higher than that in the control group,the time of labor induction and the time of labor were shorter than those in the control group,the amount of postpartum hemorrhage was less than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).The proportion of patients with positive Edinburgh postnatal depression scale(EPDS)in the control group was significantly higher than that in the observation group(t=10.818,P<0.05),and there was statistically significant differences in the ten emotional aspects of EPDS between the two groups(P<0.05).Conclusion For patients with scarred uterus in second-trimester pregnancy,COOK cervical dilation balloon for promoting cervical maturation+150 mg oral mifepristone+esacridine lactate injection 50 mg intraamniotic injection to induce labor has good effect,which is conducive to postpartum mental health.
10.The clinical value of visual evoked potential in assessing visual pathway of visual hallucinations in Parkinson disease

Objective This research was to evaluate the integrity of visual pathways in patients with Parkinson disease(PD)by visual evoked potential(VEP),especially those with visual hallucinations.Methods A total of 76 PD patients were enrolled in this study.According to the presence or absence of visual hallucinations,they were divided into two groups:24 patients with visual hallucinations and 52 patients without visual hallucinations.At the same time,22 sex-and age-matched healthy controls were selected.All subjects underwent VEP test,and Unified Parkinson's Disease Rating Scale(UPDRS),Hoehn&Yahr stage(H-Y stage)and Mini-mental State Examination(MMSE)were performed for PD patients.Results The latencies of N75,P100 and N135(88.26±10.47)ms,(118.48±8.53)ms,(144.71±9.48)ms were significantly longer in PD patients with visual hallucinations than in those without visual hallucinations[(79.00±6.96)ms,(108.60±7.01)ms,(135.95±8.21)ms](P<0.001).However,the amplitudes of N75-P100 and N135-P100[4.35(2.73,7.30)μV]、[6.40(4.15,9.90)μV]were significantly lower in PD patients with visual hallucinations than in those without visual hallucinations[7.10(5.28,9.98)μV]、[9.05(6.30,12.60)μV](P<0.001,P=0.037).Correlation analysis showed that P100 latency was positively correlated with H-Y stage,UPDRS-I,UPDRS-II,and UPDRS-III scores(r=0.537,P=0.007),(r=0.635,P=0.001),(r=0.594,P=0.004)and(r=0.558,P=0.005)in PD visual hallucinations group.Conclusion The integrity of the visual pathway is impaired in PD patients with visual hallucinations.As the progression of the disease,the impairment of visual pathway may be further deteriorated,which may extend beyond the upper pathways of visual pathway to the brain.
