1.Current situation and influencing factors of post-traumatic stress disorder among firefighters in Chongqing City

{L-End}Objective To analyze the current status of post-traumatic stress disorder (PTSD) among firefighters in Chongqing City and explore its influencing factors. {L-End}Methods A total of 1 021 firefighters in Chongqing City were selected as the study subjects using the convenient sampling method. The PTSD Checklist Civilian Version was used to assess their PTSD symptom and characteristics. The Trait Coping Style Questionnaire and the Social Support Rating Scale were used to investigate coping styles and the level of social support. {L-End}Results The positive detection rate of PTSD among the study subjects was 4.6%(47/1 021), with positive detection rates of re-experiencing, avoidance/numbing, and hyperarousal symptoms of 7.0%, 9.2%, and 16.5%, respectively. The positive detection rates of re-experiencing, avoidance/numbing, and hyperarousal symptoms in the PTSD firefighters were higher than those in non-PTSD firefighters (83.0% vs 3.3%, 93.6% vs 5.1%, 100.0% vs 12.1%, respectively; all P<0.01). The results of the multivariate logistic regression analysis showed that being injured in the past six months and adopting a negative coping style were risk factors for PTSD [odds ratio (OR) and 95% confidence interval (CI) were 2.65 (1.07-6.56) and 1.26 (1.19-1.33), respectively; both P<0.05], while adopting a positive coping style and having a higher level of social support were protective factors for PTSD [OR and 95%CI were 0.90 (0.85-0.95) and 0.95 (0.91-0.99), respectively; both P<0.05]. {L-End}Conclusion The incidence of PTSD among the firefighters in Chongqing City is relatively high, with symptoms mainly characterized by hyperarousal. Being injured in the past six months, coping styles, and the level of social support are influencing factors for PTSD.
2.Evaluation of NG-Test Carba5 for rapid detection of carbapenemases

Objective:To evaluate the clinical value of NG-Test Carba5 for rapid detection of carbapenemases produced by carbapenem-resistant Enterobacteriaceae (CRE) strains. Methods:A total of 1 210 CRE strains were collected during 2018-2022 from 77 hospitals in 21 provinces of China and were subjected to NG-Test Carba5 for rapid detection of carbapenemase. The whole genome sequencing (WGS) analysis was referenced as the gold standard method.Results:Overall, the NG-Test Carba5 demonstrated excellent performance in detection of five kinds of carbapenemases [Klebsiella pneumoniae carbapenemase (KPC), New Delhi metallo-β-lactamase (NDM), imipenemase metallo-β-lactamase (IMP), Verona integron-encoded metallo-beta-lactamase (VIM) and oxacillinase-48-type carbapenemases(OXA-48)] from CRE strains, with a sensitivity of 98.47% (1 161/1 179), specificity of 100% (31/31), and positive predictive value of 100% (1 161/1 161). The sensitivity for detection of NDM, IMP, OXA and VIM reached 100% (307/307), and 97.70% (763/781) for KPC. For 11 strains carrying blaKPC-25, blaKPC-78, or blaKPC-93, NG-Test Carba5 reported positive KPC detection (11/11). For strains carrying blaKPC-33 and blaKPC-77, however, NG-Test Carba5 delivered negative results. Additionally, for those strains co-producing two or three kinds of carbapenemases, NG-Test Carba5 was able to report all of the targets with a sensitivity of 100% (91/91). Conclusions:NG-Test Carba5 showed excellent performance in rapid and accurate detection of carbapenemases from CRE strains. Nonetheless, for those strains with negative results, some other phenotypic and genotypic methods should be implemented alongside to avoid missing targets.
3.Current situation and influencing factors of postcholecystectomy syndrome

Objective:To investigate the current situation of postcholecystectomy syndrome (PCS) and analyze its influencing factors, so as to provide basis for the formulation of postoperative nursing measures.Methods:A total of 336 patients who underwent laparoscopic cholecystectomy in Department of General Surgery in Xuanwu Hospital, Capital Medical University from June 2019 to December 2020 were selected as research objects by the convenient sampling method. General information questionnaire, lifestyle questionnaire, Self-rating Anxiety Scale and Self-rating Depression Scale were used to conduct a questionnaire survey of the patients in 1 month and 3 months after the operation. Logistic regression analysis was used to explore the influencing factors of PCS occurrence.Results:After 3 months of follow-up, 4 patients were lost to follow-up, and data of 332 patients were finally collected. The incidence of PCS was 23.5% (78/332) in 1 month after surgery and 7.2% (24/332) in 3 months after surgery. The total incidence of PCS within 3 months was 30.7% (102/332) , and the main symptoms were diarrhea, abdominal pain and abdominal distension. Logistic regression analysis showed that meat diet and irregular meals were the risk factors for PCS in one month after surgery ( P<0.05) and marriage was the protective factor for PCS in one month after surgery ( P<0.05) . Anxiety was a risk factor for PCS in 3 months after surgery ( P<0.05) . Conclusions:Patients with cholecystectomy have a high incidence of PCS. Patients are advised to reduce the intake of high-fat food, pay attention to the combination of meat and vegetables in their diet and correct irregular eating habits within 1 month after surgery. Meanwhile, family support should be enhanced and emotional management should be strengthened in 3 months after surgery to reduce the incidence of PCS.
4.Risk factors of birth weight discordance in dichorionic diamniotic twin pregnancies: analysis of 1 757 cases

Objective:To investigate the risk factors of birth weight discordance in dichorionic diamniotic (DCDA) twins.Methods:This study retrospectively analyzed 1 757 cases of DCDA twin pregnancies from 11 Chinese hospitals from January 1, 2014, to December 31, 2017. Birth weight discordance was defined as ≥ 20% difference between the twins. All cases were divided into two groups: the concordant group ( n=1 520) and discordant group ( n=237). General information was compared and the high-risk factors of birth weight discordance were analyzed. Mann-Whitney U test, Chi-square test or Fisher's exact test, and logistic regression analysis were used as statistical methods. Results:Compared with the concordant group, the discordant group showed a higher incidence of hypertensive disorders of pregnancy [24.5% (58/237) vs 12.8% (194/1 520), χ2=22.882, P<0.05], fetal structural malformations [4.2% (10/237) vs 1.0% (15/1 520), χ2=15.160, P<0.05], fetal distress [6.3% (15/237) vs 1.4% (21/1 520), χ2=22.602, P<0.05], umbilical cord abnormalities [3.8% (9/237) vs 1.2% (18/1 520), χ2=7.607, P<0.05] and abnormal placental cord insertion [3.8% (9/237) vs 1.4% (21/1 520), χ2=34.904, P<0.05], but lower incidence of premature rupture of membranes [11.0% (26/237) vs 16.5% (250/1 520), χ2=4.645, P=0.034]. Logistic regression analysis showed that the independent risk factors of birth weight discordance in DCDA twins were hypertensive disorders of pregnancy ( OR=2.258, 95% CI: 1.620-3.184, P<0.001), fetal structural malformations ( OR=4.268, 95% CI: 1.892-9.631, P<0.001), umbilical cord abnormalities ( OR=2.889, 95% CI: 1.245-6.705, P=0.014) and abnormal placental cord insertion ( OR=2.318, 95% CI: 1.012-5.311, P=0.047). Conclusions:Hypertensive disorders of pregnancy, fetal structural malformations, umbilical cord abnormalities and abnormal placental cord insertion may be the risk factors of birth weight discordance in DCDA twins.
5.Ultrasonographic diagnosis of deep infiltrating endometriosis with involvement of pelvic tissues

Objective To explore the diagnostic value of ultrasonography in diagnosing deep infiltrating endometriosis (DIE) with involvement of pelvic tissues.Methods Data of 118 DIE patients confirmed with surgery and pathology who underwent transvaginal ultrasonography before surgery were retrospectively analyzed.Taking surgical and pathologic findings as the gold standards,the sensitivity,specificity,positive predictive value (PPV) and negative predictive value (NPV) of ultrasonography in diagnosis of DIE with involvement of uterosacral ligament,intestine,vaginal rectal septum,vagina,bladder and ureter were calculated,respectively.Results Uterosacral ligament involvement was found in 85 patients,with sensitivity of 95.29% (81/85) and specificity of 90.91% (30/33).Intestinal involvement was observed in 60 patients,with sensitivity of 98.33% (59/60) and specificity of 91.38% (53/58).Vaginal rectal septum involvement was noticed in 39 patients,with sensitivity of 79.49% (31/39) and specificity of 92.41% (73/79).Vaginal involvement was detected in 18 patients,with sensitivity of 55.56 % (10/18) and specificity of 96.00 % (96/100).Bladder involvementwas found in 17 patients,with sensitivity of 76.47% (13/17) and specificity of 94.06% (95/101).Ureter involvement was also found in 9 patients,with sensitivity of 100% (9/9) and specificity of 99.08% (108/109).Conclusion Transvagina ultrasonography has great value in diagnosis of DIE with involvement of pelvic tissues.
6.Influencing factors of clinical outcome in elderly acute ischemic stroke patients after intravenous thrombolysis

Objective To study the influencing factors of clinical outcome in elderly acute ischemic stroke (AIS) patients after intravenous thrombolysis.Methods One hundred and fifty-one AIS patients admitted to our hospital for intravenous thrombolysis were divided into good outcome group (n=77) and poor outcome group (n=74) according to their modified Rankin scale score 3 months after the onset of AIS.The baseline data,thrombolysis time window,NIHSS score and ischemic stroke typing before thrombolysis,early symptom improvement,cerebral hemorrhage after thrombolysis were compared between the two groups.Results The serum levels of blood glucose and CRP,NIHSS score≥9 before thrombolysis,incidence of AF and cerebral hemorrhage were significantly higher in poor outcome group than in good outcome group (P< 0.05,P<0.01).Multivariate logistic regression analysis showed that NIHSS score,OCSP typing,blood glucose before thrombolysis,24 h symptom improvement were the independent influencing factors of clinical outcome in elderly AIS patients (OR =1.262,95 % CI:1.075-1.482,P =0.005;OR =0.203,95%CI:0.066-0.628,P=0.006;OR=1.264,95%CI:1.042-1.532,P=0.017;OR=25.764,95%CI:5.131-129.361,P=0.000).Conclusion NIHSS score,OCSP typing,blood glucose before thrombolysis and 24 h symptom improvement are the independent influencing factors of clinical outcome in elderly AIS patients after intravenous thrombolysis.
7.Resveratrol raises in vitro anticancer effects of paclitaxel in NSCLC cell line A549 through COX-2 expression.

The aim of this study was to determine the raising anticancer effects of resveratrol (Res) on paclitaxel (PA) in non-small cell lung cancer (NSCLC) cell line A549. The 10 µg/ml of Res had no effect on human fetal lung fibroblast MRC-5 cells or on A549 cancer cells and the 5 or 10 µg/ml of PA also had no effect on MRC-5 normal cells. PA-L (5 µg/ml) and PA-H (10 µg/ml) had the growth inhibitory effects in NSCLC cell line A549, and Res increased these growth inhibitory effects. By flow cytometry experiment, after Res (5 µg/ml)+PA-H (10 µg/ml) treatment, the A549 cells showed the most apoptosic cells compared to other group treatments, and after additional treatment with Res, the apoptosic cells of both two PA concentrations were raised. Res+PA could reduce the mRNA and protein expressions of COX-2, and Res+PA could reduce the COX-2 related genes of VEGF, MMP-1, MMP-2, MMP-9, NF-κB, Bcl-2, Bcl-xL, procollagen I, collagen I, collagen III and CTGF, TNF-α, IL-1β, iNOS and raise the TIMP-1, TIMP-2, TIMP-3, IκB-α, p53, p21, caspase-3, caspase-8, caspase-9, Bax genes compared to the control cells and the PA treated cells. From these results, it can be suggested that Res could raise the anticancer effects of PA in A549 cells, thus Res might be used as a good sensitizing agent for PA.

8.In vitro bactericidal assay of linezolid on multidrug-resistant tuberculosis and nontuberculous mycobacteria

Objective To investigate the mycobactericidal efficacy of linezolid on multidrugresistant tuberculosis (MDR-TB) and nontuberculous mycobacteria (NTM) in vitro.Methods Seven hundred and sixty-two Mycobacterium tuberculosis and 20 NTM isolates from Hangzhou were identified by using hsp65 gene sequencing from March to June,2013.Seventy MDR-TB isolates,which were screened by using proportion method,and twenty NTM isolates were tested the MIC to linezolid,and the differences of MICs to linezolid between MDR-TB and NTM werw analyzed in vitro.Results All 70 MDR-TB isolates were inhibited at concentrations of ≤ 1 mg/L by linezolid,MIC50 was 0.5 mg/L and MIC90 1 mg/L.Of all 8 isolates inhibited at concentration of 1 mg/L,there were 6 isolates resistant to isoniazid,rifampin,streptomycin and ethambutol,suggesting this resistance pattern may reduce sensitivity to linezolid.Among the NTM isolates,18 Mycobacterium intracellulare isolates,whose MICs to linezolid at 8-16 mg/L,were non-resistant isolates,while 2 Mycobacterium abscessus isolates displaying MICs > 32 mg/L,were resistant to linezolid.Conclusions Linezolid showed great activity to MDR-TB and there were no linezolid-resistant MDR-TB isolates identified in this study.However,MICs of MDR-TB against linezolid increased with their resistant numbers to first-line agents.This study also showed that different NTM species have varied sensitivity to linezolid.
9.Spatiotemporal expression and analysis of responding consecutive monoculture genes in Rehmannia glutinosa.

OBJECTIVEBased on previous study, authors used the suppression subtractive hybridization (SSH) technique to construct the forward and reverse subtractive cDNA libraries of consecutive monoulture Rehmannia glutinosa. Five genes related with consecutive monoculture problem of R. glutinosa were chosen from the each of two subtractive libraries. And their spatiotemporal expression was measured in order to explore the functions in consecutive monoculture problem of R. glutinosa.
METHODUsing the real-time quantitative PCR, we tested the relative expression values of the genes in different development stages and tissues of normal growth (one-year culturing) and consecutive monoculture (two-year culturing) R. glutinosa.
RESULTThe five genes (calcium-dependent protein kinase, s-adenosyl-methionine synthetase, Aminocyclopropane-1-carboxylate oxidase, methyltransferase, calpain), which were chosen from the forward library had high expression in consecutive monoculture R. glutinosa, especially in root, and were hardly expression in normal growth R. glutinosa. On the contrary, the other five genes (RNA-dependent RNA polymerase, RNA replicase, DNA-directed RNA polymerase IIa, cyclin D, RNA binding protein) chosen from the reverse library had high expression in one-year R. glutinosa, but were down regulated or shut down in consecutive monoculture R. glutinosa.
CONCLUSIONThe key genes, which regulate inessential metabolism parthway (such as cyclin D, DNA-directed RNA polymerase IIa), were restrained or shut down in consecutive monoculture R. glutinosa. Calcium and ethylene signaling might played key roles in the formation of consecutive monoculture problem, resulting in disturbing normal metabolic process and syndrome of disease in R. glutinosa appeared in turn.

10.Evaluation of the short-time effect of radiofrequency ablation on kidney by contrast-enhanced ultrasonography and magnetic resonance imaging

ObjectiveTo evaluate the accuracy of the modalities in revealing the results by observing the radiofrequency ablated (RFA) lesions with contrast-enhanced ultrasonography (CEUS),magnetic resonance image (MRI) in comparison with histopathologic findings in normal rabbit kidney.Methods RFA was performed in the left kidneys of 10 rabbits.The ablated lesions were observed with conventional ultrasound(US),CEUS,MRI at 1 day,1 week after RFA.Then the rabbits subjected to postmortem and the kidney specimens were excised,stained and processed for conventional histologic study.The maximum lesion size as measured on US,CEUS and MRI was then correlated with the histopathologically determined lesion size.The utility of different imaging modalities were evaluated.ResultsIn a relatively short time (at 1 day and 1 week),CEUS and MRI may reveal the location,shape and extent of ablated lesion.And the lesion sizes with CEUS,MRI and kidney specimens were well correlated (P>0.05).ConclusionsBoth CEUS and MRI are suitable for the accurate evaluation of the ablated lesions in the short time.CEUS can reveal the variation of flow perfusion in the ablated lesion dynamicly and in real time.
