1.Hepatitis C virus infection:surveillance report from China Healthcare-as-sociated Infection Surveillance System in 2020

		                        			Objective To investigate the infection status and changing trend of hepatitis C virus(HCV)infection in hospitalized patients in medical institutions,and provide reference for formulating HCV infection prevention and control strategies.Methods HCV infection surveillance results from cross-sectional survey data reported to China Healthcare-associated Infection(HAI)Surveillance System in 2020 were summarized and analyzed,HCV positive was serum anti-HCV positive or HCV RNA positive,survey result was compared with the survey results from 2003.Results In 2020,1 071 368 inpatients in 1 573 hospitals were surveyed,738 535 of whom underwent HCV test,4 014 patients were infected with HCV,with a detection rate of 68.93%and a HCV positive rate of 0.54%.The positive rate of HCV in male and female patients were 0.60%and 0.48%,respectively,with a statistically sig-nificant difference(x2=47.18,P<0.001).The HCV positive rate in the 50-<60 age group was the highest(0.76%),followed by the 40-<50 age group(0.71%).Difference among all age groups was statistically signifi-cant(x2=696.74,P<0.001).In 2003,91 113 inpatients were surveyed.35 145 of whom underwent HCV test,resulting in a detection rate of 38.57%;775 patients were infected with HCV,with a positive rate of 2.21%.In 2020,HCV positive rates in hospitals of different scales were 0.46%-0.63%,with the highest in hospital with bed numbers ranging 600-899.Patients'HCV positive rates in hospitals of different scales was statistically signifi-cant(X2=35.34,P<0.001).In 2020,12 provinces/municipalities had over 10 000 patients underwent HCV-rela-ted test,and HCV positive rates ranged 0.19%-0.81%,with the highest rate from Hainan Province.HCV posi-tive rates in different departments were 0.06%-0.82%,with the lowest positive rate in the department of pedia-trics and the highest in the department of internal medicine.In 2003 and 2020,HCV positive rates in the depart-ment of infectious diseases were the highest,being 7.95%and 3.48%,respectively.Followed by departments of orthopedics(7.72%),gastroenterology(3.77%),nephrology(3.57%)and general intensive care unit(ICU,3.10%)in 2003,as well as departments of gastroenterology(1.35%),nephrology(1.18%),endocrinology(0.91%),and general intensive care unit(ICU,0.79%)in 2020.Conclusion Compared with 2003,HCV positive rate decreased significantly in 2020.HCV infected patients were mainly from the department of infectious diseases,followed by departments of gastroenterology,nephrology and general ICU.HCV infection positive rate varies with gender,age,and region.
		                        		
		                        		
		                        		
		                        	
2.Development and accuracy verification of a continuous non-invasive blood pressure measurement device/

		                        			Objective:To develop a continuous non-invasive blood pressure measurement device(NC-BPM)and verify the accuracy of that in monitoring blood pressure.Methods:A NC-BPM type of non-invasively continuous blood pressure measurement device(NC-BPM for short)was developed.The entire system consisted of four modules included one sensor which shape was finger cot,one unit of signal acquisition and pressure control,one calibration system for height and one host.The Omron J760 electronic sphygmomanometer(J760)and the CNAP Monitor500 continuous non-invasive blood pressure monitoring system(CNAP Monitor500)were used as the reference sphygmomanometer of verifying accuracy of the monitored blood pressure.The blood pressures of 25 subjects at three stages included static stage,blood pressure change stage and stage post calibration were respectively measured according to the standard test procedure of wearable sleeveless blood pressure measurement device of Institute of Electrical and Electronics Engineers(IEEE).Results:Both NC-BPM and CNAP Monitor500 collected 13753 data points,and there were high correlation in systolic blood pressure(SBP),diastolic blood pressure(DBP)and mean blood pressure(MAP)between the two devices(r=0.96,0.97,0.98,P<0.05).A total of 379 data points were collected by Omron J760,and there were high correlation in SBP and DBP between NC-BPM and Omron J760(r=0.98,0.95,P<0.05).According to the standards of British Hypertension Society(BHS)and the American Association for the Advancement of Medical Instrumentation(AAMI)to conduct grade evaluation,and the proportions of the SBP differences between NC-BPM and CNAP Monitor500 within the ranges of 0~5 mmHg,0~10 mmHg and 0~15 mmHg were respectively 85.01%,97.60%and 99.47%,and the proportions of the DBP differences between them within the three ranges were respectively 84.34%,99.85%and 100%,and the proportions of the MAP differences were respectively 92.66%,99.72%and 99.96%,all of which belonged to BHS grade A.The pressure difference values of measured SBP,DBP and MAP between the two devices were respectively(0.67±5.07)mmHg,(2.43±2.87)mmHg and(1.43±2.89)mmHg,which were within the(5±8)mmHg of AAMI standard range.The proportions of the SBP differences between NC-BPM and Omron J76 within the ranges of 0~5 mmHg,0~10 mmHg and 0~15 mmHg were respectively 79.95%,97.36%and 100%,and the proportions of the DBP differences between them within the three ranges were respectively 89.71%,99.74%and 100%,all of which belonged to BHS grade A.The difference values of systolic and diastolic blood pressures of brachial artery between the two devices were respectively(1.57±4.18)mmHg and(0.57±5.20)mmHg,which were within the(5±8)mmHg of AAMI standard range.Conclusion:The NC-BPM device has passed the first stage of clinical trials in accordance with IEEE standards,which can proceed to the next stage of clinical verification trials.
		                        		
		                        		
		                        		
		                        	
3.Application value of the deep learning-based image reconstruction algorithm in combined head and neck CT angiography with low radiation dose

		                        			Objective:To explore the differences between the deep learning-based image reconstruction (DLIR) and the adaptive statistical iterative reconstruction V (ASiR-V) algorithms in the radiation dose and image quality of head and neck CT angiography (CTA).Methods:The data of 80 patients undergoing head and neck CTA due to vascular diseases in the head and neck were prospectively collected. These patients were randomly divided into groups A and B based on their examination sequence. The CTA images of group A were reconstructed based on ASiR-V 50%, with a tube voltage of 120 kV and a noise index of 11.0. In contrast, those of group B were reconstructed based on ASiR-V 50% (for group B1) and DLIR-H (for group B2), with a tube voltage of 80 kV and a noise index of 9.0. Then, the radiation doses and image quality of both groups were compared using the independent-sample t-test. The radiation doses, and both subjective and objective image quality of the two imaging method were compared through the Kruskal-Wallis test and the Wilcoxon rank-sum test. The independent- or paired-sample t-test was employed to measure inter-group vascular enhanced CT values, as well as signals and noise from regions of interest (ROIs), with signal-to-noise ratios (SNRs) and contrast-to-noise ratios (CNRs) calculated. Results:The effective doses of groups A and B were (0.77±0.08) and (0.45±0.05) mSv, respectively, with a statistically significant difference ( t = 21.96, P < 0.001). The vascular enhanced CT values, SDs, SNRs, and CNRs in the arch of the aorta, the initial and bifurcation parts of the common carotid artery, and the M1 segment of the middle cerebral artery showed statistically significant differences among groups A, B1, and B2 ( F = 67.69, 68.50, 50.52, 74.10, 63.10, 91.22, 69.16, P < 0.001). Additionally, statistically significant differences were observed in the subjective scores of image quality among groups A, B1, and B2 ( Z = 71.06, P < 0.05). Conclusions:The DLIR algorithm can further reduce the radiation dose in head and neck CTA examination while significantly reducing image noise and ensuring image quality, thus demonstrating high clinical application value.
		                        		
		                        		
		                        		
		                        	
4.A multicenter study on the prognostic value of SPECT myocardial blood flow quantitative technique in patients with intermediate stenoses of coronary arteries

		                        			Objective:To investigate the prognostic value of myocardial flow reserve (MFR) measured by SPECT myocardial blood flow (MBF) quantitative technique in patients with intermediate stenoses of coronary arteries.Methods:From September 2019 to May 2021, patients with intermediate stenoses (50% to 80%) identified by invasive coronary angiography in Fuwai Hospital, Chinese Academy of Medical Sciences, Fuwai Center China Cardiovascular Hospital, and TEDA International Cardiovascular Hospital were prospectively included. All patients underwent a one-day rest/stress SPECT myocardial perfusion imaging (MPI) and SPECT MBF quantification. The radioactivity distribution of each segment of the MPI bullseye polar maps were obtained according to the standard 5-point method to obtain the summed stress score (SSS) and the summed difference score (SDS) to determine the existence of abnormality. ROC curve analysis was used to obtain the optimal prognostic cut-off value for MFR. The primary endpoint was defined as cardiovascular endpoint events. Survival and prognostic analyses were conducted by Kaplan-Meier method and Cox proportional hazard models. The difference of AUCs was analyzed by Delong test.Results:A total of 314 patients (194 males, 120 females; age (59.4±8.6) years) were enrolled. Over a median follow-up duration of 754 (range: 628-914) d, 54 patients had endpoint events. ROC curve showed that the prediction ability of MFR was significantly better than that of conventional MPI (AUCs: 0.713 and 0.512; z=3.76, P<0.001). The optimal prognostic cut-off value for MFR to predict endpoint events in patients with intermediate stenoses was 2.04. Cox multivariate analysis showed that MFR (hazard ratio ( HR)=0.434, 95% CI: 0.282-0.669, P<0.001) was an independent predictor of endpoint events in patients with intermediate stenoses. Kaplan-Meier survival analysis showed that the prevalence of endpoint events in patients with MFR≤2.04 was significantly higher than that in patients with MFR>2.04 (25.4%(43/169) vs 7.6%(11/145); χ2=21.27, P<0.001). Conclusion:The MFR measured by SPECT MBF quantitative technique has an independent predictive value for cardiovascular endpoint events in patients with intermediate stenoses.
		                        		
		                        		
		                        		
		                        	
5.Clinical study of perceptual eye position and fixation stability in adolescents with low myopia

		                        			 AIM:To test and compare the perceptual eye position and fixation stability of adolescents with emmetropia and adolescents with low myopia, investigating the characteristics of the perceptual eye position and fixation stability of adolescents with low myopia.METHODS: Cross-sectional study. A total of 132 adolescents(264 eyes)who visited in the ophthalmology clinic of our hospital from April to December 2023 were randomly selected as the research subjects. Participants were categorized into normal control group(n=45, 90 eyes), simple low myopia group(n=45, 90 eyes)and low myopia with anisometropia group(n=42, 84 eyes)according to their refractive status and were underwent assessments for perceptual eye position and fixation stability.RESULTS: Compared with the normal control group, the static and dynamic horizontal perceptual eye position deviation of the simple low myopia group and the low myopia with anisometropia group were significantly increased(P<0.05). Compared with the simple low myopia group, the static and dynamic horizontal perceptual eye position deviation of the low myopia with anisometropia group were significantly increased(P<0.05). There was no significant difference in static and dynamic vertical perceptual eye position deviation among the three groups(P>0.05); compared with the normal control group, the horizontal and vertical fixation stability of the simple low myopia group and the low myopia with anisometropia group were significantly worse(all P<0.01), but there was no differences in the simple low myopia group and the low myopia with anisometropia group(P >0.05).CONCLUSION: Abnormalities are observed in perceptual eye position and fixation stability function in adolescents with low myopia compared with those adolescents with emmetropia, even at best corrected visual acuity. 6.Progressive Changes of Colitis Into Colorectal Cancer Associated With Changes in Neutrophil Lymphocyte Ratio, Serum, Micrornas and Infiltrate Neutrophils

		                        			Introduction: Understand the progression of colorectal cancer from the beginning until the advance stages is difficult 
and challenging. However, this could be overcome with a good animal model. Methods: In this study, a modified approach had been used to develop colorectal cancer model. The model was developed and monitored from colitis formation until the late stage of colorectal cancer. The changes of neutrophil lymphocyte ratio (NLR), serum microRNAs 
and infiltrate neutrophil in different stages of colorectal cancer were assessed in this study. Results: Results showed 
that the progression of the disease is correlated with NLR as early as the formation of colitis (r=0.121, p<0.026). 
Meanwhile, the size of the tumor at each stage is also associated with the NLR value (r=0.185, p<0.0012). In the 
serum microRNAs study, it was found microRNAs expression in blood serum change in different stages of colorectal 
cancer. In the early stage of colitis formation, miR223 (> 3 fold expression, p < 0.0025) were abundantly found in the 
blood serum. Meanwhile in others stage mild (miRNA345 > 2.5 fold, p<0.0011), moderate (miRNA347 & miR512 > 
3 fold, p<0.002) and severe (miR31 & miR145 > 2 fold, p<0.0001) microRNAs were also found expressed differently. 
The quantities of infiltrate neutrophil were varied in different stages of the disease. Conclusion: This study provides 
an insight into the immunity and molecular level of colorectal cancer and it allows a progressive monitoring on the

7.Development and performance testing of a novel transcatheter tricuspid valve interventional device

		                        			Objective  To develop a novel transcatheter tricuspid valve replacement device and test its performance. Methods The transcatheter tricuspid valve stent consisted of double-layer self-expanding nitinol stent, biotissue-derived bovine pericardial leaflets, and PTFE woven. The delivery system, mainly consisting of a handle control unit and a delivery sheath, was sent to the correct position via right atrium or jugular vein. The sheath had a visualization feature, and the handle control unit could realize the functions of stable release and partial recovery of the interventional valve. In addition, this study performed animal survival experiments on the basis of in vitro experiments. A large-white pig was used as the experimental animal. Cardiopulmonary bypass was established through median thoracotomy, then the right atrium was opened, and the interventional valve was released under direct vision without cardiac arrest. Approximately 1 month after interventional valve implantation, the maneuverability and stability of the interventional tricuspid device were evaluated by autopsy. Results Through the animal experiment, the interventional valve was successfully released, and the anchoring was satisfactory. Postoperative transthoracic echocardiography showed that the interventional valve opened and closed well, the flow rate of tricuspid valve was 0.6 m/s, and there was no obvious tricuspid regurgitation. One month after the operation, we dissected the large-white pig and found the interventional valve was not deformed or displaced, the leaflets were well aligned, and there was thrombus attachment in the groove between the inner and outer layers of the interventional valve. Conclusion Animal experiment shows that the novel device can stably and firmly attach to the tricuspid annulus, with good anchoring effect, and effectively reduce paravalvular leakage.
		                        		
		                        		
		                        		
		                        	
8.Application effect of the nursing model of doctor-nurse collaborative responsibility system in preventing catheter-related infection with tunnel-cuffed catheter

		                        			Objective To explore the clinical application effect of the nursing model of the doctor-nurse collaborative responsibility system in preventing catheter-related infection with tunnel-cuffed catheter(TCC).Methods A total of 130 hemodialysis patients using TCC were enrolled from the department of nephrology of Tianjin Hospital,Tianjin University,during the period from January 2021 to December 2021 were selected as research objects.They were divided into two groups based on different admission times,with 65 patients in each group.The control group,comprising patients admitted from January 2021 to June 2021,received traditional nursing care;while the study group,consisting of patients admitted from July 2021 to December 2021,followed a doctor-nurse collaborative responsibility nursing model.Incidence of catheter-related infection,the length of hospital stay,the duration of catheter placement and patients'satisfaction with nursing were calculated,and the results were compared between the two groups.Results Compared with the control group,the incidence of catheter exit-site infection,incidence of TCC,incidence of total infection were all significantly decreased in the study group[the incidence of catheter exit-site infection:3.08%(2/65)vs.15.38%(10/65),incidence of TCC:1.54%(1/65)vs.6.15%(4/65),incidence of total infection:6.15%(4/65)vs.24.62%(16/65),all P<0.05].There was no significant difference in incidence of catheter-related bloodstream infection between the observation group and the control group[1.54%(1/65)vs.3.08%(2/65),P>0.05].Compared with the control group,the length of hospital stay in the observation group was significantly shortened in the study group(days:12.45±3.62 vs.19.25±5.09,P<0.05),the duration of catheter placement was increased remarkably in the study group(days:83.64±14.20 vs.65.27±12.38,P<0.05),and the patients'satisfaction with nursing was significantly improved in the study group[96.92%(63/65)vs.81.54%(53/65),P<0.05].Conclusion The implementation of the nursing model of doctor-nurse collaborative responsibility system can effectively reduce the occurrence of catheter-related infection with TCC,dramatically decrease hospitalization time and significantly improve the patients'satisfaction with nursing.
		                        		
		                        		
		                        		
		                        	
9.A qualitative study on the symptom perception and coping experience of patients with low anterior resection syndrome after anus-preserving surgery for rectal cancer

		                        			Objective:To explore the symptom perception and coping experiences of patients with low anterior resection syndrome (LARS) after anus-preserving surgery for rectal cancer.Methods:This study adopted the phenomenological research method in qualitative research. From October 2022 to January 2023, purposive sampling was used to select 15 postoperative LARS patients with rectal cancer who underwent anus-preserving surgery and underwent outpatient follow-up or hospitalization at Henan Cancer Hospital as the research subject for semi-structured in-depth interviews. Colaizzi 7-step analysis method was used to analyze, summarize, and extract themes from interview data.Results:Four themes were extracted, including attitudes and emotional responses towards symptoms, daily activities influenced by symptoms, coping status with symptom management strategies, and the need for multidimensional support for symptoms.Conclusions:LARS patients after anus-preserving surgery for rectal cancer have many discomfort symptoms and are eager to receive support and assistance. Medical and nursing staff should pay attention to the complexity of patient symptom management and promptly conduct effective symptom assessment and intervention, providing patient-centered comprehensive supportive nursing, and improving their quality of life.
		                        		
		                        		
		                        		
		                        	
10.Circular RNA Circ_0000291/miR-326/PTBP1 axis affect progression of hepatocellular carcinoma

		                        			Objective:To explore the effects of Circ_0000291 on proliferation,apoptosis and immune escape profiling of he-patocellular carcinoma(HCC)cells as a molecular sponge to regulate miR-326 through targeting polypyrimidine tract-binding protein 1(PTBP1).Methods:The expressions of Circ_0000291,miR-326 and PTBP1 in tissue samples and cells were measured by qRT-PCR.Dual luciferase reporter assay was employed to confirm the targeting relationship between Circ_0000291 and miR-326 as well as miR-326 and PTBP1.CCK-8 assay,Transwell assay,flow cytometry assay and ELISA were respectively utilized to determine the prolifera-tion,invasion,apoptosis and the expressions of immune regulation related factors(TNF-α,IFN-γ).Results:Compared with normal tissue and hepatic cells,Circ_0000291 and PTBP1 expressions were overexpressed in HCC tissue and cells,but the expression of miR-326 was obviously down-regulated(all P<0.05).Moreover,functional assays indicated that downregulation of Circ_0000291 could inhibit the proliferation,invasion,and immune escape while induce apoptosis of HCC cells(all P<0.05),which was according-ly exhibited in miR-326 transfection group.Overexpression of Circ_0000291 partly attenuated the anti-tumor effects induced by miR-326(all P<0.05).In HCC,Circ_0000291 could promote the expression of PTBP1 as a molecular sponge to regulate miR-326.Conclu-sion:Circ_0000291 served as a molecular sponge of miR-326 to increase PTBP1 expression and promote the proliferation,invasion and immune escape but inhibit apoptosis of HCC.There upon then,it facilitates the progression of HCC.
