1.Association of Intensive Endoscopic Burden with Esophageal Cancer Detection: A Nationwide Cohort Study

Background/Aims:
Early diagnosis of esophageal cancer (EC) remains challenging despite the increasing frequency of endoscopic screenings globally. The rapidly increasing number of endoscopic screenings performed over a certain period might influence diagnostic performance. This study evaluated the association between the number of endoscopic screenings and EC detection rates in a nationwide cohort.
Methods:
This retrospective population-based study used the Korean National Cancer Screening Program database, comprising 32,774,742 males and females aged ≥40 years between 2015and 2019. Negative binomial regression model and least-squares mean evaluation were used to assess the association between month of the year and EC detection rates.
Results:
This study enrolled 28,032,590 participants who underwent upper endoscopy. The number of participants in the fourth quarter (October to December: 10,923,142 [39.0%]) was 2.1 times higher than that in the first quarter (January to March: 5,085,087 [18.1%]); this trend continued for all 5 years. Contrarily, detection rates for EC in the fourth quarter (0.08/1,000 person) were half that in the first quarter (0.15/1,000 person). The odds of detecting EC were lowest in November; in 2015 the odds were 0.57 (95% confidence interval, 0.41 to 0.79; p=0.001) times lower and in 2016, they were 0.51 (95% confidence interval, 0.37 to 0.68; p<0.001) times lower compared to January. The predicted detection rates showed a decreasing trend toward the end of the year (p>0.05 for all).
Conclusions
The workload of endoscopists increased excessively with the rising number of endoscopies toward the end of the year, which was reflected by the decreased EC detection rates during this period.
2.Validation of Korean Version of the Oxford Cognitive Screen (K-OCS), a Post Stroke-Specific Cognitive Screening Tool

Objective:
To establish and evaluate the validity of the recently developed Korean version of the Oxford Cognitive Screen (K-OCS), this study verified its reliability, validity, and diagnostic accuracy.
Methods:
Between November 2021 and December 2023, we recruited 72 patients with stroke from our hospital who agreed to participate in the study. The patients were repeatedly tested using K-OCS by the same or different assessors to estimate inter- and intra-rater reliability. To demonstrate the validity and usability of K-OCS, the test results of screening tools currently used in clinical practice, including the Korean-Mini Mental State Examination and the Korean version of the Montreal Cognitive Assessment, were used in comparison analyses.
Results:
The subtests of K-OCS demonstrated excellent inter-rater reliability (intra-class correlation coefficient [ICC]=0.914–0.998) and test–retest reliability (ICC=0.913–0.994). We found moderate-to-strong correlations for convergent validity for the subsets (r=0.378– 0.979, p<0.01), and low-to-moderate discriminant validity correlations. The optimal cut-offs estimated for the subtests of the K-OCS showed a good-to-high range of specificity (94.8%– 100%). The positive predictive value was 58.2%–100% and negative predictive value was 65.6%–98.4%. Sensitivity was estimated at 25.6%–86.9%.
Conclusion
The results of this study indicate that K-OCS is a reliable and valid tool for screening cognitive impairment in patients post-stroke.

This study aims to explore the trend of nursing education and major research themes in the Journal of Korean Academic Society of Nursing Education (JKASNE) over 30 years. Methods: A total of 1,026 articles, published in the JKASNE from 1995 to 2023, were analyzed using the Microsoft Excel program for descriptive statistics and NetMiner for a word cloud of Korean keywords.
Results:
Most research subjects were healthy people (n=841). Education-related research topics (n=619) were categorized into nine domains: educational methods, educational curriculum, study type and methods, knowledge level and contents, psychosocial variables related to knowledge and education, attitude, behavior, development and evaluation of educational programs, and effects of educational programs. Quantitative (n=835) and descriptive studies (n=497) were determined to be the most common research designs. Self-reported questionnaires were the most common method used in surveys (n=581) and experimental studies (n=153). Descriptive statistics, Pearson's correlation coefficient, and other statistical techniques were used. The keywords in the JKASNE reflected various aspects of nursing research. General terms related to nursing education, such as "nursing students," "nurses," and "nursing education," appeared with high frequency, while more specific terms and phrases reflecting educational competencies and characteristics, such as "self-efficacy," "clinical performance ability," and "communication knowledge," were also prominently featured.
Conclusion
The JKASNE has published mainly education-focused papers. Analyzing the full set of papers published in JKASNE and examining their characteristics is very meaningful and helpful for the future development of Korean nursing education.
