		                        			BACKGROUND: Medical skin care is essential for the treatment of skin diseases all over the world. Medical skin care is also part of medical practice and this must be differentiated from the simple skin care that is given for normal healthy skin. OBJECTIVE: We wanted to discuss medical skin care and the related medical devices and legal issues. METHODS: We reviewed the related laws and regulations, we consulted experts and associations and we analyzed the result of the survey. RESULTS: Legally, medical skin care and simple skin care are well classified. However, many illegal procedures are still performed by non-medical personnel and many adverse effects have been reported as a result. Furthermore, there are no legal restrictions for the performer based on the grade of each medical skin care procedure. CONCLUSION: For the best results and safe procedures, medical skin care must be performed by approved medical equipment under the supervision of a physician or medical personnel. 2.Partial Trisomy 1q41 Syndrome Delineated by Whole Genomic Array Comparative Genome Hybridization.

		                        			Partial trisomy 1q syndrome is a rare chromosomal abnormality. We report on a male infant with 46,XY,der(11)t(1;11)(q41;p15.5) due to unbalanced segregation of the maternal reciprocal balanced translocation 46,XX,t(1;11)(q41;p15.5). The baby presented with a mild phenotype, characterized by a triangular face, almond-shaped eyes, low ears, short stature with relatively long legs, and mild psychomotor retardation. We utilized whole genomic array comparative genome hybridization (CGH) with 4,000 selected bacterial artificial chromosomes (BACs) to define the chromosomal breakpoints and to delineate the extent of the partial trisomy in more detail. To our knowledge, this is the first case of nearly pure "partial trisomy 1q41" defined by whole genomic array CGH.

		                        			Inguinal hernia is the most common disease treated by the pediatric surgeon. There are several controversial aspects of management 1)the optimal timing of surgical repair, especially for preterm babies, 2)contralateral groin exploration during repair of a clinically unilateral hernia, 3)use of laparoscope in contralateral groin exploration, 4)timing of surgical repair of cord hydrocele, 5)perioperative pain control, 6)perioperative management of anemia. In this survey, we attempted to determine the approach of members of KAPS to these aspects of hernia treatment. A questionnaire by e-mail or FAX was sent to all members. The content of the questionnaire were adapted from the "American Academy of Pediatrics (AAP) Section on Surgery hernia survey revisited (J Pediatr Surg 40, 1009-1014, 2005)". For full-term male baby, most surgeons (85.7 %) perform an elective operation as soon as diagnosis was made. For reducible hernia found in ex-preterm infants already discharged from the neonatal intensive care unit (NICU), 76.2 % of surgeons performed an elective repair under general anesthesia (85.8 %). 42.9 % of the surgeons performed the repair just before discharge. For same-day surgery for the ex-premature baby, the opinion was evenly divided. For an inguinal hernia with a contralateral undescended testis in a preterm baby, 61.9 % of surgeons choose to 'wait and see' until 12 month of age. The most important consideration in deciding the timing of surgery of inguinal hernia in preterm baby was the existence of bronchopulmonary dysplasia (82.4 %), episode of apnea/bradycardia on home monitoring (70.6 %). Most surgeons do not explore the contralateral groin during unilateral hernia repair. Laparoscope has not been tried. 		                        			Diagnostic genetics subcommitee of KSQACP has performed two trials each for cytogenetic studies and molecular genetic studies in 2005. Cytogenetic surveys were performed by 37 laboratories and answered correctly in most laboratories. And the first trial with whole blood specimen from the patient with cytogenetic abnormality (mosaic Turner syndrome) was successfully done, which could evaluate the whole process for cytogenetic studies from cell culture, harvest and karyotyping.. The molecular genetic test surveys included many kinds of tests like M. tuberculosis, HBV, HCV, HPV, leukemias/lymphomas, ApoE genotyping, MTHFR genotyping, BRCA1 & BRCA2, Duchenne muscular dystrophy, and Huntington disease. Molecular genetic surveys showed excellent results in most of participants. 5.The effect of the application of growth factors on wound contraction : An experimental study in a fibroblast-populated collagen lattice.

		                        			Many investigators have reported that collagen gel contraction reflects the mechanism of wound contraction. In 1995, Tsai et al. reported that hypertrophic scar-derived fibroblasts in a connective tissue model possessed the greatest contraction potency when compared with those of normal skin and normal oral mucosa-derived CTMs. In this study, we studied the effect of collagen gel contraction by growth factors such as epidermal growth factor, platelet-derived growth factor, transforming growth factor-bata1, and transforming growth factor-bata3, Skin fibroblasts used in this study were obtained from the explant of rat skin culture. Fibroblasts were cultured in Dulbecco's modified Eagle's medium containing 10% fetal bovine serum. Growth factors were added per FPCL in the desired concentrations and we measured the collagen gel diameters in growth factor-treated FPCL on day 1,2,3, and 4 respectively after starting incubation. We examined the effects of EGF, PDGF, TGF-bata1, TGF-bata3 and the effects of combinations of TGF-bata1 + EGF, TGF-bata1 + PDGF, and TGF-bata1 + TGF-bata3 to contract a collagen gel. EGF has little influence on collagen gel contraction. TGF-bata1 and TGF-bata3 increase the collagen contraction. TGF-bata1 enhanced the contractility of collagen gel according to the concentrations. While TGF-bata3 alone had stimulatory contraction effects at low dose, high doses of TGF-bata3 decreased the potency of collagen gel contraction. A combination of TGF-bata1 and EGF minimally decrease TGF-bata1 activity. A combination of TGF-bata1and PDGF had an effect similar to TGF-bata1 activity. A combination of TGF-bata1 and TGF-bata3 decreased TGF-bata1 activity. According to reports that FPCL contraction is equivalent to the process of wound contraction, growth factors which enhance gel contraction may be related to wound contraction and wound healing. TGF-bata1 is reported to enhance scar formation in fetal wound. EGF accelerates wound healing and inhibits the promotion of hypertrophic scar formation. Compared to the effect of collagen gel contraction in this study, the combination of TGF-bata1 and TGF-bata3 that inhibited the promotion of collagen gel contraction are thought to diminish the formation of scar tissue. As well, EGF that has not enhanced collagen gel contraction is thought to diminish the production of scar tissue. We will study the interactive effects of TGF-bata3, EGF and TGF-bata1 on the contraction of collagen gels in the future.

6.Clinical Analysis of Breast Cancer.

		                        			The factors affecting the survival rate of breast-cancer patients were studied in 185 cases treated at the Department of Surgery, College of Medicine, Chonbuk National University from January 1987 to December 1996. The results are as follows: 1) The most prevalent age group was the 5th decade (57 cases, 30.8%). 2) The most common initial symptom was a palpable mass (167 cases, 90.3%). 3) In 134 cases (72.4%), the patients visited the hospital within 6 months of the onset of the first symptom. 4) The primary tumor was located on the left side in 101 cases (54.6%) and on the right side in 84 cases (45.4%), and the most frequent tumor location was the upper outer quadrant (113 cases, 61.1%). 5) The pathological types, according to the WHO classification, were an invasive ductal carcinoma (179 cases, 96.7%) and a medullary carcimona (22 cases, 11.9%) in that order. 6) According to the TNM system, the most common stage was stage II (100 cases, 54.1%). The overall 5-year survival rates according to pathologic stage were 96.0% for Stage I, 83.1% for Stage II, 67.8% for Stage III, and 12.5% for Stage IV. 7) The estrogen and the progesterone receptor status had no significance for the survival rate. 8) The most common type of operation was a modified radical mastectomy (Patey) (150 cases, 81.0%). 9) Axillary lymph node metastases were present in 77 cases (41.6%). 10) The most common distant metastasis was the bone (12 cases, 6.5%). 11) The overall 5-year actuarial survival rate after surgery was 76.6%. The 5-year survival rates according to the number of invaded lymph-node were 83.5% for 0 nodes, 77.6% for 1~3 nodes, 57.5% for 4~9 nodes, and 49.4% in cases of more than 10 nodes.

7.Pedunculated Lipofibroma.

		                        			A 45-year-old woman had a bean-sized, asymptomatic, soft nodule on the right buttock for 4 years. Histopafhologic evaluation revealed acanthosis and a central core of adipose tissue with thin papillary and reticular dermis containing eccrine glands. Staining with alcian blue showed a slight increase of mucopolysaccharides in the reticular dermis. 8.Phytophotodermatitis.

		                        			We describe herein a 3 family members with phytophotodermatitis. After fig (juice) was applied to skin for the purpose of treating psoriasis in a 30 year-old woman and applied incidentally in her mother and 3 year-old daughter, vesicobullous eruptions developed immediately after sun-exposure, predominantly in the sun-exposed area. ln histopathology, the epidermis shows intraepidermal separation with necrosis and reepithelization on basal layer and marked spongiosis. 9.Effect of Acupuncture on Histamine Induced Itch.

		                        			We cairried out this study to find the possibility of itch control with acupuncture under the hypothesis that antipruritic effect also exists if the itch sensation is closely allied to pain. We evaluated the effect of acupuncture on histarnine induced itch, flare and wheal in 20 healthy male volunteers aged 24 32 years old and compared it with the effect of pseudoacupuncture on them and that of no intervention on them. The results were as follows: 1. Mans of itch intensity and duration were decreased in A(session with acupuncture) and P(session with Pseudoacupuncture), compared to those of N(session with no intervention), and A was more effective in decreasing the itch intensity and its duration than P. 2. 10.Separation of Human Epidermal Cells by Using Percoll.

		                        			The epidermis, which is composed mainly of keratinocyt;es, undergoes continual proliferation and differentiation, To study keratinocyte differentiation, it is necessary to separate the keratinocytes at their sucessive stages of maturation.
