1.Shaoyaotang Restores Th17/Treg Cell Balance by Regulating Glucose Metabolism Reprogramming in Treatment of Ulcerative Colitis

		                        			ObjectiveTo investigate the effect of Shaoyaotang on T helper cell 17/regulatory T lymphocyte(Th17/Treg) cell balance in ulcerative colitis and decipher the intervention mechanism based on glucose metabolism reprogramming. MethodsThe mouse model of ulcerative colitis was established by the dextran sulfate sodium (DSS) method. Forty-eight C57BL/6 mice were randomly allocated into normal, model, Western drug control (mesalazine, 0.39 g·kg-1·d-1), Shaoyaotang (15.54 g·kg-1·d-1), inhibitor (2-deoxy-D-glucose, 2-DG, 100 mg·kg-1·d-1), and inhibitor (2-DG, 100 mg·kg-1·d-1) + Shaoyaotang (15.54 g·kg-1·d-1) groups. Mice were administrated with the corresponding drugs by gavage for 7 days. The general conditions and the colon injury degree were observed 24 h after the last administration. The expression of interleukin (IL)-10 and IL-17 in the colon tissue was detected by immunohistochemical staining. Western blot and Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) were performed to determine the protein and mRNA levels, respectively, of hypoxia-inducing factor-1α (HIF-1α), lactate dehydrogenase (LDHA), and hexokinase 2 (HK2) in the colon tissue. Th17/Treg cell differentiation was detected by flow cytometry. Enzyme-linked immunosorbent assay was employed to measure the levels of lactic acid and glucose in the colon tissue and IL-10, IL-17, and IL-6 in the serum. ResultsCompared with the normal group, the model group showed decreases in body weight and disease activity index (DAI) (P<0.05), elevations in levels of HIF-1α, LDHA, HK2, IL-17, IL-6, Th17 cells, lactic acid, and glucose in the colon tissue (P<0.05), and declines in the levels of of IL-10 and Treg cells (P<0.05). Compared with the model group, the drug administration groups showed increases in body weight and DAI (P<0.05), declines in levels of HIF-1α, LDHA, HK2, IL-17, IL-6, Th17 cells, lactic acid, and glucose in the colon tissue (P<0.05), and rises in levels of IL-10 and Treg cells (P<0.05). Shaoyaotang+2-DG group had the most obvious effect. 3.Effect of Naples prognostic score on prognosis of hepatocellular carcinoma patients undergoing hepatectomy

		                        			Objective:To assess the impact of preoperative Naples prognostic score on the prognosis of patients with hepatocellular carcinoma (HCC) after hepatic resection.Methods:Retrospective analysis was conducted on the data of 323 patients with HCC who underwent radical hepatectomy in Zhejiang Provincial People's Hospital from January 2012 to December 2017, including 281 males and 42 females, aged (56.6±11.3) years. All patients were divided into three groups according to their preoperative Naples prognostic scores: group A (0) ( n=37), group B (1-2) ( n=193), group C (3-4) ( n=93). Survival was analysed by the Kaplan-Meier method, and differences in survival were compared by the log-rank test. Univariate and multivariate Cox regression were used to analyse the effect of Naples prognostic score on prognosis. Results:The 1-, 3- and 5-year cumulative survival rates of HCC patients after hepatectomy were 91.9%, 78.4% and 68.3% in the A group, 89.1%, 76.1% and 64.4% in the B group, and 84.9%, 63.3% and 43.5% in the C group, respectively, and the cumulative survival rates showed a decreasing trend among the three groups, and the differences were statistically significant (all P<0.05). The recurrence-free survival rates at 1, 3 and 5 years after hepatectomy were 93.4%, 63.3% and 44.3% in the A group, 77.7%, 46.5% and 35.6% in the B group, and 64.1%, 41.1% and 28.2% in the C group, respectively, and the recurrence-free survival rates showed a decreasing trend among the three groups, and the differences were statistically significant (all P<0.05). On Cox multivariate analysis, patients with HCC of 3-4 had a higher risk of death after hepatic resection than those patients of 0 ( HR=2.011, 95% CI: 1.048-3.859, P=0.036), and the risk of postoperative recurrence was also higher than those patients of 0 ( HR=1.820, 95% CI: 1.081-3.066, P=0.024). Conclusion:Preoperative Naples prognostic score performs as a prognostic influence factor on survival and recurrence-free survival after hepatectomy in patients with HCC.
		                        		
		                        		
		                        		
		                        	
4.Differential diagnosis and treatment of chylothorax and pseudochylothorax after lung cancer surgery

		                        			Objective:To explore the differential diagnosis and different treatment methods of chylothorax and pseudochylothorax after lung cancer surgery.Methods:Clinical data of 1 584 surgical patients with non-small cell lung cancer from January 2016 to December 2021 were analyzed, 21 cases of chylothorax and 8 cases of pseudochylothorax were identified and analyzed to compare the differences in pleural fluid chyle test, pleural effusion biochemical values, total cholesterol, triglycerides, total cholesterol/triglyceride ratio, leukocyte count, bacterial culture and treatment.Results:The incidence of chylothorax after lung cancer surgery was 1.3%, and the incidence of pseudochylothorax was 0.5%; 80.9%% of chylothorax on the right side was significantly higher than 19.1% of chylothorax on the left side, and the difference was statistically significant( P<0.05). Pseudochylothorax occurred on the right side(100%). The difference between chylothorax and pseudochylothorax in pleural fluid tests for cholesterol and triglyceride was statistically significant( P<0.05), the leukocyte count was significantly higher in pseudochylothorax than chylothorax, and the difference was statistically significant( P<0.05). The differences in drainage before treatment, postoperative drainage time and postoperative hospitalization time between the two groups were statistically significant( P<0.05). The success rate was 61.9% in 13 cases of chylothorax treated conservatively and 38.1% in 8 cases of thoracic duct clamping; all cases of pseudochylothorax were treated conservatively with a success rate of 100%. Conclusion:In naddition to pleural fluid chyle test and pleural effusion biochemical values, total cholesterol, triglyceride and total cholesterol to triglyceride ratio in pleural fluid should be tested to identify chylothorax and pseudochylothorax, high triglyceride in pleural fluid diagnosed as chylothorax; Pseudochylothorax is diagnosed with a cholesterol/triglyceride ratio >1 in the pleural fluid, pseudochylothorax is usually treated conservatively. Chylothorax is treated conservatively and surgically according to different conditions. If the drainage flow is greater than 800 ml/day for 3 consecutive days or if it causes serious electrolyte disorders, it is recommended to perform thoracoscopic-assisted thoracic duct clamping via right-sided approach.

		                        			The vagina contains at least a billion microbial cells,dominated by lactobacilli.Here we perform metagenomic shotgun sequencing on cervical and fecal samples from a cohort of 516 Chinese women of reproductive age,as well as cervical,fecal,and salivary samples from a second cohort of 632 women.Factors such as pregnancy history,delivery history,cesarean section,and breastfeeding were all more important than menstrual cycle in shaping the microbiome,and such information would be necessary before trying to interpret differences between vagino-cervical micro-biome data.Greater proportion of Bifidobacterium breve was seen with older age at sexual debut.The relative abundance of lactobacilli especially Lactobacillus crispatus was negatively associated with pregnancy history.Potential markers for lack of menstrual regularity,heavy flow,dysmenor-rhea,and contraceptives were also identified.Lactobacilli were rare during breastfeeding or post-menopause.Other features such as mood fluctuations and facial speckles could potentially be predicted from the vagino-cervical microbiome.Gut and salivary microbiomes,plasma vitamins,metals,amino acids,and hormones showed associations with the vagino-cervical microbiome.Our results offer an unprecedented glimpse into the microbiota of the female reproductive tract and call for international collaborations to better understand its long-term health impact other than in the settings of infection or pre-term birth.
		                        		
		                        		
		                        		
		                        	
6.Establishment and Analysis of the Fingerprints of Adiantum capillusveneris from Different Producing Areas and Content Determination of Its Differential Components

		                        			OBJECTIVE:To establish the fingerprint of Adiantum capillusveneris from different producing areas ,and to conduct chemometric analysis and content determination of differential components ,so as to provide reference for quality control of A. capillusveneris . METHODS :HPLC-DAD combined with Similarity Evaluation System of TCM Chromatogramtic Fingerprint (2004 A edition )were used to establish fingerprint of 19 batches of A. capillusveneris from different producing areas (S1-S19). Common peaks were confirmed and their similarities were evaluated. Chemometric analysis methods such as cluster analysis , principle component analysis (PCA),orthogonal partial least squares discriminant analysis (OPLS-DA)were used to evaluate the quality of 19 batches of A. capillusveneris from different producing areas ,screen the differential components ,and determine the contents of some differential components. RESULTS :Among 19 batches of A. capillusveneris from different producing areas ,22 common peaks were confirmed ;peak 9 was chlorogenic acid ,peak 17 was quercetin- 3-O-β-D-glucopyranoside,peak 20 was kaempferol-3-O-rutoside;the similarity of 19 batches of sample were 0.677-0.962. Through cluster analysis ,it was found that S 7 and S 10 were clustered into one category ;S15 and S 18 were clustered into one category ;and S 1-S6,S8,S9,S11-S14,S16,S17 and S 19 were clustered into one category. PCA and OPLS-DA found that S 7 and S 10 were clustered into one category ;S15 were clustered into one category ;S18 were clustered into one category ;S1-S6,S8,S9,S11-S14,S16,S17 and S 19 were clustered into one category. Chlorogenic acid ,quercetin-3-O-β-D-glucopyranoside,kaempferol-3-O-rutoside and chemical composition represented by peak 14 were the differential components of the〔2017〕2841); medicinal material. Among 19 batches of A. capillusveneris , average contents of chlorogenic acid and quercetin- 3-O-β-D-  glucopyranoside and kaempferol- 3-O-rutoside were 0.10-4.25,  0.31-7.11,0.61-12.00 mg/g,respectively. 7. Analysis of drug resistance and homology of 226 Acinetobacter baumannii in geriatric department

		                        			Through statistical analysis of drug resistance and homology of Acinetobacter baumannii in geriatric department of our hospital, we can provide evidence for clinical experience in antibiotics use and nosocomial infection control.
		                        		
		                        			Methods:
		                        			The drug resistance of Acinetobacter baumannii isolated from the elderly rehabilitation department in Ningbo Kangning Hospital from January 2017 to December 2018 was analyzed by retrospective analysis.The homology of Acinetobacter baumannii isolated from clinical isolates was detected by PCR ERIC-PCR method and agarose gel electrophoresis method.
		                        		
		                        			Results:
		                        			The resistance rates of Acinetobacter baumannii to ciprofloxacin and levofloxacin were 41.6% and 35.4%, respectively.The resistance rates to cefepime, ceftriaxone and ceftazidime were 40.3%, 38.9% and 36.7%, respectively.The resistance rate to imipenem was 38.9%, and the sensitivity rates to cefoperazone/sulbactam, gentamicin and tobramycin were all over 70%.A total of 226 strains of Acinetobacter baumannii were genotyped by ERIC-PCR.They were mainly classified into A, B, C, D and E 5 types.Among them, 116 strains were type A, 68 strains were type B, 26 strains were type C, 8 strains were type D and E.
		                        		
		                        			Conclusion
		                        			The resistance rate of Acinetobacter baumannii to some antibiotics in geriatric rehabilitation department is still at a high level, so we should continue to strengthen the rational use of antibiotics; at the same time, the strains are highly homologous, and there is likely to be clonal transmission in the department, so we should take more effective measures to prevent and control its transmission in hospital.

		                        			Criteria for diagnosis of Alzheimer's disease (AD) is not available in China.The international criteria is not a proper choice due to issues such as translation and lead to low diagnostic rate and high rate of missed diagnosis.The research group of Alzheimer's Disease Chinese (ADC) reviewed knowledge and techniques in neuropsychology,neuroimaging,molecular biology,and clinical neurology,and systematically studied the detection techniques such as memory,language,visuospatial,executive function,and medial temporal lobe visual scores on MRI,and their optimal threshold and diagnostic value for the diagnosis of AD.Through a systematic review and consensus meeting,a diagnostic framework for screeningAD in the Chinese population was established.Among these methods,an operational standard for clinical pathology models increased the diagnostic sensitivity by 15%.The sensitivity and specificity of screening memory impairment increased by 18.1% and 11.6%,respectively.The sensitivity of screening medialtemporal lobe atrophy increased by 24.5% and missed diagnosis was decreased by 34.5%.An operational standard for clinical biology models,incorporating the latest molecular imaging and molecular biology techniques,has enabled the early diagnosis of AD in China.The framework combines a principled diagnostic guideline with an operational screening protocol,which is applicable to all clinical settings and of great significance for the early detection,early diagnosis and early treatment of AD.
