1.A Statistical Analysis of Forensic Autopsies Performed in the Jungbu Province of Korea in 2022

We analyzed the manner and cause of death in 1,193 forensic autopsies from the Jungbu province (central part of South Korea) conducted by the National Forensic Service Daejeon Institute in 2022. Analysis of the manner of deaths revealed that 43.1% (514/1,193 cases) were natural deaths; 42.8% (511/1,193 cases) were unnatural deaths; and 14.1% (168/1,193 cases) were unknown. Among the unnatural deaths, the major manner of death was 44.6% (228/511 cases) by accidents, 34.1% (174/511 cases) by suicide, 13.5% (69/511 cases) undetermined, and 7.8% (40/511 cases) by homicide. Among the unnatural deaths, the major causes of death was 38.4% (196/511 cases) by trauma, 20.4% (104/511 cases) by poisoning, and 17.6% (90/511 cases) by asphyxia. Falling was the major cause of death by trauma (58.7%, 115/196 cases), and strangulation was the major cause of death by asphyxia (75.6%, 68/90 cases). Among the natural deaths, heart disease was the major cause (46.7%, 240/514 cases), followed by endocrine, nutritional and metabolic diseases (14.0%, 72/514 cases). A time-series statistical analysis and comparison of the manner and cause of deaths in this province may facilitate more advanced interpretations relating to both public safety and healthcare in the future.

3.Cost-Effectiveness Analysis of Cataract Surgery for the Beginners Using Intraocular Illumination

Purpose:
To evaluate the cost-effectiveness of cataract surgery for the beginners using intraocular illumination compared to the case of using microscope illumination.
Methods:
A Markov model was constructed for cost-effectiveness analysis. Hypothetical Korean men and women over 50 years of age were set as a cohort, and cataract surgeons were limited to beginners with less than 2 years of experience. Cataracts were classified as severe cataracts requiring surgery and mild cataracts that did not require surgery. Cataracts was assumed to progress to severe cataracts in 10% of each year. A cost-effectiveness analysis was performed based on the difference in the complications of surgery according to intraocular and microscope illumination. For the cost, data from the National Health Insurance Corporation were used, and a micro cost calculation method was used. In the utility analysis, data from previous studies were used for utility in each cataract and complication status. Sensitivity analysis was performed to evaluate the uncertainty of the results.
Results:
The surgery using microscope intraocular illumination showed the cost of 3,168,895 won, and the quality-adjusted life years of 16.4 years. The surgery of intraocular illumination showed the cost of 3,200,552 won and the quality-adjusted life years of 16.5 years. the incremental cost-effectiveness ratio of intraocular illumination was 1,675,630 won compared to microscope illumination. In the sensitivity analysis to, the utility of postoperative state without complication had the greatest influence on the results.
Conclusions
For beginners with less than 2 years of surgical experience, the incremental cost-effectiveness ratio was 1,675,630 won, which is acceptable in Korea's health care system, when comparing the case of using intraocular illumination to the case of using conventional microscope illumination.

6.Age-related macular degeneration

Age-related macular degeneration (AMD) is the leading cause of blindness in developed countries. Recently, AMD prevalence in Korea has increased from 6.6% to 13.9% over the past decade, which may be a threat to the public health of Korean society. This study describes the current status, including risk factors, treatment, and economic burden of AMD in Korea.Current Concepts: Major AMD risk factors have been consistently reported, including age, hypertension, smoking, and ultraviolet radiation. The primary AMD treatment is intravitreal injection of anti-vascular endothelial growth factor (anti-VEGF). Anti-VEGF was developed and improved in terms of efficacy and safety. However, AMD treatment’s economic burden is significant. Therefore, several treatment strategies have been introduced, such as monthly, as-needed, and treat-and-extend injection strategies.Discussion and Conclusion: With the rapid increase in AMD prevalence in Korea, the economic burden of AMD management will be a significant challenge for the Korean healthcare system. Therefore, cost-effectiveness and resource allocation for AMD treatment needs to be investigated.
7.Postoperative Refractive Outcomes of Biometric Formulas in Phacovitrectomy with Gas Tamponade

Purpose:
To investigate the refractive accuracy of intraocular lens (IOL) power calculation for biometric formulas in phacovitrectomy.
Methods:
This retrospective study included 357 eyes of 357 patients who underwent phacovitrectomy using four commonly available IOL power formulas: Hoffer Q (87 eyes), Holladay 1 (78 eyes), Holladay 2 (91 eyes), and SRK/T (101 eyes). The mean refractive error (ME) and the mean absolute refractive error (MAE) were calculated based on the predicted postoperative refraction error, and they were compared using analysis of variance test. Subjects were divided into high myopic eyes (axial length, ≥26 mm) and nonhigh myopic eyes (axial length, <26 mm).
Results:
The ME and the MAE after phacovitrectomy did not show a significant difference among the four IOL power formulas (p = 0.546 and p = 0.495, respectively). There was no significant statistical difference in formulas when the eyes were grouped into high myopia and nonhigh myopia (ME: p = 0.526 and p = 0.482, respectively; MAE: p = 0.715 and p = 0.627, respectively). The ME showed myopic shift in all formulas regardless of IOL formula used. The ME showed greater myopic shift in high myopia group than nonhigh myopia group in all formulas.
Conclusions
Our study did not find evidence for superiority of any formula in phacovitrectomy. However, in phacovitrectomy, possible myopic shift should be considered for IOL power calculation. Especially, in phacovitrecotmy in patients with high myopia, more myopic shift should be considered when selecting IOL.
8.Targeted Liquid Biopsy Using Irradiation to Facilitate the Release of Cell-Free DNA from a Spatially Aimed Tumor Tissue

Purpose:
We investigated the feasibility of using an anatomically localized, target-enriched liquid biopsy (TLB) in mouse models of lung cancer.
Materials and Methods:
After irradiating xenograft mouse with human lung cancer cell lines, H1299 (NRAS proto-oncogene, GTPase [NRAS] Q61K) and HCC827 (epidermal growth factor receptor [EGFR] E746-750del), circulating (cell-free) tumor DNA (ctDNA) levels were monitored with quantitative polymerase chain reaction on human long interspersed nuclear element-1 and cell line-specific mutations. We checked dose-dependency at 6, 12, or 18 Gy to each tumor-bearing mouse leg using 6-MV photon beams. We also analyzed ctDNA of lung cancer patients by LiquidSCAN, a targeted deep sequencing to validated the clinical performances of TLB method.
Results:
Irradiation could enhance the detection sensitivity of NRAS Q61K in the plasma sample of H1299-xenograft mouse to 4.5- fold. While cell-free DNA (cfDNA) level was not changed at 6 Gy, ctDNA level was increased upon irradiation. Using double-xenograft mouse with H1299 and HCC827, ctDNA polymerase chain reaction analysis with local irradiation in each region could specify mutation type matched to transplanted cell types, proposing an anatomically localized, TLB. Furthermore, when we performed targeted deep sequencing of cfDNA to monitor ctDNA level in 11 patients with lung cancer who underwent radiotherapy, the average ctDNA level was increased within a week after the start of radiotherapy.
Conclusion
TLB using irradiation could temporarily amplify ctDNA release in xenograft mouse and lung cancer patients, which enables us to develop theragnostic method for cancer patients with accurate ctDNA detection.

This report provides a detailed description of the methodology for ophthalmic examinations according to the Korea National Health and Nutrition Examination Survey (KNHANES) VII and VIII (from 2017 to 2021). The KNHANES is a nationwide survey which has been performed since 1998 in representatives of whole Korean population. During the KNHANES VII and VIII, in addition to the ophthalmic questionnaire, intraocular pressure measurement, visual field test, auto refractometry, axial length and optical coherence tomography measurements were included. This new survey will provide not only provide normative and pathologic ophthalmic data including intraocular pressure, refractive error, axial length, visual field and precise measurement of anterior segment, macula and optic nerve with optical coherence tomography, but also a more accurate diagnosis for major adult blindness diseases, including age-related macular degeneration, diabetic retinopathy, glaucoma, and other ocular diseases, for the national Korean population.
