1.Prognosis and risk factors of lung cancer with acute exacerbation of COPD in Tongchuan City

Objective To analyse the prognosis and risk factors of lung cancer with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) in Tongchuan City, and to provide a theoretical basis for improving the prognosis of patients with lung cancer complicated with AECOPD. Methods A total of 280 patients with lung cancer combined with AECOPD admitted to Tongchuan People's Hospital from January 2021 to August 2022 were selected and divided into survival group and death group according to whether the patients survived during hospitalization. Serum carcinoembryonic antigen (CEA), cytokeratin 19 fragment (CYFRA 21-1) and other tumor markers were compared between the two groups. Lung function was measured by lung function instruments, and the levels of forced expiratory volume in one second (FEV1), forced vital capacity (FVC), FEV1/FVC and other lung function indicators were compared between the two groups. The patients' clinical data were collected from the medical record system, and univariate analysis and logistic regression were used to analyze the independent risk factors affecting the prognosis of patients with lung cancer complicated with AECOP. Results The values of FEV1, FVC and FEV1/FVC in the death group were significantly lower than those in the survival group (P<0.05). Serum CEA and CYFRA 21-1 levels in the death group were significantly higher than those in the survival group (P<0.05). Multiple logistic regression analysis showed that albumin level <35 g/L (OR=2.728), TNM stage III to IV (OR=2.416), multidrug-resistant bacterial infection (OR=2.982), and GOLD grade III to IV (OR=3.417) were independent risk factors for death in patients with lung cancer complicated with AECOPD in Tongchuan City (P<0.05). Conclusion Patients with lung cancer complicated with AECOPD in Tongchuan City have a high risk of death during hospitalization, especially patients with TNM stage III to IV and GOLD grade III to IV. Multi-drug resistant bacteria infection should be actively controlled to improve the albumin level of patients, which is conducive to the prognosis of patients.
2.Study on mechanism of interfering with LncRNA expressing to reduce paclitaxel resistance in non-small cell lung cancer cells

OBJECTIVE To study the mechanism of interfering with long non-coding RNA nicotinamide nucleotide transhydrogenase-antisense RNA1 (LncRNA NNT-AS1) expressing to reduce paclitaxel (TAX) resistance in non-small cell lung cancer (NSCLC) cells. METHODS NSCLC TAX-resistant cell line (A549/TAX) was constructed, and the expressions of LncRNA NNT-AS1 in normal, parental, and drug-resistant cells were observed. The targeting relationship of microRNA-582-5p (miR-582- 5p) with LncRNA NNT-AS1 and high mobility group box2 (HMGB2) was verified. A549/TAX cells were cultured in vitro to observe the effects of interfering with LncRNA NNT-AS1 alone or interfering with LncRNA NNT-AS1 and miR-582-5p on the expressions of LncRNA NNT-AS1 and miR-582-5p, the mRNA and protein expressions of HMGB2, cell viability, clone formation and apoptosis. The effects of interfering with LncRNA NNT-AS1 on tumor growth and the expression of miR-582-5p and the mRNA and protein expressions of HMGB2 in tumor tissue were observed in nude mice. RESULTS Compared with normal cells, LncRNA NNT-AS1 was highly expressed in parental and drug-resistant cells (P<0.05), showing an increasing trend. It was validated that miR-582-5p had a targeting relationship with LncRNA NNT-AS1 and HMGB2. After interfering with the expression of LncRNA NNT-AS1, the expression of LncRNA NNT-AS1 and the mRNA and protein expressions of HMGB2, cell viability and the number of cloned cells in A549/TAX cell, decreased significantly, while the expression of miR-582-5p and the apoptotic rate increased significantly (P<0.05); simultaneously interfering with the expression of miR-582-5p could reverse above changes (P< 0.05). Interfering with the expression of LncRNA NNT-AS1 in tumor cell could significantly reduce tumor volume and tumor weight of nude mice bearing tumors; at the same time, the expression of miR-582-5p was up-regulated significantly and the mRNA and protein expressions of HMGB2 were down-regulated significantly (P<0.05). CONCLUSIONS Interfering with the expression of LncRNA NNT-AS1 may alleviate TAX chemotherapy resistance in NSCLC through targeted up-regulation of miR-582-5p and down-regulation of HMGB2.
3.Construction of Rat Model of Migraine with Hyperactivity of Liver-Yang and Blood Stagnation Based on"Combination of Disease and Syndrome"

Objective To study the characteristics of a rat model of migraine with hyperactivity of liver-yang and blood stagnation,which was established by Fuzi Decoction combined with electrical stimulation of the trigeminal ganglion.Methods The 30 SD rats were divided into normal group,model group,and Zhengtian pill group(1.6 g·kg-1),with 10 rats in each group.Rats in the model group and Zhengtian pill group were fed with aconite decoction(2.00 g·kg-1)once a day for 28 day to establish the hyperthermia model;On the 15 day of intragastric administration,rats in the Zhengtai pill group were simultaneously given Zhengtai pill solution once a day for 14 day;At 29 day,the trigeminal ganglion was stimulated to create the migraine model of hyperliver and stasis.After 30 min of the last dose,the macroscopic signs and behaviors were observed,and the blood rheology was detected by blood visticometer.Positive substance P(SP)expression,mRNA and relative protein expression in the trigeminal cervical pulp complex(TCC)in rat mice by immunohistochemistry,RT-qPCR and Western blot analysis.Results Compared with the normal group,the model group has macro signs such as red eyes,irritability,cage fighting,dark tongue and stasis points;regular head positioning and frequent hair management;increased distance in the central area(P<0.05).The relative viscosity of whole blood,plasma viscosity,red blood cell aggregation index all increased(P<0.05),and the red blood cell variant index decreased(P<0.05);SP positive expression,mRNA and the relative protein expression increased in TCC(P<0.05).Compared with model group,Zhengtian pill group can improve macro representation and behavior significantly;can significantly reduce central distance(P<0.05);can reduce plasma viscosity,high-shear relative viscosity of whole blood,and red blood cell aggregation index(P<0.05);increase the Red blood cell variant index significantly(P<0.05);down-regulate SP positive expression,mRNA and relative protein expression in TCC(P<0.05,P<0.01).Conclusion SD rats were used for 4 weeks to construct a migraine model of hyperhepatic hyperactivity and stasis,which was basically consistent with the clinical manifestations.
4.Effects of health education based on evidence-based practice using knowledge to action model on patients with osteoporosis and lumbar vertebral fractures

Objective:To explore the effects of health education based on evidence-based practice using the knowledge to action model (KTA) on patients with osteoporosis and lumbar vertebral fractures.Methods:Totally 108 patients with osteoporosis and lumbar vertebral fractures admitted in the the Xingtai General Hospital of North China Medical and Health Group from October 2017 to October 2019 were selected and divided into an observation group and a control group according to the random number table, with 54 patients in each group. Patients in both groups received routine treatment and care. Patients in the control group received routine health education, while patients in the observation group received health education received health education based on evidence-based practice using KTA. The rehabilitation status [Oswestry dysfunction index (ODI) , Barthel index] between the two groups of patients, as well as the knowledge level (osteoporosis knowledge questionnaire) , living habits and quality of life [The World Health Organization Quality of Life Brief Version (WHOQOL-BREF) ] before and after the intervention were compared.Results:After 3 months of intervention, the ODI of the two groups decreased while the Barthel index increased ( P< 0.05) ; and the ODI of the observation group was lower than that of the control group, while the Barthel index score was higher than that of the control group, with statistically significant differences ( P< 0.05) . After 3 months of intervention, the risk factors, exercise knowledge, calcium knowledge scores and total scores of the osteoporosis knowledge questionnaire in the two groups increased, with statistically significant differences ( P< 0.05) , which were higher in the observation group than in the control group, with statistically significant differences ( P< 0.05) . After 3 months of intervention, the ratio of smoking and drinking in the observation group was lower than that in the control group, and the ratio of exercise and calcium supplementation was higher than that in the control group, with statistically significant differences ( P< 0.05) . After 3 months of intervention, the scores in all dimensions of the WHOQOL-BREF of the two groups increased ( P< 0.05) , and the scores in the physical, psychological and social dimensions of the observation group were higher than those of the control group, with statistically significant differences ( P< 0.05) . Conclusions:The health education based on evidence-based practice using KTA in patients with osteoporosis and lumbar vertebral fractures helps to improve patients' related knowledge levels, living habits, rehabilitation effects, and quality of life.
5.Effects of the Extract of Xiongmatang on 5-HT and CGRP in Brain Tissue of Rats with Liver-yang Hyperactivity and Blood Stagnation Migraine

ABSTRACT OBJECTIVE:To study the effects of the extract of Xiongmatang on 5-HT and CGRP in brain tissue of rats with liver-yang hyperactivity and blood stagnation migraine. METHODS:Fifty male spontaneously hypertension (SHR) rats were randomly divided into model control group,Zhengtian pill positive control group (1.6 g/kg),the extract of Xiongmatang low-dose,medium-dose and high-dose groups(4.5,9.0,18.0 g/kg,by crude drug),with 10 rats in each group. Another 10 normal rats were taken as normal control group. Administration group was given relevant medicine intragastrically,once a day,for consecutive 28 d. Normal control group and model control group were given equal volume normal saline intragastrically. After last administration,except for normal control group,other groups were used to stimulate trigeminal ganglia(10 min)to establish liver-yang hyperactivity and blood stagnation migraine model,and maintained the administration once more after making the model. 30 min later,general behavior and tongue quality of rats were observed,and the blood pressure was measured;the contents of 5-HT and CGRP in cerebral tissue of rats were determined by ELISA. The protein expression of CGRP in cerebral tissue of rats were determined by Western blot method. RESULTS: Compared with the normal control group, the rats in the model control group had behavioral symptoms,such as the color of conjunctiva deepened and reddened, excessive hairdressing,head flicking,and most of the rats had purple tongue;the systolic pressure and the content and protein expression of CGRP were all increased obviously(P<0.05 or P<0.01), while the content of 5-HT in cerebral tissue was decreased obviously(P<0.01). Compared with model control group,general behavior and tongue quality of rats were improved significantly in administration groups. Systolic pressure,the content and the protein expression of CGRP in cerebral tissue of rats were decreased significantly in Zhengtian pill positive control group and the extract of Xiongmatang high-dose group (P<0.05),while the content of 5-HT in cerebral tissue of rats were increased significantly in Zhengtian pill positive control group,the extract of Xiongmatang medium-dose and high-dose groups(P<0.05).CONCLUSIONS:The extract of Xiongmatang has obvious protective effect on liver-yang hyperactivity and blood stagnation migraine model rats,the mechanism of which may be associated with reducing the content of CGRP in cerebral tissue and raising the content of 5-HT in cerebral tissue
6.Analysis of MYOC gene variants among sporadic patients with primary open-angle glaucoma.

OBJECTIVE:
To screen for MYOC gene variants among sporadic patients with primary open angle glaucoma (POAG).
METHODS:
For 398 patients with POAG, Sanger sequencing was applied to detect potential variants of the MYOC gene.
RESULTS:
Eight patients (2.0%) were found to harbor variations of the MYOC gene. These included five types of variants, among which c.667C>T (p.Pro223Ser) and c.1138G>T (p.Asp380Tyr) were novel. c.382C>T (p.Arg128Trp), c.1109C>T(p.Pro370Leu) and c.1130C>A (p.Thr377Lys) were previously associated with POAG. Alignment of amino acid sequences of MYOC proteins of various species revealed that the two novel variants have occurred at highly conserved positions. c.1138G>T was predicted to be possible pathogenic by Bioinformatic analysis.
CONCLUSION
Two novel variants of the MYOC gene were detected among sporadic POAG patients, which enriched its variant spectrum.

7.Mutation analysis of FBN1 gene in a child with Marfan syndrome.

OBJECTIVETo detect potential mutations of fibrillin-1 (FBN1) gene in a child with Marfan syndrome (MFS) and explore its molecular pathogenesis.
METHODSThe 66 exons of the FBN1 gene were analyzed by direct sequencing. SIFT and PolyPhen-2 were used to predict the structural and functional changes at the protein level.
RESULTSA novel heterozygous mutation c.3998 G>A (p.Cys1333Tyr) was found in exon 32 in the child. The same mutation was not found among his unaffected family members and 683 healthy controls. Multiple sequence alignment showed that this novel mutation was located in a highly conserved region of the FBN1 protein across various species and may induce structural change to a functional domain.
CONCLUSIONThe novel c.3998G>A (p.Cys1333Tyr) mutation of the FBN1 gene probably predisposed the MFS in the child. Above finding has enriched the spectrum of FBN1 mutations.
8.Survey on the quality of life of disabled people in Shanghai Jing'an district and analysis of influencing factors

Objective To survey the quality of life of disabled people in Shanghai Jing'an district and to analyze the influencing factors.Methods A face-to-face interview was conducted among 1 831 disabled people from 5 communities of Jing'an district between September 2015 and January 2016.The quality of life was evaluated with Modified Barthel Index (MBI),12-Item Short Form Health Survey (SF-12) and Patient Health Questionnaire (PHQ).Results There were 941 males and 870 females among 1 815 qualified participants.There were 433 cases of visual disability (23.86%),222 cases of hearing disability (12.23%),20 cases of language disability(1.10%),742 cases of limb disability (40.88%),169 cases of intelligent disability (9.31%),198 cases of mental disability (10.91%) and 31 cases (1.71%) of multiple disabilities.The SF-12 showed that the mean scores of both physical aspect and mental aspect were 50.0 ± 10.0.Multiple linear regression showed showed that the low self-care ability of daily life (β =0.24,P =0.00),depression (β =-9.65,P =0.00) and high serum lipids (β =-2.36,P =0.02) were associated with the quality of life.Conclusion The study indicates that the quality of life of the disabled people in Jing'an district is generally low,which is affected by various factors.
9.Effects of the bioactive parts of Xiongma Decoction on CGRP-CRLR/RAMP1 pathway in TCC of rats with nitroglycerin-induced migraine

Objective To explore the effects of bioactive parts of Xiongma Decoction (parts of ethyl acetate and n-butanol extract) on the CGRP-CRLR/RAMP1 signal pathway so as to clarify its therapeutic mechanism on migraine.Methods We randomly divided 36 male SD rats into 6 groups with 6 in each:blank group,model group,groups of low-,medium-and high-dose Xiongma Decoction bioactive parts,and Sumatriptan group.By giving hypodermic injection of 10 mg/kg nitroglycerin,the migraine rat model was copied;Only 18 rats were found to have positive expressions of CGRP,CRLR,and RAMP1 in TCC with immunohistochemistry staining after heart perfusion.For the remaining 18 rats,TCC was stripped directly from the whole brain and divided into two parts,one part used to detect CGRP,CRLR,RAMP1 mRNA expressions by qPCR,and the other part to detect CGRP,CRLR,RAMP1 protein expressions by Western blot.Results The number of CGRP,CRLR and RAMP1 immunoreactive cells,the mRNA and protein expressions on TCC in model group were effectively increased,compared with those in the blank group (P<0.05),indicating that the model copying was successful.Compared with those in the model group,the number of CGRP,CRLR and RAMP1 immunoreactive cells in Xiongma Decoction bioactive parts was significantlv decreased,and the expressions of CGRP,CRLR and RAMP1 mRNA and protein were reduced (P<0.05).Conclusion The bioactive parts of Xiongma Decoction can reduce the activity of CGRP-CRLR/RAMP1 signal pathway in TCC of migraine rats.
10.Study on HPLC Fingerprints of Huobahuagen Tablets Intermediate

OBJECTIVE:To establish the HPLC fingerprints for Huobahuagen tablets intermediate. METHODS:HPLC was per-formed on Inertsil ODS-4 column with mobile phase consisted of acetonitrile-0.1% phosphoric acid (gradient elution) at the flow rate of 0.75 mL/min. The detection wavelength was set at 220 nm,and column temperature was 30 ℃. The sample size was 10 μL. Using wilforgine as reference,HPLC chromatograms of 10 batches of samples were determined. Common peak identification and similarity evaluation were performed by using Similarity Evaluation System for TCM Chromatographic Fingerprint (2004 A edi-tion). RESULTS:There were 25 common peaks in HPLC chromatograms of 10 batches of samples,and similarity degrees were higher than 0.9. After validated,HPLC chromatograms of 10 batches of samples were in good agreement with control fingerprints. CONCLUSIONS:The established fingerprint can provide reference for identification and quality evaluation of Huobahuagen tab-lets intermediate.
