1.The Effects of the Intelligent Hearing-assistive System on Hearing Benefits to Cochlear Implant Recipients

		                        			Objective To study the effects of the intelligent hearing-assistive system incorporated in Nuro-tron cochlear implants(CI),including the autonomic acoustic scene recognition(ASR),intelligent strategy config-uration as well as the objective and subjective hearing improvements on recipients.Methods ① To evaluate the per-formance of the ASR matule,in a sound-proof room,the preset five kinds of test audios,including speech,noise,speech in noise,pure music(without human voice)and non-pure Music(with human voice)were played.Each type of scenes included 6 to 9 5 min test files.The prediction accuracy and scene switching times were calculated.② In order to evaluate the noise-reduction performance of the ABeam technology in the speech enhancement module,13 Nurotron? CI recipients were recruited and their speech recognition rate when ABeam was"ON"and"OFF"with noise coming from 90°,180°or 270°were tested,individually.Also,their subjective hearing feedback was evaluated through visual analogue scale(VAS)evaluation.Results The ASR module achieved high prediction performance,with prediction accuracy 99%±4%,96%±9%,94%±12%,94%±15%,92%±13%for speech,noise,noisy speech,pure music and non-pure music,respectively.The scene transation times for each individual scene were 1.1 ±0.3,1.4±0.7,1.3±0.5,1.4±0.8 and 1.3±0.5,indicating that the prediction was also stable.When noise came from the sides and behind of recipients and speech signal from the front,the adaptive dual microphone noise re-duction algorithm ABeam significantly increased the speech recognition score(SRS)in 5 dB signal-to-noise(SNR)environment(P<0.001),with an average increase of 15.92%.Especially when the noise came from 180 degree backward,the SRS increased 28.68%when ABeam was"0N",which was significantly higher than when ABeam was"OFF"(P<0.01).Conclusion The intelligent hearing-assistive system can help CI recipients automatically configure appropriate SPSs under different environments,improving the speech intelligibility and hearing comfort.
		                        		
		                        		
		                        		
		                        	
2.Diagnosis and treatment status of perioperative anemia in patients with gastrointestinal neoplasms: a multi-center study in Hubei Province.

		                        			Objective: To investigate the incidence and treatment of perioperative anemia in patients with gastrointestinal neoplasms in Hubei Province. Methods: The clinicopathological data of 7 474 patients with gastrointestinal neoplasms in 62 hospitals in 15 cities (state) of Hubei Province in 2019 were collected in the form of network database. There were 4 749 males and 2 725 females. The median age of the patients was 62 years (range: 17 to 96 years). The hemoglobin value of the first time in hospital and the first day after operation was used as the criterion of preoperative anemia and postoperative anemia. Anemia was defined as male hemoglobin <120 g/L and female hemoglobin <110.0 g/L, mild anemia as 90 to normal, moderate anemia as 60 to <90 g/L, severe anemia as <60 g/L. The t test and χ2 test were used for inter-group comparison. Results: The overall incidence of preoperative anemia was 38.60%(2 885/7 474), and the incidences of mild anemia, moderate anemia and severe anemia were 25.09%(1 875/7 474), 11.37%(850/7 474) and 2.14%(160/7 474), respectively. The overall incidence of postoperative anemia was 61.40%(4 589/7 474). The incidence of mild anemia, moderate anemia and severe anemia were 48.73%(3 642/7 474), 12.20%(912/7 474) and 0.47%(35/7 474), respectively. The proportion of preoperative anemia patients receiving treatment was 26.86% (775/2 885), and the proportion of postoperative anemia patients receiving treatment was 14.93% (685/4 589). The proportions of preoperative anemia patients in grade ⅢA, grade ⅢB, and grade ⅡA hospitals receiving treatment were 26.12% (649/2 485), 32.32% (85/263), and 29.93% (41/137), and the proportions of postoperative anemia patients receiving treatment were 14.61% (592/4 052), 22.05% (73/331), and 9.71% (20/206). The proportion of intraoperative blood transfusion (16.74% (483/2 885) vs. 3.05% (140/4 589), χ²=434.555, P<0.01) and the incidence of postoperative complications (17.78% (513/2 885) vs. 14.08% (646/4 589), χ²=18.553, P<0.01) in the preoperative anemia group were higher than those in the non-anemia group, and the postoperative hospital stay in the preoperative anemia group was longer than that in the non-anemia group ((14.1±7.3) days vs. (13.3±6.2) days, t=5.202, P<0.01). Conclusions: The incidence of perioperative anemia in patients with gastrointestinal neoplasms is high. Preoperative anemia can increase the demand for intraoperative blood transfusion and affect the short-term prognosis of patients. At present, the concept of standardized treatment of perioperative anemia among gastrointestinal surgeons in Hubei Province needs to be improved.

4.Simultaneous determination of multiple bioactive constituents in Abelmoschi Corolla by UFLC-QTRAP-MS/MS.

		                        			A comprehensive analytical method based on ultra-fast liquid chromatography coupled with triple quadrupole/linear ion trap tandem mass spectrometry(UFLC-QTRAP-MS/MS) was established for simultaneous determination of the content of 38 active components in Abelmoschi Corolla, including flavonoids, organic acids, nucleosides and amino acids, so as to investigate the effects of different harvesting and processing methods on multi-active components in Abelmoschi Corolla. The chromatographic separation was performed on a XBridg®C_(18) column(4.6 mm×100 mm, 3.5 μm) with(0.1% formic acid water) methanol-acetonitrile(1∶1) as the mobile phase for gradient elution at 30 ℃. The flow rate was 0.5 mL·min~(-1). The components were detected in a multiple-reaction monitoring(MRM) mode. The gray relational analysis(GRA) was used to comprehensively evaluate the multiple active components of Abelmoschi Corolla at different harvesting times and drying temperatures. The results showed that 38 components had a good linearity with correlation coefficients all above 0.999 0. The method featured a good precision, repeatability and stability with the relative stan-dard deviations(RSDs) of less than 5.0%. Recoveries ranged from 98.06% to 104.4% with RSD between 0.22% and 4.9%. The results of GRA indicated that a better quality in the samples collected on September 9 th. Samples dried at 90 ℃ had a better quality. The established method is accurate and reliable, and can be used to assess the internal quality of Abelmoschi Corolla. 5.Comparative analysis of constitutes and metabolites for traditional Chinese medicine using IDA and SWATH data acquisition modes on LC-Q-TOF MS

		                        			Identification of components and metabolites of traditional Chinese medicines (TCMs) employing liquid chromatography-quadrupole time-of-flight mass spectrometry (LC-Q-TOF MS) techniques with information-dependent acquisition (IDA) approaches is increasingly frequent. A current drawback of IDA-MS is that the complexity of a sample might prevent important compounds from being triggered in IDA settings. Sequential window acquisition of all theoretical fragment-ion spectra (SWATH) is a data-independent acquisition (DIA) method where the instrument deterministically fragments all precursor ions within the predefined m/z range in a systematic and unbiased fashion. Herein, the superiority of SWATH on the detection of TCMs' components was firstly investigated by comparing the detection ef-ficiency of SWATH-MS and IDA-MS data acquisition modes, and sanguisorbin extract was used as a mode TCM. After optimizing the setting parameters of SWATH, rolling collision energy (CE) and variable Q1 isolation windows were found to be more efficient for sanguisorbin identification than the fixed CE and fixed Q1 isolation window. More importantly, the qualitative efficiency of SWATH-MS on sanguisorbins was found significantly higher than that of IDA-MS data acquisition. In IDA mode, 18 kinds of sangui-sorbins were detected in sanguisorbin extract. A total of 47 sanguisorbins were detected when SWATH-MS was used under rolling CE and flexible Q1 isolation window modes. Besides, 26 metabolites of sangui-sorbins were identified in rat plasma, and their metabolic pathways could be deduced as decarbonylation, oxidization, reduction, methylation, and glucuronidation according to their fragmental ions acquired in SWATH-MS mode. Thus, SWATH-MS data acquisition could provide more comprehensive information for the component and metabolite identification for TCMs than IDA-MS.
		                        		
		                        		
		                        		
		                        	
6.A new antiviral isoquinoline alkaloid from Thalictrum glandulosissimum.

		                        			A new isoquinoline alkaloid(1) has been isolated from the whole plant of Thalictrum glandulosissimum by using various chromatographic techniques, including silica gel, sephadex, MCI-gel resin, and RP-HPLC, and its structure was determined as 1-(6-hydroxy-7-methylisoquinolin-1-yl) ethantone by physicochemical properties and spectroscopic data. This compound was evaluated for anti-tobacco mosaic virus(TMV) activity. The results showed that it had prominent anti-TMV activity with inhibition rates of 28.4%. 		                        			Objective:Clinical study in the treatment of common bile duct stones combined with ERCP and EST.Methods:the October 2014-2017 year in February for hepatobiliary surgery inour hospital after ERCP combined with LC in the treatment of patients with cholecystolithiasis and choledocholithiasis in 158 cases.According to ERCP postoperative LC interval difference divided into ERCP definite operation LC group(104 cases) and ERCP Elective operation LC surgery group(54 cases).The definite operation for ERCP LC group is LC line 24h-72h,another is the patients were givenLC for 3 months after ERCP.The clinical efficacy,LC operation time,hemorrhage,the gallstone is discharged into the bile duct,recurrent cholecystitis,bile leakage,length of stay and medical fee were compared between the two groups (ERCP definite operation LC group of single hospitalization time;ERCP Elective operation LC surgery group of two hospital hours together),average ERCP times,medical expenses.Results:all of the two groups were cured after operation.length of sta is long and medical expensive which was selected a time to do (P<0.05).Conclusion:ERCP definite operation of LC group in the treatment of common bile duct stones and gallbladder stones surgery is safe,less medical expenses,hospitalization time is short,patients recover quickly,the burden is small.
		                        		
		                        		
		                        		
		                        	
8.MiR-21 regulates the growth and invasion of liver cancer cells through PDCD4

		                        			Purpose To evaluate the expression of miR-21 in the tissues and cell lines of hepatocellular carcinoma,and to try to find its possible target genes.Methods The expression profile of miR-21 was detected in hepatocellular carcinoma tissues and cell lines.Mter miR-21 inhibitor was used,the alterations in the vitality and invasion of hepatocellular carcinoma cells were observed.The possible target gene of miR-21 was predicted by bioinformatics analysis.The influence of miR-21 inhibitors on the target gene activity was evaluated by dual luciferase reporting gene system.Results The expression level of miR-21 was significantly higher in hepatocellular carcinoma tissues than that in the adjacent ones (P <0.05).The expression level of miR-21 in hepatocellular carcinoma cells was significantly higher than that in the hepatic cells (P <0.01).After inhibiting miR-21,the viability and invasion ability of hepatocellular carcinoma cells were decreased (P < 0.01).The expression level of programmed cell death 4 (PDCD4) in hepatocellular carcinoma tissues was significantly lower than that in the adjacent tissues (P < 0.01).Its expression level in hepatocellular carcinoma cells was significantly lower than that in the hepatic cells (P < 0.01).After interfering with PDCD4,the vitality and invasion ability of liver cancer cells were increased (P < 0.05).Dual luciferase reporter gene assay indicated that by inhibiting miR-21,the expression level of PDCD4 was up-regulated (P < 0.01).The vitality and invasion ability of liver cancer cells were reduced (P < 0.001).Conclusion MiR-21 can regulate the growth and invasion of liver cancer cells through targeting PDCD4.
		                        		
		                        		
		                        		
		                        	
9.Audiological and Otological Status of 45 Infants with Cleft Palate.

OBJECTIVETo study the audiological and otological status of cleft palate infants with the application of distortion product otoacoustic emission (DPOAE), auditory brainstem response (ABR), tympanometry with 1000 Hz and 226 Hz probe tones.
METHODSTotally 45 cleft palate infants aged 8-24 months were included in the study. Most of them were examined for DPOAE, ABR and two frequency tympanometry.
RESULTSMost infants failed the three tests,among whom 6.7% ears passed DPOAE and 33.3% of ears had normal ABR hearing threshold. In addition, 8.9% of ears turned out normal in the 1000 Hz probe-tone tympanometry, and 13.3% were type A in the 226 Hz probe-tone tympanometry. Finally, 1000 Hz tympanometry had more agreement with DPOAE and latency of ABR wave I than 226 Hz tympanometry.
CONCLUSIONMost cleft palate infants have audiological and otological problems, which should be evaluated in a more comprehensive manner.

10.Short-term Impact of Cleft Palate Repair on Hearing and Middle Ear Function in Infants

		                        			Objective To study the short-term impact of cleft palate repair surgery on hearing and middle ear function in infants .Methods Thirty-three infants with cleft palate who received cleft palate repair surgery from January 2013 to November 2014 in Peking Union Medical College Hospital were included in this study , in-cluding 22 male infants and 11 female infants .They were examined using distortion product otoacoustic emission (DPOAE), auditory brainstem response (ABR), 226 Hz probe-tone tympanometry before surgery and 1 -24 months after surgery .Results The mean age of the 33 infants (66 ears) at the time of surgery was 17 months (8.5-25 months).Sixty-four ears were examined with preoperative DPOAE , with 7 ears (10.9%) passed;postoperative DPOAE was performed in 56 ears, with 19 ears (33.9%) passed.The passing rate was significant-ly improved after operation ( P<0.01 ) .In the 54 ears for which both preoperative and postoperative DPOAE were conducted , 16 ears ( 29.6%) showed improved hearing .Sixty-three ears were examined with preoperative ABR, showing a mean hearing threshold of (43.73 ±13.65 ) dB nHL;62 ears were examined with postoperative ABR, showing a mean hearing threshold of (35.65 ±13.75 ) dB nHL, significantly improved compared with the preoperative threshold ( P<0.01 ) .In the 53 ears which received both preoperative and postoperative ABR , 29 ears (54.7%) showed reduced hearing thresholds .There was no significant difference in 226 Hz probe-tone tympanometry between preoperative and postoperative examinations ( P>0.05 ) .Conclusion In infants with cleft palate , short-term hearing improvement may be observed after repair surgery , while middle ear function may show no obvious improvement .
