BackgroundWith the acceleration of population aging in China, studies have documented an increased prevalence of dementia in the elderly. Behavioral and psychological symptoms and impaired cognitive function are important problems affecting the quality of life in the elderly with dementia. It has been noted that there are certain limitations on the management of problems mentioned above by drug treatment alone, while its combination with music intervention is emerging as an effective approach. However, the application of this approach in the elderly with moderate to severe dementia is still in its relative infancy. ObjectiveTo investigate the effect of music assisted drug treatment on behavioral and psychological symptoms and cognitive function in the elderly with moderate to severe dementia, so as to provide insights for the development of effective intervention. MethodsA randomized controlled study was utilized to enroll a coherent of 43 elderly patients with moderate to severe dementia who were hospitalized in Geriatric Psychiatry of Deyang Mental Health Center from January to July 2023 and met the International Classification of Diseases, tenth edition (ICD-10) diagnostic criteria for dementia. Eligible subjects were divided into study group (n=22) and control group (n=21) based on random allocation using SPSS 26.0. Both groups received routine drug treatment and nursing care, while the study group added music intervention, which consisted of two 30-minute sessions per week for 8 weeks. Neuro Psychiatric Inventory (NPI) and Mini-Mental State Examination (MMSE) were administered to all patients before and after intervention. ResultsA total of 36 patients completed the study, including 20 case in study group and 16 case in control group. No statistical difference was noted in baseline NPI score and MMSE score between two groups (P>0.05). After intervention, study group reported a statistical reduction in NPI total score [58.00 (49.00, 79.25) vs. 78.50 (55.00, 95.50), Z=-3.902, P<0.01)] along with a clinically unremarkable increase in MMSE score (P>0.05) compared with baseline data. After intervention, NPI total score and scores on sub-domains including agitation/aggression, depression/dysphoria, anxiety and aberrant motor behavior were all significantly lower in study group than those in control group (Z=-2.183, -2.438, -2.691, -3.716, -2.250, P<0.05 or 0.01), while there was no significant difference in MMSE score between two groups (P>0.05). ConclusionMusic assisted drugintervention may ameliorate behavioral and psychological symptoms in the elderly with moderate to severe dementia, while no significant improvement is documented on cognitive function. Journal of Clinical Hepatology 2025;41(6):1143-1149
ObjectiveTo investigate the risk factors for pyogenic liver abscess (PLA) comorbid with sepsis by analyzing clinical features, and to construct a predictive model. MethodsA retrospective analysis was performed for 489 patients who were hospitalized and diagnosed with PLA in The Affiliated Hospital of Xuzhou Medical University from January 2019 to December 2023, and according to the presence or absence of sepsis, they were divided into sepsis group with 306 patients and non-sepsis group with 183 patients. Related data were collected, including general information, laboratory markers, and outcome measures. The patients were further divided into a training set of 342 patients and a validation set of 147 patients at a ratio of 7∶3, and the training set was used for screening of variables and construction of a predictive model, while the validation set was used to test the performance of the model. An LASSO regression analysis was used for the screening of variables, and a multivariate Logistic regression analysis was used to construct the predictive model and plot a nomogram. The calibration curve, the receiver operating characteristic (ROC) curve, and the decision curve analysis were used for the validation of the model, and internal validation was performed for assessment. The independent-samples t test was used for comparison of normally distributed continuous data between two groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups; the chi-square test was used for comparison of categorical variables between groups. ResultsThere were significant differences between the sepsis group and the non-sepsis group in pulse rate, mean arterial pressure, duration pf symptoms, comorbidity of liver cirrhosis or malignant tumor, leukocyte count, neutrophil count, lymphocyte count, platelet count (PLT), activated partial thromboplastin time, fibrinogen, C-reactive protein, aspartate aminotransferase, alanine aminotransferase, albumin, total bilirubin (TBil), creatinine, potassium, and prognostic nutritional index (PNI) (all P<0.05). In the training set, the LASSO regression analysis identified four predictive factors of pulse rate, PLT, TBil and PNI, and the multivariate Logistic regression analysis showed that pulse rate (odds ratio [OR]=1.033, 95% confidence interval [CI]: 1.006 — 1.061, P=0.018), PLT (OR=0.981, 95%CI: 0.975 — 0.987, P<0.001), TBil (OR=1.086, 95%CI: 1.053 — 1.125, P<0.001), and PNI (OR=0.935, 95%CI: 0.882 — 0.988, P=0.019) were independent influencing factors for the risk of sepsis in patients with PLA. The model constructed based on these factors showed a good predictive ability, with an area under the ROC curve of 0.948 (95%CI: 0.923 — 0.973) in the training set and 0.912 (95%CI: 0.848 — 0.976) in the validation set. The decision curve analysis showed that the model has a good net benefit within the range of 0.3 — 0.9 for threshold probability. ConclusionThe nomogram prediction model constructed based on pulse rate, PLT, TBil, and PNI has a certain clinical value and can well predict the risk of sepsis in patients with PLA.

Journal of Clinical Hepatology 2023;39(12):2809-2816
ObjectiveTo establish an early predictive model using serological markers based on LASSO regression for predicting the possibility of HBsAg clearance in HBeAg-negative chronic hepatitis B (CHB) patients treated with pegylated interferon α-2b (PEG-IFNα-2b), and to investigate the diagnostic value of the model. MethodsA total of 136 HBeAg-negative CHB patients who received PEG-IFNα-2b treatment in the Affiliated Hospital of Xuzhou Medical University from April 2020 to October 2021 were enrolled, among whom 47 received PEG-IFNα-2b for the first time (previously untreated) and 89 received PEG-IFNα-2b after 48 weeks of treatment with nucleos(t)ide analogues (treatment-experienced). The patients were randomly assigned to a training set with 95 patients and a validation set with 41 patients at a ratio of 7∶3, and related data were collected for both groups, including virological markers, routine blood test results, and liver function at baseline and week 12 of treatment. According to HBsAg status at week 48 of treatment, the patients were divided into seroconversion group with 38 patients and non-seroconversion group with 98 patients. The independent-samples t test was used for comparison of normally distributed continuous data between two groups, and the Wilcoxon rank-sum test was used for comparison of non-normally distributed continuous data between two groups; the chi-square test was used for comparison of categorical variables between two groups. The LASSO regression analysis and univariate and multivariate logistic regression analyses were used to establish a nomogram model; the receiver operating characteristic (ROC) curve was used to assess its predictive ability, and the area under the ROC curve (AUC) was used for comparison of predictive value. ResultsIn the training set, 95 HBeAg-negative CHB patients were treated with PEG-IFNα-2b for 48 weeks, among whom there were 27 patients in the seroconversion group and 68 in the non-seroconversion group. The univariate Logistic regression analysis, with P<0.2 as the criterion for screening, showed that 9 indicators were included in the LASSO regression analysis, i.e., sex, baseline HBV DNA level, the reduction in HBV DNA in 0 — 12 weeks, baseline HBsAg level, the reduction in HBsAg in 0 — 12 weeks, baseline aspartate aminotransferase (AST) level, the reduction in AST in 0 — 12 weeks, baseline alanine aminotransferase (ALT) level, and the reduction in ALT in 0 — 12 weeks. The LASSO regression analysis showed that sex, baseline HBsAg level, the reduction in HBsAg in 0 — 12 weeks, and the reduction in ALT in 0 — 12 weeks were non-zero variables and were included in the multivariate Logistic regression analysis. The multivariate Logistic regression analysis obtained 4 independent predictive factors, i.e., sex (odds ratio [OR]=5.38, 95% confidence interval [CI]: 1.11 — 34.21, P=0.049), baseline HBsAg level (OR=0.12, 95%CI: 0.04 — 0.26, P<0.001), the reduction in HBsAg in 0 — 12 weeks (OR=5.54, 95%CI: 1.97 — 19.18, P=0.003), and the reduction in ALT in 0 — 12 weeks (OR=0.99, 95%CI: 0.97 — 1.00, P=0.039). A nomogram model was established based on the results of the multivariate Logistic regression analysis, and the ROC curve was used to assess the predictive value of this nomogram model. This nomogram model had an AUC of 0.934 (95%CI: 0.886 — 0.981) in the training set and an AUC of 0.921 (95%CI: 0.838 — 1.000) in the validation set. In addition, the results of calibration curve and decision curve analyses showed that the model had good consistency and accuracy. ConclusionBased on general information and serological markers, the LASSO regression analysis is used to establish a nomogram model using sex, baseline HBsAg level, the reduction in HBsAg in 0 — 12 weeks, and the reduction in ALT in 0 — 12 weeks, and this model can be used to predict the probability of achieving HBsAg clearance in HBeAg-negative CHB patients treated with PEG-IFNα-2b, which provides important reference and theoretical support for the clinical treatment of patients.

BackgroundChronic obstructive pulmonary disease (COPD) is a common chronic respiratory disease, and patients with COPD often experience substantially emotional difficulties, such as anxiety and depression, all of which may cause serious detriment to the prognosis of patients. As a non-pharmacological intervention in clinical practice, group mindfulness-based stress reduction therapy (MBSR) is beginning to emerge, while has rarely been studied in COPD patients with concurrent emotional difficulties. ObjectiveTo evaluate the effects of group MBSR on depression, state of mindfulness and pulmonary function in stable COPD patients, so as to provide references for the application of group MBSR in patients with COPD. MethodsA total of 97 patients with stable COPD who were followed up in the Department of Respiratory and Critical Care Medicine of Mianyang Third People's Hospital from January to October 2019 were selected as the study objects, and they were assigned into study group (n=50) and control group (n=47) by random number table method. All individuals received routine medication therapy and an 8-week health education, based on this, participants in study group partook an 8-week intervention comprising group MBSR. At the baseline, 4 weeks and 8 weeks of intervention, participants were assessed with Self-rating Depression Scale (SDS), Five Facet Mindfulness Questionnaire (FFMQ) and COPD Assessment Test (CAT), as well as the pulmonary function testing. ResultsThere were 41 patients in study group and 42 cases in control group completed the study. The group * time interaction was interpreted as significant between two groups for SDS, FFMQ and CAT scores (F=54.858, 86.161, 69.862, P<0.01). Baseline SDS, FFMQ and CAT scores of the two groups yielded no statistical difference between two groups (F=0.240, 0.052, 0.019, P>0.05), while study group scored lower on SDS and CAT (F=12.900, 38.511, 7.797, 28.824, P<0.01) and higher on FFMQ (F=27.324, 82.412, P<0.01) than those of the control group after 4 and 8 weeks of intervention. With the prolongation of intervention time in study group, participants demonstrated an overall reduction in SDS and CAT scores (F=109.753, 124.144, P<0.01), and an increase in FFMQ scores (F=228.194, P<0.01). There were no between-group differences in forced expiratory volume in one second as percentage of predicted volume (FEV1%pred) after 4 and 8 weeks of intervention (F=0.104, P=0.748) , and the within-group changes in FEV1%pred value over the intervention period in study group was not statistical (F=0.561, P=0.458). ConclusionGroup MBSR may help relieve depressive symptoms, enhance mindfulness level, and alleviate clinical symptoms in stable COPD patients, but has no effect on pulmonary function. OBJECTIVE:
To examine the anti-inflammatory effects and potential mechanisms of polypeptide from Moschus (PPM) in lipopolysaccharide (LPS)-induced THP-1 macrophages and BALB/c mice.
METHODS:
The polypeptide was extracted from Moschus and analyzed by high-performance liquid chromatography and sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE). Subsequently, LPS was used to induce inflammation in THP-1 macrophages and BALB/c mice. In LPS-treated or untreated THP-1 macrophages, cell viability was observed by cell counting kit 8 and lactate dehydrogenase release assays; the proinflammatory cytokines and reactive oxygen species (ROS) were measured by enzyme-linked immunosorbent assay and flow cytometry, respectively; and protein and mRNA levels were measured by Western blot and real-time quantitative polymerase chain reaction (qRT-PCR), respectively. In LPS-induced BALB/c mice, the proinflammatory cytokines were measured, and lung histology and cytokines were observed by hematoxylin and eosin (HE) and immunohistochemical (IHC) staining, respectively.
RESULTS:
The SDS-PAGE results suggested that the molecular weight of purified PPM was in the range of 10-26 kD. In vitro, PPM reduced the production of interleukin 1β (IL-1β), IL-18, tumor necrosis factor α (TNF-α), IL-6 and ROS in LPS-induced THP-1 macrophages (P<0.01). Western blot analysis demonstrated that PPM inhibited LPS-induced nuclear factor κB (NF-κB) pathway and thioredoxin interacting protein (TXNIP)/nucleotide-binding oligomerization domain, leucine-rich repeat and pyrin domain containing 3 (NLRP3) inflammasome pathway by reducing protein expression of phospho-NF-κB p65, phospho-inhibitors of NF-κB (Iκ Bs) kinase α/β (IKKα/β), TXNIP, NLRP3, apoptosis-associated speck-like protein containing a caspase recruitment domain (ASC), and pro-caspase-1 (P<0.05 or P<0.01). In addition, qRT-PCR revealed the inhibitory effects of PPM on the mRNA levels of TXNIP, NLRP3, ASC, and caspase-1 (P<0.05 or P<0.01). Furthermore, in LPS-induced BALB/c mice, PPM reduced TNF-α and IL-6 levels in serum (P<0.05 or P<0.01), decreased IL-1β and IL-18 levels in the lungs (P<0.01) and alleviated pathological injury to the lungs.
CONCLUSION
PPM could attenuate LPS-induced inflammation by inhibiting the NF-κB-ROS/NLRP3 pathway, and may be a novel potential candidate drug for treating inflammation and inflammation-related diseases.

To explore the effect of doctor-patient relationship perception on work performance among medical staff in public hospitals and its mechanism. The convenient sampling method was used to select 380 medical staff working in four public tertiary hospitals in Sichuan Province, Zhejiang Province, and Fujian Province from August 2022 to October 2022 as the research objects, and the General Information Questionnaire, Doctor-Patient Relationship Scale, General Self-Efficacy Scale, Perceived Organizational Support Scale, Work Performance Questionnaire were used. This paper showed that the doctor-patient relationship perception of medical staff was negatively correlated with self-efficacy (r=-0.392, P<0.01) and work performance (r=-0.286, P<0.01), self-efficacy was positively correlated with perceived organizational support (r=0.538, P<0.01) and work performance (r=0.507, P<0.01), perceived organizational support was positively correlated with work performance (r=0.510, P<0.01). Self-efficacy played a partial mediating role between doctor-patient relationship perception and work performance, and its effect value was -0.241, accounting for 64.78% of the total effect. Perceived organizational support weakened the negative predictive effect of doctor-patient relationship perception on self-efficacy, which moderates the first half path of the mediating model that doctor-patient relationship perception affects work performance through self-efficacy. It indicated that the doctor-patient relationship perceived by medical staff in public hospitals is poor. Measures should be taken from the aspects of policy support, hospital and society levels to alleviate the doctor-patient relationship, play the mediating role of self-efficacy and the moderating role of organizational support, minimize the negative impact of doctor-patient relationship on the work performance among medical staff, and improve the work performance of medical staff, so as to improve the overall quality of medical services.

Journal of Public Health and Preventive Medicine 2022;33(2):100-104
Objective To analyze and discuss the epidemiological characteristics of HBV infection in people with hepatitis B virus (HBV) surface antigen (HBsAg) positive in Deyang Area. Methods The health checkup information of the region in 2020 was selected to screen the subjects with the results of HBV serological markers, and 1645 HBsAg-positive patients were selected as the main subjects to analyze the epidemiological characteristics of the population. Use Epidata to enter data, and use SPSS statistical software package to analyze the data. Results Except for the differences in occupational distribution between the sexes, the differences in age, education level, family history, and vaccination history (hepatitis B vaccine) were not statistically significant among men and women in this study. Among the 1645 HBsAg-positive subjects, 41 were double-positive for anti-HBs at the same time, and the double-positive rate was 2.49%. Compared with HBs-negative people, HBs-positive people have a relatively higher proportion of males, junior high school education level, occupation as a farmer, unvaccinated, and positive family history. AmpliPrep/COBAS quantitative detection reagent was used for HBV DNA viral load detection, 92.69% were quantitatively positive (38 cases), of which 23 cases (60.53%) with HBV DNA < 20 IU/mL, and 15 cases (39.47%) with HBV DNA >20 IU/mL). The epidemiological characteristics of the double-positive population suggest that the proportion of males, junior high school education level, occupation as a farmer, unvaccinated and positive family history is relatively higher. Analyzing the potential influencing factors of anti-HBs positive (double positive) in HBsAg-positive people, it can be seen that the increase in age, the occupation is a farmer, the first injection of hepatitis B vaccine is not in time, and the family history of liver disease is positive as potential risk factors for the occurrence of double positive. Conclusion In the future, this region should pay special attention to farmers who are older, have a family history of liver disease, and lack or incomplete hepatitis B vaccination for hepatitis B prevention and control.

Chinese Medical Journal 2022;135(5):598-605
BACKGROUND:
Intensive phototherapy (IPT) and exchange transfusion (ET) are the main treatments for extreme hyperbilirubinemia. However, there is no reliable evidence on determining the thresholds for these treatments. This multicenter study compared the effectiveness and complications of IPT and ET in the treatment of extreme hyperbilirubinemia.
METHODS:
This retrospective cohort study was conducted in seven centers from January 2015 to January 2018. Patients with extreme hyperbilirubinemia that met the criteria of ET were included. Patients were divided into three subgroups (low-, medium-, and high- risk) according to gestational week and risk factors. Propensity score matching (PSM) was performed to balance the data before treatment. Study outcomes included the development of bilirubin encephalopathy, duration of hospitalization, expenses, and complications. Mortality, auditory complications, seizures, enamel dysplasia, ocular motility disorders, athetosis, motor, and language development were evaluated during follow-up at age of 3 years.
RESULTS:
A total of 1164 patients were included in this study. After PSM, 296 patients in the IPT only group and 296 patients in the IPT plus ET group were further divided into the low-, medium-, and high-risk subgroups with 188, 364, and 40 matched patients, respectively. No significant differences were found between the IPT only and IPT plus ET groups in terms of morbidity, complications, and sequelae. Hospitalization duration and expenses were lower in the low- and medium-risk subgroups in the IPT only group.
CONCLUSIONS
In this study, our results suggest that IPT is a safe and effective treatment for extreme hyperbilirubinemia. The indication of ET for patients with hyperbilirubinemia could be stricter. However, it is necessary to have a contingency plan for emergency ET as soon as IPT is commenced especially for infants with risk factors. If IPT can be guaranteed and proved to be therapeutic, ET should be avoided as much as possible.
