1.Herbal Textual Research on Bletillae Rhizoma in Famous Classical Formulas

In order to provide basic information for the utilization and development of famous classical formulas containing Bletillae Rhizoma, this article systematically analyzes the historical evolution of the name, origin, harvesting and processing of Bletillae Rhizoma by reviewing the ancient materia medica, prescription books, medical books and modern literature. The research results showed that Baiji(白及) was the main name, some scholars took Baiji(白芨) as its main name, and there were many other names such as Baiji(白给), Baigen(白根), Baiji(白苙). The mainstream source of Bletillae Rhizoma was the tubers of Bletilla striata, and drying, large, white, solid, root-free and skin removed completely were the good quality standards. With the promotion of wild to cultivated medicinal materials, there were certain differences between their traits, and the quality evaluation indexes should be adjusted accordingly. The origin of records in the past dynasties was widely distributed, with Guizhou and Sichuan having high production and good quality in modern times. The harvesting period is mostly in spring and autumn, and harvested in autumn was better. The processing and processing technology is relatively simple, and it was used fresh or powdered in past dynasties, while it is mainly sliced for raw use in modern times. Based on the results, it is suggested that the tubers of Bletilla striata of Orchidaceae should be used in the famous classical formulas, and it should be uniformly written as Baiji(白及). And if the original formula indicates the requirement of processing, it should be operated according to the requirement, if the requirement of processing is not indicated, it can be used in raw form as medicine.
2.Qualitative study on HIV/AIDS related behavioral characteristics in young cases of male students from a district of Chengdu

Objective:
To investigate the sexual behaviors and acquired immunodeficiency syndrome (AIDS) knowledge acquisition of male students with human immunodeficiency virus(HIV)/AIDS before diagnosis, so as to provide reference for formulating policies and measures for AIDS prevention and control among young students.
Methods:
Qualitative interviews were conducted among 25 newly reported HIV/AIDS male students recruited from January 2021 to December 2023 in the district of Chengdu. The contents of the interviews included highrisk behaviors related to HIV infection, access to knowledge and psychological status after infection.
Results:
All interviewees had sex with men, and the mean age of first homosexual behavior was (17.48±1.64) years old. The time from first sexual behavior to confirmation ranged 0-8 years, with an average of (2.72±1.41) years. The proportion of condom use among interviewees was low, only 2 students reported using every time, and 12 students had never been tested for HIV infection before diagnosis. All interviewees were aware of knowledge about AIDS, but their awareness of risk was low. Among them, 16 thought that they would not be infected, 8 were aware of risks but held a luck mindset, and one did not know the role of such knowledge. Schools and Internet were the main ways to obtain knowledge. Among them, 3 got knowledge through Weibo, Douyin, Zhihu, etc., 9 got knowledge through school lectures, theme class meetings, etc., and 13 got knowledge through schools and the Internet. None of the interviewees had acquired AIDS related knowledge from family education. After knowing being infected, 21 students reported depression, frustration, pessimism, despair and other negative emotions(7 students expressed anxiety and depression, 9 students had fear and terror, 5 students felt regret) and only 4 students had a relatively calm attitude.
Conclusions
The primary reasons for the infection of young students are early age of first sexual behavior, multiple sexual partners, popularity of online dating and separation of AIDS knowledge and practice among HIV/AIDS male students. Targeted prevention and control measures should be developed to suppress AIDS transmission among young students.
3.Changes of Serum FIZZ2 and IL-33 Levels in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease and Their Correlation with Clinical Prognosis

Objective To investigate the changes in serum levels of the found in inflammatory zone 2(FIZZ2)and interleukin-33(IL-33)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and their correlation with prognosis.Methods A total of 102 AECOPD patients admitted to Hefei Cancer Hospital of Chinese Academy of Sciences from January 2020 to January 2023 were selected as AECOPD group,and were divided into survival group(n=30)and death group(n=72)according to the prognosis.Meanwhile,60 stable COPD patients diagnosed and treated were selected as the stable period group,and 60 healthy individuals who underwent physical examinations at the same time were selected as the control group.Enzyme linked immunosorbent assay(ELISA)was used to detect serum levels of FIZZ2 and IL-33.Pearson correlation analysis was used to investigate the correlation between serum FIZZ2 and IL-33 with lung function indicators.Multivariate Logistic regression analysis was used to identify the factors affecting the prognosis of AECOPD patients.The predictive value of serum FIZZ2 and IL-33 in predicting the prognosis of AECOPD patients was analyzed by receiver operating characteristic(ROC)curve.Results In AECOPD group,serum FIZZ2(212.19±48.47 ng/L)and IL-33(66.19±13.41 μg/L)were higher than those in the stable phase group(101.11±31.03ng/L,40.69±9.94μg/L)and the control group(42.33±7.65ng/L,26.17±8.10μg/L),and the differences were significant(t=15.925,26.915;12.799,20.961,all P<0.05).The serum levels of FIZZ2 and IL-33 were negatively correlated with the forced expiratory volume in one second/predicted value ratio(FEV1%pred),forced expiratory volume in one second/forced vital capacity(FEV1/FVC),and arterial partial pressure of oxygen(r=-0.676,-0.707,-0.715;-0.662,-0.711,0.630,all P<0.05),but were positively correlated with arterial partial pressure of carbon dioxide(r-0.712,0.689,all P<0.05).The serum FIZZ2(254.30±53.22 ng/L)and IL-33(75.18±15.62 μg/L)in the death group were higher than those in the survival group(194.64±47.16 ng/L,62.50±12.37 μg/L),and the differences were significant(t=5.604,4.354,all P<0.05).Serum FIZZ2(OR=1.445,95%CI:1.094~1.909)and IL-33(OR=1.701,95%CI:1.111~2.602)were independent risk factors for the prognosis of AECOPD patients.The area under the curve(95%confidence interval)of the combination of serum FIZZ2 and IL-33 in the prognosis evaluation of AECOPD patients was 0.886(0.842~0.927),which was higher than 0.834(0.798~0.890)and 0.806(0.779~0.846)of single index detection,and the differences were significant(Z=4.255,4.697,all P<0.05).Conclusion The serum levels of FIZZ2 and IL-33 are elevated in AECOPD patients,which are related to lung function.The combination of the two may have high predictive value for the survival prognosis of AECOPD patients.
4.A nationwide multicenter prospective study on the perioperative impact of closure of mesen-teric fissure in laparoscopic right hemicolectomy

Objective:To investigate the perioperative impact of closure of mesenteric fissure in laparoscopic right hemicolectomy.Methods:The prospective randomized controlled trial was conducted. The clinical data of 320 patients who underwent laparoscopic right hemicolectomy in 11 medical centers, including The First Affiliated Hospital of China Medical University et al, from November 2022 to August 2023 were selected. Based on block randomization, patients were alloca-ted into the mesenteric fissure non-closure group and the mesenteric fissure closure group. Observa-tion indicators: (1) grouping of the enrolled patients; (2) intraoperative conditions; (3) postopera-tive conditions. Measurement data with skewed distribution were represented as M( Q1, Q3) and com-parison between groups was conducted using the Mann-Whitney U test. Count data were represen-ted as absolute numbers or percentages, and comparison between groups was conducted using the chi-square test or Fisher's exact probability. Comparison of ordinal data was conducted using the rank sum test. Comparison of visual analog scores was analyzed using generalized estimating equations. Results:(1) Grouping of the enrolled patients. A total of 320 patients with colon cancer were screened for eligibility, including 156 males and 164 females, aged 68(59,73)years. All the 320 patients were allocated into the mesenteric fissure non-closure group with 164 cases and the mesenteric fissure closure group with 156 cases. There was no significant difference in the age, body mass index, American Society of Anesthesiologist score, maximum tumor diameter, anastomosis location, anastomosis method, surgical approach, range of lymph node dissection, tumor staging between the two groups ( P>0.05) and there was a significant difference in the sex between them ( P<0.05). (2) Intraoperative conditions. There was no significant difference between the mesenteric fissure closure group and the mesenteric fissure non-closure group in the volume of intraoperative blood loss, operation time, conversion to laparotomy, intraoperative complication ( P>0.05). Three patients in the mesenteric fissure non-closure group were converted to laparotomy. One patient in the mesenteric fissure closure group was converted to laparotomy, and 2 cases with intraoperative complication were mesenteric hematoma. (3) Postoperative conditions. There was no significant difference between the mesenteric fissure non-closure group and the mesenteric fissure closure group in the overall postoperative complications ( χ2=0.28, P>0.05). There was no significant difference in the occurrence of postoperative intestinal obstruction, abdominal distension, ascites, pleural effusion, gastric paralysis, anastomotic bleeding, anastomotic leakage, or surgical wound infection between the two groups ( P>0.05). There was no significant difference between the two groups in the reoperation, postoperative gastric tube replacement. There was no significant differ-ence in time to postoperative first flatus, time to postoperative initial liquid food intake, time to post-operative resumption of bowel movements, duration of postoperative hospital stay, total hospital expenses between the two groups ( Z=-0.01, 0.43, 1.04, -0.54, -0.36, P>0.05). One patient in the mesenteric fissure non-closure group received reoperation. No perioperative internal hernia or death occurred in either group. The visual analog score decreased with time in both groups. There was no significant difference in the visual analog score between the mesenteric fissure closure group and the mesenteric fissure non-closure group [ β=-0.20(-0.53,0.13), P>0.05]. Conclusion:Compared with closure of mesenteric fissure, non-closure of mesenteric fissure during laparoscopic right hemi-colectomy dose not increase perioperative complications or postoperative management risk.
5.Awareness of HIV non-occupational post-exposure prophylaxis among residents in Yangzhou City

Objective:
To investigate the awareness of HIV non-occupational post-exposure prophylaxis (nPEP) among residents in Yangzhou City, Jiangsu Province, so as to provide basis for further publicity and application of nPEP in genenral population.
Methods:
Community residents at ages of 18 years and older in Yangzhou City were sampled using the multi-stage random sampling method from January to June, 2022, and a cross-sectional survey was conducted to collect their demographic information, acquired immune deficiency syndrome (AIDS)-related knowledge, attitude and behaviors and awareness of nPEP. A multivariable logistic regression model was used to identify factors affecting awareness of nPEP.
Results:
Totally 1 488 questionnaires were allocated, and 1 447 valid questionnaires were recovered, with an effective recovery rate of 97.24%. There were 644 males and 803 females, with mean age of (36.33±11.90) years. There were 585 residents (40.43%) employed by government agencies, enterprises and public institutions; 750 residents (51.83%) with awareness of AIDS; 413 (28.54%), 651 (44.99%) and 383 residents (26.47%) had low, medium and high levels of AIDS discrimination, respectively; 201 residents (13.89%) with HIV detection experience; and 792 residents (54.73%) with regular health checkup. The awareness rate of nPEP was 47.89%. Multivariable logistic regression analysis identified career (government agencies, enterprises and public institutions, OR=1.530, 95%CI: 1.170-1.999; students, OR=1.450, 95%CI: 1.027-2.047), awareness of AIDS (OR=3.201, 95%CI: 2.495-4.106), having HIV detection experience (OR=1.908, 95%CI: 1.339-2.719), regular health checkup (OR=2.308, 95%CI:1.807-2.949) and discrimination against AIDS (medium, OR=0.703, 95%CI: 0.532-0.928; high, OR=0.699, 95%CI: 0.502-0.974) as factors affecting awareness of nPEP.
Conclusions
The awareness of nPEP among residents in Yangzhou City needs to be improved. Employees in government agencies, enterprises and public institutions and students have higher awareness of nPEP.
6.Protective effects of Slit2 on corneal epithelium and nerves in diabetic mice and its mechanism

Objective:To explore the promoting effects of slit guidance ligand 2 (Slit2) on the repair of corneal epithelium and nerve damage in diabetic mice and possible molecular mechanism.Methods:Sixty SPF C57BL/6 mice aged 5-6 weeks were divided into normal control group, diabetes model group and Slit2 injection group according to the random number table method, 20 for each group.Diabectic model was prepared by intraperitoneal injection of streptozotocin in the diabetes model group and Slit2 injection group.A mouse corneal epithelial injury repair model was established using electric epithelial scraper, and Slit2 recombinant protein was subconjunctivally injected immediately following modeling in the Slit2 injection group.The equal volume of phosphate buffer saline (PBS) was used in a same way in the diabetes model group.No intervention was performed in the normal control group.Corneal epithelial healing were examined at 24, 48 and 72 hours after corneal epithelial defect by corneal fluorescin staining.Real-time fluorescent quantitative PCR was used to detect the expression of Slit2 and its related receptors in the corneal epithelium of normal and diabetic model mice.Fluorescence staining of corneal wholemount with β-tubulin Ⅲ was used to observe the changes in corneal nerve morphology.Immunofluorescence staining was performed to detect the expression and distribution of Slit2 in mouse corneal epithelium in normal control group and diabetes model group, as well as the expression and distribution of Slit2, epidermal growth factor receptor (EGFR), extracellular-signal-regulated kinase (ERK), threonine protein kinase (AKT), β-catenin and Ki67 in the healing corneal epithelium of mice after corneal epithelium damage in different groups.The mouse corneal epithelial stem/progenitor cell line (TKE2) was divided into normal control group, high-glucose group and Slit2 treatment group.Western blot was performed to detect the expression of p-EGFR/EGFR and p-AKT/AKT in the TKE2 of the three groups.The expression of p-EGFR/EGFR and p-AKT/AKT in high glucose-cultured TKE2 with 0.01, 0.1 and 0.5 μg/ml Slit2 treatment for 10 minutes, and before and 10, 20, 30, 60, 120 minutes after 0.5 μg/ml Slit2 treatment was detected by Western blot.The effects of Slit2 on the axon regeneration of mouse trigeminal ganglion cells (TGs) were observed by immunofluorescence staining.The use and care of animals complied with the ARVO statement.This study protocol was approved by an Ethics Committee of Qingdao Eye Hospital of Shandong First Medical University (No.[2020]57).Results:At 48 and 72 hours after corneal epithelial scraping, the speed of corneal epithelial repair was significantly slowed down in diabetes model group in comparison with the normal control group and Slit2 injection group.The relative expression levels of Slit2 and its receptors Robo1, Robo2 and Robo4 mRNA in the normal corneal epithelium in the diabetes model group were significantly higher than those of the normal control group (all at P<0.05). The fluorescence intensity of Slit2 in normal corneal epithelium in diabetes model group was similar to the normal control group, and the fluorescence intensity of Slit2 in damaged corneal epithelium in diabetic mice was significantly weaker than that in normal control group.Corneal nerve plexus was denser at 7 days after corneal epithelial injury and the nerve fibers were increased with more branches in Slit2 injection group compared with diabetic group.The fluorescence intensity of p-EGFR, p-ERK, β-catenin and Ki67 in damaged corneal epithelium in normal control group and Slit2 injection group was stronger than that of the diabetes model group.The relative expression levels of p-EGFR/EGFR, p-AKT/AKT, and β-catenin in TKE2 in high-glucose group were significantly lower than those in normal control group and Slit2 treatment group (all at P<0.05). The relative expression levels of p-EGFR/EGFR and p-AKT/AKT in high glucose-cultured TKE2 after Slit2 treatment were significantly increased in comparison with before Slit2 treatment (both at P<0.05), and the relative expression levels of p-EGFR/EGFR and p-AKT/AKT in TKE2 were elevated as the increase of Slit2 concentration.The activation effect of 0.5 μg/ml Slit2 on EGFR and AKT pathways was most obvious.The synapse length of TGs cultured by high glucose was (40.52±5.44) μm, which was significantly shortened than (72.14±9.48) μm in normal control group and (73.04±4.66) μm in Slit2 injection group (both at P<0.05). Conclusions:Slit2 can protect the corneal epithelium by activating EGFR signaling pathway and play a protective role to neurons by increasing the density of corneal subepithelial plexus and promoting the growth of TGs axons in diabetic mice.
7.A reinforced suture method for stapled gastrointestinal anastomosis to reduce gastrointestinal hemorrhage during Whipple operation in laparoscopy

Purpose:
Laparoscopy is being increasingly accepted for pancreaticoduodenectomy. Stapled anastomosis (SA) is used extensively to facilitate laparoscopic pancreaticoduodenectomy (LPD); however, the incidence of anastomotic bleeding after stapled gastrointestinal anastomosis is still high.
Methods:
One hundred and thirty-nine patients who underwent LPD using Whipple method were enrolled in our study. We performed the SA with our reinforced method (n = 68, R method) and without the method (n = 71, NR method). We compared the clinical characteristics and anastomosis methods of patients with or without gastrointestinal-anastomotic hemorrhage (GAH), and operative parameters were also compared between the anastomotic methods.
Results:
Of the 139 patients undergoing LPD, 15 of them developed GAH. The clinical characteristics of patients with or without GAH were not significantly different except in the anastomotic method (P < 0.001). In the univariate logistic regression analyses, only the anastomotic method was associated with GAH. Furthermore, patients with the NR method had significantly higher incidences of GAH (P < 0.001) and Clavien-Dindo grade ≥ III complications (P < 0.001).
Conclusion
Our retrospective analysis showed that the SA performed with reinforced method might be a reform of SA without the reinforcement, as indicated by the lower incidence of GAH. However, further research is necessary to evaluate the utility of this reinforced method.
8.Application value of 18F-FDG PET/CT in the diagnosis of cholangiocarcinoma

Objective To explore the value of 18F-FDG PET/CT in the diagnosis of cholangiocarcinoma. Methods Data were collected from 44 patients with cholangiocarcinoma who underwent PET/CT in Affiliated Cancer Hospital of Shandong First Medical University from September 2017 to October 2020. All patients underwent upper abdominal CT and MRI and whole-body PET/CT. The diagnostic value of three examinations was compared for primary lesions, recurrent lesions, and regional lymph node metastasis of cholangiocarcinoma. Results There were no significant differences in the diagnostic sensitivity of CT, MRI, and PET/CT in the primary lesions and regional lymph node metastasis of cholangiocarcinoma (P > 0.05). There were significant differences in the diagnostic sensitivity of the three examinations for recurrent cholangiocarcinoma lesions (P < 0.05). Conclusion PET/CT has high diagnostic value for recurrent lesions of cholangiocarcinoma, but the three examinations showed no significant differences in the diagnostic sensitivity for primary lesions and regional lymph node metastasis.

Model informed precision dosing (MIPD) is a new concept to guide precision dosing for individual patient by modeling and simulation based on the available information about the individual patient, medications and the disease. Compared to the empirical dosing, MIPD could improve the efficacy, safety, economics and adherence of the pharmacotherapy according to the individual's pathophysiology, genotyping and disease progression. This consensus report provides a brief account of the concept, methodology and implementation of MIPD as well as clinical decision supporting systems for MIPD. The status and future advancing of MIPD was also discussed to facilitate the appropriate application and development of MIPD in China.
10.Analysis on the Heavy Metal Residues in Codonopsis pilosula in Shanxi Pingshun County

OBJECTIVE:To establish a method for the residues determination of Pb,Cd,Cu,As and Hg in Codonopsis pilo-sula,and evaluate the quality evaluation of C. pilosula of Pingshun County in Shanxi province. METHODS:Microwave diges-tion-inductively coupled plasma mass spectrometry was adopted with KED scanning model,RF power was 1 550 W,sampling depth was 5.0 mm,plasma gas(argon)flow rate was 16.0 L/min,helium partial pressure was 0.1 mbar,argon gas was 0.6 mbar, the vacuum degree of 5×10-8 mbar,branch turbopump speed was 1 000 hz,sampling cone aperture was 1.0 mm,skimmer aperture was 0.5 mm,the spray chamber temperature was 2.7 ℃,the data collection was repeated 3 times. RESULTS:The linear range was 0-20 ng/ml for Pb(r=0.999 3),0-10 ng/ml for Cd(r=0.998 5),0-250 ng/ml for Cu(r=0.998 8),0-20 ng/ml for As(r=0.999 0) and 0-1.0 ng/ml for Hg(r=0.997 9);RSDs of precision,stability and reproducibility tests were lower than 3.0%;recoveries were 95.80%-100.20%(RSD=1.85%,n=6),94.50%-98.00%(RSD=1.26%,n=6),98.52%-102.43%(RSD=1.60%,n=6), 94.90%-98.70%(RSD=2.29%,n=6)and 96.00%-101.00%(RSD=1.84%,n=6);the limits of detection were 0.021 0,0.003 4, 0.043 7,0.115 6 and 0.005 6 ng/kg,respectively. Pb,Cd,Cu,and As were detetcted,and Hg was not detected,the range of total contents was 7.185 2~12.558 0 mg/kg. CONCLUSIONS:The method is simple with good precision,stability and reproducibility, and can be used for the residues determination of Pb,Cd,Cu,As and Hg in C. pilosula;heavy metal residues in C. pilosula in Shanxi Pingshun county does not exceed limit values of national and industry standards.
