OBJECTIVE:
To observe the therapeutic effect of electroacupuncture on lumbar disc herniation (LDH) with different multifidus fatty infiltration rates.
METHODS:
A total of 108 eligible LDH patients received MRI test before treatment and the multifidus fatty infiltration rate (FI) was measured. According to theresults, they were divided to group A (FI<10%), group B (10%≤FI≤20%) and group C (FI>20%), 36 cases in each one. EA was provided in all of the groups. CONCLUSION
Electroacupuncture relieves pain and improves physical and psychological health of LDH patients. Multifidus fatty infiltration rate is probably one of the factors to influence the therapeutic effect of electroacupuncture.

BACKGROUND:
Bendamustine was approved in China on May 26th, 2019 by the National Medical Product Administration for the treatment of indolent B-cell non-Hodgkin lymphoma (NHL). The current study was the registration trial and the first reported evaluation of the efficacy, safety, and pharmacokinetics of bendamustine in Chinese adult patients with indolent B-cell NHL following relapse after chemotherapy and rituximab treatment.
METHODS:
This was a prospective, multicenter, open-label, single-arm, phase 3 study (NCT01596621; C18083/3076) with a 2-year follow-up period. Eligible patients received bendamustine hydrochloride 120 mg/m2 infused intravenously on days 1 and 2 of each 21-day treatment cycle for at least six planned cycles (and up to eight cycles). The primary endpoint was the overall response rate (ORR); and secondary endpoints were duration of response (DoR), progression-free survival (PFS), safety, and pharmacokinetics. Patients were classified according to their best overall response after initiation of therapy. Proportions of patients in each response category (complete response [CR], partial response [PR], stable disease, or progressive disease) were summarized along with a two-sided binomial exact 95% confidence intervals (CIs) for the ORR.
RESULTS:
A total of 102 patients were enrolled from 20 centers between August 6th, 2012, and June 18th, 2015. At the time of the primary analysis, the ORR was 73% (95% CI: 63%-81%) per Independent Review Committee (IRC) including 19% CR and 54% PR. With the follow-up period, the median DoR was 16.2 months by IRC and 13.4 months by investigator assessment; the median PFS was 18.6 months and 15.3 months, respectively. The most common non-hematologic adverse events (AEs) were gastrointestinal toxicity, pyrexia, and rash. Grade 3/4 neutropenia was reported in 76% of patients. Serious AEs were reported in 29 patients and five patients died during the study. Pharmacokinetic analysis indicated that the characteristics of bendamustine and its metabolites M3 and M4 were generally consistent with those reported for other ethnicities.
CONCLUSION:
Bendamustine is an active and effective therapy in Chinese patients with relapsed, indolent B-cell NHL, with a comparable risk/benefit relationship to that reported in North American patients.

5.Clinical Features and Prognostic Risk Factors of Choroid Plexus Tumors in Children.

Background:
Decision-making concerning the treatment of choroid plexus tumor (CPT) in pediatric patients remains a topic of considerable debate. The aim of this work was to describe clinical features and prognostic risk factors of CPT in the pediatric population and to provide theoretical opinions regarding clinical decisions for CPT.
Methods:
The data of 96 patients with CPT and younger than 14 years were retrospectively analyzed. Clinical characteristics such as pathological type of CPTs, rate and severity of hydrocephalus, treatment and outcome, and recurrence were investigated. For categorical variables, the Pearson's Chi-square test was performed. The Mann-Whitney U-test was used for comparisons between nonnormally distributed parameters. Log-rank test was used for progression-free survival (PFS).
Results:
The study included 70 choroid plexus papilloma (CPP) cases, 17 atypical choroid plexus papilloma (aCPP) cases, and 9 choroid plexus carcinoma (CPC) cases. Compared with patients with CPP or aCPP, patients with CPC had a shorter disease course (median: CPP, 4 months; aCPP, 2 months; CPC, 1 month; H: 23.5, P < 0.001), higher rate of acute hydrocephalus (CPP, 27.1%; aCPP, 52.9%; CPC, 77.8%; χ = 10.9, P < 0.05), and lower incidence of cure rate (CPP, 85.7%; aCPP, 70.5%; CPC, 33.3%; χ = 13.5, P < 0.05). The severity of hydrocephalus with tumor in the lateral or third ventricle was significantly higher than that with tumors in the fourth ventricle (severe hydrocephalus: lateral ventricle, 51.7%; third ventricle, 47.0%; fourth ventricle, 11.1%; χ = 26.0, P < 0.001). Patients with gross total surgical resection had no better PFS than those with partial resection because of the use of adjuvant therapy in the latter (χ = 4.0, P > 0.05). Patients with CPC experienced shorter time for recurrence than those with CPP or aCPP (χ = 40.1, P < 0.0001).
Conclusions
Our results indicated that CPP in the fourth ventricle could trigger serious clinical symptoms at an early stage, requiring early intervention. Adjuvant treatment might be necessary for patients with partially resected CPP, aCPP, and CPC to achieve a favorable outcome.

6.Research of DNA barcode of Teseudinis Carapax et Planstrum and its adulterants based on COI gene sequence.

OBJECTIVETo use COI gene on the Mauremys reevesii and its adulterants by molecular identification. Search a rapid, accurate method of identification of Teseudinis Carapax et Planstrum and its adulterants.
METHODWe collected 8 species of the authentic and adulterants of teseudinis carapax et planstrum in a nationwide then, extracted DNA, got the COI sequences. Use ContigExpress, Dnaman, Edit Sequence and Mega 5 to analyze the variable site and construct the N-J tree.
RESULTCompare with the authentic Teseudinis Carapax et Planstrum, the adulterant exist lots of variable site. The N-J tree Indicates that the same genus belong together and each species belong to relatively independent branch.
CONCLUSIONBased on the COI gene, the technology of DNA bar code can be a excellent identification of Teseudinis Carapax et Planstrum and its adulterants.

7.Etoricoxib versus indometacin in the treatment of Chinese patients with acute gouty arthritis: a randomized double-blind trial.

BACKGROUNDAcute gout is an intensely painful, inflammatory arthritis. Although the non-steroidal anti-inflammatory drugs (NSAIDs) are widely used for this condition, the efficacy is based on only a few studies, particularly in China. We tried to assess the safety and efficacy of etoricoxib in the treatment of acute gouty arthritis in China.
METHODSA randomized, double-blind, active comparator study was conducted at 10 sites in China. Patients (n = 178; ≥ 18 years of age) with acute gouty attack (< 48 hours) were treated for 5 days with etoricoxib (120 mg/d; n = 89) or indometacin (75 mg twice daily; n = 89). The primary efficacy end point was self-assessed pain in the affected joint (0-4 point Likert scale) from days 2 - 5. Secondary end points included investigator assessments of tenderness and swelling, patient/ investigator global assessments of response to therapy, and patients discontinuing treatment. Safety was assessed by adverse events (AEs).
RESULTSEtoricoxib and indometacin had comparable primary and secondary end points. Mean change difference from baseline from days 2 - 5 was 0.03 (95% confidence interval (CI) -0.19 to 0.25; P = 0.6364), which fell within the prespecified comparative bounds of -0.5 to 0.5. No severe AEs were associated with etoricoxib use. Non-severe AEs were mainly digestive and general, and most (73.7%) were mild, although they caused withdrawal of two subjects in the etoricoxib group, due to bilateral renal calculi and uronephrosis of the left kidney (unrelated to etoricoxib) and fever and chills (potentially etoricoxib-related). Overall, AEs were similar, although the absolute number of AEs in the etoricoxib group (n = 31) was less than the indometacin group (n = 34).
CONCLUSIONSEtoricoxib (120 mg once daily) is effective in treating acute gout, is generally safe and well-tolerated, and is comparable in efficacy to indometacin (75 mg twice daily).

8.Impact of Pax-8 gene interference on mitochondrial function and cardiomyocyte apoptosis

Objective To observe the effects of paired box gene 8 (Pax-8) silencing by RNA interference on mitochondrial function and cardiomyocytes apoptosis.Methods The cultured H9C2 (2-1) myocytes were divided into 3 groups:short interference RNA targeting Pax-8 (Pax-8 siRNA) group,nonspecific siRNA group as the negative control (NC siRNA),and blank control group (BC siRNA).Fluo rescence spectrophotometry was used to detect the activity of caspase-3.RT-PCR was performed to detect mRNA expression of Bcl2 and Bax.The protein expression of Bcl2、Bax and cytoplasm of Cytothrome was examined by Western blot.Changes of △ψm were detected by flow cytometry.△ψm with JC-1 monomer/polymer ratio was calculated for measuring mitochondrial depolarization proportion.Results Compared to NC siRNA and BC siRNA group(0.075 ±0.021,0.072 ±0.019),the activity of caspase-3 in Pax-8 siRNA group(0.167 ±0.012) was significantly increased (P <0.05) ; Bcl2 mRNA and protein expression in Pax-8siRNA group (0.61 ±0.06,0.94 ±0.11) were significantly downregulated compared with NC siRNA group (0.90±0.070,1.39±0.15) and BC siRNA group (0.94 ±0.087,1.49±0.20) (P<0.05); Bax mRNA and protein expression in Pax-8 siRNA group (1.05 ± 0.10,1.25 ± 0.12) were markedly upregulated compared with NC siRNA group (0.72 ± 0.03,0.99 ± 0.12) and BC siRNA group (0.64 ±0.03,0.92 ± 0.06),P < 0.05 ; cytosolic cytochrome expression in Pax-8 siRN A group (0.75 ± 0.14) was significantly upregulated compared with NC siRNA group (0.51 ± 0.06) and BC siRNA group (0.48 ±0.07) (P < 0.05) ; JC-1 monomer/polymer ratio in Pax-8 siRNA group (0.163 ± 0.011) was significantly increased compared with NC siRNA group(0.092 ±0.015) and BC siRNA group (0.072 ± 0.025) (P <0.05) indicating mitochondrial membrane potential was significantly reduced in Pax-8 siRNA group.Above parameters were similar between NC siRNA group and BC siRNA group (P > 0.05).Conclusion Inhibiting Pax-8 results in enhanced cardiomyocytes apoptosis through the mitochondrial pathway.
9.Initial experience of percutaneous coronary intervention guided by computed tomography coronary angiography derived roadmap and magnetic navigation system

Objective To evaluate the feasibility,efficacy and safety of the percutaneous coronary intervention(PCI) guided by computed tomography(CT) coronary angiography derived roadmap and magnetic navigation system(MNS).Methods During June 2011 and May 2012,thirty consecutive patients receiving elective PCI were enrolled,coronary artery disease was primarily diagnosed by dual-source CT coronary angiography(DSCT-CA) at outpatient clinic and successively proved by coronary artery angiography in the hospital.Target vessels from pre-procedure DSCT-CA were transferred to the magnetic navigation system,and consequently edited,reconstructed,and projected onto the live fluoroscopic screen as roadmaps.Parameters including characters of the target lesions,time,contrast volume,radiation dosage for guidewire crossing,and complications of the procedure were recorded.Results Thirty patients with 36 lesions were recruited and intervened by PCI.Among the target lesions,sixteen were classified as type A,11 as type B1,8 as type B2,1 as type C.The average length of the target lesions was (22.0 ± 9.8) mm,and the average stenosis of the target lesions was(81.3 ± 10.3)%.Under the guidance of CT roadmap and MNS,36 target lesions were crossed by the magnetic guidewires,with a lesion crossing ratio of 100%.The time of placement of the magnetic guidewires was 92.5 (56.6-131.3)seconds.The contrast volume and the radiation dosage for guidewire placement were 0.0 (0.0-3.0) ml and 235.0 (123.5-395.1) μGym2/36.5 (21.3-67.8)mGy,respectively.Guidewires were successfully placed in 21 (58.3%) lesions without contrast agent.All enrolled vessels were successfully treated,and there were no MNS associated complications.Conclusions It is feasible,effective and safe to initiate PCI under the guidance of CT derived roadmap and MNS.This method might be helpful for the guidewire placement in the treatment of total occlusions.

The column chromatography on silica gel, Sephadex LH-20 and semi-preparative HPLC were used to separate and purify the compounds from the EtOAc extract of medium and MeOH extract of cell cultures of Morus alba. Eight compounds were isolated. Based on physico-chemical properties and spectroscopic data, their structures were identified as isobavachalcone (1), genistein (2), norartocarpetin (3), albanin A (4), guangsangon E (5), mulberrofuran F (6), chalcomoracin (7), kuwanon J (8). Compounds 3-6 were isolated from the cell cultures of M. alba for the first time.
