1.Analysis of the efficacy and safety of Saccharomyces boulardii in the treatment of pediatric diarrhea

Objective To compare the efficacy and safety of Saccharomyces boulardii and other probiotics in the treatment of pediatric diarrhea.Methods Retrieved from PubMed,Embase,CBM,Wanfang data,CNKI and VIP,randomized controlled trial(RCTs)about Saccharomyces boulardii(treatment group)vs other probiotics(control group)were collected.After screening the literature,extracting data and evaluating the quality,Meta-analysis was performed by using RevMan 5.3 and Stata 17.0 software.Results A total of 30 RCTs were included,involving 3 082 children.Results of Meta-analysis showed there was no statistical significance in the duration of diarrhea[mean difference(MD)=-0.65,95%confidence interval(CI)=-1.44-0.14,P>0.05]or the total incidence of adverse reactions[odds ratio(OR)=0.85,95%CI=0.44-1.62,P>0.05].The total effective rate(OR=1.60,95%CI=1.08-2.36,P<0.05)and the number of stools(MD=-0.66,95%CI=-1.00--0.32,P<0.01)in the treatment group were significantly better than control group.There was statistically significant difference in the duration of diarrhea between the two groups treated with diarrhea with fever(MD=1.81,95%CI=1.12-2.49,P<0.01)and rotavirus gastroenteritis(MD=-0.92,95%CI=-1.20--0.64,P<0.01).In the treatment of diarrhea with fever,the duration of diarrhea in the control group was significantly shorter than that in the treatment group.The duration of diarrhea in the treatment group was significantly shorter than that control group.At the same time,the duration of diarrhea in children with diarrhea treated with treatment group was significantly shorter than that of control group Bifidobacterium tetragenous viable(MD=-0.84,95%CI=-1.09--0.58,P<0.01)and control group combined Bacillus subtilis and Enterococcus faecium enteric-coated(MD=-1.35,95%CI=-2.30-0.39,P<0.01).Conclusion The safety of Saccharomyces boulardii are similar to other probiotics.The efficacy and the number of stools with Saccharomyces boulardii are significantly better than other probiotics in the treatment of diarrhea.The duration of diarrhea in Saccharomyces boulardii group was significantly shorter than other probiotics,while in the treatment of antibiotic-associated diarrhea,the duration of diarrhea of Saccharomyces boulardii was the same as that of other probiotics.However,the duration of diarrhea in children with diarrhea treated with Saccharomyces boulardii was significantly longer than other probiotics.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
3.Transcriptome Analysis of Campylobacter jejuni-Induced Colorectal Cancer

ObjectiveTo explore the carcinogenic mechanism of Campylobacter jejuni. MethodsEighteen female C57BL/6 ApcMin/+ mice were randomly divided into the C. jejuni-infected group and the non-infection control group, each group with nine mice. Colorectal cancer of ApcMin/+ mice was induced by dextran sulfate sodium and gavage of C. jejuni (or PBS as a control). At the end of the experiment, the number of tumors in colorectal tissues of mice in each group was counted, and RNA was extracted from colorectal tumors, along with para-cancer tissues as controls. Transcriptome sequencing was performed by RNA-Seq technology, and data were analyzed for differentially expressed genes (DEGs). Further, selected DEGs were subjected to GO (gene ontology) enrichment analysis and KEGG pathway enrichment analysis. ResultsCompared with that of the non-infection control group, the incidence of tumor in C. jejuni-infected group was significantly higher (P < 0.01), which indicated the success of recreating the C. jejuni-induced CRC model. RNA-seq results showed that there were 394 and 501 DEGs (fold change > 4 and P < 0.05) in the C. jejuni-infected group compared with the two control groups, respectively. In GO enrichment analysis, DEGs were mainly enriched in immune response regulation and activation pathways, multiple protein transport pathways and receptor binding pathways. Cancer-related pathways and metabolic pathways were significant enriched in KEGG pathway enrichment analysis. Among these DEGs, 17 genes were found in comparisons with both control groups. The 17 genes were further selected, resulting in 14 “core” DEGs. In further validation of qRT-PCR, 9 genes were significantly differentially expressed, among which 3 genes were up-regulated (Gm1987, Saxo1 and Plekhs1) and 6 were down-regulated (Lrp2, Serpina3c, Fabp4, Tmem52, Lrrn4 and Upk3b). ConclusionThis study emphasizes 9 host genes that may play important and unique roles in the occurrence and development of colorectal cancer induced by C. jejuni, which provides new insights for further studies on the carcinogenic mechanism of C. jejuni.
4.The antimicrobial peptide YD attenuates inflammation via miR-155 targeting CASP12 during liver fibrosis

The antimicrobial peptide APKGVQGPNG (named YD), a natural peptide originating from Bacillus amyloliquefaciens CBSYD1, exhibited excellent antibacterial and antioxidant properties in vitro. These characteristics are closely related to inflammatory responses which is the central trigger for liver fibrosis. However, the therapeutic effects of YD against hepatic fibrosis and the underlying mechanisms are rarely studied. In this study, we show that YD improved liver function and inhibited the progression of liver fibrosis by measuring the serum transaminase activity and the expression of α-smooth muscle actin and collagen I in carbon tetrachloride-induced mice. Then we found that YD inhibited the level of miR-155, which plays an important role in inflammation and liver fibrosis. Bioinformatics analysis and luciferase reporter assay indicate that Casp12 is a new target of miR-155. We demonstrate that YD significantly decreases the contents of inflammatory cytokines and suppresses the NF-κB signaling pathway. Further studies show that transfection of the miR-155 mimic in RAW264.7 cells partially reversed the YD-mediated CASP12 upregulation, the downregulated levels of inflammatory cytokines, and the inactivation of the NF-κB pathways. Collectively, our study indicates that YD reduces inflammation through the miR-155–Casp12–NF-κB axis during liver fibrosis and provides a promising therapeutic candidate for hepatic fibrosis.

Adhesion G protein-coupled receptors(aGPCRs) play a significant role in cognitive impairment related diseases. As an important member of aGPCRs, brain-specific angiogenesis inhibitor 1(BAI1) has a prominent impact on anti-angiogenesis, anti-tumor and participating in immune phagocytosis. Recent research found out that BAI1 exerts a great influence on synaptogenesis and synaptic plasticity, but few studying concerning BAI1 in nervous system. Nowadays, the aging of population aggravates the occurrence of cognitive impairment. The pathogenesis of Alzheimer's disease and vascular cognitive impairment remains elusive, and identification of cognitive impairment at an early stage faces challenges. In the stage of mild cognitive impairment, synaptic damage is evident. BAI1 can regulate the function of postsynaptic membrane, synaptogenesis, synaptic signal transmission and the morphological development of dendritic spines. Therefore, it may potentially act as an early-warning index and intervention target for cognitive impairment.
8.Therapeutic effects on primary dysmenorrhea treated with moxibustion at Shenque (CV 8) and warm needling at Guanyuan (CV 4) and Sanyinjiao (SP 6).

OBJECTIVE:
To explore the clinical effects on primary dysmenorrhea treated with moxibustion at Shenque (CV 8) and warm needling at Guanyuan (CV 4) and Sanyinjiao (SP 6).
METHODS:
A total of 120 patients with primary dysmenorrhea were randomized into an observation group and a control group, 60 cases in each one. In the control group, the warm needling technique was used at Guanyuan (CV 4) and Sanyinjiao (SP 6). In the observation group, besides the same treatment as the control group, moxibustion was added at Shenque (CV 8). The treatment was given for 4 menstrual cycles consecutively. Before and after treatment, the score of the severity and the score of the total frequency in the retrospective scale of dysmenorrhea symptoms as well as the score of the visual analog scale (VAS) were recorded and compared in the patients between the two groups. Additionally, the safety of the two therapeutic methods was evaluated.
RESULTS:
After treatment, the score of severity and the score of total frequency as well as VAS score of menstrual pain were all reduced as compared with those before treatment in the patients of the two groups (all <0.05). The scores in the observation group were more obvious as compared with the control group (all <0.05). Regarding the safety evaluation, the difference was not significant between the two groups (>0.05).
CONCLUSION
The combined treatment of moxibustion at Shenque (CV 8) with the warm needling technique at Guanyuan (CV 4) and Sanyinjiao (SP 6) achieves the better clinical effects on primary dysmenorrhea as compared with the simple application of the warm needling technique at Guanyuan (CV 4) and Sanyinjiao (SP 6). This therapy is safety in clinical practice.

10.Effect of Two Kinds of Kinesio Taping Methods on Shoulder Pain in Hemiplegic Patients with Stroke during Spasm Period

Objective To observe the effect of two kinds of Kinesio taping methods on shoulder pain in hemiplegic patients with stroke during spasm period. Methods From October,2015 to September,2017,75 stroke hemiplegic patients with shoulder pain during spasm period were randomly divided into group A(n=25),group B(n=25)and control group(n=25).Three groups were given conventional treatment,and group A and group B received two kinds of Kinesio taping methods,respectively.Be-fore and four weeks after treatment,the shoulder pain was assessed with Visual Analogue Scale(VAS),the motor function of upper limb was assessed with Fugl-Meyer Assessment(FMA),and their activities of daily living was assessed with modified Barthel Index(MBI). Results After treatment, the score of VAS significantly decreased in all groups (P<0.001), and was lower in groups A and B than in the control group(P<0.05),especially in group A(P<0.01);the scores of FMA and MBI signifi-cantly increased(P<0.001),and was higher in groups A and B than in the control group(P<0.05),especially in group A(P<0.05). Conclusion Kinesio taping combined with rehabilitation training for the treatment of stroke patients following hemiple-gia during spasm period can relieve the shoulder pain and improve the motor function of upper limb and activi-ties of daily living.The curative effect varies with different taping methods.
