Objective To design a post-structured technology-based clinical disease database system to solve the problems of the traditional disease database system in dependence on manual judgment,lack of auxiliary annotation and poor availability of electronic medical record data.Methods An entity recognition model was constructed with I2B2 standard and bi-directional long short-term memory(BiLSTM)model to form a medical record template library,and some relational templates were generated and complex medical entities were extracted to realize post-structuring of electronic medical records.Then a clinical disease database system was established based on the post-structured electronic medical record technology,which was composed of the modules for medical record structuring,structured assessment,data annotation,routine functions and system management.Results The system developed transformed the text of electronic medical records into structured language,contributed to data element extraction and intelligent structured service and enhanced the efficiency of clinical treatment and scientific research.Conclusion The system developed improves the data availability of clinical diseases,reduces the workload of user data processing,ensures the quality of data application and lays a foundation for assisted decision making during clinical treatment and scientific research.[Chinese Medical Equipment Journal,2024,45(4):20-26]

Purpose: Human breast milk (HBM) contains immune components that produced and delivered from the mother along with nutrients necessary for the baby. MicroRNA (miRNA) is a small noncoding RNA molecule, that is used as an ideal biomarker for diagnosis and prognosis of various diseases and are more abundant in HBM. We analyzed and compared the immune components and miRNAs of HBM. Methods: HBM were collected from 20 healthy breastfeeding mothers. We measured the amount of lactoferrin, lysozyme, and immunoglobulin A (IgA) and extracted the miRNAs from each breast milk samples. Next, the top 5 and bottom 5 expressed miRNAs were compared and analyzed based on the amounts of the 3 immune components. Results: The mean levels and ranges of lactoferrin, lysozyme, and IgA were 6.33 (2.24–14.77)×106 ng/mL, 9.90 (1.42–17.59)×107 pg/mL, and 6.64 (0.48–20.01)×105 ng/mL, respectively. The miRNAs concentration per 1 mL of skim milk was 40.54 (14.95–110.01) ng/μL. Comparing the bottom 5 and top 5 groups of each immune component, 19 miRNAs were significantly upregulated (6, 9, and 4 targeting lactoferrin, lysozyme, and IgA, respectively) and 21 were significantly downregulated (4, 9, and 8 targeting lactoferrin, lysozyme, and IgA, respectively). There were no miRNAs that were expressed significantly higher or lower in common to all 3 components.However, 2 and 3 miRNAs were commonly overexpressed and underexpressed, in the top 5 groups of lysozyme and IgA concentrations. Conclusion We identified the immune components and miRNAs in breast milk and found that each individual has different ingredients.

Tuberculous pericarditis is an extrapulmonary manifestation of tuberculosis that is most commonly associated with pericardial thickening, effusion, and calcification. We present a case of tuberculous pericarditis mimicking a malignant pericardial tumor in a 77-year-old male. CT revealed an irregular and nodular pericardial thickening. MRI revealed high signal intensity on T1-weighted fat-suppressed images and peripheral rim enhancement after gadolinium administration. MRI can be helpful in determining the differential diagnoses in cases of tuberculous pericarditis with nonspecific imaging findings.

Respiratory epithelial adenomatoid hamartoma (REAH) in the head and neck is a rare benign lesion containing glandular tissue covered with ciliated respiratory epithelium. In the head and neck, REAH of the nasal cavity, paranasal sinuses, and nasopharynx have been reported in literature. Due to rareness of REAH and insufficient knowledge of its imaging features, the diagnosis can be challenging when we encounter a non-specific cystic mass at an uncommon site in the head or neck. Here, we report the case of a pathologically confirmed REAH showing a cystic mass centered at the buccal space (retromaxillary fat pad) with CT and MRI findings.

Purpose: This study aimed to identify the influential factors of nursing home nurses that impact fall-prevention behaviors, thereby providing basic data to prepare effective measures that can increase nurses’ fall-prevention behaviors. Method: The participants were 136 nurses from 4 long-term care hospitals with more than 200 beds. The relevant data were collected from August 2 to August 31, 2021, and analyzed using independent t-test, one-way ANOVA, Pearson's Correlation Coefficient, and Multiple Regression. Result: The factors that had a significant impact on the fall prevention behaviors of participants included not only the perception of patient safety culture (β=.18, p=.039) and the age (β=.21, p=.046) but also the perceived seriousness (β=.22 p=.012), the perceived barrier (β=-.17, p=.044), and the motivation of action (β=.18, p=.028), which are the sub-domains of fall-prevention behavior health beliefs. The regression model was revealed to be significant, and the total explanatory power of the variables for fall-prevention behaviors was 44.5% (p<.001). Conclusion The influential factors on fall prevention behaviors of long-term care hospital nurses included perception of patient safety culture, fall-prevention behavior health beliefs, and age. This study has significance in that the results helped identify the influential factors on nursing home nurses’ fall-prevention behaviors, thereby providing fundamental data for effective fall prevention behaviors.

This study aims to identify factors influencing the caring behavior of the primary caregivers of older adults with dementia at home. Methods: A total of 125 primary family caregivers, responsible for the care of older adults with dementia across eight dementia care centers, participated in the study. Data were collected from August 1 through September 8, 2022, and analyzed using t-tests, analysis of variance, the Scheffé test, Pearson’s correlation, and multiple linear regression. Results: Participants had a mean age of 47.4±14.2 years, and the mean score for caring behavior was 3.89±0.40. Caring behavior was influenced by the availability of help (β=.22, p=.015), the educational level of older adults with dementia (middle school β=.32, p=.008; high school β=.24, p=.045), care experience evaluation (β=.21, p=.023), and family resilience (β=.22, p=.014). The explanatory power was 25.4% (F=6.27, p<.001, adjusted R²=.25). Conclusion: To enhance the caring behavior of primary caregivers for older adults with dementia at home, nursing interventions that positively impact dementia care experiences are necessary. Regular access to community resources for assistance is crucial. Additionally, programs should be developed to strengthen family resilience, enabling family members to support each other during challenging situations. Emphasizing education is essential to enhance awareness of care aspects among primary caregivers of older adults with dementia.
