1.Comparative Analysis of Three Massive Medical Image Storage Technologies.

Nowadays, the massive medical data have already influenced the information construction in medical institutes, so it is not enough to solely rely on traditional local storage system to solve the problems like the read/write speed, visualization, and economy brought about by the massive data. Furthermore, various medical cloud services have been developed at home and abroad that patients' medical data can be shared through all medical institutes on the cloud, which makes a higher demand on the transmission speed of the medical data. This article analyzes from multiple aspects like high availability and costs by performing a medical image transmission speed test on the three mainstream storage technologies to provide an optional storage system for future medical image big data in the access process. The experimental result shows that it can be found that in the process of accessing medical image big data, the access speed and performance of object storage system is better than those of the existing local storage systems. However, with comprehensive consideration, it is recommended that the distributed file storage system like HDFS be the first choice for the storage system of the medical images.

2.Design and Implementation of Medical Wireless Gateway Based on Several Communication Protocols.

We designed a multi-functional medical wireless gateway based on Zigbee Bluetooth and WiFi Technology to monitor the medical physiological status of subjects in real time. On the one hand, the gateway acts as the Zigbee coordinator, organizing the Zigbee network and controlling multiple medical digital terminals. On the other hand, the gateway implements a heterogeneous wireless network. It establishes the communication between the medical digital terminals and the host computers or hand-held devices. In addition, we developed a display interface based on the LabVIEW platform, implemented the display and storage of various physiological data on the host computer, and verified the availability of the medical wireless gateway.

3.Action Research on Development and Application of Internet of Things Services in Hospital.

OBJECTIVES: Services based on the Internet of Things (IoT) technologies have emerged in various business environments. To enhance health service quality and maximize benefits, this study applied an IoT technology based on NFC and iBeacon as an omni-channel service for patient care in hospitals. METHODS: Application of the IoT technology based on NFC and iBeacon was conducted in a general hospital during August 2015 through June 2016, and the development and evaluation results were aligned to an action research framework. The five phases in the action research included diagnosing, planning action, taking action, evaluating action, and specifying learning phases. RESULTS: During the first two phases, problems of functional operations in a hospital were diagnosed and eight service models were designed by using iBeacon and NFC to solve the problems. Service models were applied to the hospital by installing beacons, wearable beacons, beacon scanners, and NFC tags during the third phase. During the fourth and fifth phases, the roles and benefits of stakeholders participating in the service models were evaluated, and issues and knowledge of the whole application process were derived and summarized from technological, economic, social and legal perspectives, respectively. CONCLUSIONS: From an action research perspective, IoT-based healthcare services were developed and verified. IoT-based services enable the hospital to acquire lifelog data for precision medicine and ultimately be able to go one step closer to precision medical care. The derived service models could provide patients more enhanced healthcare services and improve the work efficiency and effectiveness of the hospital.

4.Fall Detection System for the Elderly Based on the Classification of Shimmer Sensor Prototype Data.

OBJECTIVES: Falling in the elderly is considered a major cause of death. In recent years, ambient and wireless sensor platforms have been extensively used in developed countries for the detection of falls in the elderly. However, we believe extra efforts are required to address this issue in developing countries, such as Pakistan, where most deaths due to falls are not even reported. Considering this, in this paper, we propose a fall detection system prototype that s based on the classification on real time shimmer sensor data. METHODS: We first developed a data set, ‘SMotion’ of certain postures that could lead to falls in the elderly by using a body area network of Shimmer sensors and categorized the items in this data set into age and weight groups. We developed a feature selection and classification system using three classifiers, namely, support vector machine (SVM), K-nearest neighbor (KNN), and neural network (NN). Finally, a prototype was fabricated to generate alerts to caregivers, health experts, or emergency services in case of fall. RESULTS: To evaluate the proposed system, SVM, KNN, and NN were used. The results of this study identified KNN as the most accurate classifier with maximum accuracy of 96% for age groups and 93% for weight groups. CONCLUSIONS: In this paper, a classification-based fall detection system is proposed. For this purpose, the SMotion data set was developed and categorized into two groups (age and weight groups). The proposed fall detection system for the elderly is implemented through a body area sensor network using third-generation sensors. The evaluation results demonstrate the reasonable performance of the proposed fall detection prototype system in the tested scenarios.

5.The Change of Lacrimal Gland Volume in Korean Patients with Thyroid-associated Ophthalmopathy.

PURPOSE: To describe the change of lacrimal gland volumes in Korean patients with thyroid-associated ophthalmopathy (TAO) via computed tomography (CT). METHODS: A retrospective review of CT images from 217 TAO patients and 135 control subjects was performed. The TAO patients were diagnosed between May 2005 and May 2014 and had a CT performed on initial presentation (330 orbital CT scans). These images were compared with 270 orbital CT scans from the control group, obtained between May 2013 and May 2014. An open source DICOM viewer was used to calculate the volume of the lacrimal gland. RESULTS: The mean volume of the lacrimal gland in TAO patients was 0.816 cm³ in the right orbit (standard deviation [SD], 0.048) and 0.811 cm3 in the left orbit (SD, 0.051), with no significant difference between right and left (p = 0.192). However, significant differences were observed between TAO patients and healthy individuals (p < 0.001). There was no significant difference between mean lacrimal gland volumes of males (0.812 cm³; SD, 0.037) and females (0.816 cm³; SD, 0.029) (p = 0.513). There was a negative correlation between gland volume and age in TAO patients (Pearson r = -0.479, p = 0.00). The subjective tearing (right: r = 0.244, p = 0.018; left: r = 0.226, p = 0.024), corneal superficial punctate keratopathy (right: r = 0.192, p = 0.040; left: r = 0.206, p = 0.036), and exophthalmometry (right: r = 0.182, p = 0.032; left: r = 0.180, p = 0.046) correlated with lacrimal gland volume. CONCLUSIONS: This study is the first to use CT images to calculate the lacrimal gland volume of Korean TAO patients. In TAO patients, the lacrimal gland volume was notably increased compared to control subjects. The lacrimal gland volume decreased with age, but there was no difference between gender and no difference between left and right. The lacrimal gland volume correlated with subjective tearing, corneal superficial punctate keratopathy and exophthalmometry.

6.Establishment of a Daily E-mail Learning Program Using Intranet and It's Initial Evaluation.

BACKGROUND/AIMS: We started a daily web-based E-mail learning program about gastrointestinal (GI) endoscopy. We surveyed the opinion of the E-mail recipients about the usefulness of the daily E-mail learning program. MATERIALS AND METHODS: Using the intranet system, PDF-file based education materials about interesting endoscopic cases were sent to 144 members of Samsung Medical Center every day. It included every resident of the Department of Medicine, and every fellow of the Division of Gastroenterology. They were invited to answer the questionnaires using E-mail. RESULTS: A total of 92 subjects replied the questionnaire (response rate: 64%). About half of the members (47%) answered that they read the education material in the morning. They answered that they read about more than 75% of the material (48%). It took less than 5 minutes to read a daily material (87%). However, only 13 percent answered that they have sent questions to the editor of the program via E-mail. The members wanted more information about GI pharmacologic treatment (54%). Also, they wanted more cases of benign and malignant upper GI diseases (54%). CONCLUSIONS: In our endoscopy unit, we have successfully established a daily E-mail learning program. It seems to be very useful for teaching and learning interesting cases during busy working hours.

7.A Case of Cervical Spine Fracture with Imitating Social Media Video.

With increasing users of SNS (Social Network Service), the video known as 'couple play' has been popular recently. People imitate the scene of couple play in the video. However, some of them failed to reproduce some actions. We report a case of cervical spine fracture with 'couple play'. A 14-year-old female presented to the emergency department complaining of posterior neck pain. She had a history of trauma with 'couple play' three days ago. She had no neurologic deficit. Cervical spine x-ray and computed tomography showed spinous process fracture of the seventh cervical spine (clay-shoveler fracture). This case demonstrates the need for heightened awareness concerning videos containing dangerous actions that may be harmful when imitated. The authors believe that an institutional strategy for notification of the risks is needed.

8.The Development of Information Centralization and Management Integration System for Monitors Based on Wireless Sensor Network.

Developed the information centralization and management integration system for monitors of different brands and models with wireless sensor network technologies such as wireless location and wireless communication, based on the existing wireless network. With adaptive implementation and low cost, the system which possesses the advantages of real-time, efficiency and elaboration is able to collect status and data of the monitors, locate the monitors, and provide services with web server, video server and locating server via local network. Using an intranet computer, the clinical and device management staffs can access the status and parameters of monitors. Applications of this system provide convenience and save human resource for clinical departments, as well as promote the efficiency, accuracy and elaboration for the device management. The successful achievement of this system provides solution for integrated and elaborated management of the mobile devices including ventilator and infusion pump.

9.Impact of an Antimicrobial Stewardship Program on Unnecessary Double Anaerobic Coverage Prescription.

BACKGROUND: Co-administration of two or more antimicrobials with anti-anaerobic activity is not recommended except in certain circumstances. We therefore conducted an intervention to reduce unnecessary double anaerobic coverage (DAC) prescription. MATERIALS AND METHODS: The intervention consisted of education using an institutional intranet and prospective audits and feedback provided through collaboration between a pharmacist and an infectious diseases physician in Seoul National University Bundang Hospital, a tertiary hospital in Seongnam, Republic of Korea, in 2013. The study period was 1 year which contained 6 months of pre-intervention period and 6 months of intervention period. To estimate the overall effect of the intervention, we compared the monthly number of patients receiving unnecessary DAC for more than 3 days and the proportion of patients receiving unnecessary DAC for more than 3 days among all patients receiving DAC. RESULTS: The average monthly number of patients receiving unnecessary DAC for more than 3 days after screening decreased by 73.9% in the intervention period from 26.8 to 7.0. Wilcoxon rank sum test revealed there was a significant statistical difference in the monthly number of patients receiving unnecessary DAC for more than 3 days (P = 0.005). The proportion of patients receiving unnecessary DAC for more than 3 days after screening among all patients identified as receiving necessary or unnecessary DAC also decreased by 67.8% in the intervention period from 42.3% to 13.6% (P < 0.001). CONCLUSION: The multidisciplinary antimicrobial stewardship program with combined methods reduced unnecessary DAC prescription successfully.

10.Wireless human body communication technology.

The Wireless Body Area Network (WBAN) is a key part of the wearable monitoring technologies, which has many communication technologies to choose from, like Bluetooth, ZigBee, Ultra Wideband, and Wireless Human Body Communication (WHBC). As for the WHBC developed in recent years, it is worthy to be further studied. The WHBC has a strong momentum of growth and a natural advantage in the formation of WBAN. In this paper, we first briefly describe the technical background of WHBC, then introduce theoretical model of human-channel communication and digital transmission machine based on human channel. And finally we analyze various of the interference of the WHBC and show the AFH (Adaptive Frequency Hopping) technology which can effectively deal with the interference.
