1.Clinical experience of V-shaped incision for excision of scalp subcutaneous tumors

		                        			Objective:To summarize the clinical experience of using V-shaped incision to excise scalp subcutaneous tumors.Methods:A retrospective analysis of the clinical data of patients with subcutaneous benign tumors admitted to the Department of Burns and Plastic Surgery of the Northern Theater General Hospital from August 2020 to December 2021 was conducted. The tumors were all above 3 cm in diameter and closely adhered to surrounding tissues, and all were removed using a V-shaped incision. During the operation, the scalp was incised along the design line, the V-shaped skin flap was lifted to affiliate the tumor removed. The incision was closed directly from the tip of "V" until the moderate tension attained. The scalp flap was spread smoothly on the wound surface, and the excess scalp tissue was removed to form a Y-shaped suture. The flap survival, complications, incision scar, hair growth around the incision and tumor recurrence were observed and followed up after operation.Results:A total of 20 cases were included, including 12 males and 8 females, with an average medical history of (3.6±1.9) years and a tumor diameter of (4.1±1.1) cm. There were 8 sebaceous cysts, 5 lipomas, and 7 epidermoid cysts. After operation, 18 flaps survived well, and 2 cases had epidermal necrosis at the tip of the flap, which healed after dressing change. During the follow-up period of 6-12 months, 3 patients experienced local scalp numbness, which improved within 3-6 months. In 20 cases, the scalp was flat, the incision scar was not hyperplastic, the scar was short visually, the surrounding hair grew normally, and no tumor recurrence happened.Conclusion:V-shaped incision is relatively easy to expose the tumor and can accurately remove excess skin. After operation, the scalp has a good flat appearance, so it is an ideal choice for large scalp subcutaneous tumors with close adhesion to the surrounding tissue.

3.Effects of modified proper digital artery island flap in repairing complex fingertip defects

		                        			Objective:To investigate the surgical method and clinical effects of the modified proper digital artery island flap in repairing complex fingertip defects.Methods:A retrospective observational study was conducted. From January 2017 to December 2021, 15 patients (15 fingers) with complex fingertip defects, involving the pulp, nail bed, and lateral wall of the nail, who met the inclusion criteria were admitted into General Hospital of Northern Theater Command, including 11 males and 4 females, aged from 18 to 55 years. The area of the post debridement wound was from 2.5 cm×2.0 cm to 3.5 cm×3.5 cm, and all the wounds were repaired by using modified proper digital artery island flap (including 3 parts: main flap, tongue-shaped flap, and triangular flap), of which the main flap was used to cover the finger pulp defect, the tongue-shaped flap was used to cover the nail bed and the nail lateral wall defect, and the triangular flap was inserted into the edge of the finger pulp wound to cover the vessel pedicle. The range of the flap ranged from 3.0 cm×2.0 cm to 4.5 cm×3.0 cm. The wound at the donor site was repaired with full-thickness skin graft of the groin, and the donor site of the skin graft was sutured directly. After operation, the survival of the flap and skin graft as well as and the appearance of the affected finger were observed. During the follow-up, the fingertip morphology of the affected finger was observed, two-point discrimination distance of the affected finger pulp was measured, and the patients' satisfaction with the efficacy (including very satisfied, satisfied, and dissatisfied) was asked, and the affected finger function was evaluated by the total active movement (TAM) system evaluation standard recommended by American Academy for Surgery of Hand.Results:After operation, the main flaps and skin grafts in 15 patients all survived; but the incision at the edge of tongue-shaped flap in one patient healed poorly, and one patient developed venous stasis at the distal end of the tongue-shaped flap; the triangular flap at the pedicle was slightly bloated in the early postoperative period and became smooth after 2 to 3 months. Overall, two patients developed subcutaneous hematoma in their flaps. All the complications were healed by appropriate dressing change, suture removal, or compression bandaging. After operation, the appearance of the flap was full and formed a prominent fingertip shape. During the follow-up of 6 months to 5 years, the fingertips of the affected fingers were prominent and full; the two-point discrimination distance of the affected finger pulp was (8.6±1.4) mm; 8 patients were very satisfied with the efficacy, 6 patients were satisfied, and one patient was dissatisfied; the functional assessment of the affected fingers were all excellent.Conclusions:The modified proper digital artery island flap can repair complex fingertip defects involving the pulp, nail bed, and lateral wall of the nail. 4.Management experience of a designated hospital for children with coronavirus disease 2019.

		                        			The global pandemic of coronavirus disease 2019 (COVID-19) has brought great challenges to the traditional medical model. During the outbreak of COVID-19 in Shanghai, China, from March to May, 2022, there was a significant increase in the number of pediatric cases due to high transmissibility, immune escape, and vaccine breakthrough capacity of Omicron variants. The designated hospitals for children with COVID-19 served as a connecting link between children's specialized hospitals and mobile cabin hospitals. From April 7 to June 2, 2022, a total of 871 children with COVID-19 were admitted to Renji Hospital, Shanghai Jiao Tong University School of Medicine (South Branch), a designated hospital for children with COVID-19. Among these patients, 568 (65.2%) were children under 3 years old, 870 (99.9%) were mild or moderate, and 1 was severe. This article reports the experience in the management of pediatric cases in this designated hospital, which included the following aspects: establishing an optimal case-admission process; strengthening multidisciplinary standardized diagnosis and treatment; optimizing the management, warning, and rescue system for severe COVID-19; implementing family-centered nursing care; formulating an individualized traditional Chinese medicine treatment regimen; optimizing the discharge process and strengthening bed turnover; implementing strict whole-process control to reduce the risk of nosocomial infection; constructing a structured medical record system and using information platforms to adapt to the work mode of large-volume cases; conducting scientific research and sharing the experience in diagnosis and treatment.

5.Effect of Toddalia asiatica Alcohol Extract on Apoptosis of Non-small Cell Lung Cancer A549 Cells Based on Autophagy Pathway

		                        			ObjectiveTo study the effects of Toddalia asiatica alcohol extract on autophagy and apoptosis of non-small cell lung cancer A549 cells, and to explore its possible mechanism. MethodA549 cells were cultured in vitro. Cell counting kit-8 (CCK-8) was used to detect the proliferation of A549 cells, and cell survival rate was calculated to screen the drug concentration. The apoptosis in each dose group and that after the use of 3-methyladenine (3-MA), an autophagy inhibitor, were detected by flow cytometry combined with Annexin V-FITC/PI double staining. Western blot was used to detect the expression levels of apoptosis-related proteins such as B cell lymphocytoma-2(Bcl-2), Bcl-2-associated X protein(Bax), microtubule-associated protein 1 light chain 3 (LC3), cleaved cysteinyl aspartate-specific protease-3 (cleaved Caspase-3), activated poly (Adenosine diphosphate) ribonucleotide polymerase (cleaved PARP1), PARP1, activated death activator (t-Bid), Bid, and ubiquitin-binding protein p62 in each group and those after the use of 3-MA. ResultCompared with the conditions in the control group, the cell survival rate in 0.25 g·L-1 group (P<0.05), and 0.5, 1, 2, 4 g·L-1 groups (P<0.01) was decreased after 24 h intervention. Additionally, the cell survival rate was reduced in a concentration-dependent manner at 48 h and it was less than 10% at 4 g·L-1 (P<0.01). Compared with the conditions in the control group, the total apoptosis rate in 0.5 g·L-1 group was increased (P<0.05), and the apoptosis rate in 1 and 2 g·L-1 groups was also increased (P<0.01). Compared with the 2 g·L-1 group and 3-MA group, the 3-MA combined with T. asiatica alcohol extract had significantly decreased apoptosis rate (P<0.01). Compared with the conditions in the control group, elevated expression of pro-apoptotic proteins cleaved PARP1, Bax and t-Bid in 1 and 2 g·L-1 groups (P<0.05, P<0.01), and reduced expression of Bid in the 2 g·L-1 group (P<0.01) were found. Compared with the conditions in the control group, the expression of anti-apoptotic protein Bcl-2 (P<0.05, P<0.01) and the level of p62 (P<0.01) were down-regulated in 0.5, 1, 2 g·L-1 groups, while the level of LC3 Ⅱ protein was up-regulated (P<0.01), with certain concentration dependence. 7. Effect of NLRP3 mediated pyroptosis in myocardial cells undergoing hypoxia/deoxygenation injury

		                        			To investigate the effect of NACHT-LRR-PYD- containing proteins 3 (NLRP3) mediated pyroptosis in myocardial cells undergoing hypoxia/deoxygenation (H/R) injury.

		                        			In order to observe whether H/R-treatment could cause pyroptosis, H9c2 cells were divided into 2 groups randomly using the lottery method: control group(without H/R-treatment) and H/R group (in which the H9c2 cells were underwent H/R-treatment). In order to clarify the role of pyroptosis in H/R-injury, H9c2 cells were divided into 4 groups randomly using the lottery method: control group(in which the H9c2 cells were cultivated with normal medium); YVAD group(in which the H9c2 cells were pretreated with z-Val-Ala-Asp(Ome)-fluoromethylketone (Z-YVAD-FMK) 20 μm for 4 hours, then replaced with normal medium); H/R group(H9c2 cells underwent H/R-treatment); YVAD+H/R group (in which the H9c2 cells were pretreated with 20 μm Z-YVAD-FMK for 4 hours before H/R-treatment). To determine whether H/R-induced cell pyroptosis is associated with NLRP3, H9c2 cells were divided into 4 groups randomly using the lottery method: control group (in which cells were transfected with a control nonspecific siRNA); si-NLRP3 group (in which cells were transfected with NLRP3-targeting siRNA); H/R group(in which cells were transfected with a control nonspecific siRNA before H/R-treatment); si-NLRP3+H/R group(in which the H9c2 cells were transfected with NLRP3-targeting siRNA before H/R-treatment). Pore formation on cell membrane was detected by propidium iodide (PI) staining. Cell viability was detected by CCK8 reagent. 8.Agricultural Risk Factors Influence Microbial Ecology in Honghu Lake.

		                        			Agricultural activities, including stock-farming, planting industry, and fish aquaculture, can affect the physicochemical and biological characters of freshwater lakes. However, the effects of pollution producing by agricultural activities on microbial ecosystem of lakes remain unclear. Hence, in this work, we selected Honghu Lake as a typical lake that is influenced by agriculture activities. We collected water and sediment samples from 18 sites, which span a wide range of areas from impacted and less-impacted areas. We performed a geospatial analysis on the composition of microbial communities associated with physicochemical properties and antibiotic pollution of samples. The co-occurrence networks of water and sediment were also built and analyzed. Our results showed that the microbial communities of impacted and less-impacted samples of water were largely driven by the concentrations of TN, TP, NO-N, and NO-N, while those of sediment were affected by the concentrations of Sed-OM and Sed-TN. Antibiotics have also played important roles in shaping these microbial communities: the concentrations of oxytetracycline and tetracycline clearly reflected the variance in taxonomic diversity and predicted functional diversity between impacted and less-impacted sites in water and sediment samples, respectively. Furthermore, for samples from both water and sediment, large differences of network topology structures between impacted and less-impacted were also observed. Our results provide compelling evidence that the microbial community can be used as a sentinel of eutrophication and antibiotics pollution risk associated with agricultural activity; and that proper monitoring of this environment is vital to maintain a sustainable environment in Honghu Lake.

9.The Effect of Different Weeks of High Intensity Interval Training and Moderate-intensity Aerobic Exercise on Skeletal Muscle's AMPK and PGC-1α of Rats

		                        			Objective To explore the effects of different weeks of high intensity interval training (HIIT) and moderate intensity aerobic exercise on maximal oxygen uptake (VO2max) and the expression of the skeletal muscle oxidative capacity related factor AMP-activated protein kinase (AMPK) and peroxisome proliferators γactivated receptor coativator-1-α (PGC-1α),so as to provide the basis for the choice of effective load intensity.Methods One hundred and twenty 6-week-old SPF male Wistar rats were equally randomized into 3 groups according to their body weight:a sedentary control group (not receiving any exercise),a moderate-intensity exercise group (undergoing 50-min continuous running at an intensity of 60%-70% VO2max) and a HIIT group (conducting 3-min running at 90% VO2max interspersed with 3-min recovery periods at 50% VO2max and repeating that process 6 times,with a 7-min warm-up and cool-down period at 70% VO2max).All rats except those in the control group exercised five days a week and 50min per day.All rats were measured their weight at 8:00-9:00am every Sunday.Soleus was taken from ten randomly chosen rats of each group at 2nd,4th,6th and 10th week after the onset of the intervention.The VO2max test was done before taking muscles.Western blotting was used to detect the protein expression of AMPK and PGC-1α.Results (1) Both the exercise duration and mode had a significant impact on rats' body weight,and they had interaction with each other (P<0.01).(2) The average PGC-1α expression in the skeletal muscle of 10-week HIIT group were significantly higher than those in the control group (P<0.05),and the average VO2max at the same time was also significantly higher than the other two groups.(3) In the HIIT group,the average VO2max at the 10th week was significantly higher than that at the 2nd,4th and 6th week (P< 0.05).The average protein expression of AMPK in the skeletal muscle at the 4th week in the moderate-intensity exercise group was significantly higher than that at the 2nd and 6th week of the same group,and that at the 2nd week of the control group (P<0.05).The average protein expression of PGC-1α in the HIIT group from the 2nd to the 10th week was significantly higher than the control group at the 2nd week (P<0.05).(4)The protein expression of PGC-1α was significantly correlated with temporal variation of VO2max in the HIIT group (P<0.05).Conclusion Ten-week HIIT can effectively promote the expression of AMPK and PGC-1α in skeletal muscles and the maximal oxygen uptake.It plays a more quick and effective role in improving oxidant capacity and cardiorespiratory endurance of skeletal muscles than traditional moderate-intensity exercises.
