1.Analysis of clinical maintenance effect of 76 organ donors

		                        			Objective By analyzing the means of donor maintenance from the perspective of intensive care medicine,the difference of clinical indicators before and after donor treatment is obtained,summarizing the treatment experience and effect of clinical maintenance of donor donors,and providing more clinical evidence for the maintenance and treatment of organ donation donors.Methods A retrospective analysis was performed on 76 organ donors collected from the Department of Intensive Care Medicine,Army Medical Center,Chinese People's Liberation Army from January 2014 to December 2021,collected donor maintenance treatment,including mechanical ventilation,continuous renal replacement therapy(CRRT)treatment,enteral nutrition and parenteral nutrition,enteral and parenteral nutrition,plasma exchange,at the same time,the paired t test was used to compare the organ function before and after treatment.Results The lung,liver and kidney of 76 donors were evaluated before and after treatment to determine the number of eligible donors.The difference between the number of eligible donors of kidney and liver before and after treatment was statistically significant(P<0.05).There was no significant difference in lung evaluation(P>0.05).Conclusion Timely and effective organ support therapy for organ transplantation donors is beneficial to the recovery of organ function and improve the utilization rate of organ.
		                        		
		                        		
		                        		
		                        	
2.In vitro oral simulation evaluation of palatability and chewability of chewable tablets

		                        			OBJECTIVE To evaluate the palatability and chewability of chewable tablets, and provide reference for the quality evaluation of various types of chewable tablets. METHODS Using self-made Glucosamine hydrochloride chewable tablets as the model drug, the quality test was conducted. The in vitro simulation system for chewable tablets was established by using a texture analyzer and rheometer, and an oral simulation experiment was conducted on chewable tablets. The texture analyzer was used to measure the force required for chewing and simulate the static disintegration process of chewable tablets; the rheometer was adopted to measure the viscoelasticity, thixotropy, and deformability of chewable tablets during the chewing process. RESULTS The disintegration time limit, principal component content, and dissolution of self-made Glucosamine hydrochloride chewable tablets all met the limit requirements. The in vitro simulation results of the texture analyzer showed that self-made chewable tablets were easy to chew in both axial and radial directions, and the force required for chewing was within the range of the chewing force of the teeth; chewable tablets could disintegrate at an appropriate time without being chewed and only taken in the oral cavity. The in vitro simulation results of the rheometer showed that the chewable tablets in the oral cavity exhibited a behavior of elasticity as the main factor and viscosity as the secondary factor through the continuous stirring of the tongue, and the viscosity of the chewable tablets gradually decreased with tongue stirring or tooth chewing; when chewing with teeth, the internal force of the chewing tablets decreased, causing plastic deformation and crushing. After being crushed, the shape couldn’t be restored, making it easy to chew and swallow. CONCLUSIONS The combination of texture analyzer and rheometer can be used to simulate the oral chewing process and evaluate the palatability and chewability of self-made Glucosamine hydrochloride chewable tablets. 3.Application value of papaverine combined with staged rehabilitation in patients after free flap transplantation for hand trauma

		                        			Objective To explore the application value of popaverine combined with stage rehabilitation for patients after free flap grafting for hand trauma.Methods Data of postoperative patients who underwent hand trauma from January 2021 to January 2022 in The 903rd Hospital of the Joint Logistics Support Force of the People's Liberation Army of Chinawere retrospectively collected,and they were divided into the combined rehabilitation group(poppadine combined with stage rehabilitation care)and the stage rehabilitation group(only stage rehabilitation care was applied)according to the treatment methods.Visual analog scale(VAS)scores,rehabilitation exercise adherence scores,postoperative rates of excellent cutaneous sensory function of the hand,total active mobility(TAM)of the fingers,Jamar grip strength,upper extremity functional assessment(DASH)scale scores,and the incidence of vascular crises were compared between the two groups.Results A total of 120 cases were included in the study,with 57 cases in the combined rehabilitation group and 63 cases in the stage rehabilitation group.There was no statistically significant difference in VAS scores,Jamar grip strength and DASH scores between the two groups before surgery(P>0.05);at 14 days and 3 months of intervention,VAS scores of the two groups decreased compared with the pre-intervention period(P<0.05),and VAS scores of the patients in the combined rehabilitation group were significantly lower than those in the stage rehabilitation group(P<0.05).After 3 months of intervention,the rehabilitation exercise adherence score,the rate of excellent hand skin sensory function,and finger TAM of the patients in the combined rehabilitation group were higher than those in the stage rehabilitation group(P<0.05);Jamar grip strength were elevated in both groups compared with the pre-intervention period,but the combined rehabilitation group were higher than those in the stage rehabilitation group(P<0.05);DSAH scores were lower in both groups compared to pre-intervention,and were lower in the combined rehabilitation group than in the stage rehabilitation group;and during the 3 months of intervention,the incidence of vascular crisis was significantly lower in patients in the combined rehabilitation group than in the stage rehabilitation group(P<0.05).Conclusion Opium poppy alkaloids combined with stage rehabilitation can effectively reduce patients'postoperative pain,enhance patients'adherence to rehabilitation exercises,effectively improve postoperative hand function and upper limb motor function,and reduce the incidence of vascular crisis.
		                        		
		                        		
		                        		
		                        	
4.Characteristics and outcomes of fetuses with narrow aorta on prenatal ultrasound

		                        			Objective To observe the characteristics and outcomes of fetuses with narrow aorta on prenatal ultrasound.Methods Data of 29 fetuses with narrow aorta on prenatal ultrasound were retrospectively analyzed.The prenatal ultrasound and echocardiographic parameters of newborns were comparatively observed.Results All 29 fetuses with narrow aorta on prenatal ultrasound had thinner aortic isthmus,25 with aortic isthmus diameter larger than,1 equal to and 3 smaller than that of the left subclavian artery.Meanwhile,thinner aortic valve annulus was found in 28 fetuses,thinner ascending aorta in 26 and widened pulmonary valve annulus in 27 and abnormal proportion of ventricular chambers in 10 fetuses(1 with enlarged right ventricle and 9 with smaller left ventricle),and 24 with malformations.Postnatal follow-up detected aortic stenosis(AS)or coarctation of aorta(CO A)in 4 cases and narrow aorta without AS nor CO A in 6 cases,while 19 newborns were found with normal aortic diameter,including 2 cases with increased aortic blood flow velocity.Conclusion Fetuses with narrow aorta on prenatal ultrasound often complicated with thinner aortic isthmus,aortic valve annulus and ascending aorta as well as widened pulmonary valve annulus,sometimes with abnormal proportion of ventricular chambers.Normal aorta diameter could be observed in some cases after birth.
		                        		
		                        		
		                        		
		                        	
5.Analysis of Whole Exome Sequencing Result in Adult Genetic Diseases

		                        			Objective:To investigate the effectiveness and feasibility of whole exome sequencing (WES), as a molecular diagnosis technique, for adult patients with genetic diseases.Methods:The present retrospective analysis included 445 adult patients (ages 18-80 years) with suspected genetic diseases who underwent whole exome sequencing (WES) from August 2021 to December 2022. The pathogenicity classification of each variant was assessed in accordance with the recommendations developed by the American Society of Medical Genetics and Genomics.Results:The overall positive rate of WES among adult patients with suspected genetic diseases was 28.08% (125/445). The highest positive rate was observed in the age group of 41-50 years (34.33%, 23/67). Among the diagnosed genetic diseases, those affecting the cardiovascular system (63.16%, 84/133), nervous system (18.05%, 24/133), and endocrine system (13.53%, 18/133) ranked as the top three. The most common genetic diseases identified through WES in adult patients were hypertrophic cardiomyopathy (18.80%, 25/133), dilated cardiomyopathy (16.54%, 22/133), Marfan syndrome (15.04%, 20/133), epilepsy (9.02%, 12/133), and familial hypercholesterolemia (4.51%, 6/133). The main causative genes identified included FBN1 (14.29%, 19/133), MYBPC3 (9.02%, 12/133), MYH7 (9.02%, 12/133), LDLR (3.76%, 5/133), TTN (3.76%, 5/133), and TNNI3 (3.01%, 4/133).Conclusion:Applying the WES technique in clinical practice can improve the diagnostic rate of adult genetic diseases, especially in adult patients with suspected genetic conditions involving the cardiovascular system, nervous system, and endocrine system.
		                        		
		                        		
		                        		
		                        	
6.A consensus on the management of allergy in kindergartens and primary schools

		                        			Allergic diseases can occur in all systems of the body, covering the whole life cycle, from children to adults and to old age, can be lifelong onset and even fatal in severe cases. Children account for the largest proportion of the victims of allergic disease, Children s allergies start from scratch, ranging from mild to severe, from less to more, from single to multiple systems and systemic performance, so the prevention and treatment of allergic diseases in children is of great importance, which can not only prevent high risk allergic conditions from developing into allergic diseases, but also further block the process of allergy. At present, there is no consensus on the management system of allergic children in kindergartens and primary schools. The "Consensus on Allergy Management and Prevention in Kindergartens and Primary Schools", which includes the organizational structure, system construction and management of allergic children, provides evidence informed recommendations for the long term comprehensive management of allergic children in kindergartens and primary schools, and provides a basis for the establishment of the prevention system for allergic children.
		                        		
		                        		
		                        		
		                        	
7.Wendantang Treats Inflammation in Obesity (Syndrome of Phlegm-dampness) by Regulating PI3K/Akt/mTOR Pathway-mediated Adipocyte Autophagy

		                        			ObjectiveTo observe the effects of Wendantang on the expression of inflammatory cytokines, autophagy markers, and key molecules of phosphatidylinositol-3-kinase/protein kinase B/mammalian target of rapamycin (PI3K/Akt/mTOR) signaling pathway in the adipocytes of the rat model of obesity (syndrome of phlegm-dampness) and to explore the material basis of inflammation in obesity (syndrome of phlegm-dampness) and the underlying mechanism of Wendantang intervention. MethodA total of 126 SD rats were randomized into 2 groups: 16 rats in the blank group and 110 rats in the modeling group. The blank group was fed with a basic diet while the modeling group with a high-fat diet to establish the animal model of obesity (syndrome of phlegm-dampness) for 8 weeks. After successful modeling, 48 obese rats were selected according to their body mass and randomized into a model control group, an orlistat (ORLI, 32.40 mg·kg-1) group, a rapamycin (RAPA, 2 mg·kg-1) group, and low-, medium-, and high-dose (4.45, 8.90, 17.80 g·kg-1, respectively) Wendantang groups, with 8 rats in each group. In addition, 8 rats were randomly selected from the blank group to be set as the normal control group. The corresponding agents in each group were administrated by gavage and the model and control groups were administrated with equal amounts of distilled water once daily for 6 weeks. The body mass, Lee's index, body fat ratio, and obesity rate were measured or calculated. The expression of UNC51-like kinase-1 (ULK1), Beclin1, human autophagy-related protein 5 (Atg5), p62, and microtubule-associated protein 1 light chain 3 (LC3) Ⅰ/Ⅱ (markers of autophagy in adipocytes) was detected by the immunohistochemical two-step method. Enzyme-linked immunosorbent assay (ELISA) was employed to determine the expression of tumor necrosis factor (TNF)-α, interleukin-6 (IL-6), IL-1β, monocyte chemotactic protein-1 (MCP-1), IL-4, IL-10, IL-13, and transforming growth factor (TGF)-β in adipocytes. Western blot was employed to measure the protein levels of classⅠ-PI3K, phosphatidylinositol triphosphate (PIP3), Akt, mTORC1, ULK1, TSC1, and TSC2 in adipocytes. ResultCompared with the blank group, the modeling group showed increased body mass and Lee's index (P<0.01), the obesity rate >20%, and phlegm-dampness syndrome manifestations such as physical obesity, decreased mobility, decreased appetite, lusterless and tight fur, loose stools, decreased responsiveness to the outside world, and decreased water intake. Compared with the normal control group, the model control group showed increased body mass, Lee's index, body fat ratio, adipocyte autophagy marker expression, pro- and anti-inflammatory cytokine levels (P<0.05, P<0.01), down-regulated protein levels of classⅠ-PI3K, PIP3, Akt, mTORC1, TSC1, and TSC2 (P<0.01), and up-regulated protein level of ULK1 (P<0.01). The intervention groups showed lower body mass, body fat ratio, adipocyte autophagy marker protein expression, and protein levels of TNF-α, IL-6, IL-1β, MCP-1, IL-4, and IL-13 than the model control group (P<0.05, P<0.01). Moreover, the RAPA and Wendantang (medium and high dose) groups showed lowered levels of IL-10 and TGF-β (P<0.01), and the ORLI group showed down-regulated expression of TGF-β (P<0.01). The expression of key molecules of the signaling pathway was up-regulated (P<0.05, P<0.01) while that of ULK1 was down-regulated (P<0.01) in all the intervention groups. Compared with the RAPA group, the Wendantang groups showed up-regulated expression of all autophagy marker proteins in adipocytes (P<0.01). In addition, the low-dose Wendantang group showed elevated levels of inflammatory cytokines (except TNF-α) (P<0.05, P<0.01) and down-regulated expression of all key molecules of the signaling pathway (P<0.05, P<0.01). The levels of inflammatory cytokines (except IL-16, MCP-1, and IL-10) were elevated in the medium-dose Wendantang group (P<0.05, P<0.01). The expression of key molecules except PI3K of the signaling pathway was down-regulated in the medium- and high-dose Wendantang groups (P<0.05, P<0.01). Compared with the ORLI group, low- and medium-dose Wendantang groups showed up-regulated expression of autophagy markers in adipocytes (P<0.01), and the low-dose group showed elevated levels of inflammatory cytokines (IL-6, IL-4, and TGF-β) (P<0.01) and down-regulated expression of all key molecules of the signaling pathway (P<0.01). The medium-dose Wendantang group showed up-regulated expression of IL-4 (P<0.01) and down-regulated expression of key molecules except PI3K of the signaling pathway (P<0.05, P<0.01). The high-dose Wendantang group showed increased body mass, up-regulated expression levels of autophagy markers (ULK1, LC3 Ⅰ/Ⅱ) (P<0.05, P<0.01), down-regulated expression of PIP3, mTORC1, and TSC1 (P<0.05, P<0.01), and lowered levels of Beclin1, Atg5, TNF-α, and IL-13 (P<0.05, P<0.01). ConclusionThe inflammation in obesity (syndrome of phlegm-dampness) is closely associated with the PI3K/Akt/mTOR pathway-mediated adipocyte autophagy. 8.A family of Niemann-Pick disease type C caused by compound heterozygous mutations in NPC1 gene

		                        			Objective:To report the clinical and genetic characteristics of a family with Niemann-Pick disease type C caused by novel compound heterozygous mutations in the NPC1 gene to improve the clinicians′ recognition of the disease. Methods:Two patients from the family with non-consanguineous marriages admitted to the Department of Neurology of the First Affiliated Hospital of Kunming Medical University in 2020 were examined in detail. Peripheral blood DNA was extracted, and whole exome sequencing was performed on the patients, combined with Sanger sequencing for verification. The mutation and protein function predictor softwares were applied to analyze the mutation sites.Results:The inheritance was autosomal recessive in this family. The onset age of the proband was 9 years, and the main clinical manifestations were dysarthria, dysphagia, cognitive impairment, ataxia, bilateral pyramidal tract impairment, vertical supranuclear gaze palsy and splenomegaly. The clinical phenotype of the proband′s younger brother was similar to that of the proband, but it was more severe than that of the proband. The younger brother of the proband had an earlier age of onset and severe psychomotor retardation. Whole exome sequencing showed that both brothers carried 2 rare variants of NPC1 gene:1 pathogenic, stop gain at c.352_353del, p.Gln119ValfsTer8, and a missense change, c.593A>G, p.Asn198Ser, of suspected pathogenic. Sanger sequencing confirmed that compound heterozygous mutations were derived from the proband′s parents. According to the American College of Medical Genetics and Genomics guidelines, the above variants were rated as pathogenic and suspected pathogenic, respectively. And the c.593A>G, p.Asn198Ser mutation found in this family was a novel one which had not been reported yet. The proband had delayed diagnosis for 7 years from the onset of symptoms. After taking megastat for 1 year, the symptoms of dysphagia, ataxia and vertical eye movement disorder were significantly improved. Conclusions:The clinical phenotype of the pedigree was consistent with the clinical phenotype of Niemann-Pick disease type C. Compound heterozygous mutations of NPC1 gene (c.352_353del; c.593A>G) were found to be the genetic cause of the family.
		                        		
		                        		
		                        		
		                        	
9.Risk factors associated with low-level viremia in human immunodeficiency virus/acquired immunodeficiency syndrome patients after anti-retroviral therapy

		                        			Objective:To investigate the risk factors of low-level viremia (LLV) among human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) patients after combined anti-retroviral therapy (ART), and to provide evidence for reducing the risk of LLV.Methods:It was a cross-sectional observation study that enrolled HIV/AIDS patients with LLV (plasma HIV-1 RNA was 50 to 1 000 copies/mL) receiving ART over one year (LLV group) from January 2019 to December 2020 in Guangzhou Eighth People′s Hospital, Guangzhou Medical University. Contemporaneous patients with ART over one year and successful viral suppression (plasma HIV-1 RNA<50 copies/mL) were randomly selected as the control group (suppression group) with a ratio of 1∶2.5, and the risk factors for LLV were analyzed by unconditional logistic regression.Results:A total of 128 and 297 patients were enrolled in LLV group and the suppression group, respectively.ART durations were 3.62(1.83, 4.89) years and 4.91(2.90, 5.88) years, respectively. Multivariate logistic regression analysis showed that the risk factors associated with LLV included the age of initial ART treatment above 50 years old (odds ratio ( OR)=1.82, 95% confidence interval ( CI) 1.01 to 3.26, P=0.046), the baseline HIV-1 RNA over 1×10 5 copies/mL ( OR=2.18, 95% CI 1.30 to 3.68, P=0.003), using the simplified initial ART regimen ( OR=1.82, 95% CI 1.02 to 3.26, P=0.044), missing medication more than three times per year ( OR=2.49, 95% CI 1.55 to 4.01, P<0.001) and changing regimen during ART ( OR=1.90, 95% CI 1.14 to 3.14, P=0.013), while the duration of ART longer than five years could reduce the risk of LLV ( OR=0.37, 95% CI 0.22 to 0.64, P<0.001). In patients with simplified initial ART regimen, the baseline CD4 + T lymphocyte count of whom with LLV was lower than that of whom with viral suppression, and the difference was statistically significant (94.00 (24.00, 281.00)/μL vs 375.00 (310.00, 435.00)/μL, Z=-2.60, P<0.001). Conclusions:The occurrence of LLV is related to the age of initial ART treatment, the baseline HIV-1 RNA, the initial ART regimen, the medication adherence and the change of ART regimen during ART. Strategies may be beneficial to reducing the risk of LLV for HIV/AIDS patients, such as initiating ART as soon as possible, using simplified regimen as initial regimen with caution in patients with low baseline CD4 + T lymphocyte counts, strengthening compliance education, avoiding unnecessary ART regimen changes.
		                        		
		                        		
		                        		
		                        	
10.Intervention Effect of Ruyi Zhenbao Pills on Mice with Central Pain After Thalamic Stroke

		                        			ObjectiveTo observe the intervention effect of Ruyi Zhenbao pills (RYZBP) on central pain after thalamic stroke in mice and explore the underlying mechanism. MethodThe central post-stroke pain syndrome (CPSP) model was induced by stereotactic injection of type Ⅳ collagenase into the hypothalamus in mice. The mice were divided into a sham group, a model group, low-, medium-, and high-dose RYZBP groups (0.65, 1.3, 2.6 g·kg-1), and a pregabalin group (0.075 g·kg-1). Seven days after modeling, the mice in the groups with drug intervention were administered with corresponding drugs by gavage according to the body mass, once per day for 25 days, while those in the sham group and the model group received an equal volume of normal saline. During this period, mechanical pain and cold pain were detected at different time points, and the apoptotic state of brain tissue cells was detected by in situ terminal deoxynucleotidyl transferase-mediated dUTP nick end-labeling (TUNEL). The 36 classical broad-spectrum inflammatory factors were quantitatively analyzed by liquid-phase chip technology, and differential molecules were screened out and verified by Western blot and enzyme-linked immunosorbent assay (ELISA). ResultCompared with sham operation group, mechanical pain threshold and cold sensitive pain threshold in model group were significantly changed (P<0.01). TUNEL results showed that apoptosis of brain cells was obvious. Western blot and ELISA results showed that the expressions of interleukin-1α (IL-1α) and chemokine ligand 5 (CCL5) increased in hypothalamus tissue and serum, while the expressions of Ang-2, granulocyte-colony-stimulating factor (G-CSF) and IL-4 decreased significantly (P<0.01). Compared with model group, RYZBW dose groups significantly increased mechanical pain threshold, decreased cold sensitivity pain threshold, decreased hypothalamus cell apoptosis ratio (P<0.01), decreased the expression of IL-1α and CCL5 in hypothalamus tissue and serum, while the expression of ANG-2, G-CSF and IL-4 were significantly increased (P<0.05). ConclusionRYZBP can relieve hyperalgesia in CPSP mice, and its mechanism is related to the regulation of the expression of pro-/anti-inflammatory factors IL-1α, CCL5, IL-4, G-CSF, and Ang-2.
