1.Changes in serum levels of CCL19,sB7-H3,and sPD-1 in patients with recurrent oral ulcers and their predictive value for recurrence

Objective To investigate the changes in serum levels of chemokine ligand 19(CCL19),soluble co stimulatory molecule B7-H3(sB7-H3),and soluble programmed death protein 1(sPD-1)in patients with recurrent oral ulcer(ROU)and their predictive value for recurrence.Methods A total of 320 patients with ROU accepted by our hospital from April 2021 to April 2022 were collected as the study subjects,namely the ROU group.During the same period,320 healthy volunteers without oral diseases who underwent physical ex-amination in our hospital were regarded as the reference group.Enzyme linked immunosorbent assay(ELISA)was applied to detect levels of CCL19,sB7-H3,and sPD-1 in serum.Pearson method was applied to analyze the correlation between the expression levels of CCL19,sB7-H3,and sPD-1 in the serum of ROU patients and the levels of interleukin-6(IL-6),interleukin-10(IL-10),and tumor necrosis factor-α(TNF-α).ROC curve was applied to analyze the predictive value of serum levels of CCL19,sB7-H3,and sPD-1 in patients with ROU for recurrence.Results Compared with the reference group,the serum levels of CCL19 and sPD-1 in the ROU group were obviously increased,while the level of sB7-H3 was obviously reduced(P<0.05).The serum lev-els of IL-6,IL-10,and TNF-α in the ROU group were obviously higher than those in the reference group(P<0.05).Correlation analysis showed that the levels of CCL19 and sPD-1 in the serum of ROU patients were positively correlated with the levels of IL-6,IL-10,and TNF-α,while the level of sB7-H3 was negatively corre-lated with the levels of IL-6,IL-10,and TNF-α(P<0.05).Compared with the non recurrent group,the serum levels of CCL19 and sPD-1 in the recurrent group were obviously increased,while the level of sB7-H3 was ob-viously reduced(P<0.05).The AUC of combined detection of CCL19,sB7-H3,and sPD-1 in serum for pre-dicting the recurrence of ROU patients was higher than the AUC of single detection of three factors[ZCCL19-the combination of the three-3.-294,P=0.001;ZsB7-H3-the combination of the three=4.348,P<0.001;ZsPD-1-the combination of the three 4.990,P<0.001].Conclusion CCL19 and sPD-1 are highly expressed in the serum of ROU patients,while sB7-H3 is lowly expressed.These three factors have certain value in predicting the recurrence of ROU.
2.Correlation analysis of polyclonal plasma cell proportion in the bone marrow with clinical characteristics of patients with newly diagnosed multiple myeloma

Objective:To explore the correlation of bone marrow polychonal plasma cell proportion (pPC% ) and clinical features in newly diagnosed multiple myeloma (NDMM) patients.Methods:A retrospective analysis of 317 patients with NDMM admitted to Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology from January 2018 to January 2023 was performed. The results of the pPC% in all patients were clear. The relationship between the pPC% and clinical characteristics was analyzed.Results:A total of 317 patients were included, comprising 180 males and 137 females. The median age at diagnosis was 61 (26-91) years, and 55.8% were 60 years or older. The pPC% in the bone marrow of patients with NDMM was different in the DS, International Staging System (ISS), and revised ISS (R-ISS) stages ( P=0.002, 0.010, and 0.049, respectively), whereas no statistical difference in pPC% was observed among patients with different FISH risk stratigrams ( P=0.971). The correlation coefficient between pPC% and hemoglobin (HGB) at the first diagnosis in patients was 0.211 ( P<0.01). The correlation coefficients with serum calcium, serum creatinine, M protein level, and β 2-microglobulin were -0.141, -0.120, -0.181, and -0.207, respectively, and the results of the significance test were P=0.012, 0.033, 0.004, and 0.002, respectively, indicating a negative correlation. Compared with the patients with a pPC% of ≥2.5%, the group of patients with a pPC% of <2.5% had significantly higher levels of light chain, serum calcium, serum creatinine, M protein, and β 2-microglobulin at the initial diagnosis ( P<0.05) ; lower HGB level ( P<0.001) ; and a higher proportion of patients in ISS stage Ⅲ ( P=0.034) . Conclusion:In this study, the pPC% in patients with NDMM was associated with clinical features of good prognosis, including higher HGB, lower serum calcium, serum creatinine, M protein quantity, β 2-microglobulin, light chain involvement, lower proportion of advanced disease (DS stage and ISS stage Ⅲ), and clinical features showing lower tumor burden.
3.Role of endogenous heme oxygenase-1/carbon monoxide signaling pathway in endoplasmic reticulum stress during endotoxin-induced acute lung injury in rats

Objective To evaluate the role of endogenous heme oxygenase-1/carbon monoxide ( HO-1/CO) signaling pathway in endoplasmic reticulum stress during endotoxin-induced acute lung injury ( ALI) in rats. Methods Forty healthy clean-grade male Sprague-Dawley rats, aged 8 weeks, weighing 190-210 g, were divided into 4 groups ( n=10 each) using a random number table method: control group (group C), ALI group, ALI plus ZnPP-IX group (group AZ), and ALI plus vehicle sodium bicarbonate group ( group AV) . ALI was induced by intravenously injecting lipopolysaccharide 5 mg/kg in anesthetized rats. At 30 min before establishing the model, ZnPP-IX 10μmol/kg (diluted to 1 ml in 50 mmol/L sodium bicarbonate) was intraperitoneally injected in group AZ, and 50 mmol/L sodium bicarbonate 1 ml was intra-peritoneally injected in group AV. After injecting lipopolysaccharide for 6 h, blood samples were collected from the common carotid artery for determination of plasma CO concentration, the rats were then sacrificed, and lungs were removed for microscopic examination of the pathological changes which were scored and for determination of CO level, wet to dry weight ratio ( W/D ratio) , cell apoptosis ( by TUNEL) , and expres-sion of heme oxygenase-1 ( HO-1) , glucose-regulated protein 78 ( GRP78) , phosphorylated protein kinase R-like endoplasmie reticulum kinase (p-PERK), phosphorylated eukaryotic translation initiation factor 2 alpha ( p-elF2 ) , CCAAT/enhancer-binding protein homologous protein ( CHOP ) and caspase-12 in lung tissues ( by Western blot) . Apoptosis index ( AI) was calculated. Results Compared with group C, the lung injury scores, W/D ratio, AI and CO levels in plasma and lung tissues were significantly increased, and the expression of HO-1, GRP78, p-PERK, p-elF2, CHOP and caspase-12 was up-regulated in the other three groups ( P<0. 05) . Compared with group ALI, lung injury scores, W/D ratio and AI were sig-nificantly increased, CO levels in plasma and lung tissues were decreased, the expression of HO-1 was down-regulated, and the expression of GRP78, p-PERK, p-elF2, CHOP and caspase-12 was up-regula-ted in group AZ (P<0. 05), and no significant change was found in the parameters mentioned above in group AV ( P>0. 05) . Conclusion HO-1/CO signaling pathway produces endogenous protection possibly through inhibiting endoplasmic reticulum stress during endotoxin-induced ALI in rats.
4.Correlation analysis of the clinical and sonographic characteristics of papillary thyroid microcarcinomas with its aggressiveness

Objective To compare the clinical and sonographic characteristics of aggressive and nonaggressive papillary thyroid microcarcinomas(PTMC)in order to improve the preoperative predictive value of aggressive PTMC.Methods A total of 309 patients with PTMC from January 2014 to December 2016 were included in this study.Patients with lymphatic metastasis,extrathyroidal invasion,reccurence, distant metastasis and death were classified into aggressive PTMC group,and patients without above characteristics were classified into nonaggressive group.Clinical and sonographic features were reviewed and compared between the two groups.Results Among the 309 patients,76 cases(24.6%)were aggressive PTMC,and 233 cases(75.4%)were nonaggressive.Patients were younger and larger cancerous nodules, microcalcification,capsular inconnection and multifocality were seen more frequently in aggressive PTMC group compared with nonaggressive group.The best cut-off value of age and diameter were 44.5 years and 0.66 cm respectively.Advanced age was the protective factor and larger tumor size and multifocality were independent risk factors for PTMC aggressiveness.The capsular invasion was related with the lateral cervical lymph node metastasis while other features were not.Conclusions Extra attention should be paid to patients with age<44.5 years,tumor size>0.66 cm and multifocal cancerous nodules because their PTMCs are more likely to be aggressive.Thyroid capsule adjacent to the cancerous nodule should be observed carefully.If there is interruption in the capsule,lateral cervical lymph nodes should be carefully examed.
5.Significant decrease in inferior vena cava pressure predicts high postoperative artificial blood vessel patency in type Ⅱ Budd-Chiari syndrome patients undergoing atrial caval shunting

Objective To investigate the change of inferior vena cava pressure (IVCP) in type Ⅱ Budd-Chiari syndrome patients undergoing atrial caval shunting and its relationship with postoperative artificial blood vessel (ABV) patency rate.Methods We recruited 209 patients who had undergone atrial caval shunting for type Ⅱ Budd-Chiari syndrome and evaluated IVCP,right atrial pressure (RAP) and free portal vein pressure (PFP) before and after ABV opening.Presure changes were compared by t-test.These patients were followed up by color Doppler ultrasonograthy for ABV patency.The correlation between IVCP and postoperative ABV patency were analyzed By Kaplan-Meier test.Results IVCP (t =0.56,P < 0.05)and PFP (t =0.72,P < 0.05) decreased and RAP increased significantly after ABV opening (t =0.52,P < 0.05).Follow up result showed that ABV patency rate was lower in patients with IVCP descent < 1 kPa than those with IVCP descent > 1 kPa (P < 0.05).Conclusions Significant IVCP descent correlates with high ABV patency rate after atrial caval shunting in type Ⅱ Budd-Chiari syndrome patients.
6.The influence of liver outflow veins diameter on postoperative portal venous pressure and graft patency in Budd-Chiari syndrome patients after atrial caval shunting

Objective To investigate the influence of diameter of liver outflow vein on portal hypertension and artificial blood vessel (ABV) patency rate in Budd-Chiari syndrome (BCS) patients undergoing atrial caval shunting (ACS).Methods We recruited 209 patients,who had undergone ACS for Ⅱ type of BCS.Those patients with unobstructed liver outflow vein were included into group A and the patients with stenosed liver outflow vein into group B.Free portal pressure (FPP) was measured before and after ABV opening.Portal vein velocity (Vpv),liver function,spleen volume and function,esophagogastric varices and ABV patency were evaluated postoperatively.Results After ABV opening,FPP decreased significantly in group A than group B (t =10.45,P < 0.05).Vpv accelerated significantly in group A 2 weeks after operation than group B (t =12.81,P < 0.05).Apparent improvement of liver function,spleen function and esophagogastric varices and reduction of spleen volume were observed in group A patients than group B patients (P < 0.05).Reduction of esophagogastric varices in group A was better than in group B (x2 =44.73,P < 0.05).By postoperative follow up,ABV patency of group A was higher than group B (P < 0.05).Conclusions Patency status of liver outflow vein significantly influences postoperative portal vein pressure and closely correlats to ABV patency rate after ACS.
