1.Establishment and application of clinical pharmaceutical pathway of anti-infective treatment for high-risk populations of antibiotic-associated encephalopathy

OBJECTIVE To establish a clinical pharmaceutical pathway of anti-infective therapy for high-risk populations of antibiotic-associated encephalopathy (AAE), and analyze its application effects. METHODS Clinical pharmacists developed the “AAE High-Risk Population Screening Form” and “Antibiotic AAE Risk Comparison Form” based on literature and expert opinions, and established the “Clinical Pharmaceutical Pathway of Anti-infective Treatment for AAE High-Risk Population” in our hospital. A prospective, non-randomized controlled study was conducted from May 2023 to April 2024, including 50 cases in the observation group and 50 cases in the control group among patients with pulmonary infections admitted to the Dept. of Internal Medicine in our hospital. The observation group was involved in the development of an anti-infective treatment following the clinical pharmaceutical pathway by clinical pharmacists, while the control group received routine anti-infective treatment by clinical physicians. The occurrence of AAE, the rational antibiotic drug use, and the effectiveness of initial anti-infective treatment in the two groups were observed, and the intervention measures and outcomes of AAE cases were summarized. RESULTS The anti-infective treatment clinical pharmaceutical pathway for AAE high-risk population was preliminarily established in our hospital. The analysis of the application effects showed that there was 1 case of AAE in the observation group and 8 cases in the control group, with a significantly lower incidence of AAE in the observation group than in the control group; the rational antibiotic drug use and the effectiveness of initial anti-infective treatment in the observation group were both significantly superior to those in the control group (P<0.05). Drug withdrawal and dressing change were the preferred effective intervention measures for AAE, and encephalopathy treatment drugs could be used as auxiliary measures for symptom relief. Timely and effective intervention was conducive to rapid symptom relief, with a total improvement rate of AAE of 88.89%. CONCLUSIONS The anti-infective treatment clinical pharmaceutical pathway for AAE high-risk population can effectively prevent the occurrence of AAE as well as contribute to promoting rational drug use and the effectiveness of initial anti-infection plans and strengthening treatment outcomes.
2.Effect of Buzhong Yiqiwan on NLRP3 Inflammasome Pathway of DSS-induced Colitis Model Mice at Different Pathological Stages

ObjectiveTo explore the intervention effect and mechanism of Buzhong Yiqiwan (BZYQ) on colitis mice. MethodSixty-four C57BL/6 mice were randomly divided into 2 weeks blank group, 2 weeks model group, 2 weeks high-dose BZYQ (12 g·kg-1) group, 2 weeks low-dose BZYQ (6 g·kg-1) group, 4 weeks blank group, 4 weeks model group, 4 weeks high-dose BZYQ (12 g·kg-1) group, and 4 weeks low-dose BZYQ (6 g·kg-1) group. The colitis model was induced in mice by feeding 3% dextran sodium sulfate (DSS) for 7 days. The mice received BZYQ (12 and 6 g·kg-1) by gavage on the 8th day after modeling, once per day, and sacrificed on the 2nd and 4th weeks, correspondingly. The colon length and weight of mice in each group were measured. Hematoxylin-eosin (HE) staining was used for pathological observation and colonic mucosal inflammation was scored. The mRNA and protein expression of NOD-like receptor thermoprotein domain 3 (NLRP3), apoptosis-associated speck-like protein containing a CARD (ASC), and cysteinyl aspartate-specific protease-1 (Caspase-1) was detected by real-time quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. Enzyme-linked immunosorbent assay (ELISA) was used to detect the content of inflammatory cytokines, such as interleukin (IL)-1β, IL-18, and IL-33 in colonic tissues. ResultCompared with the 2 weeks blank group, the 2 weeks model group showed shortened colon length, increased colon weight (P<0.05), loss of epithelial cells, destruction of gland structure, infiltration of a large number of inflammatory cells in mucosa and submucosa, local crypt abscess, and increase in mucosal inflammation score (P<0.01) as revealed by light microscopy, elevated levels of IL-1β, IL-18, and IL-33 in colonic tissues (P<0.05), and increased mRNA and protein expression of NLRP3, ASC, and Caspase-1 (P<0.05). The intervention of BZYQ (12 g·kg-1) restored colon length, alleviated mucosal injury (P<0.05), down-regulated the content of IL-18 (P<0.05), reduced the mRNA expression of NLRP3 and ASC as well as the protein expression of ASC and Caspase-1 compared with the conditions in the 2 weeks model group. Compared with the 4 weeks blank group, the 4 weeks model group showed decreased colon length, increased colon weight (P<0.05), decreased glands in the mucosal layer, expansion of glandular cavity, atrophy of crypt, local connective tissue hyperplasia and lymphocyte infiltration, increased inflammation score (P<0.01) as revealed by the light microscopy, increased expression of IL-1β, IL-18, and IL-33 (P<0.05), and elevated mRNA and expression of NLRP3 inflammasome complex (P<0.05). Compared with the conditions in the 4 weeks model group, the intervention of BZYQ (12 and 6 g·kg-1) could improve colon length and weight (P<0.05), and the intervention of BZYQ (12 g·kg-1) could also improve the inflammation score of the colon (P<0.05). Different from the acute stage, the intervention of BZYQ (12 and 6 g·kg-1) increased the content of IL-33 in the intestinal mucosa and up-regulated the mRNA and protein expression of NLRP3 inflammasome complexes ASC and Caspase-1 (P<0.05). ConclusionBZYQ can relieve the injury of colitis induced by DSS in mice. The mechanism is related to the regulation of intestinal immune response mediated by NLRP3 inflammasome, and it has different regulatory effects in acute and chronic inflammation stages.

To address this challenge, an expert committee was established on behalf of the Chinese Pancreatic Surgery Association, Chinese Society of Surgery, Chinese Medical Association, which consisted of surgical oncologists, gastroenterologists, medical oncologists, endocrinologists, radiologists, pathologists, and nuclear medicine specialists. By reviewing the important issues regarding the diagnosis and treatment of pNENs, the committee concluded evidence-based statements and recommendations in this article, in order to further improve the management of pNENs patients in China.
5.βE2 activates Nrf2 pathway to resist light-induced retinal dysfunction in rats

【Objective】 To investigate whether 17β-estradiol (βE2) can activate the NF-E2-related factor 2(Nrf2) pathway to resist the downregulation of retinal function induced by light damage. 【Methods】 Two weeks after female SD rats were castrated, they were divided into the following six groups: control group (control), light-damage group (LD), saline group, saline light-damage group (saline-LD), βE2 group, and βE2 light-damage group (βE2-LD). Rats in the light-damage were exposed to 8000-lux fluorescence for 12 h after 18 h of dark adaptation. Then electroretinogram (ERG), immunofluorescence, Real-time PCR and immunohistochemical detection were performed after one day of dark recovery. 【Results】 The results of ERG showed that ERG was lower in LD group than in control group (P<0.05). After light damage, ROS was increased and the mRNA expressions of antioxidant genes, such as Sod1, Sod2, Cat, Glrx1, Glrx2, Txn1, and Txn2 were decreased (all P<0.05). After βE2 administration, compared with those in saline-LD, ROS level was decreased, the levels of Nrf2 protein and antioxidant genes were increased, and ERG was recovered to a certain extent in βE2-LD (P<0.05). 【Conclusion】 βE2 can restore the function of rat retina, and its mechanism might be related to the upregulation of Nrf2 and antioxidant genes.

8.Clinical analysis of blood purification in the treatment of 102 patients with moderately severe acute pancreatitis and severe acute pancreatitis

Objective:To analyze the efficacy of blood purification (BP) in the treatment of moderately severe acute pancreatitis (MSAP) and severe acute pancreatitis (SAP).Methods:The clinical data of 102 patients of MSAP and SAP in Changhai Hospital affiliated with Navy Medical University from December 2015 to March 2019 were retrospectively analyzed. According to the treatment method and prognostic outcome, patients were divided into control group ( n=46) and BP group ( n=56), control survival group ( n=43), and control death group ( n=3), BP survival group ( n=37) and BP death group ( n=19), respectively. Based on BP treatment duration each time, BP group was sub-divided into BP short-term group (4 h≤T<8 h, n=38) and BP long-term group (8 h≤T≤12 h, n=18). General data, laboratory findings, complications, average daily hospitalization cost, prognosis and effects of BP duration on the blood urea nitrogen improvement were recorded. Results:Compared with control survival group, BP survival group had lower levels of lactic acid, urea nitrogen and other metabolites, and SIRS duration was significantly shortened [12(7, 16) days vs 5(3, 9) days, HR=0.19, 95% CI 0.11-0.35, P<0.0001]. The clearance of urea nitrogen in BP short-term group was more obvious than BP long-term group (χ 2==4.44, P=0.035). APACHEⅡ score on admission( OR=1.33, 95% CI 1.087-1.617, P=0.005) and numbers of organ failure ( OR=3.445, 95% CI 1.426-8.323, P=0.006) were important risk factors affecting MSAP and SAP prognosis under BP therapy. For patients with extremely poor prognosis, it was still difficult for BP to reverse the progression trend and prognosis of MSAP and SAP. Conclusions:BP can effectively remove accumulated metabolites at acute stage and shorten the duration of SIRS. BP duration of 4-8 h each time may be more conducive for improving the severity of the disease.
9.Effects of Mycobacterium tuberculosis liposaccharide ManLAM on CE protein-induced B cell activation

Objective:To investigate the influence and potential mechanism of mannose-capped lipoarabinomannan (ManLAM) to B cells responding to Mycobacterium tuberculosis (Mtb) infection. Methods:B cells were separated from patients with active pulmonary tuberculosis using magnetic beads and then stimulated with ManLAM in combination with CE protein. Flow cytometry was performed to evaluate the apoptosis, proliferation and activation of B cells. The secretion of cytokines and CE protein-specific IgG subclasses were detected by ELISA. ELISPOT assay was used to analyze the influence on the differentiation of B cells into CE protein-specific antibody-secreting cells (ASCs).Results:ManLAM inhibited the CE protein-induced proliferation and activation of B cells and the production of pro-inflammatory cytokines, resulting in significantly increased secretion of the immunosuppressive cytokine IL-10. It also inhibited the differentiation of B cells into CE protein-specific IgG secretory cells, but had no significant influence on the differentiation to IgM secretory cells. Moreover, ManLAM inhibited the secretion of CE protein-specific IgG1 and IgG3 and induced the secretion of immunosuppressive IgG4 via TLR2.Conclusions:This study suggested that ManLAM could inhibit the anti-tuberculosis immune response of B cells, which provided new theoretical reference for better understanding the immune escape mechanism in Mtb infection.
10.Application of laparoscopic hepatic portal exposure in children with type Ⅲ biliary atresia

Objective To explore the clinical value of portal exposure in laparoscopic treatment of children with type Ⅲ biliary atresia (BA).Methods From June 2013 to October 2017,30 infants with type Ⅲ BA who treated with laparoscopic portoenterostomy in Huai'an Women and Children's Hospital were selected.A percutaneous suture was used to snare the round ligament and retract the liver,other percutaneous stay sutures were then introduced and fundus and neck of gallbladder were sutured to elevate the liver to expose the portal hepatis.The fibro cord and hepatic vessels were mobilized,and then two rubber bands were put around the left and right portal veins and hepatic arteries.The portal hepatis was exposed by laterally stretching the two elastic rubber bands.The fibro cord was removed and then laparoscopic portoenterostomy was accomplished.In 20 cases,the liver was enlarged,part of hepatic lobus quadratus was removed laparoscopiclly for exposure of the portal hepatis.Results There were 30 cases in this group,2 cases were converted to open surgery by a micro transverse incision.There was no surgical death.Time of laparoscopic procedure varied from 210 to 280 min.All cases survived the surgery without any intraoperative complications.Blood loss during operation was minimal,without necessity for blood transfusion.One case died of respiratory failure one week after surgery.Two cases were lost follow-up.Twenty-five cases were followed up for 3~51 months(mean 22.4 months).Three cases died because of repeated cholangitis and liver failure at 10,16,35 months postoperatively.Nineteen patients' total bilirubin had dropped to normal,three others' bilirubin levels dropped significantly after surgery.Conclusion The technique of laparoscopic hepatic porta exposure can help to complete hepatic portoenterostomy successfully,reduce the conversion rate of laparoscopic surgery,and improve the surgical effect.
