1.Mechanism of Modified Erxian Decoction Regulating Perimenopausal Syndrome via SIRT1/Kisspeptin/GnRH Signaling Pathway

ObjectiveTo explore the regulation of hypothalamic-pituitary-gonadal (HPG) axis by modified Erxian decoction in rats with perimenopausal syndrome (PMS) and to further analyze the expression of proteins related to the silent information regulator 1 (SIRT1)/hypothalamic kisspeptin (Kisspeptin)/gonadotropin-releasing hormone (GnRH) signaling pathway in the arcuate nucleus region (ARC) of the hypothalamus, so as to reveal the potential target of action and molecular biological mechanism of modified Erxian decoction for the treatment of perimenopausal syndrome. MethodsAn animal model was established via the incomplete castration method, with successful modeling confirmed by the exfoliated cervical cell smear method. The 48 rats were divided into six groups based on the randomization principle after successful modeling, including a sham operation group, a model group, an estradiol valerate group (0.09 mg∙kg-1∙d-1), high-, medium-, and low-dose modified Erxian decoction groups (7.614, 3.807,1.903 5 g∙kg-1∙d-1), with 8 rats in each group. The estradiol valerate group and the high-, medium- and low-dose modified Erxian decoction groups were continuously administered by gavage for 28 days, and the indicators were detected 24 hours after the last administration. Body weights and uterine indices were measured. The pathological changes of the uterus were observed by hematoxylin-eosin (HE) staining. Enzyme-linked immunosorbent assay (ELISA) was performed to measure the levels of estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and gonadotropin-releasing hormone (GnRH). Real-time quantitative polymerase chain reaction (Real-time PCR) and Western blot were used to determine the expression levels of SIRT1, Kisspeptin, kisspeptin receptor (GPR54), and GnRH in the ARC region of the hypothalamus and gonadotropin-releasing hormone receptor (GnRH-R) in pituitary. ResultsCompared with the sham operation group, rats in the model group had a significantly increased body weight (P0.01), reduced wet weight and index of uterus (P0.01), endometrial thinning or atrophy, glandular atrophy, and a decreasing number of glands. Additionally, serum levels of E2 and the expression of SIRT1 in the ARC region of the hypothalamus significantly decreased (P0.01). Serum levels of FSH, LH, and GnRH, the expression of Kisspeptin, GPR54, and GnRH in the ARC region of the hypothalamus, and GnRH-R in pituitary significantly increased (P0.01). Compared with the model group, the estradiol valerate group and the high-, medium-dose modified Erxian decoction groups had significantly reduced body weight, serum levels of FSH, LH, and GnRH, and expression of Kisspeptin, GPR54, and GnRH in the ARC region of the hypothalamus and GnRH-R in pituitary (P0.05, P0.01) and significantly increased wet weight and index of uterus, serum level of E2, and expression of SIRT1 in the ARC region of the hypothalamus (P0.05, P0.01). In addition, they showed thickened endometrium, increased number of endometrial glands, and improved glandular atrophy. ConclusionModified Erxian decoction regulates the function of the HPG axis through multi-targets, and its mechanism of action may be related to the up-regulation of the expression of SIRT1 in the ARC region of the hypothalamus, the inhibition of the over-activation of the Kisspeptin/GnRH signaling pathway, the regulation of the expression of GnRH-R in the pituitary, the restoration of secretion balance of gonadotropins, and the elevation of the estrogen level. This study provides an experimental basis for the interpretation of the scientific connotation of modified Erxian decoction in the treatment of perimenopausal syndrome and a theoretical reference for the development of a novel therapeutic strategy based on the SIRT1/Kisspeptin/GnRH pathway.
2.Role of Endoplasmic Reticulum Stress in Atherosclerosis and Its Regulation by Traditional Chinese Medicine: A Review

Atherosclerosis (AS) is a chronic inflammatory pathological process in which lipid and/or fibrous substances are deposited in the intima of arteries, and it is one of the pathological bases of many cardiovascular and cerebrovascular diseases. Endoplasmic reticulum stress (ERS) is a protective mechanism of cell adaptation. Moderate ERS can reduce abnormal protein aggregation and increase the degradation of misfolded proteins to repair and stabilize the internal environment, while excessive ERS can cause unfolded protein reaction, activate inflammation, oxidative stress, apoptosis, autophagy, and other downstream pathways, and lead to cell damage, or even apoptosis. A large number of studies have shown that ERS mediates a variety of pathological processes related to AS, affects endothelial cells, smooth muscle cells, macrophages, endothelial progenitor cells, and other cell components closely related to its occurrence and development, influences the progress of AS by regulating cell function, and promotes the formation of AS plaque, the transformation of stable plaque to unstable plaque, and the rupture of unstable plaque. Regulation of ERS may be a key target for the prevention and treatment of AS, and it is a research hotspot at present. Traditional Chinese medicine (TCM) believes that the origin of AS is the imbalance of Yin and Yang, the disharmony of Zangfu organs, and the abnormal operation of Qi, blood, and body fluid, which leads to the accumulation of phlegm, blood stasis, and other pathological products in the pulse channels, making the blood flow blocked or misfunction and causing the disease, which belongs to the syndrome of deficiency in origin and excess in superficiality. As the pathogenesis of AS is complex, and the symptoms are diverse, TCM has significant advantages in treating AS because of its multiple targets, multiple pathways, stable efficacy, strong individualization, and high safety. This paper systematically elaborated on the role of ERS in the occurrence and development of AS and summarized the mechanism research on the regulation and control of ERS by Chinese herbal monomer, Chinese herbal extract, Chinese herbal compound, and proprietary medicine, so as to provide a theoretical basis for clinical research and drug development in the prevention and treatment of AS.
3.Determination of 27 volatile organic compounds in exhaled breath by thermal desorption-gas chromatography-mass spectrometry

Background Volatile organic compounds (VOCs) in exhaled breath are closely associated with respiratory diseases and are linked to various metabolic reactions in the human body. A quantitative analytical method can provide technical support for studying VOCs related to various diseases. Objective To establish a thermal desorption-gas chromatography-mass spectrometry (TD-GC-MS) method for the determination of 27 VOCs in exhaled breath. Methods VOCs in exhaled breath were collected using a Bio-VOC sampler and enriched with Tenax TA thermal desorption tubes before TD-GC-MS analysis. Standards were collected using thermal desorption tubes and optimized for thermal desorption conditions as well as chromatographic and mass spectrometric conditions: The separation of the 27 VOCs was achieved by an optimized temperature program, the improvement of sensitivity by optimizing quantitative ions, and the increase of VOCs desorption efficiency by optimizing thermal desorption time and temperature. Limit of detection, limit of quantification, accuracy, precision, and stability of the proposed method were investigated by spiking with a blank gas bag, and exhaled breath samples from 20 healthy individuals were collected for an application study of the proposed method. Results The thermal desorption temperature was 280 ℃, and desorption time was 6 min. A VF-624ms chromatographic column was selected for the separation of target substances. The initial temperature of heating program was 35 ℃, maintained for 1 min, and then increased to 100 ℃ at a heating rate of 3 ℃·min−1 for 1 min, followed by increasing to 210 ℃ at a heating rate of 28 ℃·min−1 for 5 min. A quantitative analysis was conducted with a single ion monitoring (SIM) mode. Under these conditions, the 27 VOCs showed good linear relationships in their respective concentration ranges and the correlation coefficients were higher than 0.9990. The limits of detection of the method were in the range of 0.01-0.13 nmol·mol−1, the limits of quantification were in the range of 0.02-0.44 nmol·mol−1, and the spiked recoveries were in the range of 80.1%-120.5%, with intra-batch and inter-batch precision ≤ 18.8% and 17.9% respectively. All substances can be stored at room temperature (23-28 °C) for 7 d and at 4 °C for 14 d. The proposed method was applied to exhaled breath samples from 20 subjects with detection rates≥ 80% (except for trans-2-pentene and decane) and a concentration range of 0.00-465.50 nmol·mol−1. Conclusion The established TD-GC-MS method for quantification of VOCs in exhaled breath is characterized by high sensitivity and good accuracy, and is suitable for quantitative determination of VOCs in exhaled breath, which can provide technical support for the study of exhaled breath VOCs.
4.The mediating effect of self-management ability on stress perception, anxiety and depression in young and middle-aged patients with type 2 diabetes

Objective:To analyze the interaction between self-management ability, stress perception, anxiety and depression in young and middle-aged patients with type 2 diabetes, and provide guidance for relieving anxiety and depression.Methods:A total of 198 young and middle-aged patients with type 2 diabetes who were treated in the First People′s Hospital of Suqian City from December 2021 to June 2022 were included by convenience sampling method. The general information questionnaire, Self-Behavior Management Scale for Diabetic Patients, Stress Perception Scale, Self-rating Anxiety Scale and self-rating Depression Scale were used to investigate the patients. Pearson correlation was used to analyze the correlation between patients′ self-management ability, perceived stress and anxiety and depression, and to construct a structural model of patients′ self-management ability in perceived stress, anxiety and depression.Results:Finally 185 valid questionnaires were collected. The mean age of the patients was (39.09 ± 5.36) years old, with 102 males and 83 females, respectively. The patients′ self-management ability was (15.79 ± 1.34) points, stress perception (36.46 ± 10.53) points, anxiety (47.39 ± 7.47) points, depression (52.35 ± 8.50) points. Stress perception was positively correlated with anxiety and depression ( r = 0.779, 0.932, both P<0.05), while self-management was negatively correlated with stress perception ( r = - 0.656, P<0.05), and self-management was negatively correlated with anxiety and depression ( r = - 0.631, - 0.684, both P<0.05). There was a partial mediating effect between stress perception and anxiety, accounting for 17.72% of the total effect. There was a partial mediating effect between stress perception and depression, accounting for 20.92% of the total effect. Conclusions:Middle-aged and young people with type 2 diabetes generally have greater stress and higher anxiety and depression, and perceived stress can directly affect the degree of anxiety and depression, and perceived stress can indirectly affect the degree of anxiety and depression through the mediating role of self-management ability, and can reduce the degree of anxiety and depression by strengthening the self-management ability of patients.
5.Relationship Between NLRP3 Inflammasome and Diabetic Cardiomyopathy and Intervention Effect of Traditional Chinese Medicine: A Review

Diabetic cardiomyopathy (DCM) is one of the complications of diabetes. It refers to a specific type of idiopathic cardiomyopathy that occurs in individuals with diabetes, distinct from other cardiovascular diseases such as coronary heart disease, valvular heart disease, or congenital heart disease. It has also been identified as one of the leading causes of death in diabetic patients for many years. Research has shown that the pathogenesis of DCM is closely associated with insulin resistance, activation of various inflammatory responses, increased oxidative stress, impaired coronary microcirculation, and accumulation of advanced glycation end products (AGEs). Among various inflammatory responses, the activation of the NOD-like receptor protein 3 (NLRP3) inflammasome can induce the secretion of a large amount of pro-inflammatory cytokines through the cascade reaction of inflammation, subsequently mediating cellular pyroptosis and promoting myocardial damage. Currently, extensive experimental studies on traditional Chinese medicine (TCM) have been conducted in China and abroad based on the significant role of the NLRP3 inflammasome in the prevention and treatment of DCM. These studies have demonstrated that Chinese medicinal extracts, such as Astragalus polysaccharide and ginsenoside Rb1, single drugs like Coriolus and Cordyceps, and Chinese medicinal formulas like Didangtang and modified Taohe Chengqitang, as well as acupuncture and TCM exercise therapy, can regulate the relevant pathways of the NLRP3 inflammasome to inhibit its assembly or activation, reduce inflammatory responses, inhibit myocardial remodeling in DCM, and improve cardiac function. This article reviewed the relationship between the NLRP3 inflammasome and DCM, as well as the research progress on TCM in exerting anti-inflammatory effects in this field, aiming to provide new insights for the development of therapeutic approaches for DCM.
6.Autophagy of Vascular Endothelial Cells Influences Diabetic Macroangiopathy: Based on Theory of Qi Deficiency and Stagnation

The basic pathological change of diabetic macroangiopathy is atherosclerosis (AS), which is mainly associated with vascular endothelial cells (VECs) injury, oxidative stress, glucose and lipid metabolism disorders, hemorheological abnormalities, and endoplasmic reticulum stress. The injury and dysfunction of VECs are the initiating factors of diabetic macroangiopathy. Autophagy is a subcellular self-protection mechanism that regulates basic intracellular metabolism through lysosome-mediated degradation of proteins and damaged organelles to maintain homeostasis. Insufficient autophagy of VECs leads to enhanced inflammation, apoptosis, and oxidative stress of VECs, which promotes AS. According to the theory of traditional Chinese medicine (TCM), diabetic macroangiopathy corresponds to the syndrome of internal deficiency and pathogen invasion, with Qi deficiency and stagnation as the key pathogenesis. Qi deficiency is the root cause, and Qi stagnation is the manifestation. The disease occurs with the initial cause of nutrient-defense disharmony and instability of vessels, the main cause of the deficiency of kidney Qi and the lack of source for generation and transformation, the internal cause of Qi and blood loss in the viscera and the stagnation of Qi, blood, and fluid, and the superficial cause of the stagnation of pathological products and the damage of vessels. Autophagy is a microscopic manifestation of Qi, which has the function of dispelling pathogens and maintaining homeostasis. Insufficient autophagy of VECs leads to Qi deficiency and stagnation, and the gradual deficiency and heavy stagnation of Qi lead to insufficient autophagy, which form a vicious cycle. Modern research has demonstrated that regulating the autophagy of VECs is the main way to prevent and treat AS, and TCM can exert the therapeutic effect in a multi-target and multi-pathway manner. Therefore, based on the theory of Qi deficiency and stagnation, the method of tonifying deficiency of and removing stagnation can be adopted to select prescriptions for regulating the autophagy of VECs and treating AS, which can slow down the procession of diabetic macroangiopathy.
7.Prevention and Treatment of Diabetic Nephropathy by Regulating Endoplasmic Reticulum Stress with Traditional Chinese Medicine: A Review

Diabetic nephropathy (DN) is one of the serious and common microvascular complications of diabetes mellitus (DM) and the main cause of end-stage renal disease (ESRD). Endoplasmic reticulum stress (ERS) is a common stress defense mechanism in eukaryotic cells. In the ERS state, cells activate the unfolded protein response (UPR) to enhance the folding of unfolded proteins and the degradation of misfolded proteins, so as to restore the normal physiological function of the endoplasmic reticulum and avoid cell damage. However, excessive or chronic persistent ERS can induce apoptosis, inflammation, oxidative stress and other pathways to eventually cause cell damage. In recent years, a large number of studies have confirmed that ERS is closely associated with the occurrence and development of DN. In the case of DN, ERS is involved in the damage or protection of podocytes, glomerular mesangial cells, renal tubular epithelial cells, and glomerular endothelial cells. The regulation of ERS has become one of the hotspots in the prevention and treatment of DN and has received extensive attention in the field of traditional Chinese medicine. This paper systematically expounds the role of ERS in the occurrence and development of DN and summarizes the ERS-targeted regulation of DN by traditional Chinese medicine, with a view to providing certain research ideas for the prevention and control of DN with traditional Chinese medicine.
8.Visual analysis of Chinese medicine nursing of coronary heart disease research based on CiteSpace

Objective:To provide a reference for in-depth application and research in the field of coronary heart disease traditional Chinese medicine (TCM) nursing. Based on the CiteSpace software, the development trend and research hotspots of the literature are displayed and analyzed in this field.Methods:The China Knowledge Network (CNKI) database was used to retrieve the literature related to the research in this field published in the past 20 years from January 1st 2001 to October 5th 2021, and CiteSpace was used to visually analyze the number of articles, research institutions, authors and keywords, and to interpret its meaning in combination with the content of the atlas.Results:The amount of research literature in this field was generally on the rise, but the cooperation between various research institutions and authors needs to be strengthened. Research hotspots mainly focused on five areas: coronary heart disease-related symptom care, the application of characteristic TCM nursing techniques, the outcome indicators of the research, the TCM health management of patients with coronary heart disease, and the application model of TCM nursing.Conclusions:It is recommended to strengthen multidisciplinary and teamwork in this field, carry out high-quality randomized controlled studies, and focus on related research on multiple nursing models, evaluation tools, and symptom management.
9.Evaluation on the usability of a gamification coronary heart disease health management platform based on WeChat

Objective:To evaluate the usability of gamification coronary heart disease health management platform based on WeChat, so as to provide support for self-management of coronary heart disease patients.Methods:From November to December 2022, 10 patients with coronary heart disease who were treated at Dongzhimen Hospital of Beijing University of Chinese Medicine were selected using convenience sampling. The research subjects registered for the health management platform by scanning the QR code and underwent a 2-month trial experience. Two months later, researchers collected data from the platform and used a combination of questionnaire surveys and qualitative interviews to investigate the user's experience and feedback of the research subjects, in order to evaluate the usability of the platform.Results:Within two months, the health management platform had 226 visits, an average visit time of 26 minutes, and an availability questionnaire score of (71.8±2.8). The research subjects had an acceptable attitude towards the usability of this platform, had a good overall user experience, and had also provided improvement suggestions for the platform while they gained beneficial experiences.Conclusions:The gamification coronary heart disease health management platform based on WeChat has good usability, providing a feasible means for the health management of heart disease patients, and also providing reference for other chronic disease health management projects involving lifestyle changes and incentives.
10.Evaluation of the determination of hydrogen sulfide in the air of workplace by the detection tube method

Objective:To evaluate the accuracy and applicability of detection tube method for quantitative detection of hydrogen sulfide in workplace air.Methods:In September 2021, the lower limit of quantification, accuracy, precision, environmental factors, interfering gases and other performance indicators of the method for determining hydrogen sulfide in the air of workplace were verified by the detection tube, and the results were compared with those of GB 11742-89 "Standard method for hygienic examination of hydrogen sulfide in air of residential areas-methylene blue spectrophotometric method" to evaluate the application effect of the detection tube method for quantitative detection of hydrogen sulfide in workplace air.Results:There was no significant difference in the results of 2.83 mg/m 3, 4.25 mg/m 3 and 17.00 mg/m 3 hydrogen sulfide concentration between the two methods ( P>0.05) , but there was significant difference in the results of 8.50 mg/m 3 concentration ( P<0.05) . The lower limit of quantification of hydrogen sulfide in workplace air was 2.83 mg/m 3, the accuracy was 96.0%-111.0%, and the precision was 0.70%-6.64%. Under the condition of 4 ℃, the measured results decreased by 3.39%-13.10%. When the humidity was 50%-80%, the relative error of the average measured value was -1.67%-4.44%. Interference gases that may exist in the workplace (including carbon dioxide, carbon monoxide, mercaptans, nitrogen oxides, sulfur dioxide, etc.) did not interfere with the results of the test tube. Conclusion:The accuracy and precision of the detection tube method meet the detection requirements. The method is simple, rapid and easy to be popularized, and can be used for the rapid detection of hydrogen sulfide gas concentration in the workplace.
