1.The evaluation value of serum PTX3 and CK-MB levels on volume load in patients with chronic decompensated heart failure

Objective:To explore the evaluation value of serum levels of positive pentameric protein 3 (PTX3) and creatine kinase isoenzyme MB (CK-MB) on volume load in patients with chronic decompensated heart failure (CDHF).Methods:A total of 300 CDHF patients who visited the Xingtai Central Hospital from July 2019 to July 2022 were selected and divided into a capacity overload group ( n=182) and a non capacity overload group ( n=118) based on their capacity balance level. Two clinical data sets were compared and analyzed. The receiver operating characteristic (ROC) curve was used to analyze the evaluation value of serum PTX3 and CK-MB levels on the volume load of CDHF patients. The clinical disease characteristics of the two groups of patients were analyzed using univariate analysis, and the influencing factors of volume load of CDHF patients were analyzed using logistic regression. A column chart model was constructed and validated. Results:The body mass index (BMI), waist circumference, systolic blood pressure, diastolic blood pressure, fasting blood glucose (FBG), glycosylated hemoglobin (HbA 1c), C-reactive protein (CRP), uric acid (UA), homeostasis model assessment of insulin resistance (HOMA-IR) of patients in the capacity overload group were higher than those in the non-capacity overload group, and the differences were statistically significant (all P<0.05). The PTX3, CK-MB, pulmonary capillary wedge pressure (PCWP), and CVP levels of patients in the capacity overload group were higher than those in the non-capacity overload group, while albumin, hemoglobin, and hematocrit were lower than those in the non-capacity overload group, and the differences were statistically significant (all P<0.05). The ROC curve showed that the area under the curve (AUC) of PTX3 and CK-MB for predicting capacity overload in CDHF patients are 0.795 and 0.718, with sensitivity of 86.2% and 83.7%, specificity of 65.4% and 68.6%, respectively, indicating high predictive accuracy; The AUC of the two joint predictions is 0.817, the sensitivity was 92.5%, and the specificity was 70.6%. The prediction accuracy was higher than PTX3 ( Z=3.812, P<0.05) and CK-MB ( Z=3.365, P<0.05). PTX3, CK-MB, albumin, hemoglobin, hematocrit, PCWP, and central venous pressure (CVP) were all influencing factors of volume load status in CDHF patients (all P<0.05). The column chart risk prediction model established based on these factors had high accuracy and strong applicability in clinical treatment. Conclusions:Serum PTX3 and CK-MB levels are influencing factors for volume overload in CDHF patients. A column chart model constructed in combination with indicators such as albumin, hemoglobin, hematocrit, PCWP, and CVP has high predictive value for the volume overload status of CDHF.

Cow′s milk protein allergy (CMPA) is one of the most common presentations of food allergy seen in early childhood.It is an abnormal immune response caused by cow′s milk protein.CMPA can be clinically subdivided into either immediate-onset IgE mediated or delayed onset non-IgE mediated, or both.At present, concerns regarding the early and timely diagnosis of CMPA have been high-lighted over the years and there are many expert consensus on CMPA in China, but these consensus did not distinguish IgE mediated or non-IgE mediated CMPA.In view of the obvious clinical differences between the two type of CMPA and non-IgE mediated CMPA is more common in infancy, experts focus on pediatric gastroenterology, allergy/immunology, dermatology, nutrition and child healthcare convened by the Allergy Prevention and Control Professional Committee of Chinese Preventive Medicine Association present this guideline to help practitioners in primary care settings to early recognize and make suitable management of non-IgE mediated CMPA in China.The guideline incorporates the cutting-edge international guidance and the actual situation of Chinese children describing in detail the types, clinical features, diagnosis and nutritional intervention of non-IgE mediated CMPA.There are 42 recommendations in 7 categories in total referring to the common questions related to non-IgE mediated CMPA.
3. Effect of different compatibility ratios of Centella asiatica-Rhubarb on the dissolution rate of effective components of Centella asiatica

Objective:
To study the effects of different compatibility ratios of Centella asiatica- Rhubarb on the dissolution rates of madecassoside and asiaticoside.
Methods:
By using the HPLC method to analyze the content of madecassoside and asiaticoside in the Centella asiatica extract solution as well as Centella asiatica - Rhubarb extract solution with 6 different kinds of compatibility ratios. The tests were carried out by Thermo C18 (250 mm×4.6 mm, 5 μm) by isocratic elution with acetonitrile and 2 mmol/L of beta-cyclodextrin solution as mobile phase at a flow rate of 1 ml/min, the column temperature was 30 ℃ and the detection wavelength was 205 nm.
Results:
Compared with the Centella asiatica extract solution, the dissolution rate of madecassoside decreased significantly with the increase of the proportion of rhubarb. The combination of Centella asiatica and Rhubarb had little effect on the content of asiaticoside.
Conclusions
Considering the clinical application and efficacy of Centella asiatica, the optimal ratio of Centella asiatica-Rhubarb is 10:1 and 15:1.
4.Simultaneous determination of five compounds in Ganmaoning granules by UPLC-MS/MS

Objective To establish a method for the simultaneous determination of adenosine, chlorogenic acid, caffeic acid, baicalin and wogonin in Ganmaoning granules. Methods The UPLC-MS/MS method was adopted. The separation was performed on ZOBAX SB C18 (2.1 mm×150 mm, 5μm) column with mobile phase consisted of methanol-2 mmol/L ammonium acetate aqueous solution (isocratic elution) at the flow rate of 0.2 ml/min. The column temperature was set at 35℃, and the injection volume was 2μl. The electrospray ionization source (ESI) was used. Multiple reaction monitoring (MRM) mode was adopted, using positive and negative ions scanning. The ion source temperature was 300℃, the drying gas temperature was 400℃, the drying gas flow was 20 L/min, the atomizing gas pressure was 55 psi, and the capillary voltage was 4000 V.Results The linear ranges of adenosine, chlorogenic acid, caffeic acid, baicalin and wogonin in Ganmaoling granules were 0.20-12.80 ng (r=0.9996), 1.00-64.00 ng (r=0.9998), 0.40-25.60 ng (r=0.9996), 4.00-256.00 ng (r=0.9992), 0.20-12.8 ng (r=0.9991), which showed the linear relationship was good. The limits of detection were 0.02, 0.10, 0.04, 0.40, 0.02 ng, respectively. The limits of quantitation were 0.04, 0.20, 0.08, 0.80, 0.04 ng, separately. TheRSDs of precision, reproducibility and stability (24 h) tests were no more than 3.5%. The recovery rates were 99.3%-100.75% (RSD=2.09%-3.17%).Conclusions The method established in this experiment is simple, rapid, sensitive and accurate. It can be used to simultaneously determine the contents of five components inGanmaoling granules, and can be used for quality control ofGanmaoninggranules.
5.Determination of the protein-binding rates of gardenioside, paeoniflorin and naringin in Weiyanling granules in different species of plasma

Objective To study the protein binding rates ofgardenioside,paeoniflorin and naringin in Weiyanling granules in bovine serum albumin,rat plasma and human plasma.Methods The equilibrium dialysis method was carried to determine the binding-rates of three components in Weiyanling granules in bovine serum albumin,rat plasma and human plasma.The HPLC method was used to determine the mass concentration of three components in weiyanling granules in inner and outer dialysis and to calculate the plasma protein-binding rates.Results The linear relationship,precision,extraction recovery and stability of gardenoside,paeoniflorin and naringin all conformed to the methodological requirements.Within the low,middle and high concentration (5.084,10.04 and 20.08 g/L) of Weiyanling granules,the protein-binding rates of gardenioside and bovine serum albumin were 23.77% ± 13.39%,10.75% ± 1.34%,5.82% ± 6.53%,respectively;the protein-binding rates of paeoniflorin were 51.81% ± 6.53%,30.12% ± 6.61%,21.39% ± 9.45%,respectively;the protein-binding rates of naringin were 70.21% ± 3.31%,67.61% ± 1.31%,56.00% ± 3.46%,respectively;the protein-binding rates of gardenioside and rat plasma were 71.56% ± 3.00 %,43.56% ± 12.22%,40.63% ± 4.73%,respectively;the protein-binding rates ofpaeoniflorin and rat plasma were 55.72% ± 1.60%,47.59% ± 6.33%,40.07% ± 0.72%,respectively;the protein-binding rates of naringin and rat plasma were 85.85% ± 3.53%,85.38% ± 0.99%,79.99% ± 2.57%,respectively;the protein-binding rates of gardenioside and human plasma were 13.56% ± 2.40%,3.02% ± 2.41%,5.82% ± 2.08%,respectively;the protein-binding rates of paeoniflorin and human plasma were 9.49% ± 5.23%,5.01% ± 1.10%,3.98% ± 1.54%,respectively;the protein-binding rates of naringin and human plasma were 25.04% ± 2.61%,26.09% ± 13.82%,20.20% ± 2.24%,respectively.Conclusions Within 5.084,10.04 and 20.08 g/L,the protein binding-rate of gardenioside,paeoniflorin and naringin were shown asfollow,rat plasma >bovine serum albumin > human plasma.There were significant differences in species.
6.Quality standard analysis and research for Wuji-Jianwei granules

Objective To establish the quality standard of Wuji-Jianwei granules. Methods The contents of Bupleuri Radix, Codonopsis Radix, Paeoniae Radix Alba, Corydalis Rhizoma in Wuji-Jianwei granules were identified by using the thin layer chromatography (TLC) method. The contents of peoniflorin was determined by the high performance liquid chromatography (HPLC) method. Results The TLC had strong specificity and high separation, negative control without interference. The linear range was 0.124 8-0.748 8 μg for peoniflorin (r=0.999 6). The average recovery was 94.42% (RSD=1.46%) and the content of peoniflorin was 0.725 3 mg/g. Conclusions The method is accurate and reasonable, and can be used for the quality control of Wuji-Jianwei granules.
7.Simultaneous determination of three compounds in penthorum Chinense pursh by HPLC

Objective To establish a method for the simultaneous content determination ofquercetin,kaempferol and pinocembrin in Penthorum Chinense Pursh.Methods The HPLC analysis was carried out on Alltima C18 (250 mm × 4.6 mm,5 μm) with a mixture of methanol-0.2% phosphate (45:55) as the mobile phase.The determination wavelength was set at 270 nm with the flow rate of 1.0 ml/min.The column temperature was set at 30 ℃.Results The quercetin,kaempferol and pinocembrin showed good linearity in the range of 0.316-10.120 μg (r=0.999 9),0.012-0.397 μg (r=0.999 6) and 0.063-2.016 μg (r=0.999 8) respectively.The average recovery rates of quercetin,kaempferol and pinocembrin were 99.34% (RSD=1.51%),99.76% (RSD=l.96%) and 98.34% (RSD=1.56%) respectively.Conclusions The method is accurate and sensitive,which can be used for the content determination of quercetin,kaempferol and pinocembrin in Penthorum Chinense Pursh.
8. The study on behaviour of protective equipment utilization of workers exposed to benzene and factors based on Planned Behavior Theory

Objective:
To investigate and predict the behavioral intention and mode of the protective equipment utilization selection of the workers who used Benzene, the Theory of Planned Behavior (TPB) was applied to establish the behavioral model to enhance the theoretical foundation for long-term intervention.
Methods:
Questionnaires were used to survey the 707 workers, and all the behaviors of using protective equipment were investigated. Evaluate the relationships between each variable and obtain the influence affects by structural equation model.
Results:
The investigation showed that 38.47% of the total workers (272 cases) used whole body protection, 13.58% used partially, and 16.69% didn't use any body protection. There were significant difference between the varying degrees in the four dimensions (behavioral attitude, perceived behavior control, subjective norm, and behavioral intention) (P<0.01) . The results of structural equation model revealed that perceived behavior control was the most important influencing factor, subjective norm, positive attitude, negative attitude were the other three respects in sequence. The path co-efficient were 0.600、0.215、0.141 and 0.046 respectively.
Conclusion
The study show that the theory of planned behavior can effectively explain the behavioral intention and behavior of protective equipment utilization. Therefore, combining the subjective initiative of individuals with the supervision of enterprises, In order to effectively enhance the protective equipment utilization of benzene workers.
9.Application of the continuous language rehabilitation nursing in the motor aphasia patients with abrain stroke

Objective To investigate the clinical effect of continuous language rehabilitation nursing on the language function in patients with stroke motor aphasia. Methods A total of 60 motor aphasia patients with a brain stroke were recruited and divided into experimental group and control group with 30 cases each by random digits table method. The control group was treated with the routine stroke care in traditional Chinese medicine, while the experimental group with the continuing language rehabilitation nursing base on the routine stroke care. The language function of both groups were detected by the Chinese Rehabilitation Research Center Standard Aphasia Examination before and after 3 months treatment, and a survey of nursing satisfaction of patients when discharged was conducted. Results The right rate of hear and understand, retelling, say, read aloud, reading comprehension, transcription, description, dictation, calculation in the Chinese Rehabilitation Research Center Standard Aphasia Examination was (44.35 ± 18.69), (32.37 ± 22.25), (21.49 ± 14.91), (27.63 ± 12.54), (46.87 ± 15.30), (14.25 ± 6.11), (7.33±3.23), (11.63±4.82), (18.93±6.82)%before the treatment and (62.14±10.81), (55.36±19.53), (41.42 ± 13.75), (59.16 ± 11.05), (55.57 ± 13.90), (32.39 ± 7.12), (9.73 ± 3.92), (27.45 ± 4.71), (37.46 ± 6.91)%after the treatment in the experimental group, except for description, the differences were significant ( t=2.096-14.540, P<0.01 or 0.05). The right rate of hear and understand, retelling, say, read aloud, reading comprehension, transcription, description, dictation, calculation in the Chinese Rehabilitation Research Center Standard Aphasia Examination was (43.86±15.89), (34.28±20.27), (22.84±15.40), (28.63±12.45), (47.23±11.30), (13.40±6.24), (7.27±3.70), (11.10±4.73), (19.50±6.80)%before the treatment and (49.46± 14.27), (42.36±20.58), (30.33±13.36), (40.31±11.88), (49.97±10.13), (15.37±5.59), (8.50±3.74), (12.83± 4.36), (21.47 ± 7.74)% after the treatment in the control group, the differences of hear and understand, retelling, say, read aloud were significant (t=2.263-3.991, P<0.05 or 0.01). There were significant differences in hear and understand, retelling, say, read aloud, transcription, dictation, calculation after the treatment between two groups (t=2.510-10.298, all P<0.05). The score of satisfaction was (94.36 ± 5.55) in the experimental group and (88.25 ± 5.46) points in the control group, and there was significant difference (t=4.299, P<0.01). Conclusions Continuing language rehabilitation nursing can improve the recovery of the language function of the motor aphasia patients with a brain stroke.
10.Dissolution Characteristics of Trepibutone Tablets

Objective To compare the dissolution behavior between domestic trepibutone tablets and original reference product, and provide a basis for evaluating the quality consistency of generic drugs. Methods Four dissolution media recommended by Japanese Orange Book and a domestic standard dissolution media were selected to determined the dissolution profile,and f2 factor was calculated to investigate the consistency of stripping curves. Results In water,pH 4.0 and pH 1.2,the f2 of domestic formulation and reference formulation was under 50,and the dissolution profile was inconsistent.Dissolution behavior of domestic preparations of different manufacturers was dissimilar.In water,the f2 of domestic preparations of different batches of the same manufacturer was over 99.9,and the dissolution behavior was similar. Conclusion The dissolution method of existing domestic standard can not distinguish the dissolution behavior of different products,and it should be revised and completed.There is still great difference in quality between the domestic preparations and reference preparations.
