1.The Application of Adeno-asscociated Virus in Lipid Metabolism Research and Lipid-lowering Gene Therapy

Cardiovascular and cerebrovascular diseases, usually result from atherosclerosis, has the highest mortality rate globally. Lipid metabolism disorder is the main cause of atherosclerotic cardiovascular and cerebrovascular diseases, which not only lead to acute diseases such as myocardial infarction, stroke, acute pancreatitis, but also chronic kidney disease. In recent years, the advancement of gene therapy technologies has provided novel means for lipid metabolism study, and has also made it possible to cure patients with congenital lipid metabolism abnormalities. Adeno-associatd virus has a wide host range, high safety, low immunogenicity, and especially the ability of long-term stable expression in vivo, making it the preferred delivery tool for gene therapy of monogenic genetic diseases. Alipogene triprivec, also known as Glybera, was approved by the European Medicines Agency in 2012. It is the first gene therapy drug that uses recombinant AAV1 vector to directly deliver a highly active LPL protein S447X mutant to muscle cells for the treatment of patients with hereditary LPL deficiency. To enhance the targeted transduction efficiency of AAV carriers, recombinantAAV8.TBG.hLDLR utilizes the tissue tropsim of AAV8 to liver, meanwhile utilizes a liver specific thyroxine binding globulin promoter to control gene transcription, thereby achieving liver cell specific high expressionof human low-density lipoprotein receptors (LDLR). In patients with familial hypercholesterolemia,AAV8.TBG.hLDLR treatment effectively lower the level of plasma LDL for a long time, thus preventing the occurrence of atherosclerosis.Proprotein convert subunit kexin 9 (PCSK9) is secreted by liver cells. PCSK9 binds and transports LDLR to lysosomes for degradation, preventing the circulation and regeneration of LDLR, leading to accelerated degradation of LDLR and finally resulting in the accumulation of low-density lipoprotein cholesterol in plasma. Using AAV to deliver Cas9 of Staphylococcus aureus and gRNA targeting the Pcsk9 gene can knock out Pcsk9 in mouse liver, leading to a long-term significant decrease in plasma cholesterol levels in mice. Hepatocyte specific angiopoietin related protein 3 (Angptl3) is an endogenous inhibitor of LPL. Using the AAV9 mediated AncBE4max system and the dCas9 mediated single base gene editing system to introduce early termination codons, the knockout of Angptal3 in liver cells was achieved with an average knockout efficiency of 63.3%. After 2-4 weeks of administration in mice, the Angptl3 protein was completely undetectable in the peripheral blood, and serum triglycerides and total cholesterol decreased by 58% and 61%, respectively. Ring finger containing protein 130 (RNF130) is an E3 ubiquitin ligase. Research has shown that overexpression of RNF130 using AAV2/8 leads to ubiquitination degradation and redistribution of LDLR on the cell membrane, significantly reducing LDLR expression on liver cells and increasing plasma LDLC levels, while knocking out Rnf130 gene using the AAV-CRISPR system results in the opposite effect. This AAV mediated RNF130 function study proves that RNF130 is a posttranslational regulatory protein of LDLR and plays an important role in the regulation of serum LDLC. As mentioned above, recently, various lipid-lowering gene therapy drugs carried by different serotypes of adeno-associated virus have been applied in clinic or are undergoing clinical trials, and adeno-associated virus has emerging to be an important tool for lipid metabolism research.This article reviews the new progress of adeno-associated virus vectors in lipid metabolism study and lipid-lowering gene therapy.
2.Development and Application of Detection Methods for Capture and Transcription Elongation Rate of Bacterial Nascent RNA

ObjectiveDetection and quantification of RNA synthesis in cells is a widely used technique for monitoring cell viability, health, and metabolic rate.After exposure to environmental stimuli, both the internal reference gene and target gene would be degraded. As a result, it is imperative to consider the accurate capture of nascent RNA and the detection of transcriptional levels of RNA following environmental stimulation. This study aims to create a Click Chemistry method that utilizes its property to capture nascent RNA from total RNA that was stimulated by the environment. MethodsThe new RNA was labeled with 5-ethyluridine (5-EU) instead of uracil, and the azido-biotin medium ligand was connected to the magnetic sphere using a combination of “Click Chemistry” and magnetic bead screening. Then the new RNA was captured and the transcription rate of 16S rRNA was detected by fluorescence molecular beacon (M.B.) and quantitative reverse transcription PCR (qRT-PCR). ResultsThe bacterial nascent RNA captured by “Click Chemistry” screening can be used as a reverse transcription template to form cDNA. Combined with the fluorescent molecular beacon M.B.1, the synthesis rate of rRNA at 37℃ is 1.2 times higher than that at 15℃. The 16S rRNA gene and cspI gene can be detected by fluorescent quantitative PCR,it was found that the measured relative gene expression changes were significantly enhanced at 25℃ and 16℃ when analyzed with nascent RNA rather than total RNA, enabling accurate detection of RNA transcription rates. ConclusionCompared to other article reported experimental methods that utilize screening magnetic columns, the technical scheme employed in this study is more suitable for bacteria, and the operation steps are simple and easy to implement, making it an effective RNA capture method for researchers.

4.Efficacy and safety of Liqingtong granules in the treatment of hyperuricemia: A randomized double-blind placebo-controlled pilot clinical trial

Objective:
To investigate the efficacy and safety of Liqingtong (LQT) granules in patients with dampness-heat hyperuricemia.
Methods:
A randomized, double-blind, placebo-controlled pilot trial was conducted at the 983rd Hospital of the Joint Logistic Support Force of the People's Liberation Army from March 15, 2023, to August 10, 2023. In total, 119 participants were enrolled in this trial, and participants were given either LQT granules or placebo for 60 days based on a health education. The primary outcome was serum uric acid (SUA) level, and the secondary outcome was the traditional Chinese medicine (TCM) symptom score, measured on days 0, 30, and 60. Safety indicators, including liver function, kidney function, blood routine, glucose, blood lipid, blood pressure, and heart rate were tested on days 0 and 60 of the trial. The data were analyzed using Prism 9 software, and the significance level was set at P < .05.
Results:
Among 119 participants, six in the LQT granule group and seven in the placebo group dropped out, and 106 participants completed clinical observation. Baseline information, including SUA levels, TCM symptom scores, and other clinical characteristics, did not differ between the groups. At the end of the trial, compared with baseline values, the SUA levels in the LQT granule group decreased (P < .001), and no significant change was observed in the placebo group (P = .422); compared with the placebo group, the SUA levels decreased in the LQT granule group (P = .001). Compared with baseline values, the total TCM symptom scores in the LQT granule group decreased (P < .001), with no change in the placebo group (P = .136). Safety indicators did not differ significantly between the two groups.
Conclusion
The pilot trial demonstrated the potential of LQT granules to lower SUA levels and improve symptoms of dampness and heat.
5.Value of two-dimensional shear wave elastography in diagnosis of liver fibrosis in patients with type 2 diabetes mellitus and non-alcoholic fatty liver

Objective To evaluate the value of two-dimensional shear wave elastography(2D-SWE)in the diagnosis of liver fibrosis in patients with type 2 diabetes mellitus complicated with non-alcoholic fatty liver disease(NAFLD).Methods A total of 100 patients with T2DM complicated with NAFLD from some hospital were selected and was divided into three groups based on the degree of NAFLD including a mild group(38 cases),a moderate group(35 cases)and a severe group(27 cases).The values of liver stiffness measurement(LSM)in different degrees of fatty liver of the three groups were compared;the receiver operating characteristic(ROC)curve was used to evaluate the efficacy of 2D-SWE technique in diagnosing progressive liver fibrosis in T2DM patients with NAFLD.SPSS 26.0 statistical software was used for data analysis.Results The three groups had significant differences in LSM value(P<0.001),and the differences of the LSM values between the mild and moderate groups and between the mild and severe groups were all statistically significant(P<0.001),while the moderate and severe groups had no obvious difference in LSM value(P=0.065),with the LSM value increasing with the aggravation of NAFLD.There were 16 patients with progressive hepatic fibrosis and 84 ones with non-progressive hepatic fibrosis,and the median LSM value of patients with progressive heaptic fibrosis(8.3 kPa)significantly higher than that of patients with non-progressive hepatic fibrosis(5.0 kPa)(P<0.001).The ROC curve showed that the area under the curve(AUC)of 2D-SWE for the diagnosis of progressive liver fibrosis in T2DM patients with moderate and severe fatty liver was 0.896[95%confidence interval(CI):0.819-0.974],and the diagnostic sensitivity and the specificity was 92.3%and 83.7%respectively when the optimal cut-off value was 7.85 kPa.Conclusion The quantitative results of the LSM values measured by 2D-SWE technology differ in patients with different degrees of NAFLD,which can be used for auxiliary diagnosis of the degree of NAFLD and behaves well in diagnosing progressive hepatic fibrosis of patients with type 2 diabetes mellitus complicated with NAFLD.[Chinese Medical Equipment Journal,2024,45(9):73-77]
6.Multivariate analysis of influencing functional recovery after ankle fracture of type Ⅱ and above supination-external rotation

Objective To explore influencing factors of functional recovery after ankle fracture of type Ⅱ degree and above supination-external rotation.Methods Clinical data of 120 patients with ankle fractures of type Ⅱ degree and above supination-external rotation admitted from February 2019 to April 2021 were retrospectively analyzed.According to American Orthopaedic Foot and Ankle Society(AOFAS),patients were divided into excellent group with 73 patients(90 to 100 points),good group with 35 patients(75 to 89 points),and fair group with 12 patients(<50 points).The differences of ankle active range of motion(ROM)and AOFAS score were compared among three groups at the latest follow-up.Multivariate Logistic regression analy-sis was performed to analyze the factors related to functional recovery after ankle fracture of supination-external rotation.Results There were significant differences in postoperative ROM(dorsoextension,plantar flexion,varus and valgus)and com-plications between excellent group and good and acceptable group(P<0.05).Univariate analysis showed there were differences in age above 50 years old,Ⅳ degree of supination-external rotation fracture,lower tibiofibular ligament injury,posterior ankle fracture,no drainage tube placed,infection,antibiotic use time above 7 days(P<0.05).Multivariate Logistic regression analy-sis showed age above 50 years old[OR=2.829,95%CI(1.049,7.628),P=0.040],Ⅳ degree fracture of supination-external ro-tation[OR=6.13,95%CI(1.153,32.593),P=0.033],lower tibiofibular ligament injury[OR=10.785,95%CI(3.338,34.894),P=0.000],and posterior ankle fracture[OR=6.349,95%CI(1.869,21.560),P=0.003]were independent risk factors for func-tional recovery after ankle fracture of supination-external rotation(P<0.05).Conclusion The postoperative excellent outcome of ankle fracture was good,and the recovery of joint motion was better.The older age of patient,Ⅳ degree of supination-exter-nal rotation fracture,the lower tibiofibular ligament injury,and posterior ankle fracture are all adverse factors affecting func-tional recovery after supination-external rotation ankle fracture.In clinical,effective measures should be taken to deal with these influencing factors,and strive to improve the functional recovery after the operation of this type of fracture and reduce the occurrence of related complications.

Post-endoscopic retrograde cholangiopancreatography pancreatitis(PEP)is one of the most common complications after endoscopic retrograde cholangiopancreatography(ERCP).Numerous PEP prediction models have been established based on different statistical methods at home and abroad.The PEP prediction model,as a tool for evaluating and screening high-risk populations,can provide a basis for medical staff to find high-risk PEP patients early and take effective preventive measures.In recent years,new PEP prediction models have appeared one after another,but there is still a lack of recognized reliable prediction models in clinic.This article reviews the research status of PEP risk prediction models,aim to provide a direction for establishing a more reliable,accurate,and practical PEP risk prediction model in the later period.
8.Clinical Significance of Genetic and Molecular Changes in Primary Myeloid Sarcoma

Objective:To investigate the clinical significance of genetic and molecular changes in primary myeloid sarcoma(MS).Methods:Fourteen patients with primary MS were selected in Jiading District Central Hospital Affiliated Shanghai University of Medicine & Health Sciences,The First People's Hospital of Lianyungang from September 2010 to December 2021.AML1-ETO fusion,PML-RARα fusion and CBFβ breakage were detected by fluorescence in situ hybridization(FISH),and the mutations of NPM1,CEBPA,FLT3,RUNX1,ASXL1,KIT and TP53 genes were detected by new generation sequencing(NGS).Results:Among 14 patients,the MS occurred in bone,breast,epididymis,lung,chest wall,cervix,small intestine,ovary,lymph nodes and central nervous system.The tumor cells expressed MPO(13 cases),CD34(7 cases),CD43(8 cases),CD68(7 cases),CD99(8 cases)and CD117(6 cases).Cytogenetic abnormalities were observed in 4 cases,including 3 cases of AML1-ETO fusion and 1 case of CBF β breakage,while no PML-RAR α fusion was detected.There were no significant differences in overall survival(OS)and leukemia-free survival(LFS)between patients with and without AML1-ETO fusion/CBFβ breakage(both P>0.05).Among the 14 patients,the number of NPM1,CEBPA,FLT3-ITD,RUNX1,ASXL1,KIT and TP53 gene mutations was 5,3,5,3,2,2,1.respectively,of which 7 cases had at least one mutation in FLT3-ITD,RUNX1,ASXL1 and TP53 gene.The OS and LFS of patients with FLT3-ITD,RUNX1,ASXL1 or TP53 mutation were shorter than those without mutations(both P<0.01).Conclusion:The genetic and molecular abnormalities of primary MS can be detected by FISH and NGS techniques.FLT3-ITD,RUNX1,ASXL1 or TP53 mutation indicates a worse prognosis,but further clinical studies are needed to confirm it.
9.Effects of Tao Hong Si Wu decoction on circular RNA expression profiles in rats with middle cerebral artery occlusion

Aim To screen and study the effects of Tao Hong Si Wu decoction(THSWD)-mediated treat-ment on circular RNA(circRNA)expression profiles in rats with middle cerebral artery occlusion(MCAO),and investigate the possible roles and molecular mecha-nisms of THSWD.Methods Next-generation RNA sequencing was conducted to identify circRNA expres-sion profiles in MCAO rats after treatment with THSWD and compared with the MCAO model group and control group.Bioinformatics analysis was performed to predict the potential target microRNAs and mRNAs.Gene On-tology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway analyses for the potential target mRNAs were applied to explore the potential roles of differentially expressed circRNAs.RT-qPCR was performed to verify circRNAs with significant differences in expression.Results We identified 87 significantly differentially expressed circRNAs between the MCAO group versus the control group,and 86 sig-nificantly differentially expressed circRNAs between the MCAO group versus the THSWD group.respective-ly.Among them,17 circRNAs induced by the MCAO model were reversed via treatment with THSWD.To demonstrate the roles of mRNAs targeted by DECs,the GO and KEGG databases were used.Further analysis revealed that five circRNAs may play important roles in the development of MCAO.Conclusions The com-prehensive expression profile of circRNAs in rats with middle cerebral artery occlusion after THSWD treat-ment is determined for the first time,suggesting that the therapeutic effect of THSWD on MCAO may be a-chieved by regulating the expression of circRNAs.
10.The regulatory role and mechanism of CP-25 on OAT1 in rats with collagen-induced arthritis

Aim The present study was conducted to investigate the expression of organic anion transporter 1(OAT1)in synovial tissue of collagen-induced arthritis(CIA)rats and the effect of paeoniflorin-6'-O-benzene sulfonate(CP-25),and meanwhile reveal its potential regulatory mechanism.Methods The rat CIA model was established and treated with CP-25(50 mg·kg-1·d-1),paroxetine(15 mg·kg-1·d-),prosta-glandin E receptor 4(EP4)antagonist(3 mg·kg-1·d-1),or a combination of CP-25(50 mg·kg-1·d-1)plus EP4 antagonist(3 mg·kg-1·d-1),while methotrexate(0.5 mg·kg-1·3d-1)was used as the positive control drug.Clinical manifestation of CIA rats,including arthritis index,numbers of swollen joints,paw volume of non-injected hind and clinical scores was measured.Moreover,X-ray imaging of paw,pathological examination of the ankle joint and quantitative assessment were conducted.The concen-tration of serum prostaglandin E2(PGE2)was quanti-fied using the ELISA.Western blot analysis was em-ployed to assess the levels of phospho-G protein-cou-pled receptor kinase 2(p-GRK2),protein kinase A(PKA),EP4 and OAT1 membrane proteins of rat syn-ovial tissues following CP-25 treatment.Results The administration of CP-25 resulted in a reduction in foot swelling,global score,arthritis index,paw volume of non-injected,and numbers of swollen joints in CIA rats.Pathological examination and X-ray analysis re-vealed that CP-25 significantly ameliorated the patho-logical changes and bone erosion degree of CIA rats.CP-25 significantly lowered PGE2 concentration in the serum of CIA rats.In the CIA model group,membrane expression of OAT1 and EP4 in synovial tissues of rats was significantly downregulated.Treatment with CP-25 or paroxetine resulted in a significant upregulation of OAT1 and EP4 expression and the effect of CP-25 on OAT1 was blocked when EP4 antagonist was used to-gether.Furthermore,the expression of p-GRK2 in syn-ovial tissue from CIA rats was significantly upregulat-ed,and however,CP-25 or paroxetine interventions led to a significant reduction in p-GRK2 expression.The expression trend of PKA was similar to that of OAT1.Conclusion OAT1 membrane expression is reduced in the synovial tissue of CIA rats,and CP-25 treatment increases OAT1 membrane expression.The PGE2-EP4-PKA pathway may be involved in the regu-lation of OAT1 by CP-25.
