1.Meaning, Efficacy and Mechanism of Huoxin Pills in Treating Coronary Heart Disease with Qi Deficiency and Blood Stasis Syndrome

Cardiovascular diseases, with high incidence and high mortality, belong to the category of "chest impediment and heart pain" in traditional Chinese medicine (TCM). Chinese medicines have unique effect on the prevention and treatment of cardiovascular diseases with little side effects. Huoxin pills, one of the National Essential Drugs, is formulated based on the basic pathogenesis of weak pulse at Yang and wiry pulse at Yin and the pathological basis of myocardial ischemia and hypoxia and used for treating angina pectoris of coronary heart disease (Qi deficiency and blood stasis syndrome). This medicine is derived from the classic famous prescription and is composed of ten precious Chinese medicinal herbs. It can replenish Qi, activate blood, and warm collaterals to diffuse impediment by enhancing myocardial contractility and cardiac output to improve micro-circulation and increase coronary blood flow, regulating immune functions, alleviating inflammation, detoxifying, and tranquilizing mind. Clinically, it is suitable for patients with angina pectoris caused by the lack of heart Yang, chest tightness, shortness of breath, palpitation, fear of cold for limbs and so on, especially for the elderly with Yang deficiency or the patients with a history of myocardial infarction. On the basis of the available research reports, this paper explains the formula meaning of Huoxin pills from the perspective of the basic pathogenesis of coronary heart disease and predicts its action targets, location and links. Furthermore, we expound the mechanism of action of Huoxin pills based on basic research and clinical evidence-based research, aiming to provide data support and evidence for the clinical application of this medicine.

Fatigue is a widespread and complex physiological phenomenon. Chronic fatigue can lead to cardiovascular dysfunction, mental disorders and other serious pathological reactions. Therefore, how to relieve fatigue accurately and effectively is an important proposition to implement the concept of "Healthy China" in the new era. As an important part of Chinese medicine health industry, Chinese medicine health food has been developing rapidly in recent years. At present, there are 1 157 kinds of anti-fatigue health food on the market in China, most of which are single Chinese medicine and its compound. However, their functions are generally labeled as "anti-fatigue", and their function positioning is too extensive and unclear. With the deepened understanding of fatigue classification and its physiological and pathological basis, it is urgent to be combined with the progress of modern chemical and pharmacological stu-dies to differentiate and precisely position the anti-fatigue health effects of traditional Chinese medicine. For this purpose, the classifications of fatigue were summarized in this paper, and the mechanism of fatigue was explained from the aspects of energy metabolism, accumulation of metabolites, oxidative stress, inflammation, hypothalamus pituitary adrenal axis and so on. We selected 10 traditional Chinese medicines which are most frequently used in health food, analyzed their anti-fatigue effect mechanisms, and summarized the best types of anti-fatigue food, so as to promote the scientific development of anti-fatigue health food industry, expand the market application scope of anti-fatigue health food, better respond to the construction of a healthy China and serve for people's health.

4. Pharmacological Effect and Toxicity of Xanthii Fructus

Xanthii Fructus (XF) is one of important drugs for the treatment of sinusitis and headache.It is commonly used in the treatment of diseases relating to immune abnormalities,such as rheumatoid arthritis,acute and chronic rhinitis,allergic rhinitis,and skin diseases,with a high medicinal value.Modern pharmacological studies have showed a wide range of pharmacological effect and a high medicinal value in XF.However,due to long-term or excessive intake,and improper processing of medicinal materials,toxic reactions have often occurred.Toxicological studies have shown that XF poisoning can cause substantial damage to organs,such as the liver,heart and kidney,especially to liver.This paper reviews the pharmacological action,toxic substances and hepatotoxicity mechanism of XF by systematically reviewing and summarizing relevant literatures on XF both at home and abroad.It is concluded that XF has anti-hypertension,anti-allergic,anti-bacterial,anti-inflammatory,analgesic,anti-tumor and lipid-decreasing effects.The toxic components are mainly atractyloside,carboxy atractyloside and 4'-desulphate-atractyloside.The mechanism of hepatotoxicity induced by XF is closely related to lipid peroxidation,bile cholestasis and hepatocyte energy metabolism.Meanwhile,the discovery of novel biomarkers of hepatotoxicity,such as miRNA-122,also provides new ideals for medical research.Toxic traditional Chinese medicine (TCM) is an important part of TCM.Compatibility or processing can reduce or eliminate toxicity and preserve or increase efficacy.At present,there are few reports on the principle of attenuating the production of XF.The author suggests further strengthening the study on the principle of attenuation of XF,giving full play to the unique curative effect of XF and developing its greater medicinal value.
5.Comparative study on the suitability of chemical fingerprint and anti-platelet aggregation/anti-inflammatory biopotency in the overall quality evaluation of Xiaojin pills

Xiaojin pills, the first choice for clinical treatment of breast hyperplasia, were selected to explore the suitability of a bioactivity assay with chemical fingerprinting for the development of an overall quality evaluation assay. The liposoluble and water-soluble fraction fingerprints of Xiaojin pills were established. The ability to inhibit platelet aggregation and the rate of inhibition of cyclooxygenase-2 (COX-2) for 16 batches of Xiaojin pills from several manufacturers was analyzed; the chemical fingerprints of these samples were correlated with the bioactivity and chemical analysis. The animal protocol was approved by the Committee on the Ethics of Animal Experiments of Affiliated Hospital of Chengdu University of Traditional Chinese Medicine Approval, ID: 2018BL-002. Results showed that the antiplatelet aggregation activity of 16 batches was 0.712-1.278 U∙mg-1, with a relative standard deviation (RSD) of 15.4%. COX-2 inhibition was 52.07%-68.95% and the RSD was 8.91%. The results showed that there was little difference in the biological effects of these samples. However, the chemical fingerprint consistency of these 16 batches of Xiaojin pills was poor, and the similarity of nearly half of the samples was less than 0.9. The total peak area of Xiaojin pills was 32.74%-165.37% across samples, showing very poor chemical consistency. In order to explore the reasons for the poor chemical consistency despite good consistency in the biological assays, the fingerprint chromatogram was analyzed by multivariate statistical analysis. 6.Effects of adult cleft palate surgery combined with pharyngoplasty

Objective:
To assess the voice recoverment of pharynplasty given at the same time with the velopharyngeal closure surgery.
Methods:
36 cases of cleft palate patients aged 18-32 years were divided into control group (18 cases) and experimental group (18 cases). Only cleft palate surgery was given in the control group, while pharyngoplasty combined with the palate closure surgery were given in the experimental group. Wound healing assessment were performed after 1 week , and voice improvement were compared in the 2 groups after 3 months.
Results:
Well wound healings without any infections were achieved in all the cases, except for 1 case had a palate dehiscence 1 week after the operation. Dysarthria and nasal leak were improved significantly in the experiment group (P < 0.05), however the nasal resonance improvement between the 2 groups were not significantly different (P > 0.05).
Conclusion
Adult cleft palate surgery combined with the pharyngeal angioplasty is a recommended method.

8.A Safe and Efficacious Alternative to Roux-en-Y Gastric Bypass for the Treatment of Morbid Obesity and Type 2 Diabetes - One Anastamosis / Mini Gastric Bypass.

Roux-en-y gastric bypass (RYGB) is currently used to treat obesity and metabolic syndrome. It is however technically challenging with a steep learning curve and long operating times. Laparoscopitc mini-gastric bypass (LMGB) is another surgical method that is acclaimed to achieve similar efficacy and yet safe with acceptable complication rates. We reviewedcurrent literature on LMGB on its efficacy and safety profile. Comprehensive search of available literature using a combination of key words was performed, looking out for efficacy and safety end points. Efficacy end points include excess weight loss, change in body mass index (BMI), resolution of metabolic syndrome or T2DM remission. Safety end points include mortality and morbidity rates, short and long term complications. 18 studies were selected with a total of 9392 patients. Follow up range was from 1 year to 6 years with majority of studies achieving 57%-92% excess weight loss (%EWL) within 1 year. Remission of T2DM rates were mostly more than 84%. Several studies reported better %EWL and T2DM remission when compared to SG and RYGB. Overall mortality rate was 0.152%. Morbidity rates vary from 2.7%-12.5%. Some studies reported lower mortality and complication rates in LMGB when compared to SG and RYGB. In summary, MGB is a safe and effective metabolic-bariatric procedure in treating morbid obesity and T2DM. It should be considered an alternative to standard RYGB. Risk of bile reflux, marginal ulcer and anemia needs to be explained to the patient when counselling for such procedure.

9.Stroke-like Migraine Attacks after Radiation Therapy Syndrome.

OBJECTIVETo summarize the clinical presentation, pathogenesis, neuroimaging, treatment, and outcome of stroke-like migraine attacks after radiation therapy (SMART) syndrome, and to propose diagnostic criteria for this disorder.
DATA SOURCESWe searched the PubMed database for articles in English published from 1995 to 2015 using the terms of "stroke-like AND migraine AND radiation." Reference lists of the identified articles and reviews were used to retrieve additional articles.
STUDY SELECTIONData and articles related to late-onset effects of cerebral radiation were selected and reviewed.
RESULTSSMART is a rare condition that involves complex migraines with focal neurologic deficits following cranial irradiation for central nervous system malignancies. The recovery, which ranges from hours to days to weeks, can be partial or complete. We propose the following diagnostic criteria for SMART: (1) Remote history of therapeutic external beam cranial irradiation for malignancy; (2) prolonged, reversible clinical manifestations mostly years after irradiation, which may include migraine, seizures, hemiparesis, hemisensory deficits, visuospatial defect, aphasia, confusion and so on; (3) reversible, transient, unilateral cortical gadolinium enhancement correlative abnormal T2 and fluid-attenuated inversion recovery signal of the affected cerebral region; (4) eventual complete or partial recovery, the length of duration of recovery ranging from hours to days to weeks; (5) no evidence of residual or recurrent tumor; (6) not attributable to another disease. To date, no specific treatment has been identified for this syndrome.
CONCLUSIONSSMART is an extremely rare delayed complication of brain irradiation. However, improvements in cancer survival rates have resulted in a rise in its frequency. Hence, awareness and recognition of the syndrome is important to make a rapid diagnosis and avoid aggressive interventions such as brain biopsy and cerebral angiography.

10.Investigation on cosmetic dermatosis and pathogenic ingredients tested by diagnostic patches in a district of Shanghai City

Objective To ascertain pathogenic ingredients in cosmetics resulting in dermatosis in Changning District of Shanghai. Methods By means of questionnaire, a survey was done among volunteers ,with the history of unwelcome cosmetic reactions in ten communities of Changning District of Shanghai.They received cosmetic series diagnostic patch test. Results Of a total of 122 volunteers,34 were observed with positive reactions, the positive rate being 27.87%.The top five antigens in positive rates were Shellac, Thimerosal, Methylisothiazolinone +Methylchloro -isothiazolinone, Dodecyl Gallate, and Sorbitan Sesquioleate.Meanwhile, The top five different categories of antigens in positive rates were preservatives,colorants,emulsifying agent, antioxidant and surfactant.There were no significant differences found between genders and ages. Conclusion Patch test is an important basis for diagnosis of cosmetic dermatosis.By large sample investigation, common cosmetic pathogenic ingredients in Shanghai can be determined.Then, pathogenic database can be established, which is important in active prevention of cosmetic dermatosis.
