1.The sclerotherapy of polidocanol foam combined with absolute ethanol in the treatment of lower extremity venous malformation in children

		                        			Objective To explore the application value of polidocanol foam combined with absolute ethanol in the treatment of lower extremity venous malformation(VM)in children.Methods The clinical data of 20 children with lower extremity VM were analyzed retrospectively.All of these children were diagnosed with VM of lower extremity by MRI and were treated with digital subtraction angiography(DSA)guided percutaneous sclerotherapy.Different hardening agents were selected according to the classification,the size of the lesion range and the flow rate during the operation.The VM with small lesions range,shallow location and slower velocity were treated with polidocanol foam only,while the VM with larger lesion range and faster velocity were combined treated with abso-lute ethanol.Clinical symptoms and MRI were used to evaluate the curative effect one month after operation.Results Twenty chil-dren received sclerotherapy.After the treatment,15 cases were cured,3 cases showed significantly effective,and 2 cases were effec-tive.Some of the children had pain and swelling of different degrees within 48 hours after operation,who were given symptomatic treatment,and the symptoms disappeared in the short time.The skin of the lesion ulcerated in one case and the ulceration healed after treatment.Conclusion Polidocanol foam combined with absolute ethanol is safe and effective in the treatment of lower extremity VM in children,and the combination can improve the curative effect and reduce the occurrence of postoperative complications.
		                        		
		                        		
		                        		
		                        	
2.Application of MR T1-mapping imaging in the quantitative assessment of renal insufficiency in children

		                        			Objective To explore the possibility of MR Ti-mapping imaging technology to provide additional indicators for clinical quantitative evaluation of renal function in children with renal insufficiency.Methods A total of 13 children with renal insufficiency diagnosed and underwent renal function MRI examination were selected(renal insufficiency group),and 23 children with normal renal function were included as the control group.The difference of renal cortex T1 value between the control group and the renal insuffi-ciency group was compared,and the correlation between renal cortex T1 value and biochemical indexes such as serum creatinine(SCr)and cystatin C(CysC)in the renal insufficiency group was analyzed.Results There was significant difference in T1 value of renal cortex between the renal insufficiency group and the control group(P<0.01).The correlation between T1 value of renal cortex and SCr and CysC in renal insufficiency group was higher(r=0.75,P<0.01;r=0.68,P<0.01).Conclusion MR T1-mapping technol-ogy can non-invasively monitor the renal function status of children with renal insufficiency,and has a suggestive value for clinical quantitative evaluation of renal function in children.
		                        		
		                        		
		                        		
		                        	
3.MRI findings and pathological analysis of testicular teratoma in children

		                        			Objective To analyze the clinical and MRI images of testicular teratoma in children and to compare with pathological results.Methods The clinical and MRI data of 12 children with testicular teratoma confirmed by pathology were analyzed retrospec-tively.Results Of the 12 cases,6 cases were on the left testicle and 6 cases were on the right testicle.The clinical manifestations were all painless testicle enlargement.Of these,6 cases were accompanied by a slight increase of neuron-specific enolase,3 cases had increased alpha-fetoprotein(AFP),10 cases were mature teratoma,and 2 cases were immature teratoma.MRI findings showed that the lesions in 6 cases were cystic,with long T1 and T2 signals as the main signals,no reduction in lipid pressure image signals,no enhance-ment on the enhanced scan,and low signal on diffusion weighted imaging(DWI).The other 6 cases were cystic-solid lesions,mainly with mixed T1 and T2 signals,and showed uneven high signal on DWI.The cystic components were not enhanced on the enhanced scan,and the solid components were mildly to significantly enhanced.Among them,3 cases were accompanied by irregular flaky short T1 and long T2 signals,decreased lipid pressure image signals,1 case was accompanied by speckle long T1 and short T2 signals.And the envelope intact in all 12 cases.Conclusion The MRI findings of testicular teratoma in children are mostly cystic lesions,with few signs of fat and calcification,and no obvious invasion of peripheral structures.AFP examination is helpful for diagnosis.
		                        		
		                        		
		                        		
		                        	
4.Application value of interventional embolization in the treatment of giant infant parotid hemangioma

		                        			Objective To explore the clinical application value of interventional embolization in the treatment of giant infant parotid hemangioma.Methods The clinical data of 17 cases of giant infant parotid hemangioma were analyzed retrospectively.All childrens were performed relevant preoperative examination and CT enhancement examination,with no absolute contraindications for the treat-ment.Interventional embolization was performed under static aspiration compound anesthesia,propranolol was given orally on the same postoperative day.One month after surgery,the clinical effect was evaluated according to the clinical symptoms and ultrasound examination.Results A total of 18 interventional embolization were performed on 17 childrens,who were followed up for 6 months after surgery.The tumor body shrank by more than 80%,after treatment,the child had a symmetrical facial face,no soft tissue rupture,and normal skin color with no impairment of the parotid gland and facial nerve function.Conclusion Interventional embolization is safe and effective in the treatment of giant infant parotid hemangioma,and can become one of the preferred treatment methods for giant infant parotid hemangioma.
		                        		
		                        		
		                        		
		                        	
5.TACE combined with propranolol for treating giant infantile hepatic hemangioma

		                        			Objective To explore the efficacy and safety of TACE combined with propranolol for treating giant infantile hepatic hemangioma(IHH).Methods Ten infants with giant IHH who underwent TACE combined with propranolol were retrospectively enrolled.The technical success rate,complications and recurrence were recorded.The therapeutic effect was evaluated according to clinical symptoms and changes of IHH volume before and 6 months after TACE.Results Totally 15 times of TACE were successfully performed in 10 infants,with technical success rate of 100%.Bleomycin-lipiodol emulsion+poly(vinyl alcohol)(PVA)+coil embolization were performed in 3 cases,while bleomycin-lipiodol emulsion+PVA embolization were performed in 7 cases.Complications of TACE included subcutaneous hematoma around the puncture site in 1 case and transient hypothermia within 24 h after TACE in 2 cases,all alleviated after symptomatic treatments.No other complication occurred.Six months after the last TACE,significantly effective was observed in 9 cases(9/10,90.00%),while effective was noticed in 1 case(1/10,10.00%),with total therapeutic effective rate of 100%.No serious complication such as gallbladder necrosis,liver necrosis,death,nor recurrence of hepatic hemangioma occurred during follow-up.Conclusion TACE combined with propranolol was effective and safe for treating giant IHH.
		                        		
		                        		
		                        		
		                        	
6.Measurement of 239Pu in fecal samples based on inductively coupled plasma-mass spectrometry

		                        			Objective To establish a method for measurement of 239Pu in fecal samples based on inductively coupled plasma-mass spectrometry (ICP-MS), and to provide a novel method for assessing the internal exposure of workers. Methods Fecal samples were collected from workers and labeled. The samples were pretreated with carbonization ashing and microwave digestion devices, purified on TEVA resin, and measured using ICP-MS. Results The detection limit of 239Pu in fecal samples based on ICP-MS was 1.91 × 10−4 Bq. Conclusion In the routine monitoring of class S substances characterized by a 5 μm aerodynamic diameter during 12 months, the committed effective dose corresponding to the detection limit is 0.17 mSv. This value meets the requirements of relevant national standards and ICP-MS can be used as a novel means for accurate evaluation of internal exposure for workers.
		                        		
		                        		
		                        		
		                        	
7.Exploration of 241Am measurement in fecal samples

		                        			Objective To preliminarily study and establish a method for measurement of the transuranic nuclide 241Am in fecal samples, and to provide technical support for internal radiation monitoring of staff. Methods Fecal samples were collected with a self-made stool sampler and treated with a self-made carbonization and ashing furnace. DGA resin was used to separate and purify 241Am from fecal samples. With 243Am as the tracer, the orthogonal method was used for condition optimization. Results The optimum conditions for separation and purification were: the acidity of HNO3 added into the column, 6 mol/L; column flow rate, 0.6 mL/min; and the volume of analytical solution,12 mL. The method based on inductively coupled plasma mass spectrometry showed a detection limit of 9.79×10−4 Bq for 241Am in fecal samples, which was satisfactory and feasible. Conclusion This method fills the vacancy of 241Am measurement in fecal samples to some extent, which is of practical significance for internal radiation monitoring and protection for analysts.
		                        		
		                        		
		                        		
		                        	
8.The value of MRI in the diagnosis of fetal esophageal atresia

		                        			Objective To explore the value of MRI in prenatal diagnosis of fetal esophageal atresia and to provide assistance for prenatal counseling.Methods A retrospective analysis of fetal MRI data of 1 1 cases due to ultrasound findings of hydramnios,small gastric vacuoles or unclear findings,and 22 to 3 5 gestational weeks was performed.MRI sequences included singleGshot fast spin echo (SShFSE),sequence and balanced fast field echo (BFFE),freedom cervical and thoracic cross,sagittal and coronal sectional scans, especially sagittal scans were used to observe fetal esophagus and trachea.Results In 1 1 cases prenatal ultrasound showed that the gastric cavities were small in 6 cases,not seen in 3 cases,and normal in 2 cases;5 cases of amniotic fluid increased,and 6 cases were normal;2 cases were diagnosed as susepected esophageal atersia by ultrasound.Eight patients were diagnosed as fetal esophageal atresia by MRI;one case was missed;two patients underwent MRI to show normal esophageal.9 cases of fetus were diagnosed as esophageal atresia by autopsy or postnatal examination.The MRI findings of fetal esophageal atresia showed that the upper esophagus was expanded in 6 cases,the esophagus was never showed in 3 cases,of which 1 case was missed by MRI,with the esophagus not shown,normal size of stomach bubble and normal amount of amniotic fluid.Among 11 cases,combined with cardiac macrovascular malformation in 4 cases;bilateral kidney volume decreased in 1 case;ventricular dilated in 3 cases;and single umbilical artery in 1 case.6 patients underwent induction of labor during followGup;4 patients were born successfully,including 2 patients undergoing surgery;2 patients with normal esophagus;and 1 patient with 40 weeks of fetal death.The accuracy of MRI in diagnosis of esophageal atresia was 88.9%.Conclusion Using repeated scanning of SShFSE and dynamic BFFE sequence,we can accurately observe fetal esophageal track.Fetal MRI is helpful in the diagnosis of fetal esophageal atresia,which can be used as a complement of prenatal ultrasound diagnosis method.
		                        		
		                        		
		                        		
		                        	
9.Construction of artificial intelligence assisted diagnosis model for colonoscopy

		                        			Objective To establish an artificial intelligence deep learning model using clinical colonoscopy images and video to assist the diagnosis by colonoscopy. Methods More than 600000 colonoscopy images were collected in endoscopic center of the Second Affiliated Hospital of Zhejiang University School of Medicine from 2014 to 2018, and endoscopic experts recorded a large number of high-quality operation video of colonoscopy as analysis data. After repeated discussion by six experts, the classified intestinal sites and pathological features were determined, and fuzzy and confusable images were deleted. The final selection result was approximately 1 out of 4. And then the features of images were marked using an independently developed software. The deep learning algorithm was developed using TensorFlow platform of Google. Results After repeated comparison and analysis of the results of machine training and judgment results combined with pathology from endoscopic experts, the sensitivity of the model for some diseases ( such as colon polyps) was 99% under laboratory conditions. In the clinical colonoscopy test, the sensitivity, specificity, and overall accuracy of this model for diagnosis of colon polyps were 98. 30%(4187/4259), 88. 10% (17620/20000), and 92. 92% [2×98. 30%×88. 10%/(98. 30%+88. 10%)], respectively. The sensitivity and specificity for ulcerative colitis were 78. 32% ( 2671/3410) , and 67. 06%(13412/20000), respectively. The diagnosis time spent on a single image was 0. 5±0. 03 s, and it was the real time for application, including system recognition, text prompt in video image, background record and storage. Conclusion The artificial intelligence assisted diagnosis model developed by our team can identify colonic polyps, colorectal cancer, colorectal eminence, colonic diverticulum, ulcerative colitis, etc. The auxiliary diagnosis model of colon disease can guide beginners to carry out colonoscopy, and can improve lesion detection rate, reduce misdiagnosis rate, and improve the overall operating efficiency of endoscopic center, which is conducive to the quality control of colonoscopy.
		                        		
		                        		
		                        		
		                        	
10.Application of MSCT in the diagnosis of neonatal type (Ⅲ) congenital esophageal atresia

		                        			Objective To study the value of MSCT in the diagnosis of neonatal type (Ⅲ) congenital esophageal atresia.Methods MSCT data of 25 cases with neonatal type (Ⅲ) congenital esophageal atresia confirmed by surgery and pathology were analyzed retrospectively.The full preparation before examination and the image post-processing function were checked,and MSCT data were compared with the findings of surgery.Results By full preparation and application of M PR,SSD technology after scanning,the esophageal proximal blind end,and the distance between the two esophageal blind ends were clearly displayed,furthermore,the opening position,morphology and diameter of the distal tracheoesophageal fistula were also showed,and bronchial tree morphology and inflammation in the lung can be visible.Conclusion MSCT full preparation and application of image post processing function are potential to the clinical diagnosis of congenital esophageal atresia and tracheoesophageal fistula.
