1.Risk Factors for Intracranial Injury Caused by Falls at Home in Korea Using Data from the Emergency Department-based Injury In-depth Surveillance (2011–2018)

Background:
Most unintentional injuries that occur at home are preventable. However, it may be difficult to sufficiently reduce the number of falls occurring at home by only identifying risk factors focused on specific age groups. Therefore, this study aimed to identify the risk factors (especially age and places where injuries occurred at home) for intracranial injury (ICI) caused by unintentional falls at home.
Methods:
Using the Emergency Department (ED)-Based Injury In-depth Surveillance, we analyzed the data of patients who visited the ED due to unintentional falls at home. Risk factors were identified using multivariable logistic regression according to age groups and interactions between place of injury occurrence and age groups, and sex and age groups were assessed.
Results:
In total, 232,124 patients were included in the analysis; older adults had a higher adjusted odds ratio (aOR) 14.05 (95% confidence interval [CI], 12.74–15.49) of ICI than infants. The corridor was associated with ICI in the male pediatric group (aOR, 2.71; 95% CI, 1.08–6.84) and the balcony with the female pediatric group (aOR, 2.04; 95% CI, 1.03–4.04). In the adult group, aOR of kitchen was 1.38 (95% CI, 1.02–1.88) in females and 0.56 (95% CI, 0.48–0.66) in males.
Conclusion
In this study, we identified the risk factors of ICI caused by falls at home using ED-based injury surveillance data. The risk of ICI was different among places of occurrence in the home depending on the age groups and sex.

3.A Retrospective Clinical Epidemiological Study of the Health Care Utilization of the Outpatients in the Dermatologic Field after 1 year's Separation of Dispensary from Medical Practice.

BACKGROUND: Despite 2 years' separation of dispensary from medical practice since year of 2000, many outpatients in Korea have a lack of understanding this separation. In addition, the illegal practices of pharmacists such as medical examination have contributed to the recent problematic situation. OBJECTIVE: We performed this study to evaluate the actual situation of the separation of dispensary from medical practice in the dermatologic field. METHODS: The clinico-epidemiologic analysis of 17 months' data of 18, 230 outpatients was conducted through the available medical records and serial questionnaires. RESULTS: The medical or non-medical institutions that 18, 230 outpatients with skin diseases had chosen at first, were as follows by the order of frequency; pharmacy(78.5%), folk remedies or self-medication(9.8%), dermatologic institutions(5.5%), non-dermatologic medical clinics(3.4%), herb clinics(2.8%). Accordingly, most(94.5%, 17, 223) of the new patients did not select a dermatologic institution for the care of their skin diseases. The patterns of health care utilization of the patients mostly(72.9%) showed a fixed tendency to visit the one particular institution or formula continuously prior to final visit to the research hospitals. Most of the patients(62.8%) firstly visited a pharmacy for their disease care and did not revisit another institution. Since the first visit to a pharmacy, 9.6% of the patients repetitively utilized one or more herb clinic(s) or folk remedies in addition to one or more medical institution(s). The patients utilizing non-dermatologic measures for skin disease care at first, were mostly in their fifties(25.3%). The patients seeking herb medicine or non-dermatologic medical clinics, were in their teens(27.3% and 24.3%, respectively). Of the cases misdiagnosed as another disease or aggravated in the patients choosing non-dermatologic care, fungal infections are most common(24.0%). In front of 97.1% of the patients seeking pharmacy at first, the pharmacists practiced medicine like a physician in a wrong way instead of dispensing a prescription. CONCLUSION: In Korea, the majority of dermatologic clinics has been deprived of a position as an institution for primary care of skin diseases. It is imperative that dermatologists should be granted independent and unconstrained authority in the medical profession for the benefit of their patients.

4.A Case of Metastatic Small Cell Neuroendocrine Carcinoma of the Lung Mimicking a Merkel Cell Carcinoma.

Metastatic small cell neuroendocrine carcinomas(MSCNC) from distant sites, such as the lung, are high grade tumors with poor prognosis. Histopathologically, it is difficult to distinguish between MSCNC and Merkel cell carcinoma solely on the basis of histologic morphology, electron microscopy, and immunohistochemistry. In recent years, differential expression of cytokeratin(CK) 20 and thyroid transcription factor(TTF)-1 has been proposed as useful distinguishing features. It must be emphasized that clinical correlation(e.g. symptomatology, chest X-ray) and a combination of CK 20 and TTF-1 markers, are essential to differentiate these two disease entities. We herein present a case of MSCNC of the lung that might be misdiagnosed as Merkel cell carcinoma.

5.A Case of Angioma Serpiginosum Accompanied with Ipsilateral Nevus Flammeus on the Hand.

Angioma serpiginosum is an acquired benign vascular neoplasm characterized by minute purple-colored, grouped nonpalpable punta on the extremities. However, nevus flammeus is a congenital vascular malformation that shows unilateral red macules. Although the evidence for association between vascular tumor and malformation have been posed, angioma serpiginosum is known to be rarely associated with nevus flammeus. We herein report a rare case of angioma serpiginosum accompanied with nevus flammeus on the ipsilateral hand.

6.Survival Rates and Prognostic Factors in Childhood Medulloblastoma and Supratentorial Primitive Neuroectodermal Tumor.

PURPOSE: Children under 3 years old with medulloblastoma (MB) or supratentorial primitive neuroectodermal tumor (SPNET) have a poor prognosis. They are more susceptible to the deleterious effects of craniospinal radiation (CSR). This study was undertaken to determine the survival rates and prognostic factors of currently used treatment. METHPDS: Retrospective review of 19 children (MB 16, SPNET 3) who were newly diagnosed as MB or SPNET at Seoul National University Children's Hospital from April 1996 to August 2000 was done. Eligible Patients were treated with postoperative induction chemotherapy using Children's Cancer Group (CCG) 9921A or 9931. Patients over 3 year of age received CSR as well. RESULTS: Three Patients with SPNET are all alive without disease. However, 6 patients under 3 years old at diagnosis with MB were all dead whereas only one patient had an event among 10 patients over 3 years old at diagnosis. Among patients with MB, age under 3 years at diagnosis or residual tumors after induction chemotherapy correlated with poor outcome. Metastasis at diagnosis, postoperative residual disease or extent of excision did not have a significant effect on survival statistically. Three-year event-free survival (EFS) of patients with MB was 53%. Three-year EFS of patients over 3 years old at diagnosis with MB was 85.7% and 1-year EFS of under 3 years old at diagnosis with MB was 16.7%. CONCLUSION: The combined use of chemotherapy and CSR in the treatment of MB and SPNET improved survival in children over 3 years old, whereas any treatment failed to improve survival in children under 3 years old. So, there's a need for a new treatment modality to improve survival of children under 3 year of age.

7.Usefulness of Tc-99m MIBI Scan in the Postoperative Follow Up Of Well-Differentiated Thyroid Cancer.

To evaluate the clinical utility of Tc-99m MIBI scan in the detection of residual thyroid tissue or metastatic lesion in postoperative thyroid cancer patients, we compared Tc-99m MIBI scan with I-131 diagnostic and therapeutic scan in 30 postoperative well-differentiated thyroid cancer patients. Thyroglobulin levels of both on and off thyroid hormone medication and antithyroglobulin antibody were tested [Tg(on), Tg(off)l Positive rates for I-131 diagnostic and Tc-99m MIBI scan were 81% and 68% respectively. Concordance between I-131 diagnostic and Tc-99m MIBI scan was observed in 15 cases; 12 positive and 3 negative respectively Among the 6 cases with negative I-131 diagnostic scan and positive Tc-99m MIBI scan, 4 were positive in the therapeutic I-131 scan. We concluded that Tc-99m MIBI scan maybe a useful complementary modality to the currently established method of I131 scan and serum thyroglobulin level in the detection of recurrent or metastatic thyroid cancer, especially in the case of negative I-131 diagnostic scan.

8.Comparison of I-131 Scintigraphy , T1-201 Scintigraphy , and Serum Thyroglobulin in the Postoperative Follow-Up of Differentiated Thyroid Cancer.

To evaluate the utility of I-131, T1-201 scintigraphy, and serum thyroglobulin(Tg) in the follow-up of differentiated thyroid cancer, we compared retrospectively the data from 33 patients who underwent total or subtotal thyroidectomy. I-131 scintigraphy was performed after optimal endogenous TSH stimulation (TSH>50microgram/ml). Total 41 cases of I-l3l and T1-201 scintigraphy pairs were examined. Concomitant serum thyroglobulin levels were measured for 41 pairs of scan. Tg-off levels(that measured after discontinuation of the thyroid hormone) higher than 40ng/ml were considered positive, and Tg-on levels(that measured during the thyroid hormone replacement) higher than 5ng/ml were considered positive. The concordance rates between I-131 therapeutic scintigraphy and T1-201 scintigraphy was 48% in the 38 case of total scan pairs(59% in the 17 cases of postoperative preablation group, and 38% in the 21 cases of postoperative postablation group), Of 17 studies before the I-131 ablation therapy (preablation group), 7 showed positive I-131 therapeutic scintigraphy despite of negative T1-201 scintigraphy. Among patients with negative I-131 therapeutic scintigraphy, no patients had abnormal T1-201 uptake. However, of 21 studies which were done after radioiodine therapy(postablation group) 6 had abnormal uptake on T1-201 scintigraphy which were not seen on I-131 therapeutic scintigraphy, and Tg-off levels also elevated in this 4 of 6 cases. As a result, I-131 therapeutic scintigraphy showed highest positive rate at postoperative preablation follow-up study in differentiated thyroid cancer patients. T1-201 scintigraphy may be useful in postablation studies, and the use of the combined modalities(T1-201 and Tg levels) provides a higher diagnostic yield.

9.Urodynamic Observation In Neuropathic Bladder.

Recently the conception of Neuropathic bladder is being changed with the aid of the modern medico- engineering development and the advanced neurological knowledge. In this observation, we tried to investigate and prove the bladder detrusor and urethral sphincteric function in our suspected neuropathic bladder and its interrelationship on voiding. From Jan. 1980 to April 1981, cystometric-electromyographic (C.M.G.) investigation was performed in 44 cases with voiding difficulty without obstruction in lower urinary tract and suspected neuropathic bladder. The following results were obtained. 1. Cystosphincteric dyssynergia was noted in 12 cases (67%) of detrusor hyperreflexic 18 cases and in 10 cases (57%) of detrusor hyporeflexic 14 cases. Even though they have voiding difficulty or suspected neuropathic 14 cases. Even though they have voiding difficulty or suspected neuropathic bladder, normal C.M.G. findings were noted in 27%, but cystosphincteric dyssynergia in 33%. 2. In cervical vertebra injury, all of 12 cases were detrusor hyperreflexia, when cystosphincteric dyssynergia has noted in 18 cases (67%) and in thoracic vertebra injury 14 cases, detrusor hyperreflexia was noted in 2 cases (14%) and detrusor hyporeflexia in 8 cases (57%), and also cystosphincteric dyssynergia was noted in 6 cases (43%). 3. In sacral lesion 8 cases, detrusor hyperreflexia and detrusor hyporeflexia were noted in 2 cases respectively, and cystosphincteric dyssynergia (75%) was noted higher than that of cervical or thoracic cases. 4. The cases of less than 2 years in duration after vertebral injury or disease have cystosphincteric dyssynergia in 27%, but the cases of more than 2 years have cystosphincteric dyssynergia in 82%.

10.Clinical Observation of Renal Adenocarcinoma.

The clinical observation was made on 34 cases of renal adenocarcinoma in Department of Urology, Yonsei University College of Medicine, during the period from 1970 to 1980 (June). 1) Male to female ratio was 1.8:1, and the age distribution showed that the high peak incidence were fifth decade or later. 2)Chief complaints of its patients were palpable mass(64.7%), hematuria(61.8%), and flank pain(35.3%). 3) Left to right ratio of renal adenocarcinoma was 1.6:1, and 57.1% of the cases were developed in the upper portion of kidney, 35.7% in the lower portion, and 7.2% in the middle portion. 4) The cell types of renal adenocarcinoma showed that 75% of all the cases were clear cell type, 20% in granular cell type, and 5% in spindle cell type. The mean weight of nephrectomized kidney with renal adenocarcinoma was 680 gram. 5) The clinical stage coincided with final stage in 73% of all cases, and the accuracy of clinical staging in stage I and stage III was lower. 6) In the stage VI cases of renal adenocarcinoma, 60% of these cases were metastased to lung at the diagnosed period, 50% to bone, 40% to liver and 20% to lymph nodes except the regional area of affected kidney.
