1.Effects of Bruner's constructivist theory on critical thinking and practice performance of student nurse interns in internal medicine

Objective:To investigate the effects of Bruner's constructivist theory on the critical thinking and practice performance of nursing students interning in internal medicine.Methods:One hundred and fifty-three undergraduate nursing students practicing in the department of internal medicine of a Beijing grade A tertiary hospital were selected by convenience sampling and divided into control group ( n=76) and experimental group ( n=77) according to the order of rotation time. The experimental group received teaching based on Bruner's constructivist theory, while the control group was given traditional teaching. SPSS 24.0 was used to perform the t test. Results:Compared with the control group, the experimental group showed a significantly higher critical thinking score [(306.13±33.00) vs. (325.03±32.09)], a significantly higher exit theory assessment score [(94.74±3.24) vs. (96.94±1.79)], a significantly higher exit skills assessment score [(86.68±11.95) vs. (90.23±9.17)], and a significantly higher degree of satisfaction with teaching [(180.08±13.35) vs. (187.91±14.50); all P<0.05]. Conclusions:Bruner's constructivist theory can help nurse students improve their critical thinking, theoretical and practical performance, and satisfaction with teaching, which enhances the effects of internal medicine nursing teaching.

Purpose/Significance To solve the problem that regional clinical research data are difficult to integrate efficiently,and to promote"Chinese evidence"and"Chinese protocol"in the global clinical research community.Method/Process Based on the standard-ization theory,the data standardization system is proposed,and the construction and application methods of the regional clinical research data platform are explored with the integration of multi-center clinical research data as the starting point.Result/Conclusion The theo-retical framework of the regional clinical research data platform has been preliminarily established,and the clinical research capabilities of tertiary hospitals in Shanghai have been significantly improved.
3.Expression levels of serum CCL19 and sCD163 in patients with systemic lupus erythematosus during pregnancy and their impact on maternal and infant outcomes

Objective:To discuss the changes in the levels of chemokine ligand 19(CCL19)and soluble CD163(sCD163)in serum of the patients with systemic lupus erythematosus(SLE)during pregnancy,and to clarify their effects on the maternal and infant outcomes.Methods:A total of 180 pregnant SLE patients were selected as SLE group and then divided into successful pregnancy group(n=132)and pregnancy failure group(n=48)based on the maternal and infant outcomes.A total of 180 healthy pregnant women underwent prenatal checks during the same period were randomly selected as control group.The general data of the patients in two groups were collected,and the serum levels of CCL19 and sCD163,along with related serum factors,were detected by kits.Multivariate Logistic regression analysis was used to detect the risk factors for pregnancy failure in the SLE patients,and receiver operating characteristic(ROC)curve was used to evaluate the effectiveness of serum CCL19 and sCD163 levels in predicting the pregnancy outcomes of the patients in SLE group.Results:Compared with control group,the levels of complements C3 and C4 in the serum of the patients in SLE group were significantly decreased(P<0.05),and the levels of erythrocyte sedimentation rate(ESR),creatinine(CR),anti-cardiolipin antibody(ACA)-IgG,anti-β2 glycoprotein Ⅰ(anti-β2GPⅠ),CCL19,and sCD163 of the patients were significantly increased(P<0.01).Compared with successful pregnancy group,the levels of complement C3 and C4 pregnancy of the patients in failure group were significantly decreased(P<0.01),and the levels of ESR,CR,ACA-IgG,anti-β2GPⅠ,CCL19,and sCD163 were significantly increased(P<0.01).The serum levels of CCL19,sCD163,ESR,CR,ACA-IgG,and anti-β2GPⅠ were the risk factors for pregnancy failure of the SLE patients(P<0.05 or P<0.01),while the levels of complement C3 and C4 were the protective factors(P<0.01).The area under the ROC curve(AUC)of the serum CCL19 level for predicting the pregnancy failure of the SLE patients was 0.726,and the AUC of serum SCD163 level for predicting the pregnancy failure of the SLE patients was 0.789;the AUC of combination of both markers for predicting the pregnancy failure of the SLE patients was 0.835.The predictive performance of CCL19 and sCD163 for pregnancy outcomes of the SLE patients was superior to either marker alone(Zcombined-CCL19=3.066,P=0.002;Zcombined-sCD163=2.087,P=0.037).Conclusion:The serum levels of CCL19 and sCD163 in the SLE patients during pregnancy are significantly increased,which may cause the poor outcomes in the patients.
4.The predictive value of systemic immune-inflammatory response index combined with tumor burden score in the prognosis of patients after radical resection for intrahepatic cholangiocarcinoma

Objective:To explore the prognostic value of systemic immune-inflammatory index(SII)combined with tumor burden score (TBS) (hereinafter referred to as STS) in patients with intrahepatic cholangiocarcinoma (ICC) after radical resection, and to construct a nomogram model.Methods:The clinical data (including the degree of tumor differentiation, vascular cancer thrombus, and lymph node metastasis, etc.) of 258 ICC patients who received radical resection at People′s Hospital of Zhengzhou University (170 cases, training set) and Cancer Hospital of Zhengzhou University (88 cases, validation set) from January 1, 2016 to January 31, 2020 were retrospectively analyzed and graded by SII, TBS and STS. Multivariate Cox regression analysis were used to identify independent factors affecting the prognosis of patients with ICC. Kaplan-Meier survival curve and receiver operating characteristic curve (ROC) were drawn to evaluate the predictive efficiency of SII, TBS and STS in the overall survival of patients with ICC after radical resection. The nomogram prediction model was constructed and evaluate the performance of nomogram model using consistency index (C-index) and calibration curve.Results:Among 170 ICC patients in the training set, there were 106 cases of SII grade 1 and 64 cases of SII grade 2; 137 cases of TBS grade 1 and 33 cases of TBS grade 2; and 98 cases of STS grade 1, 47 cases of STS grade 2, and 25 cases of STS grade 3. Among 88 ICC patients in the validation set, there were 33 cases of SII grade 1 and 55 cases of SII grade 2; 66 cases of TBS grade 1 and 22 cases of TBS grade 2; and 30 case of STS grade 1, 39 cases of TBS grade 2, and 19 cases of TBS grade 3.The multivariate Cox regression analysis showed that tumor differentiation degree (highly differentiated vs. moderately differentiated HR=0.157, 95% confidence interval(95% CI) 0.045 to 0.546, highly differentiated vs. poorly differentiated HR=0.452, 95% CI 0.273 to 0.750), STS (grade 3 vs. grade 2 HR=1.966, 95% CI 1.148 to 3.469; grade 3 vs. grade 1 HR=1.405, 95% CI 0.890 to 2.216), vascular cancer thrombus ( HR=2.006, 95% CI 1.313 to 3.066), nerve invasion ( HR=1.865, 95% CI 1.221 to 2.850), and lymph node metastasis ( HR=1.802, 95% CI 1.121 to 2.896) were independent influencing factors of overall survival in ICC patients after radical resection (all P<0.05). The Kaplan-Meier survival curve showed that SII, TBS, and STS were independent influencing factors of overall survival in ICC patients (all P<0.05). The results of ROC analysis showed that the areas under the curve of SII, TBS and STS in predicting overall survival of ICC patients after radical resection were 0.566 (95% CI 0.479 to 0.652), 0.585 (95% CI 0.499 to 0.672), and 0.657 (95% CI 0.522 to 0.692), respectively. Tumor differentiation, vascular tumor thrombus, nerve invassion, lymph node metastasis, and STS were included to constract the nomogram model. The C-indexes of the training set and validation set based on the nomogram model were 0.792 (95% CI 0.699 to 0.825) and 0.776 (95% CI 0.716 to 0.833), respectively. The calibration curves of the survival rate of the training set and the validation set were close to the reference lines, and the nomogram model had better predictive ability in both the training set and the validation set. Conclusions:Preoperative STS grading is an effective and practical predictor of overall survival in ICC patients after radical section. Compared with SII and TBS alone, it has better predictive value for the prognosis of patients with ICC.
5.MSCs-derived apoptotic extracellular vesicles promote muscle regeneration by inducing Pannexin 1 channel-dependent creatine release by myoblasts.

Severe muscle injury is hard to heal and always results in a poor prognosis. Recent studies found that extracellular vesicle-based therapy has promising prospects for regeneration medicine, however, whether extracellular vesicles have therapeutic effects on severe muscle injury is still unknown. Herein, we extracted apoptotic extracellular vesicles derived from mesenchymal stem cells (MSCs-ApoEVs) to treat cardiotoxin induced tibialis anterior (TA) injury and found that MSCs-ApoEVs promoted muscles regeneration and increased the proportion of multinucleated cells. Besides that, we also found that apoptosis was synchronized during myoblasts fusion and MSCs-ApoEVs promoted the apoptosis ratio as well as the fusion index of myoblasts. Furthermore, we revealed that MSCs-ApoEVs increased the relative level of creatine during myoblasts fusion, which was released via activated Pannexin 1 channel. Moreover, we also found that activated Pannexin 1 channel was highly expressed on the membrane of myoblasts-derived ApoEVs (Myo-ApoEVs) instead of apoptotic myoblasts, and creatine was the pivotal metabolite involved in myoblasts fusion. Collectively, our findings firstly revealed that MSCs-ApoEVs can promote muscle regeneration and elucidated that the new function of ApoEVs as passing inter-cell messages through releasing metabolites from activated Pannexin 1 channel, which will provide new evidence for extracellular vesicles-based therapy as well as improving the understanding of new functions of extracellular vesicles.

6.Effect and potential mechanism of sodium ferulate on corneal endothelial dysfunction and CEC injury

OBJECTIVE To investigate the effect and potential mechanism of sodium ferulate (SF) on corneal endothelial dysfunction and corneal endothelial cell (CEC) injury. METHODS The male New Zealand rabbits were divided into control group, benzalkonium chloride (BAK) group and BAK+SF group, with 6 rabbits in each group. Except for control group, the other groups were given BAK into the anterior chamber to induce bullous keratopathy model, and BAK+SF group then given SF solution 200 mg/kg intraperitoneally the next day after surgery, twice a day, for consecutive 14 d. The transparency of corneal and edema of corneal stroma in each group of rabbits (before and on the 1st, 7th, and 14th day after surgery) were observed, and the corneal thickness (14th day after surgery) and intraocular pressure (1st to 14th day after surgery) were measured. On the 14th day after operation, the corneal endothelial structure was evaluated and the expressions of functionally related proteins [phalloidin, zonula occludens-1 (ZO-1), Na+/K+-ATPase, Ki67] were detected. On the 14th day after surgery, the corneal tissue was collected in BAK group, the primary rabbit CECs were isolated and cultured, and they were divided into blank group and SF groups with different mass concentrations. The cell viabilities after being cultured for different time, and the protein expressions of Ras homologous gene family A (RhoA), bone morphogenetic protein receptor 1A (BMPR1A) and BMRP2 were determined in each group. RESULTS Compared with BAK group, the transparency of corneal and edema of corneal stroma were gradually improved, and the corneal thickness was significantly decreased in BAK+SF group (P<0.05). The rabbit CECs in BAK+SF group were only damaged to zone B and showed a normal hexagonal endothelial cells structure. The protein expressions of phalloidin, ZO-1, Na+/K+-ATPase and Ki67 in BAK+SF group were significantly increased (P<0.05). When SF concentration was lower than and equal to 200 mg/L, it could promote the proliferation of rabbit CEC, in concentration manner (P<0.05) and time-dependent trend. SF at concentrations of 50, 100, and 200 mg/L could up-regulate the protein expressions of RhoA, BMPR1A and BMPR2 in concentration-dependent manner (P<0.05). CONCLUSIONS SF can improve the transparency of corneal and edema of corneal stroma in bullous keratopathy model rabbits, reduce corneal thickness, maintain the integrity of corneal endothelium structure, and promote the recovery of corneal endothelial function; this compound can promote the proliferation of CEC, the mechanism of which may be related to the activation of RhoA-ROCK-BMP pathway.
7.Construction and evaluation of a predictive nomogram model for the prognosis of intrahepatic cholangiocarcinoma patients undergoing curative resection based on the albumin-bilirubin score and tumor burden score grade

Objective:A predictive nomogram model for the prognosis of intrahepatic cholangiocarcinoma (ICC) patients after curative resection was constructed based on the albumin-bilirubin score and tumor burden score (ATS) grade, and the predictive performance of the nomogram model was evaluated.Methods:Retrospective analysis of clinical data was made, from ICC patients who underwent curative resection at Zhengzhou University People's Hospital and Zhengzhou University Cancer Hospital from January 2016 to January 2020. A total of 258 patients were included in the study, with 140 males and 118 females, with an average age of (56.5±9.5) years. The 258 ICC patients were randomly divided into a training set ( n=174) and a testing set ( n=84) in a 7∶3 ratio. Single-factor and multi-factor Cox regression analyses were performed to identify prognostic factors for ICC patients of the training set, and then a nomogram model was constructed. The performance of the nomogram model was evaluated by using the concordance index (C-index), calibration curve, and risky decision curve analysis. Results:In the training set, univariate Cox regression analysis indicated that albumin-bilirubin (ALBI), tumor burden score (TBS), carcinoembryonic antigen (CEA), tumor differentitation, lymphvascular invasion and ATS significantly influenced overall survival after radical resection for ICC (all P<0.05). Multifactorial Cox regression analysis revealed that ATS grade, CEA, tumor differentiation, lymphovascular invasion, and AJCC N stage are independent risk factors for the prognosis of ICC patients after curative resection (all P<0.05). Assessment of the postoperative survival prediction model based on multifactorial Cox regression yielded a C-index of 0.775(95% CI: 0.747-0.841) for the training set and 0.731(95% CI: 0.668-0.828) for the testing set. The calibration curves for both the training and testing sets indicated strong predictive capability of the model. Additionally, the risk decision curve also suggested high net benefit of the model. Conclusions:The preoperative ATS grade is an independent factor affecting the survival after ICC radical resection. The nomogram model constructed based on ATS grade demonstrates excellent predictive value for postoperative prognosis in ICC patients.

Objective:To compare the differences in the prevalence of mild micro-hepatic encephalopathy (MHE) among patients with cirrhosis by using the psychometric hepatic encephalopathy score (PHES) and the Stroop smartphone application (Encephal App) test.Methods:This prospective, multi-center, real-world study was initiated by the National Clinical Medical Research Center for Infectious Diseases and the Portal Hypertension Alliance and registered with International ClinicalTrials.gov (NCT05140837). 354 cases of cirrhosis were enrolled in 19 hospitals across the country. PHES (including digital connection tests A and B, digital symbol tests, trajectory drawing tests, and serial management tests) and the Stroop test were conducted in all of them. PHES was differentiated using standard diagnostic criteria established by the two studies in China and South Korea. The Stroop test was evaluated based on the criteria of the research and development team. The impact of different diagnostic standards or methods on the incidence of MHE in patients with cirrhosis was analyzed. Data between groups were differentiated using the t-test, Mann-Whitney U test, and χ2 test. A kappa test was used to compare the consistency between groups. Results:After PHES, the prevalence of MHE among 354 cases of cirrhosis was 78.53% and 15.25%, respectively, based on Chinese research standards and Korean research normal value standards. However, the prevalence of MHE was 56.78% based on the Stroop test, and the differences in pairwise comparisons among the three groups were statistically significant (kappa = -0.064, P < 0.001). Stratified analysis revealed that the MHE prevalence in three groups of patients with Child-Pugh classes A, B, and C was 74.14%, 83.33%, and 88.24%, respectively, according to the normal value standards of Chinese researchers, while the MHE prevalence rates in three groups of patients with Child-Pugh classes A, B, and C were 8.29%, 23.53%, and 38.24%, respectively, according to the normal value standards of Korean researchers. Furthermore, the prevalence rates of MHE in the three groups of patients with Child-Pugh grades A, B, and C were 52.68%, 58.82%, and 73.53%, respectively, according to the Stroop test standard. However, among the results of each diagnostic standard, the prevalence of MHE showed an increasing trend with an increasing Child-Pugh grade. Further comparison demonstrated that the scores obtained by the number connection test A and the number symbol test were consistent according to the normal value standards of the two studies in China and South Korea ( Z = -0.982, -1.702; P = 0.326, 0.089), while the other three sub-tests had significant differences ( P < 0.001). Conclusion:The prevalence rate of MHE in the cirrhotic population is high, but the prevalence of MHE obtained by using different diagnostic criteria or methods varies greatly. Therefore, in line with the current changes in demographics and disease spectrum, it is necessary to enroll a larger sample size of a healthy population as a control. Moreover, the establishment of more reliable diagnostic scoring criteria will serve as a basis for obtaining accurate MHE incidence and formulating diagnosis and treatment strategies in cirrhotic populations.
9.The mechanism by which dagglitazine protects cardiovascular and renal function in patients with type 2 diabetes mellitus

Objective:To investigate the protective effect of dagglitazine on cardiovascular and renal function in patients with type 2 diabetes mellitus.Methods:A total of 200 patients with type 2 diabetes patients with poor blood glucose control who received treatment in Huxi Hospital Affiliated to Jining Medical University (Shanxian Central Hospital) from January to December 2019 were included in this study. They were randomly assigned to receive treatment with either metformin ( n = 100, control group) or dagglitazine ( n = 100, observation group) based on diet control and exercise therapy for 12 weeks. Before and after treatment, blood glucose, blood lipid, blood pressure, body mass index, cardiac function and renal function were compared between the control and observation groups. Adverse reactions were also monitored in each group. Results:After treatment, blood glucose, blood lipid, body mass index, cardiac function and renal function in each group were improved compared with before treatment (all P < 0.05). Fasting blood glucose, 2-h postprandial blood glucose, glycosylated hemoglobin, systolic blood pressure, diastolic blood pressure, total cholesterol, triglyceride, low density lipoprotein cholesterol, body mass index, left ventricular end diastolic diameter, serum creatinine, uric acid and cystatin C in the observation group were (6.48 ± 0.72) mmol/L, (8.03 ± 0.77) mmol/L, (6.16 ± 0.63)%, (126.03 ± 3.86) mmHg, (75.62 ± 2.87) mmHg, (3.83 ± 0.17) mmol/L, (2.21 ± 0.36) mmol/L, (2.34 ± 0.15) mmol/L, (23.03 ± 0.55) kg/m 2, (52.10 ± 2.13) mm, (39.97 ± 1.62) μmol/L, (237.17 ± 20.34) μmol/L, (0.64 ± 0.06) mg/L, respectively, which were significantly lower than (8.01 ± 0.84) mmol/L, (10.03 ± 0.90) mmol/L, (7.30 ± 0.72)%, (130.06 ± 4.79) mmHg, (79.60 ± 3.19) mmHg, (4.67 ± 0.37) mmol/L, (2.51 ± 0.57) mmol/L, (2.74 ± 0.19) mmol/L, (24.03 ± 0.60) kg/m 2, (57.22 ± 1.74) mm, (80.00 ± 6.88) μmol/L, (281.62 ± 40.52) μmol/L, (0.76 ± 0.09) mg/L, t = 13.850, 16.866, 11.933, 6.549, 9.263, 20.879, 4.469, 16.982, 12.410, 18.634, 10.626, 9.804, 18.876, all P < 0.001). After treatment, high density lipoprotein cholesterol, left ventricular ejection fraction, and the average distance walked by patients during the 6-min walk test in the observation group were (2.47 ± 0.15) mmol/L, (39.97 ± 1.62)%, (366.50 ± 17.74) m, which were significantly superior to those in the control group [(1.70 ± 0.20) mmol/L, (36.77 ± 1.21)%, (323.70 ± 12.48) m, t = 30.255, 16.435, 19.733, all P < 0.001). There was no significant difference in the incidence of adverse reactions between the two groups ( P > 0.05). Conclusion:Dagglitazine protects cardiovascular and renal function by reducing blood glucose, blood pressure, blood lipid, serum creatinine, serum uric acid levels and decreasing body mass index in patients with type 2 diabetes mellitus.
10.Genetic study of an X-linked agammaglobulinemia pedigree caused by an BTK mutation.

OBJECTIVE:
To explore the genetic pathogenesis of X-linked agammaglobulinemia in two patients for clinical diagnosis and family counseling.
METHODS:
Data was collected from the patients' family including clinical information, blood immunoglobulin level, as well as classification and subgrouping of B lymphocytes. Gene mutations were screened by whole exome sequencing (WES) through next-generation sequencing (NGS), the result was verified with Sanger sequencing.
RESULTS:
A BTK c.1627T>C (p.Ser543Pro) variant was found in the pedigree. The phenotype and variant have co-segregated in the pedigree. The variant was not found in population database. The variant has affected in the kinase domain which contained no benign variants and is harmful as predicted through bioinformatic analysis.
CONCLUSION
