1.Spatial and temporal analysis of MDR-TB epidemic based on SaTScan in Nanning in 2017 - 2021

Objectives To analyze the spatial and temporal aggregation of multidrug resistant pulmonary tuberculosis (MDR-TB) incidence in Nanning at the township / street scale from 2017 to 2021, to explore the spatial and temporal characteristics of the spread of MDR-TB in Nanning, and to provide a scientific reference basis for the health administrative departments to achieve the precise implementation of MDR-TB prevention and control. Methods Based on the data of MDR-TB cases in Nanning from 2017 to 2021, the spatial-temporal scanning analysis software SaTScan v9.7 was used to retrospectively detect and analyze the areas where MDR-TB cases gathered. Results Through simple spatial scanning analysis, it was found that there were three first-class aggregation areas (the aggregation center was Fujiayuan Street, Jiangnan District, 2017, Xinyang Street, Xixiangtang District, 2019, and Zhonghe Town, Yongning District, 2020), and one second-class aggregation area (the aggregation center was Jinchai Town, Mashan County, 2020). Simple time scanning showed that the clustering occurred from May 2019 to December 2020. Temporal and spatial aggregation analysis showed that Xinyang Street in Xixiangtang District was the center of the first-class aggregation area, Zhonghe Town in Yongning District was the center of the second-class aggregation area, and Jinchai Town in Mashan County was the center of the third-class aggregation area. Conclusion The multidrug resistant pulmonary tuberculosis epidemic in Nanning is distributed in an aggregated manner, especially in Xinyang Street, Xixiangtang District, which has the highest spatial and temporal aggregation. It is necessary to focus on and take regional prevention and control measures to control the epidemic.

Glioma is the most common primary central nervous system tumor,mainly derived from glial cells,with strong invasiveness,easy recurrence,and poor prognosis.Glioblastoma is a high-grade glioma with the highest degree of malignancy.The clinical treatment method is mainly surgical resection,supplemented by compre-hensive treatment such as radiotherapy,chemotherapy,and electric field therapy,but the treatment effect is not satisfactory.In recent years,with the rapid development of the field of tumor immunotherapy,CD73 is a novel immune checkpoint related to adenosine metabolism,which can promote tumor progression by inhibiting anti-tumor immune responses and promoting angiogenesis.This article systematically reviews the mechanism of action of CD73 and discusses its biological role and application in glioma,aiming to provide potential treatment options for glioma patients.
3.Establishment of suckling rabbits model for monkeypox virus infection

OBJECTIVE To establish a model of monkeypox virus(MPXV)infection in suckling rabbits and analyze its pathological changes.METHODS Ten-day-old suckling rabbits were divided into two groups:the MPXV group(intranasally inoculated with 100 μL of MPXV at 106 PFU·mL-1)and the normal control group(intranasally inoculated with 100 μL of PBS solution),with 10 rabbits in each group.After infection,such indicators as body weight,survival rate,and mental state of the suckling rabbits were monitored daily for 15 days.On days 3,9,and 15 post-infection,samples of the heart,liver,spleen,lung,kidney,brain,small intestine,and peripheral blood were collected from each group for RT-qPCR to detect viral the load.Histopathological analysis using HE staining was performed on samples collected on day 9 post-infection.RESULTS Suckling rabbits infected with MPXV exhibited weight loss,reduced survival rates,and such symptoms as lethargy and chilliness,which were similar to the clinical manifes-tations observed in humans infected with MPXV.Viral load results indicated the presence of the virus in the heart,liver,spleen,lung,kidney,brain,small intestine,and peripheral blood,with varying degrees of pathological changes in each tissue and organ.CONCLUSION Intranasal inoculation can help estab-lish a model of MPXV infection in suckling rabbits.
4.Assessment of different bone marrow infiltration patterns with MR whole-body diffusion weighted imaging in multiple myeloma

Objective To evaluate the apparent diffusion coefficient(ADC)values of different bone marrow infiltration patterns in multiple myeloma(MM)patients with MR whole-body diffusion weighted imaging(WB-DWI)and to determine the ADC thresholds for different bone marrow infiltration patterns.Methods Nineteen MM patients diagnosed for the first time were selected.The lesions types of each site(cervical spine,ribs,sternum,humerus,scapula,sacral spine,ilium,femur,thoracic spine,and lumbar spine)after the WB-DWI images were visually evaluated,which were divided into focal group(including focal lesion in combined focal and diffuse infiltration)[region of interest(ROI)=141],pure diffuse infiltration group(ROI=150),diffuse lesion in combined focal and diffuse infiltration group(ROI=127),"salt-and-pepper"group(ROI=54),and normal appearance group(ROI=68).ADC values were measured and compared between each group and the receiver operating characteristic(ROC)curve was drawn to distinguish different patterns of bone marrow infiltration.Results There was no statistically significant difference in ADC values between the diffuse lesion in combined focal and diffuse infiltration group and the"salt-and-pepper"group(P>0.99),and there was statistically significant difference in ADC values between the other groups(P<0.05).The ROC curve showed that the area under the curve(AUC)for identifying focal group and the"salt-and-pepper"group was 0.889[95%confidence interval(CI)0.844-0.934],the AUC for identifying pure diffuse infiltration group and the normal appearance group was 0.968(95%CI 0.949-0.987).ADC values were able to accurately and visually differentiate between the different patterns of bone marrow infiltration.Conclusion The ADC values can be used as a quantitative tool to objectively distinguish different bone marrow infiltration patterns in MM patients.
5.Opinion on the definition of "occult hepatitis B virus infection"

The presence of replication-competent HBV DNA in the liver and/or serum of HBsAg-negative individuals is a sufficient and necessary condition for the diagnosis of occult hepatitis B virus infection (OBI). In recent years, Chinese scholars have proposed what is considered a more "rigorous" definition, i.e., on this basis, HBV window period (WP) infection is excluded, which corresponds to a serum HBV DNA level of below the lower limit of detection or a low positive value (< 200 IU/mL). As the definition of WP for HBV infection remains unclear and its duration is highly variable, "HBV DNA < 200 IU/mL" is not the only criterion in OBI patients. Therefore, it is believed that there is still a lack of sufficient basis and operability for the definition of OBI based on "the exclusion of HBV WP infection" and "HBV DNA < 200 IU/mL" as "rigorous" conditions for the diagnosis of OBI.

BACKGROUND:
LY01005 (Goserelin acetate sustained-release microsphere injection) is a modified gonadotropin-releasing hormone (GnRH) agonist injected monthly. This phase III trial study aimed to evaluated the efficacy and safety of LY01005 in Chinese patients with prostate cancer.
METHODS:
We conducted a randomized controlled, open-label, non-inferiority trial across 49 sites in China. This study included 290 patients with prostate cancer who received either LY01005 or goserelin implants every 28 days for three injections. The primary efficacy endpoints were the percentage of patients with testosterone suppression ≤50 ng/dL at day 29 and the cumulative probability of testosterone ≤50 ng/dL from day 29 to 85. Non-inferiority was prespecified at a margin of -10%. Secondary endpoints included significant castration (≤20 ng/dL), testosterone surge within 72 h following repeated dosing, and changes in luteinizing hormone, follicle-stimulating hormone, and prostate specific antigen levels.
RESULTS:
On day 29, in the LY01005 and goserelin implant groups, testosterone concentrations fell below medical-castration levels in 99.3% (142/143) and 100% (140/140) of patients, respectively, with a difference of -0.7% (95% confidence interval [CI], -3.9% to 2.0%) between the two groups. The cumulative probabilities of maintaining castration from days 29 to 85 were 99.3% and 97.8%, respectively, with a between-group difference of 1.5% (95% CI, -1.3% to 4.4%). Both results met the criterion for non-inferiority. Secondary endpoints were similar between groups. Both treatments were well-tolerated. LY01005 was associated with fewer injection-site reactions than the goserelin implant (0% vs . 1.4% [2/145]).
CONCLUSION:
LY01005 is as effective as goserelin implants in reducing testosterone to castration levels, with a similar safety profile.

7.Two decomposition algorithms of dual-energy cone beam CT and their dependence on the phantom sizes

Objective:To analyze the effects of two decomposition algorithms of dual-energy cone beam CT (DECBCT) (direct decomposition and iterative decomposition) on the image quality and material decomposition accuracy of different sizes of phantoms.Methods:Different sizes of imaging parts of patients were simulated using the combination of CatPhan604 phantoms and customized annuluses. CBCT with high energy of 140 kVp and low energy of 100 kVp were acquired using the Varian Edge CBCT system. Then the material decomposition of DECBCT images was performed using the two algorithms. The electron density (ED) and contrast-to-noise ratio (CNR) of each material in the CTP682 module were calculated. They were used to assess the decomposition accuracy and image quality of the two algorithms.Results:Based on the values in the Catphan604 manual, both algorithms have high ED accuracy. Only the ED accuracy of four materials of the smallest sized phantom showed statistical difference ( z = -4.21, 4.30, 2.87, 5.45, P < 0.05), but the average relative error was less than 1%. The CNR of the iterative decomposition algorithm was significantly higher than that of the direct decomposition, increasing by 51.8%-703.47%. The increase in the phantom size significantly reduced the accuracy of ED, and the increased amplitude of the relative error was up to a maximum of 2.52%. The large phantom size also reduced the image quality of iterative decomposition, and the decreased amplitude of CNR was up to a maximum of 39.71. Conclusions:Compared with the direct decomposition, the iterative decomposition algorithm can significantly reduce the image noise and improve the contrast without losing the accuracy of electron density in the DECBCT construction of different sizes of phantoms.
8.Incidence of pretransplant sarcopenia and its effect on prognosis in patients receiving hematopoietic stem cell transplantation: a meta-analysis

Objective:To evaluate the incidence of pretransplant sarcopenia and its effect on prognosis in patients receiving hematopoietic stem cell transplantation (HSCT).Methods:Several electronic databases (PubMed, Embase, Web of Science, Cochrane Library, EBSCO, CINAHL, CBM, CNKI, VIP, WanFang data) were searched from inception to March 2022. Cohort and case-control studies on the outcomes of HSCT patients with pre-transplant sarcopenia were collected, and the quality of the studies was evaluated using the Newcastle-Ottawa Scale. After literature screening, data extraction and quality evaluation, meta-analysis was performed using RevMan 5.3.Results:9 cohort studies were included, of which 6 were of high quality and 3 were of medium quality. The total sample size was 2,255 cases, including 862 cases in the sarcopenia group. The incidence of pretransplant sarcopenia in HSCT patients was 40% (95% CI: 0.35 to 0.46). Pretransplant sarcopenia was associated with decreased overall survival rate ( HR = 1.73, 95% CI: 1.38 to 2.04, P = 0.04) and increased non-relapse mortality after transplantation ( HR = 1.84,95% CI: 1.47 to 2.32, P < 0.01). There was no significant correlation between pretransplant sarcopenia and the incidence of acute graft-versus-host disease ( OR = 1.08, 95% CI: 0.84 to 1.39, P = 0.55). Sarcopenia before transplantation had no significant effect on the duration of hospital stay ( MD = 3.57, 95% CI: -0.13 to 7.26, P = 0.06). Conclusions:Pretransplant sarcopenia was associated with reduced overall survival and increased non-relapse mortality after transplantation. More attention to pretransplant sarcopenia is needed domestically and large-scale, multi-center, prospective studies assessing early screening for sarcopenia are necessary to provide guidance about prevention and treatment strategies.
9.The study on bone marrow-derived mesenchymal stem cells in alleviating tubulointerstitial injury in unilateral ureteral obstruction model

Objective:To investigate the effect and mechanism of bone marrow-derived mesenchymal stem cells (MSCs) in alleviating renal injury in unilateral ureteral obstruction (UUO) model.Methods:MSCs were cultured, and then transplanted into a UUO model through the tail vein. Histology, cell apoptosis, peritubular capillary (PTC) loss and β-catenin expression were examined on the fourteen day after surgery.Results:Renal interstitial fibrosis in the MSCs group was significantly attenuated compared with that in the UUO group (HE: 1.60 ± 0.35 vs. 3.34 ± 0.23; MASSON: 21.32 ± 7.54 vs. 51.08 ± 4.45). Moreover, MSCs treatment inhibited the loss of peritubular capillaries (PTC) (13.56 ± 4.65 vs. 60.16 ± 10.24), cell apoptosis (14.32 ± 3.54 vs. 28.16 ± 6.21) and β-catenin expression (29.33 ± 6.45 vs. 39.51 ± 8.42).Conclusions:MSCs infusion is a promising therapeutic strategy for promoting kidney repair in chronic renal fibrosis model. The mechanism maybe that it inhibits the loss of peritubular capillaries (PTC) , cell apoptosis and β-catenin expression.
10.Clinical analysis of bronchogenic cyst

Objective:To discuss the diagnostic methods, clinical features and treatment options of bronchogenic cysts.Methods:A total of 86 patients with bronchogenic cysts and 5 patients with esophageal cysts and esophageal cysts were selected from January 2011 to May 2020 in the Affiliated Tumor Hospital of Harbin Medical University. There were 37 males and 49 females with bronchogenic cysts, aged 23 to 70(49.27±10.70)years old. According to the location of the disease, the patients were divided into mediastinal type(65 cases, 75.6%); intrapulmonary type(21 cases, 24.4%); bronchogenic cyst originating from the esophagus(9 cases, 10.5%).Results:The preoperative diagnosis coincidence rate was 9.3% in 8 cases. The rate of thoracoscopic surgery(59.3% in 51 cases), compared with the indwelling time of thoracic drainage tube after thoracotomy[(3.80±1.25) days vs.(4.97±1.54)days, P<0.001] and hospital stay[(7.08±1.75) days vs.(9.60±2.58)days, P<0.001] significantly shortened. 65 cases(71.4%, 65/91) were successfully followed up, with a median follow-up time of 34(2-111) months, and no recurrence was found. Conclusion:Bronchial cysts have no characteristic clinical manifestations, and it is difficult to make a clear diagnosis before surgery. Chest MRI has a great advantage in the diagnosis of cysts. For most cases, thoracoscopic surgery can achieve better clinical treatment results and has minimally invasive advantages. It is difficult to distinguish between bronchogenic cysts that originated in the esophagus and esophageal cysts, and there is no significant difference in clinical characteristics.
