A common pathological features of Alzheimer's disease (AD) is the excessive activation of inositol-requiring enzyme-1α (IRE1α) downstream of endoplasmic reticulum stress (ERS) and the abnormal expression of microRNA (miRNA) with a variety of cellular physiological functions. Studies have found that IRE1α and miRNA are not alone in the AD pathologenesis. IRE1α can regulate the expression of miR-200, miR-7, miR-17 and miR-34, and participate in most AD brain diseases such as Aβ deposition, Tau protein hyperphosphorylation and neuronal apoptosis. It is suggested that IRE1α-miRNA signal abnormality is one of the pathological mechanisms of AD. Exercise can prevent and delay AD, but its mechanism is unclear. Studies have found that exercise could interfere with AD by regulating the IRE1αmiRNA signaling pathway. The specific mechanisms of action include: (1) Exercise improves the adaptation of AD brain energy metabolism and alleviates the excessive activation of brain IRE1α signals. (2) Exercise regulates the expression of miRNA in the brain, exerts epigenetic effects, and reduces pathologies such as Aβ and Tau protein aggregation. (3) The IRE1α-miRNA pathway and its downstream protein changes can mediate exercise to resist the development of AD. This article will review the relationship between IRE1α-miRNA and AD pathology and its exercise feedback mechanism, aiming to provide evidence and ideas for AD diagnosis and treatment strategies.

OBJECTIVETo study the role of proximal nerve stimulation at Erb point in the early diagnosis of Guillain-Barré syndrome (GBS) in children.
METHODSThirty-two children who were diagnosed with GBS between October 2013 and December 2014 received neurophysiological examination. Thirty healthy children were used as controls. Compound muscle action potentials and distal motor latency of the median and ulnar nerves were determined and analyzed after nerve stimulation at the wrist, elbow, and Erb point in the two groups. Moreover, F-wave latency of the median nerve and H-reflex latency of the tibial nerve were measured and analyzed in the two groups.
RESULTSThe F-wave and H-reflex latencies were significantly longer in the patient group than in the control group (P<0.05). In thirty-two patients, the numbers of patients with abnormal amplitude, abnormal latency, and conduction block at Erb's point were 24 (75%), 22 (69%), and 20 (62%), respectively. The patient group had significantly lower amplitudes but significantly longer latencies of the ulnar and median nerves at Erb point than the control group (P<0.05). There were no significant differences in the amplitudes and latencies at the wrist and elbow between the two groups (P>0.05).
CONCLUSIONSThe nerve stimulation at Erb point holds promise as a routine examination for the early diagnosis of GBS.

Objective To dynamically analyze the evolutionary process of cerebral edema absorption and the level of local iron in rats with intra-cerebral hematoma by high-field strength 7 Tesla MRI and explore the characteristics and mechanism of secondary injury after intra-cerebral hematoma. Methods Sixteen adult SD rats (about 150 g) were randomly divided into experimental group (n=10) and control group (n=6).Rat models in the experimental group were established by performing injection of 50 μL their own venous blood into their right caudate nucleus accurately. Rats in the control group were used normal saline, instead. After that, head MRI (T2 and T2-star scans) was performed 1,2,3,7 and 14 d after the injection; their imaging features were compared. Results Nine rats in the experimental group survived and 1 died after the operation; in the early days (within 3 d), the T2 weighing imaging showed that the time of relaxation surrounding the hematoma was longer than that in control group, suggesting that the zone of the edema surrounding the hematoma became more clearly. In the early days (within 3 d),T2-weighted imaging was clear, and the time of relaxation surrounding the hematoma increased rapidly, steadily improved 3 d after the operation and reached its peak level 7 dafter the operation; the damage area absorption decreased steadily but turned widening 3 d later and reached the peak 7 d later.T2-star value reached the peak rapidly 3 d after the operation, and then, moderated the downturn. The rats in the control group showed no obvious signal changes under MRI,except those with needle tract injury. Conclusion Secondary injury after intra-cerebral hemorrhage shows a rapidly injury progress in the short terrn at first, and then, has intensify again after a stable period; the local iron diffusion trend is synchronized to the secondary injury, suggesting that iron may play a key role in the mechanism of secondary brain edema.

BACKGROUNDOff-label application of drug-eluting stents (DES) during percutaneous coronary intervention (PCI) was not uncommon in daily practice, however DES in treating Chinese patients with complex lesion subset was under-investigated. The primary objective of the FIREMAN registry was to evaluate the long term efficacy and safety of the Firebird sirolimus-eluting stent (SES) in treating patients with complex coronary lesions. Here we report the mid-term of one-year clinical outcomes and eight-month angiographic follow-up results of FIREMAN registry.
METHODSThe FIREMAN registry was a prospective multi-center registry, which included 1029 consecutive patients undergoing PCI with Firebird SES implantation between September 2006 and July 2007 in 45 centers in China. The clinical follow-up was designed to be performed at 1, 6, 12, 18, 24, 30 and 36 months post index procedure, and non-mandatory angiographic follow-up at 8 months was planned. One hundred percent site monitoring was conducted.
RESULTSLong lesions (59.2%), multi-vessel disease (50.4%), and small vessel disease (31.6%) were mostly found in angiography. Major adverse cardiac events (MACE) occurred in 51 (5.1%) patients at 1 year clinical follow-up, including cardiac mortality in 6 (0.6%), non-fatal myocardial infarction in 11 (1.1%), and target lesion revascularization in 36 (3.5%) of the patients. Definite and probable stent thrombosis (ST) by Academic Research Consortium (ARC) definition occurred in 12 (1.36%) patients at one-year clinical follow-up. The 8-month binary restenosis rate was 5.7% in-segment and 4.3% in-stent, respectively. Late lumen loss was (0.21 ± 0.40) mm in-segment and (0.23 ± 0.36) mm in-stent, respectively. Furthermore, Cox regression analysis revealed that diabetes, small vessel diameter, and chronic total occlusion were independent predictors of ST.
CONCLUSIONSThe results showed that the Firebird SES was effective and safe in treating Chinese patients with complex coronary lesions and occurrence of ST rate at one-year clinical follow-up was acceptable, however further long-term follow-up was still necessary. (NCT00552656)

Objective To investigate the indication and result of parental artery occlusion, embolization with coils, stents in treatment of the traumatic carotid artery pseudoaneurysm. Methods There were six patients with traumatic carotid artery pseudoaneurysm including three patients of cavernous pseudoaneurysm combined with carotid-cavernous fistula (CCF), two with simple traumatic carotid artery pseudoaneurysm and one with traumatic carotid artery pseudoaneurysm that was found after CCF embolization with detachable balloon. The treatment included balloon occlusion for CCF and traumatic carotid pseudoaneurysm in one patient, coil embolization in three and intenal carotid artery balloon occlusion in two. The Clinical manifestations, imaging data, choice of treatment, clinical efficacy, follow-up data and literatures were analyzed to discuss the indications for three treatments. Results There was no cerebral ischemia or surgically-related complication. No epistaxis occurred. The eyeball protrusion restoration was found in three patients and intracranial bruit vanishing in three. Vision was improved one patient after half a year follow-up. The pupils shrank in three patients during follow - up. Digital subtraction angiography (DSA) showed no recurrence of pseudoaneurysm. Conclusions Endovascular treatment is the preferred choice of treatment for traumatic carotid artery pseudoaneurysm. The occlusion or parent artery balloon occlusion can be used when the pseudoaneurysm is combined with CCF. Coil embolization can be used for shoes with narrow neck, stent technology combined with coil embolization for those with wide neck and the covered stent for the rock section of the internal carotid artery.

Objective To analyze the temporospatial characteristics of traumatic brain injury (TBI) caused by Wenchuan earthquake, the distribution of the complex injuries, and the surgical timing and approaches. Methods The clinical data were retrospectively analyzed among 92 patients sustaining TBI in Wenchuan earthquake admitted in our department within 20 days after the injury. Results All the 92 patients came from the northwest Chengdu Champaign. Seventy-six patients were admitted within the initial 12 hours after earthquake, and 10 patients underwent surgical procedures. Forty-seven patients were cured, 31 were transferred to other provinces for further medical care, and 3 died. Conclusions Early TBI accounted for a large proportion of the TBI cases in Wenchuan earthquake, characterized by rapid progression of the condition. Early emergency surgeries can significantly improve the success rate of operations and lower the mortality rate.
