1.Early experience with mechanical hemodynamic support for catheter ablation of malignant ventricular tachycardia

Objective:To explore the role of mechanical hemodynamic support (MHS) in mapping and catheter ablation of patients with hemodynamically unstable ventricular tachycardia (VT), report single-center experience in a cohort of consecutive patients receiving VT ablation during MHS therapy, and provide evidence-based medical evidence for clinical practice.Methods:This was a retrospective cohort study. Patients with hemodynamically unstable VT who underwent catheter ablation with MHS at Beijing Anzhen Hospital, Capital Medical University between August 2021 and December 2023 were included. Patients were divided into rescue group and preventive group according to the purpose of treatment. Their demographic data, periprocedural details, and clinical outcomes were collected and analyzed.Results:A total of 15 patients with hemodynamically unstable VT were included (8 patients in the rescue group and 7 patients in the preventive group). The acute procedure was successful in all patients. One patient in the rescue group had surgical left ventricular assist device (LVAD) implantation, remaining 14 patients received extracorporeal membrane oxygenation (ECMO) for circulation support. ECMO decannulation was performed in 12 patients due to clinical and hemodynamic stability, of which 6 patients were decannulation immediately after surgery and the remaining patients were decannulation at 2.0 (2.5) d after surgery. Two patients in the rescue group died during the index admission due to refractory heart failure and cerebral hemorrhage. During a median follow-up of 30 d (1 d to 12 months), one patient with LVAD had one episode of ventricular fibrillation at 6 months after discharge, and no further episodes of ventricular fibrillation and/or VT occurred after treatment with antiarrhythmic drugs. No malignant ventricular arrhythmia occurred in the remaining 12 patients who were followed up.Conclusions:MHS contributes to the successful completion of mapping and catheter ablation in patients with hemodynamically unstable VT, providing desirable hemodynamic status for emergency and elective conditions.

3.One-stage operation surgical efficacy observation of congenital preauricular fistula infection and static period of inflammation in children.

Objective:To compare the clinical effect of surgical treatment of congenital preauricular fistulas in children during the local infection period and static inflammatory period. Methods:Forty children with congenital preauricular fistula infection treated in our hospital from January 2020 to December 2022 were selected as the experimental group, and 39 children with congenital preauricular fistula inflammation at static period were selected as the control group. The fistula of the two groups of children aged between 1-14 years old was located in front of the foot of the ear wheel or the foot of the ear wheel, and all were unilateral fistulas. The postoperative follow-up was 6 months to 2 years, and the efficacy of the two groups was compared. Results:There was no significant difference in the healing rate of stage Ⅰ and stage Ⅱ between the two groups(P>0.05). There was no significant difference in fistula recurrence rate and satisfaction with the preauricular scar between the two groups after treatment(P>0.05). There was no significant difference in postoperative hospital stay between the experimental group and the control group(P>0.05). Conclusion:The effect of surgical treatment of congenital preauricular fistula in the infected period is similar to that of surgical treatment in the static period of inflammation, and it can reduce the pain of dressing change under local anesthesia in children, avoid the second operation in children, and reduce the economic cost. This treatment method is worthy of clinical promotion. Appropriate incision and resection method were designed according to the fistula and infection sites.

4.Effect of painting psychology training assisted with antipsychotics on recovery effects of patients with schizophrenia during convalescence

Objective:To explore the effects of painting psychology training assisted with antipsychotics on recovery effects of patients with schizophrenia during convalescence.Methods:A total of 160 patients with schizophrenia in Kailuan Mental Health Center from September 2018 to August 2019 during convalescence were randomly divided into painting group and control group, with 80 cases in each group. The control group was treated with antipsychotics, and painting group was given painting psychology training assisted with antipsychotics. At 12 weeks of treatment, the changes of mental symptoms, cognitive function, and intelligence and behaviors were compared between the two groups.Results:After treatment, the positive and negative syndrome scale (PANSS) negative symptom score, general psychopathological symptom score and PANSS total score in painting group were significantly lower than those in control group ( P<0.05), and there was no significant difference in PANSS positive symptom score between the two groups ( P>0.05). The scores of number of persistent responses, number of persistent errors, number of completed classifications, number of correct responses, percentage of conceptual level responses of Wsiconsin card sorting test (WCST) scale were significantly better than those in control group ( P<0.05). The mini-mental state examination (MMSE) score in painting group was significantly higher than that in control group ( P<0.05), and the scores of positive factors of personal cleanliness, social interest and social ability and negative factors of depression, delay, irritability and psychiatric performance of nurse observation scale for inpatient evaluation (NOISE-30) scale were significantly better than those in control group ( P<0.05). Conclusions:Painting psychology training assisted with antipsychotics can significantly improve the negative symptoms of patients with schizophrenia during convalescence, enhance cognitive function, and promote the recovery of intelligence and behaviors of patients.
5.Application of portable eye-fixing device in patients with out-of-hospital A-ultrasonography examination

Objective? To explore the application effects of portable eye-fixing device in patients with out-of-hospital A-ultrasonography examination. Methods? By convenience sampling, 100 patients who underwent A-ultrasonography examination in "Lifeline Express" in Guilin stop of Guangxi in 2017 were selected as subjects in the study and randomly divided into the observation group and control group with 50 cases in each group.Compared with the control group, the observation group was given portable eye-fixing device to help patients fix their eyes during the A-ultrasound examination. The fixing state and examination cooperation between the two groups were compared. Results? After using portable eye-fixing device, the frequency of eye position adjustment in the observation group was (1.34±1.24) times, and that in the control group was (1.86±1.29) times, the difference was statistically significant (t=-2.05, P< 0.05). The cooperation degree of patients in the observation group was better than that in the control group with statistical significance (χ2=7.484, P<0.05). Conclusions? Portable eye-fixing device solves the communication difficulties between doctors and patients caused by regional cultural differences. It can quickly let patients accept instructions and cooperate with doctors to complete the corresponding ophthalmic examinations. It improves the working efficiency of medical staff, especially in out-of-hospital services.
6.Establishment and verification of pectus carinatum model by finite element methods

Objective To explore the establishment of 3D thoracic model by finite element methods,and study the mechanical mechanism of minimally invasive surgery for correcting the chest wall deformity,and provide personalized surgery solution in the future.Methods According to admission and exclusion criteria,we selected 10 cases of pectus carinatum that received chest CT scan.The finite element model of pectus carinatum was established and analyzed by Mimics,ABAQUS,etc.The validity of finite element method for chest wall was verified by comparing the sternal displacement between the simulated values and actual values with the same force.Results The 3D finite element model of pectus carinatum was successfully established and analyzed.The stress distribution of the 10 cases in the posterior ribs was mainly in the 1-6 ribs on both sides,mostly concentrated in the 4th to 6th ribs,and the stress was symmetrical on both sides.Statistical analysis showed that the displacement value of the sternum was correlated,and the validity of the model was verified.Conclusion Mimics,ABAQUS and other finite element modeling and analysis software can effectively establish the thoracic 3D finite element model and mechanical analysis,which can help the further development of personalized minimally invasive surgery for correcting chest wall deformities.
7.Antimicrobial resistance profile and genotypes of Haemophilus influenzae isolates from respiratory tract in children

Objective To investigate the antibiotic resistance profile and ftsⅠ genotypes of Haemophilus influenzae isolates from respiratory tract in children.Methods A total of 141 consecutive nonduplicate clinical strains of H.influenzae were collected from January to March 2016.Antimicrobial susceptibility was determined using Kirby-Bauer method.Beta-lactamase production was detected by nitrocefin disk test.PCR technique was employed for ftsⅠ genotyping.Antimicrobial resistance was compared between differentfisⅠ genotypes.Results The prevalence of β-lactamases was 40.4% (57/141) in H.influenzae isolates.More than half (53.2%,75/141) of the strains were resistant to ampicillin.Mutation offtsI gene was positive in 72.3% (102/141) of the isolates.The dominant genotype of genomic beta-lactamase-negative ampicillin-resistant (gBLNAR) strains was type Ⅲ (72/102,70.6%).The gBLNAR strains showed higher resistance rate to ampicillin and cefuroxime than the genomic beta-lactamase-negative ampicillin susceptible (gBLNAS) strains (P<0.05).Conclusions High prevalence offisⅠgene mutation is found in the strains ofH.influenzae isolated from respiratory tract in children.The dominant genotype of gBLNAR strains was type Ⅲ.Mutation offtsⅠ gene in H.influenzae is associated with higher resistance rate to ampicillin and cefuroxime.
8.Study on application of massive blood transfusion program in massive hemorrhage and early stage of coagulopathy

Objective To study the effect of massive blood transfusion program(MTP) in early massive blood transfusion and preventing coagulopathy.Methods Fifty cases of massive blood transfusion were clinically collected and performed the statistical analysis and processing before and after intervention according to the blood transfusion scheme of MTP.The statistical comparison was performed by using the data before and after intervention.Results PLT was decreased with the increase of transfusing of RBC.After transfusing 15 U of RBC,the PT test,INR and APTT were increased significant(P<0.05).After transfusing 20 U of RBC,the Fib was decreased significant(P<0.05).The MTP had statistical difference between before and after intervention(P<0.01).Conclusion The MTP application has an important significance to prevent the occurrence of coagulopathy and evaluate the coagulation status.
9.Nursing care of a patient with combined cicatricial contracture of skin of large area

Objective To discuss the efficacy of nursing care after penetrating keratoplasty of patients with bilateral adherent leucoma complicated with cicatricial contracture of skin of large area.Methods Mental health care before operation and eye care after operation were taken,which includes eye cleansing,eye medications,the observation and treatment of amniotic membrane grafting,the prevention and treatment of eye complications,skin care,the protection of blood vessels,and discharge instructions.Results Through a series of specific nursing care,no complications developed after operation,and the corneal graft was well healed. Conclusions Penetrating keratoplasty is the most effective treatment for adherent leukoma,and specific nursing care given timely according to patient's condition is beneficial to the healing of corneae.
10.R language-based analysis of big data about drugs prescribed in grass root clinics

Objective To provide the evidence for health management decision-making and rational use of drugs grass root clinics by studying their drug prescription rules.Methods The prescribed drugs in clinics of 5 township health centers from September 2012 to September 2014 were retrieved from The Management Information System of Sichuan Grass Root Medical Institutions.Their big data were analyzed using R language.Results The commonly pre-scribed drugs in clinics were vitamin B6, vitamin C and cefixime tablets, which were usually used in combination. Conclusion Health administrative organizations can strengthen their supervision and management of prescribed drugs and promote their rational use in grass root clinics using unified management information system of grass root medical institutions in combination with information technology .
