1.Infection status and molecular characteristics of Campylobacter isolated from diarrhea patients in Changping District of Beijing

Objective To understand the infection status and molecular characteristics of Campylobacter, and to provide evidence for the prevention and control of Campylobacter infection. Methods A total of 382 stool samples from diarrhea patients in a sentinel hospital in Changping District, Beijing, were collected in 2019. Campylobacter, Salmonella, Diarrhea Escherichia coli, and Vibrio Parahaemolyticus were detected, and the isolated Campylobacter jejuni strains were analyzed by pulsed-field gel electrophoresis (PFGE). Results The positive detection rate of Campylobacter (58 strains, 15.18%) was higher than that of Diarrhea Escherichia coli (37 strains, 9.69%), Salmonella (24 strains, 6.28%) and Vibrio Parahaemolyticus (21 strains, 5.50%), and the difference was statistically significant (χ2=26.735, P=0.000). Campylobacter strains were found in spring (14.71%), summer (18.33%), autumn (15.00%) and winter (10.00%), but there was no significant difference (χ2=2.197, P=0.533). PFGE analysis showed that 30 strains of Campylobacter jejuni had 26 PFGE patterns, and the similarity coefficient was 31.00% -100.00%. Conclusion In the study, the detection rate of Campylobacter is in the first place, and it is detected in four seasons, indicating that the infection of Campylobacter is serious in Changping District of Beijing. It is necessary to pay more attention to the infection of Campylobacter and formulate corresponding policies to prevent the outbreak caused by Campylobacter infection..
2.Multicenter postmarketing clinical study on using pegylated recombinant human gran-ulocyte-colony stimulating factor to prevent chemotherapy-induced neutropenia

Objective: To investigate the efficacy and safety of using pegylated recombinant human granulocyte-colonystimulating factor (PEG-rhG-CSF) in preventing neutropenia in multiple chemotherapy cycles. Methods: A multicenter, prospective, open-label, singlearmstudy was designed. Patients with malignant tumors, such as lung, ovarian, and colorectal cancers, who received multiple cycles of chemotherapy with the prophylactic use of PEG-rhG-CSF for 2-4 consecutive cycles participated in the study. Results: After the prophylactic use of PEG-rhG-CSF, the incidence of grade IV neutropenia decreased from 4.76% (13/273) in the first cycle to 1.83% (5/273), 1.15% (2/174), and 2.08% (2/96) in subsequent cycles. Meanwhile, the incidence of grade III neutropenia decreased from 11.36% (31/ 273) in the first cycle to 6.23% (17/273), 2.87% (5/174), and 3.13% (3/96) in subsequent cycles. The incidence of febrile neutropenia (FN) during the first cycle was 0.73% (2/273). The duration of FN was 2 days in one case and 5 days in another case. FN was not observed during the second, third, or fourth cycle. After the secondary prophylactic use of PEG-rhG-CSF, the incidence of grade IV neutropenia decreased from 25% (7/28) to 3.57% (1/28), 0% (0/28), and 6.67% (1/15) in subsequent cycles. Meanwhile, the incidence of grade III neutropenia decreased from 71.43% (20/28) to 10.71% (3/28), 14.29% (4/28), and 0% (0/15) in subsequent cycles. The proportion of patients who received antibiotic therapy during the entire chemotherapy period was 10.48% (44/420). Conclusion: The application of PEG-rhG-CSF once per chemotherapy cycle can effectively reduce the occurrence of neutropenia in patients under multiple cycles of chemotherapy treatment with good safety.
3.Exploration and practice of experimental skills training for young teachers of basic medical science

With basic medicine teaching faculty becoming younger,it is still in accordance with the traditional training mode about training the young teachers' experimental skills, which brings about some problems such as nonstandard basic operation, weak basic knowledge, lack of systematic skills training and relative lack of research.In practice,we strengthen the basic skills of young teachers through skills training and assessment, focus on the cultivation of young teachers' basic quality by paying attention to theoretical teaching skills and experimental skills; guide teachers to emphasize experimental skills through holding ex-perimental skills competition; promote teachers' experimental skills by integrating the subject research into experiment practice which proved that the experimental skills and teaching level of young teachers get sig-nificantly improved.
4.Evaluation of Xpert MTB/RIF for the Diagnosis of Extrapulmonary Tuberculosis in China.

We evaluate the performance of Xpert MTB/RIF for the diagnosis of extrapulmonary tuberculosis (EPTB) in China. The performance of Xpert was evaluated compared to the composite reference standard (CRS), drug susceptibility testing (DST), and imaging examination. The overall sensitivity and specificity of Xpert were 64.1% (195/304) and 100% (24/24), respectively, using CRS as the gold standard. The sensitivity was significantly higher than that of culture for pus (P<0.05). The proportion of EPTB-positive cases diagnosed by imaging was two times more than that diagnosed using Xpert; however, 6 out of 19 cases may have been overdiagnosed by imaging. Compared to phenotypic DST, the sensitivity and specificity of Xpert were 80% (12/15) and 100% (75/75), respectively. Considering its high sensitivity and specificity, Xpert MTB/RIF may be used as a rapid initial test for EPTB diagnosis, and may also support a quicker decision on the treatment regimen. The combination of imaging and Xpert testing could provide high efficiency and accurate diagnosis of suspected EPTB.

5.Effect of bacailin on the angiogenic regulating factor and migration of laryngeal cancer cells

Objective To explore the effects of bacailin on the angiogenic regulating factor and migration activity of laryngeal cancer cells.Methods Hep-2 was cultured in vitro,(1)in the groups adding phorbol ester,gelatin-zymogram and Transwell assay were used to observe the effects of bacailin on the gelatinase activity of matrix metalloproteinase (MMP) and the migration ability of Hep-2,respectively; (2)in the group without phorbol ester,enzyme-linked immunosorbent assay (ELISA) was used to detect the effects of bacailin on the protein level of endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF).Results Among the control group,the low-dose group,the middle-dose group,and the high-dose group,the activity of MMP-3 of each corresponding group was (100.0 ±0.0) %,(95.0 ±4.9) %,(93.1± 6.2)%,and (90.7 ± 7.3)% with statistically significant difference (t =2.123,2.309,2.412,P <0.05) ; the activity of MMP-9 of each corresponding group was (100.0 ± 0.0) %,(92.3 ± 7.5) %,(89.5 ± 9.3) %,and (85.6 ± 6.1) % with statistically significant difference (t =2.253,2.302,3.708,P <0.05 or P <0.01) ;the protein level of VEGF of each corresponding group was (242.7 ±9.5)ng/L,(230.6 ± 12.7)ng/L,(212.1 ± 15.9)ng/L,and (184.2 ± 23.5)ng/L with statistically significant difference (t =2.408,3.733,4.146,P <0.05 or P <0.01) ; the protein level of bFGF of each corresponding group was (190.7±8.2) ng/L,(181.2±13.0) ng/L,(169.9±17.3)ng/L,and (140.7±9.2)ng/Lwith statistically significant difference (t =2.330,2.922,5.145,P <0.05 or P <0.01) ; Cellular migration rate of each corresponding group was (38.3 ± 5.6) %,(32.9 ± 7.1) %,(27.9 ± 4.4) %,and (23.3 ± 6.0) % with statistically significant difference (t =2.141,3.365,4.027,P < 0.05 or P <0.01).Conclusions Bacailin could suppress the migration of Hep-2 through inhibiting expression of angiogenic regulating factor,which might has clinical value in the treatment of laryngeal cancer.
6.The association between genetic polymorphisms of IL-6 and the susceptibility of chronic rhinosinusitis.

OBJECTIVE:
To investigate the relationship between the promoter polymorphism of IL6 (-174G > C, -572G > C and -597G > A) and chronic rhinosinusitis (CRS).
METHOD:
The case-control study consisted of 123 patients with CRS and 239 controls from a Chinese Han population from Shanghai. The genotypes of the subjects were determined by polymerase chain reaction-restriction fragment length polymorphism and gene sequencing. Besides, the concentrations of the totle immunoglobulin E (TIgE) and eosinophilic cationic protein (ECP) in the blood were also determined.
RESULT:
The -174G > C and -597G > A polymorphisms were not detected in this study population. Significant differences in genotype and allele frequencies of -572C/G were observed between CRS patients and control groups. In CRS patients, the CC, CG, GG genotype frequencies were 69.1%, 29.3%, 1.6%, C, G allele frequencies were 83.7%, 16.3%. In control group, the genotype frequencies were 55.2%, 42.3%, 2.5%, the allele frequencies were 76.4%, 23.6%, respectively. The -572CC genotype was associated with an increased risk of developing CRS (P < 0.05, OR = 1.932, 95% CI, 1.205-3.097). There was no significant differences in the concentrations of the TIgE and ECP among each genotype.
CONCLUSION
IL-6 gene -572G > C polymorphism is associated with the susceptibility to CRS. CC genotype could be an independent risk factor.

7.Analysis on clinical characteristics, treatments and prognostic factors of head and neck non-Hodgkin's lymphoma.

OBJECTIVE:
To study the clinical characteristics, treatments and prognoses of the primary non-Hodgkin's lymphoma (NHL) in head and neck.
METHOD:
Clinical manifestations and clinicopathology characteristics of 107 NHL patients in head and neck were analyzed retrospectively.
RESULT:
The median age of patients with head and neck NHL was 54 years. The most commonly primary site was tonsil (29 cases, 27.10%), and the secondly primary site was nasal cavity (28 cases, 26.17%). The most common histologic subtype was diffuse large B cell lymphoma(46 cases, 42.99%), and the secondly one was NK/T cell lymphoma(24 cases, 22.43%). We found that the combination of rituximab was superior than that of chemotherapy alone. Prognosis depended on both IPI and histological subtype, and IPI was a more dangerous factor than histological subtype.
CONCLUSION
The primary NHL is a common neoplasm in the head and neck region. Characteristic in age, primary site, histologic subtype, treatment and prognostic factors were helpful to understand and treat non-Hodgkin's lymphoma in head and neck.

8.Endoscopic orbital decompression for thyroid-associated ophthalmopathy.

OBJECTIVE:
To evaluate the therapeutic results of endoscopic orbital decompression for thyroid-associated ophthalmopathy.
METHOD:
The records of nine patients (twelve orbits) received endoscopic orbital decompression for thyroid-associated ophthalmopathy were analyzed for changes in visual acuity, intraocular pressure, proptosis, corneal ulceration and movement. The follow-ups ranged from two months to thirty-six months.
RESULT:
Twelve orbits (100%) had improvement in visual acuity (range 0.1-0.7). Ten orbits (83.3%) decreased in intraocular pressure (range 0.2-21.4 mm Hg). Eight orbits (66.70%) decreased in proptosis (one-five mm). The orbit with corneal ulcer was healed after decompression. Diplopia was cured in one of four patients.
CONCLUSION
Endoscopic orbital decompression is a safe and effective procedure for the treatment of thyroid-associated ophthalmopathy.

9.Association of IL-1beta gene -31T/C polymorphism with chronic rhinosinusitis.

OBJECTIVE:
To investigate the relationship between polymorphism of IL-1beta gene 31T/C and chronic rhinosinusitis.
METHOD:
One hundred and twenty-three patients with chronic rhinosinusitis and 239 healthy controls were collected to determine the genotypes by polymerase chain reaction-restriction fragment length polymorphism.
RESULT:
There was significant difference in genotype and allelic frequencies between the CRS and control group (P < 0.05). -31CC genotype was an independent risk factor with CRS (P < 0.05, OR = 1.639). There was no significant difference in TIgE and ECP among these genotypes (P > 0.05).
CONCLUSION
IL-1beta gene 31CC genotype may be an independent risk factor with chronic rhinosinusitis.

10.Nasal anatomic abnormalities and bacterial infection in chronic rhino sinusitis.

OBJECTIVE:
To study the relationship between nasal anatomic abnormalities and the bacteria infection status of maxillary sinus.
METHOD:
The anatomic abnormalities of 115 cases of maxillary sinuses were detected with the CT images and confirmed with chronic infection, which were divided into two groups: high anatomic abnormality group and low anatomic abnormality group. The sinusal contents were sent to bacteria culture, compare the bacteria infection rate and the distribution of bacteria between the two groups.
RESULT:
The bacteria positive rate of the high anatomic abnormality group and low anatomic abnormality group was 90.32% and 56.60% (P < 0.01) respectively. The contribution of gram-positive bacteria and gram-negative bacteria are 47.76% and 52.24% in high anatomic abnormality group, 62.16% and 37.84% in low anatomic abnormality group.
CONCLUSION
Nasal anatomic abnormalities can improve the bacteria infectious rate of maxillary sinuses. High anatomic abnormality may more induces gram-negative bacteria infections,while low anatomic abnormality may more induces gram-positive bacteria infections.
