1.Relationship between Phenotypic Changes of Dendritic Cell Subsets and the Onset of Plateau Phase during Intermittent Interferon Therapy in Patients with CHB

Objective This study aimed to evaluate whether the onset of the plateau phase of slow hepatitis B surface antigen decline in patients with chronic hepatitis B treated with intermittent interferon therapy is related to the frequency of dendritic cell subsets and expression of the costimulatory molecules CD40,CD80,CD83,and CD86. Method This was a cross-sectional study in which patients were divided into a natural history group(namely NH group),a long-term oral nucleoside analogs treatment group(namely NA group),and a plateau-arriving group(namely P group).The percentage of plasmacytoid dendritic cell and myeloid dendritic cell subsets in peripheral blood lymphocytes and monocytes and the mean fluorescence intensity of their surface costimulatory molecules were detected using a flow cytometer. Results In total,143 patients were enrolled(NH group,n = 49;NA group,n = 47;P group,n = 47).The results demonstrated that CD141/CD1c double negative myeloid dendritic cell(DNmDC)/lymphocytes and monocytes(%)in P group(0.041[0.024,0.069])was significantly lower than that in NH group(0.270[0.135,0.407])and NA group(0.273[0.150,0.443]),and CD86 mean fluorescence intensity of DNmDCs in P group(1832.0[1484.0,2793.0])was significantly lower than that in NH group(4316.0[2958.0,5169.0])and NA group(3299.0[2534.0,4371.0]),Adjusted P all<0.001. Conclusion Reduced DNmDCs and impaired maturation may be associated with the onset of the plateau phase during intermittent interferon therapy in patients with chronic hepatitis B.
2.Association of Cytokines with Clinical Indicators in Patients with Drug-Induced Liver Injury

Objective To explore characteristics of clinical parameters and cytokines in patients with drug-induced liver injury(DILI)caused by different drugs and their correlation with clinical indicators. Method The study was conducted on patients who were up to Review of Uncertainties in Confidence Assessment for Medical Tests(RUCAM)scoring criteria and clinically diagnosed with DILI.Based on Chinese herbal medicine,cardiovascular drugs,non-steroidal anti-inflammatory drugs(NSAIDs),anti-infective drugs,and other drugs,patients were divided into five groups.Cytokines were measured by Luminex technology.Baseline characteristics of clinical biochemical indicators and cytokines in DILI patients and their correlation were analyzed. Results 73 patients were enrolled.Age among five groups was statistically different(P=0.032).Alanine aminotransferase(ALT)(P=0.033)and aspartate aminotransferase(AST)(P=0.007)in NSAIDs group were higher than those in chinese herbal medicine group.Interleukin-6(IL-6)and tumor necrosis factor alpha(TNF-α)in patients with Chinese herbal medicine(IL-6:P<0.001;TNF-α:P<0.001)and cardiovascular medicine(IL-6:P=0.020;TNF-α:P=0.001)were lower than those in NSAIDs group.There was a positive correlation between ALT(r=0.697,P=0.025),AST(r=0.721,P=0.019),and IL-6 in NSAIDs group. Conclusion Older age may be more prone to DILI.Patients with NSAIDs have more severe liver damage in early stages of DILI,TNF-α and IL-6 may partake the inflammatory process of DILI.
3.Effect of Tongnao Yizhi Formula (通脑益智方) on Cognitive Function and Gut Microflora in Treating Vascular Cognitive Impairment No Dementia Patients with Spleen and Kidney Depletion,Phlegm and Stasis Obstructing Collaterals Syndrome

ObjectiveTo observe the clinical efficacy and mechanism of Tongnao Yizhi Formula (通脑益智方, TYF) in treating vascular cognitive impairment no dementia (VCIND) with spleen and kidney depletion, phlegm and stasis obstructing collaterals syndrome. MethodsNinety-two VCIND patients with spleen and kidney depletion, phlegm and stasis obstructing collaterals syndrome were randomly divided into control group (42 cases) and treatment group (52 cases). Both groups received routine basic treatment. The control group was given donapezil hydrochloride capsules orally, 5 mg each time, once at night, while the treatment group was given TYF orally, 1 dose per day. Both groups were treated continuously for 3 months. The scores of Mini-Mental State Examination (MMSE), Vascular Dementia Assessment Scale-Cognitive Subscale (VaDAS-Cog), Activity of Daily Living Scale (ADL), and TCM syndrome scores (the primary symptoms such as sluggish thinking, forgetfulness, temperament changes, and language confusion, and secondary symptoms such as weakness of waist and knees, dizziness and headache, occasional tinnitus, fatigue, heaviness of limbs, insomnia and irritability, poor appetite and abdominal distension, numbness of face) were observed before and after treatment in both groups. The changes in gut microflora diversity and flora abundance structure as well as fecal short-chain fatty acids (SCFAs) levels including acetic acid, propionic acid, butyric acid, isobutyric acid, isovaleric acid, valeric acid, and caproic acid were compared between groups. The feces of 20 healthy subjects in the same period were included as reference. Safety was evaluated during the study. ResultsAfter treatment, both groups exhibited significant increases in MMSE scores and decreases in VaDAS-cog scores (P<0.05 or P<0.01), and ADL scores in the treatment group significantly increased (P<0.05). Scores of symptoms including sluggish thinking, forgetfulness, temperament change, language confusion, heaviness of limbs, insomnia, irritability, poor appetite, abdominal distension, and facial numbness as well as the total score significantly decreased in both groups after treatment (P<0.05 or P<0.01). When compared between groups, the treatment group showed substantial reductions in scores of weakness of waist and knees, tinnitus, fatigue, heaviness of limbs, insomnia, irritability, loss of appetite and abdominal distension (P<0.05 or P<0.01). The gut microflora diversity analysis showed that the Shannon index of the treatment group significantly increased after treatment (P<0.05).PCoA analysis and ANOSIM test indicated significant differences between groups, suggesting changes in microflora species (P<0.01). After treatment, the relative abundance of Bacteroidetes and Fusobacteria in the treatment group increased, while the relative abundance of Actinobacteria, Verrucomicrobia and Cyanobacteria decreased (P<0.05); the relative abundance of Faecalibacterium prausnitzii, Bifidobacterium, Lactobacillus, and Ruminococcus increased significantly (P<0.05). Compared to the the gut microflora species diversity of the healthy people, it is indicated that the gut microflora structure in the treatment group was close to that of the healthy people, while there was no such trend in the control group. In the treatment group, acetic acid, propionic acid, and butyric acid in the treatment group were all higher after treatment (P<0.05 or P<0.01). ConclusionsTYF can improve the cognitive ability and quality of life of VCIND patients with spleen and kidney depletion, phlegm and stasis obstructing collaterals syndrome, and this improvement may be related to regulating intestinal microecology.
4.Psychosocial factors influencing recurrence in patients with myasthenia gravis

Objective To explore the psychosocial factors affecting the recurrence of the patients with myasthenia gravis (MG).Methods The retrospective analysis was adopted.One hundred and sixty-one pa-tients with MG treated in the neurosurgery department of this hospital from January 2019 to January 2021 were selected as the research subjects.The psychosocial data of the patients were collected.The correlation be-tween the gender,age of onset,history of underlying diseases,body mass index (BMI),education level,occu-pational nature,per capita monthly family income,living environment,payment method of medical expenses and mental state with the disease recurrence was analyzed.Results Among 161 patients,111 cases developed the recurrence (included in the recurrence group) and 50 cases (included into the stable group).There were statistically significant differences in the age,gender and mental status between the two groups (P<0.05),but there were no statistically significant differences in the history of underlying diseases,BMI,education lev-el,occupational nature,per capita monthly household income,living environment and payment method of med-ical expenses between the two groups (P>0.05).The multivariate logistic regression analysis results showed that the age of onset (OR=0.304,95%CI:0.146-0.634) and anxiety combined with depression (OR=2.706,95%CI:1.090-6.719) were the influencing factors of the recurrence in the patients with MG (P<0.05).Conclusion It is necessary to pay special attention to MG patients with young onset age,anxiety and depression status,which are prone to relapse.
5.Determination of Ultra-Low Levels of 239Pu and 240Pu in Biological Samples

In this work,a reliable analytical method for ultra-low 239,240Pu in biological samples was developed.Sodium carbonate and magnesium acetate were used as saponification agent and ashing aid,respectively,and then Fe(OH)2-Fe(OH)3 was used for co-precipitation separation and enrichment of plutonium extracted from the ashing samples.Then aluminum nitrate was used as a masking agent to eliminate the interference of a large amount of phosphorus in the loading solution on plutonium during the anion exchange separation process.The results showed that the average recovery of plutonium was more than 73%and the decontamination factor for 238U was 3.3×104-5.4×105 in biological samples.The ultimate sample introduction technology(APEX-Ω)was used in determination of plutonium isotopes by inductively coupled plasma mass spectrometry(ICP-MS/MS)with NH3-He as the collision/reaction gas.The 242Pu standard added before sample separation was used as the yield tracer and isotope diluent to calculate the concentration of 239Pu and 240Pu in samples.This method not only effectively suppressed the 238U peak tail and 238U1H+interference,but also greatly improved the analytical sensitivity of plutonium from 850 cps/(pg/g)(ICP-MS/MS)to 8000 cps/(pg/g)(APEX-ICP-MS/MS).The detection limits of 239Pu and 240Pu in 3.5 kg fresh fish samples were as low as 2.56×10-4 mBq/(kg·wet)and 7.33×10-4 mBq/(kg·wet),respectively.The detection limits of 239Pu and 240Pu in 300 g fresh seaweed samples(30 g dry weight)were 2.98×10-3 mBq/(kg·wet)and 8.55×10-3 mBq/(kg·wet).The concentrations of 239Pu and 240Pu in marine biological samples from the China Sea have been successfully analyzed by using the established analytical method.

Ulcerative colitis (UC) is a chronic inflammation of the gastrointestinal tract and is characterized by alternating periods of inflammation and remission. Although UC incidence is lower in Taiwan than in Western countries, its impact remains considerable, demanding updated guidelines for addressing local healthcare challenges and patient needs. The revised guidelines employ international standards and recent research, emphasizing practical implementation within the Taiwanese healthcare system. Since the inception of the guidelines in 2017, the Taiwan Society of Inflammatory Bowel Disease has acknowledged the need for ongoing revisions to incorporate emerging therapeutic options and evolving disease management practices. This updated guideline aims to align UC management with local contexts, ensuring comprehensive and context-specific recommendations, thereby raising the standard of care for UC patients in Taiwan. By adapting and optimizing international protocols for local relevance, these efforts seek to enhance health outcomes for patients with UC.

Crohn’s disease (CD) is a chronic, fluctuating inflammatory condition that primarily affects the gastrointestinal tract. Although the incidence of CD in Taiwan is lower than that in Western countries, the severity of CD presentation appears to be similar between Asia and the West. This observation indicates the urgency for devising revised guidelines tailored to the unique reimbursement system, and patient requirements in Taiwan. The core objectives of these updated guidelines include the updated treatment choices and the integration of the treat-to-target strategy into CD management, promoting the achievement of deep remission to mitigate complications and enhance the overall quality of life. Given the diversity in disease prevalence, severity, insurance policies, and access to medical treatments in Taiwan, a customized approach is imperative for formulating these guidelines. Such tailored strategies ensure that international standards are not only adapted but also optimized to local contexts. Since the inception of its initial guidelines in 2017, the Taiwan Society of Inflammatory Bowel Disease (TSIBD) has acknowledged the importance of continuous revisions for incorporating new therapeutic options and evolving disease management practices. The latest update leverages international standards and recent research findings focused on practical implementation within the Taiwanese healthcare system.

Fermented Chinese medicine has long been used. Amid the advance for preservation of experience, the connotation of fermented Chinese medicine has been enriched and improved. However, fermented Chinese medicine prescriptions generally contain a lot of medicinals. The fermentation process is complicated and the conventional fermentation conditions fail to be strictly controlled. In addition, the judgment of the fermentation end point is highly subjective. As a result, quality of fermented Chinese medicine is of great difference among regions and unstable. At the moment, the quality standards of fermented Chinese medicine are generally outdated and different among regions, with simple quality control methods and lacking objective safe fermentation-specific evaluation indictors. It is difficult to comprehensively evaluate and control the quality of fermented medicine. These problems have aroused concern in the industry and also affected the clinical application of fermented Chinese medicine. This article summarized and analyzed the application, quality standards, and the modernization of fermentation technology and quality control methods of fermented Chinese medicine and proposed suggestions for improving the quality standards of the medicine, with a view to improving the overall quality of it.

9.Performance of screening of contacts of COVID-19 cases in same flight.

Objective: To understand the performance of 2019-nCoV nucleic acid detection in screening of contacts of COVID-19 cases in same flights and provide evidence for the effective screening of persons at high risk for the infection in domestic flights. Methods: The information of passengers who took same domestic flights with COVID-19 cases in China from April 1, 2020 to April 30, 2022 were retrospectively collected,and χ2 test was used to analyze positive nucleic acid detection rates in the passengers in different times before the onsets of the index cases, in different seat rows and in epidemic periods of different 2019-nCoV variants. Results: During the study period, a total of 433 index cases were identified among 23 548 passengers in 370 flights. Subsequently, 72 positive cases of 2019-nCoV nucleic acid were detected in the passengers, in whom 57 were accompanying persons of the index cases. Further analysis of the another 15 passengers who tested positive for the nucleic acid showed that 86.67% of them had onsets or positive detections within 3 days after the diagnosis of the index cases, and the boarding times were all within 4 days before the onsets of the index cases. The positive detection rate in the passengers who seated in first three rows before and after the index cases was 0.15% (95%CI: 0.08%-0.27%), significantly higher than in the passengers in other rows (0.04%, 95%CI: 0.02%-0.10%, P=0.007),and there was no significant difference in the positive detection rate among the passengers in each of the 3 rows before and after the index cases (P=0.577). No significant differences were found in the positive detection rate in the passengers, except the accompanying persons, among the epidemics caused by different 2019-nCoV variants (P=0.565). During the Omicron epidemic period, all the positive detections in the passengers, except the accompanying persons, were within 3 days before the onset of the index cases. Conclusions: The screening test of 2019-nCoV nucleic acid can be conducted in the passengers took the same flights within 4 days before the onsets of the index cases on board. Passengers who seated within 3 rows from the index cases can considered as the close contacts at high risk for 2019-nCoV, for whom screening should be conducted first and special managements are needed. The passengers in other rows can be classified as general risk persons for screening and management.
