1.Simulation-based verification of evaporation efficiency for an air carrier discharge with purified tritium-containing wastewater using high pressure spray technology

Objective:To confirm the transportation and evaporation efficiency of micro droplets sprayed by high-pressure nozzles from purified tritium-containing wastewater with carrier gas in the horizontal evaporation chamber.Methods:A scale test bench with a single high-pressure nozzle had been built based on design conditions and similarity criteria to explore the designed evaporation characteristics of generated micro droplets in the horizontal evaporation chamber. At the same time, combined with experimental data, a suitable evaporation model was constructed using the Discrete Phase Model (DPM) in Ansys Fluent. On this basis, further simulation and analysis were conducted on the evaporation process and efficiency under the condition of multiple nozzles in the desighed horizontal evaporation chamber.Results:For single nozzle tests, the particle size of micro droplets along the nozzle centerline was between 12-50 μm and the particle size distribution was similar to the Rosin-Rammler distribution. Besides, the relative light intensity decreased exponentially with distance, indicating that the particle size and concentration of micro droplets both decreased rapidly, which means the evaporation rate of micro droplets was rapid. For the simulation of multiple nozzles injection in the desighed horizontal evaporation chamber, even for the hardest design condition, the evaporation percentage reached up to 99%, and small amount of the escaping micro droplets continued to evaporate during the process of mixing with other process exhaust until complete evaporation in the vertical chimney section.Conclusions:Under the conditions of desighed sprayed particle size distribution, typical operating conditions and the incoming flow similar to the supposed area, the complete evaporation of the micro droplets can be basically achieved in the horizontal evaporation chamber, so as to ensure the complete evaporation of purified tritium-containing wastewater from the outlet of the chimney.
2.Raoultella ornithinolytica neonatal sepsis: case report and literature review

Objective:To study the clinical characteristics of neonatal sepsis caused by raoultella ornithinolytica.Methods:From January 2010 to December 2020, clinical data of seven cases of neonates with raoultella ornithinolytica sepsis in the Department of Neonatology of our hospital were analyzed. Literature published from the establishment of the databases to December 31, 2020 were searched and reviewed on this topic. The databases included PubMed, Web of Science, Embase database, Wanfang Database, CNKI, National Science and Technology Library and Chinese Science Paper Online.Results:Among the 7 cases admitted to our hospital, 6 male and 1 female, 6 premature and 1 full-term small-for-gestational-age (SGA), 6 patients presented with lethargy, 5 patients had fever and 3 showed dyspnea. 4 patients had necrotizing enterocolitis (NEC), 1 congenital intestinal malrotation, 1 congenital jejunal atresia, 1 intestinal adhesion and stricture. 4 patients had history of surgery. Leucocytosis was found in 3 cases and leukopenia in 1 case. Thrombocytopenia and increased inflammatory indicators were found in all cases. All 7 patients recovered and were discharged. 4 articles on 4 newborn cases (3 males, 1 female including two premature infants) were found for literature review. 3 cases had skin flushing, 3 cases showed dyspnea, 2 cases had fever and 1 case presented with lethargy. 1 case received surgery for congenital heart disease. Leucocytosis was found in 2 cases, leukopenia in 1 case, thrombocytopenia in 2 cases and elevated inflammatory indicators in 3 cases. 1 patient died due to septic shock and the other three recovered and were discharged.Conclusions:Raoultella ornithinolytica neonatal sepsis may occur in infants with intestinal comorbidities, history of invasive procedures, premature birth or full-term SGA and congenital malformations. Most anti-infective therapies are effective. However, if the patient had septic shock, the prognosis is poor.
3.Protective effect of PGSF on myocardial ischemia injury induced by ISO in neonatal rats and its possible mechanism

Objective:To study the protective effect of polygalasaponin F (PGSF) on isoproterenol (ISO)-induced myocardial ischemia (MI) injury in neonatal rats and its possible mechanism.Methods:Fifty newborn Sprague Dawley (SD) rats were randomly divided into control group, model group, low, medium and high dose PGSF groups (5, 10, 50 mg/kg), with 10 rats in each group. The rats in the control group were treated with normal saline; Myocardial ischemia (MI) model was established in model group by subcutaneous injection of isoproterenol (ISO, 85 mg/kg, once a day); The MI model was established in rats of low, medium and high dose PGSF group after intraperitoneal injection of 5, 10 and 50 mg/kg PGSF for 7 days. The cardiac function of rats in each group was evaluated by echocardiography; pathological changes of myocardial tissue of rats in each group were observed by hematoxylin and eosin (HE) staining; The serum activities of troponin I (cTnI), creatine kinase isoenzyme (CK-MB), myoglobin (Mb) and lactate dehydrogenase (LDH) of rats in each group were detected by enzyme linked immunosorbent assay (ELISA); the content of malondialdehyde (MDA) and active oxygen species (ROS) in myocardial tissue were detected ; the expression of nuclear proliferation antigen (Ki67) and caspase-3 protein in myocardial tissue was detected by immunohistochemical staining; The expression of protein kinase B (AKT) and nuclear factor erythroid 2-related factor 2 (Nrf2) protein in myocardium was detected by Western blot.Results:In the model group, the myocardial structure was disordered, the cells were congested and swollen, and there were a lot of inflammatory cells infiltrating and large necrotic foci. The left ventricular wall thickness (LVWT), left ventricular ejection fraction (LVEF), fractional shortening (FS), heart rate (HR) and expression of Ki67 positive protein in the model group were lower than those in the control group (all P<0.05), while the left ventricular end systolic volume (LVESV), activities of cTnI, CK-MB, Mb, LDH in serum, content of ROS and MDA in myocardial tissue and caspase-3 positive protein in the model group were higher than those in the control group (all P<0.05). Compared with the model group, the degree of myocardial pathological changes in neonatal rats of the low, medium and high dose PGSF groups gradually decreased. Compared with model group, the LVWT, LVEF, FS, HR and expression of Ki67 positive protein increased in low, medium and high dose PGSF groups (all P<0.05), while the LVESV, activities of cTnI, CK-MB, Mb and LDH in serum, content of ROS and MDA in myocardial tissue and the expression of caspase-3 positive protein decreased (all P<0.05); Western blot results showed that the relative expression of phosphorylated(p)-AKT/AKT, p-Nrf2/Nrf2 protein in myocardium of model group was lower than that of control group (all P<0.05); The relative expression of p-AKT/AKT, p-Nrf2/Nrf2 protein in myocardium of low, medium and high dose PGSF groups were higher than that in the model group (all P<0.05). Conclusions:PGSF has protective effect on MI injury in neonatal rats, and its mechanism may be related to anti-apoptosis and anti-oxidative stress.
4.Genetic analysis of three fetusesaffected with cardiac rhabdomyomas.

OBJECTIVE:
To report on echocardiographic finding and genetic testing of three fetuses with cardiac rhabdomyoma.
METHODS:
Clinical data of the three fetuses was collected. High-throughput sequencing was carried out to analyze the whole exomes of the three fetuses. Suspected variants were confirmed by Sanger sequencing.
RESULTS:
Multiple hyperechoic masses were found in both ventricles of the three fetuses, suggesting the presence of fetal cardiac rhabdomyoma. Genetic testing revealed that fetus 1 carried a heterozygous c.740G>A (p.W247*) variant of the TSC1 gene, fetus 2 carried a previously known heterozygous c.3352C>T (p.Q1118*) variant of the TSC2 gene. Fetus 3 carried a previously known heterozygous c.1579C>T (p.Q527*) variant of the TSC1 gene. None of their parents carried the same variant. Literature review has identified 109 fetuses with relatively complete data. Cardiac rhabdomyomas in ventricles and ventricular septum was reported in 89, and multiple cardiac rhabdomyoma was reported in 79. Out of the 94 cases who underwent genetic testing, 74 have carried variants of the TSC1 or TSC2 genes.
CONCLUSION
Fetal cardiac rhabdomyoma may present as multiple hyperechoic intraventricular masses. Most of them are associated with other manifestation of tuberous sclerosis. Such cases may warrant prenatal genetic testing.
5.Three-dimensional modeling of female urinary system based on MRI and CT data.

OBJECTIVE:
To reconstruct a three-dimensional model of female urinary system based on magnetic resonance imaging (MRI) and tomography angiography (CTA) data.
METHODS:
MRI and CTA datasets were collected from 20 patients in our department in 2018 for reconstructing 3D models of the bladder urethra in resting state using Mimics19.0 software combined with engineering software. The metric parameters of the bladder urethra were analyzed in the reconstructed 3D model.
RESULTS:
The bladder and urethra were successfully reconstructed using 10 MRI datasets, and the kidney, ureter and bladder were reconstructed using 10 CTA datasets. Using engineering software, we measured a number of cysto-urethral geometric parameters, including the cysto-urethral posterior angle (151.1±17.9°), beta angle (137.3±14.0°), urethral pubic angle (47.8± 12.1°), urethral tilt angle (21.5±7.3°), alpha angle (83.8±13.8°), the posterior pubic space (15.3±3.0 mm), and the urethral striated muscle thickness (2.6±0.6 mm).
CONCLUSIONS
Three-dimensional reconstruction of the anatomical model of the human urinary system provides a platform for studying the fine anatomy of the female urinary system and allows measurement of multiple parameters to better understand the functional differences of the bladder and urethra in different populations.

7.Effects of early enteral nutrition combined with synbiotics on CRP and PCT in patients with severe head injure

Objective:To evaluate the effect of early enteral nutrition supplemented with synbiotics on the levels of C reactive protein (CRP) and Procalcitonin(PCT)level of patients with severe brain injury.The clinical outcomes were also observed.Methods:Forty-seven patients with identified severe brain injury were randomized into study group (n =24) and control group (n =23).All patients received enteral nutrition via nasogastric tube within 24 to 48 h following admission.Patients in the study group were also given synbiotics.Fasting blood samples were collected for detecting the levels of CRP,PCT on day 1,4,7 and 15,respectively.The incidence of lung infection,the length of ICU stay,the cost,the GCS score,the APACHE Ⅱ score,and the mortality in 30 days after administration were collected and compared between 2 groups.Results:Patients in the study group had a lower level of PCT than control group on day 7 and 15 (P <0.05,P <0.05),and a lower level of CRP on day 15 (P <0.01)was also found in the study group.The incidence of lung infection of the study group was also significantly lower than the control group (P < 0.05).A reduced length of hospital stay and a lower cost were found in the study group (P <0.05).The GCS scores in the study group was higher on day 15 when compared with that in the control group (P < 0.05).However,There was no significant change in APACHE Ⅱ score and mortality in 30 days(P > 0.05).Conclusion:Nutrition supplemented with synbiotics leads to a lower rate of infection,a shorter length of ICU stay,a reduced the cost,and a better outcome in patients with severe brain injury.
8.The Current Status of Sexual Education and Needs Assessment among College Freshmen in Kunming

Objective To explore the current status of sexual education and real needs of college freshmen in Kunming.Methods By applying random sampling,350 students from three universities in Kunming were investigated by seff-administered questionnaire,and 310 valid questionnaires were recovered.Fesults 23.9% students confirmed that the schools do not provide courses related to sexual health,and 22.9% of the surveyed students claimed that they even did not heard any sexual education courses in their schools.The main sources of students to acquire information in terms of sexuality and sexual health are network television,video magazines,friends and classmates.Both male and female students are more acceptable to carry out sex education lectures.There were significant differences between male and female students in the form of obtaining sexual education,the content of sexual education and the demand of sexual education in three aspects (P<0.05).Girls tend to carry out sexual education in the form of major courses,but boys tend to the form of playing video or distributing books and periodicals (P<0.05).Girls tend to obtain the courses on sexually disease,AIDS,heterosexual intercourse,or love psychology,but boys tend to contraceptive knowledge and skilIs,or Sexual value orientation (P<0.05).Girls are more likely to acquire the knowledge of masturbation,sexual health,or sexual health,but boys prefer to the knowledge of sexual psychology,parasexuality,or homosexuality (P<0.05).Conclusion As a main body of educational institute for young people,all universities should provide training courses related to sexuality and sexual health at the early stage of higher education in order to equip college students with essential knowledge and skills to have safer and positive impact on their college studies.
9.Prenatal diagnosis and intervention to fetal hydrothorax:five cases analysis

Objective To research the important of prenatal diagnosis and effect of intervention to fetal hydrothorax. Methods The cases of fetal hydrothorax (n=5) were obtained from the Shengjing Hospital,China Medical University between December 2014 and May 2015. All pregnancies were uncomplicated, excluded congenital organic and chromosomal abnormalities during prenatal diagnosis and with a 37 average gestational weeks. The case 1, 2, 4 were unilateral hydrothorax and the case 3, 5 were bilateral. We performed an antenatal thracocentesis to case 1, 2, 3. In case1, the hydrothorax increased rapidly after 5 days, and the patient underwent a cesarean section and ex utero intrapartum treatment (EXIT);in case 2, the fetal heart rate was decreased to 40-50 bpm suddenly during thracocentesis, and we performed an emergent cesarean section and EXIT for the patient; in case 3, the patient underwent thracocentesis and a meanwhile cesarean section and EXIT procedure. We performed a conservative management to case 4, 5, the hydrothorax resolved spontaneously during the pregnancy and after birth, both patients underwent cesarean section. Results All fetuses were survived, the neonates of case 1, 2 and 3 underwent assited mechanical ventilation, thoracic close drainage, then discharged after hydrothorax resolved and feeding tolerance;in case 4, there was no respiratory distress and hospital treatment;in case 5, the neonate underwent assited mechanical ventilation and conservative management, the hydrothorax has resolved gradually. Conclusions The prenatal diagnosis and antenatal intervention (thracocentesis) may play an important role in fetal hydrothorax treatment. In clinical, we should choose different plan according to the gestation weeks and classification of hydrothorax of the patient.
10.Effect of continued nursing on postpartum breastfeeding within 6 months

Objective To explore the effect of continued nursing on postpartum breastfeeding within 6 months. Methods Two hundred and twenty parturient women were randomly divided into the observation group and the control group using random digits table. The control group received breastfeeding knowledge education and nursing skills instruction. The observation group was given continued nursing and breastfeeding guidance . The two groups were compared in terms of breastfeeding rates at 1 , 2 , 3 , 4 , 5 , 6 months . Result The breastfeeding rates of the observation group were significantly higher than those of the control group at each time point ( all P < 0 . 05 ) . Conclusion According to reasons why the parturient women don′t breastfeed , they are given continued nursing and detailed solutions after discharge , to make them confident in breast feeding and improving breastfeeding rate within six months after delivery .
