1.Study on the impact of the specialized centralized procurement for insulin on the daily cost and affordability of insulin in China

OBJECTIVE To evaluate the impact of the specialized centralized procurement policy for insulin on daily cost and affordability of insulin, and provide data support for the enhancement of relevant policies. METHODS In this research, the insulin purchasing data were obtained from provincial centralized procurement platforms in provinces before and after the specialized centralized procurement of insulin (October-December 2021 and October-December 2022), and the cost variations of insulin before and after the centralized procurement were analyzed by the defined daily dose cost (DDDc) of various types of insulins. The changes in the affordability of various types of insulins before and after the specialized centralized procurement were evaluated, using the percentage of annual expenditure on various types of insulins relative to annual per capita disposable income (i.e. the proportion of annual expenditure) as an indicator. RESULTS After the specialized centralized procurement, DDDc of various types of insulins decreased by 20.7%-71.8%, with an average reduction of 45.7%. Moreover, the reduction in DDDc for third-generation insulin exceeded that for second-generation insulin. The reduction in the proportion of annual expenditure on insulin ranged from 24.3% to 73.4%, with an average decrease of 48.5%. Premixed insulin analogs experienced the greatest reduction (73.4%). Following the specialized centralized procurement, DDDc of insulin decreased in all provinces. Except for Guangxi (10.2%), the average proportion of annual expenditure on insulin in the remaining provinces dropped to below 10%. CONCLUSIONS The specialized centralized procurement policy for insulin has significantly reduced insulin costs and improved affordability, thereby alleviating the economic pressure on patients with diabetes. There are notable cost disparities among provinces and among insulin categories, which require attention.
2.The Distribution and Drug Resistance of Pathogenic Bacteria in Respiratory Tract Infections in Children from 2019 to 2022

Objective To analyze the clinical characteristics and the antimicrobial resistance of respiratory tract infection in children in Baoshan City,guide clinicians to rationally apply antibiotics,and improve the success rate of treatment.Methods Retrospective analysis of the distribution characteristics and drug sensitivity results of 1039 strains of pathogens detected in pediatric inpatients of hospitals from 2019 to 2022 was conducted.Results The main pathogens causing the respiratory infections in children in Baoshan area were Streptococcus pneumoniae,Escherichia coli,Staphylococcus aureus,Haemophilus influenzae,Klebsiella pneumoniae and Pseudomonas aeruginosa.Analysis of the drug sensitivity results of pathogenic bacteria with a detected quantity greater than 80 revealed that Streptococcus pneumoniae had a high resistance rate to erythromycin,clindamycin,and compound sulfamethoxazole.The resistance rates of penicillin,ceftriaxone,cefotaxime,and meropenem were P<0.05,and the difference was statistically significan.Methicillin-resistant Staphylococcus aureus(MRSA)was11.1%;CTX/CRO-R-ECO,CTX/CRO-R-KPN,CR-ECO and CR-KPN were lower than the 2021 ISPED level;The P.aeruginosa drug resistance rate and H.influenzae's ampicillin and ampicillin/sulbactam were higher than the 2021 ISPED level.Conclusion The treatment of respiratory tract infections in pediatric patients faces great challenges.The non-standard use of empirical medication has led to the emergence of multidrug-resistant bacteria,and the selection of anti infection treatment drugs is limited.Therefore,it is imperative to grasp the epidemic characteristics and drug resistance of pathogenic bacteria in the local area.
3.Clinical characteristics of patients >65 years old with acute exacerbation of chronic obstructive pulmonary disease and COVID-19 infection

Objective:To investigate the clinical characteristics of patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and corona virus disease 2019 (COVID-19) infection.Methods:Clinical data of AECOPD patients over 65 years old who were diagnosed in the Respiratory and Emergency Departments of the Dongzhimen Hospital, Beijing University of Chinese Medicine from September 2022 to September 2023 were collected. AECOPD patients were divided into a COVID-19 group ( n=29) and a non COVID-19 group ( n=31). The platelet count, white blood cell count, lymphocyte count, neutrophil count, neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), C-reactive protein (CRP), procalcitonin (PCT), partial pressure of oxygen (PO 2), partial pressure of carbon dioxide (PCO 2), D-dimer (D-D), and interleukin-6 (IL-6) were compared between two groups of patients upon admission Confusion, Uremia, Respiratory, BP, Age 65 Years (CURB-65) was used to compare length of hospital stay, AECOPD grading, and mortality endpoint days. Results:There was no statistically significant difference in platelet count, white blood cell count, lymphocyte count, neutrophil count, NLR, and PLR between the COVID-19 group and the non COVID-19 group (all P>0.05). The proportion of males, CRP, PCO 2, D-D, IL-6, and CURB-65 scores in the COVID-19 group were higher than those in the non COVID-19 group, while PCT and PO 2 were lower than those in the non COVID-19 group, with statistically significant difference (all P<0.05). The proportion of AECOPD grade Ⅲ in the COVID-19 group was significantly higher than that in the non COVID-19 group, and the progression rate of the disease was higher in the COVID-19 group (37.9% vs 22.6%, P<0.05). COVID-19 was an independent influencing factor for the progression of AECOPD. Conclusions:Patients over 65 years old with AECOPD infected with COVID-19 have a more pronounced inflammatory response, and CRP, IL-6, and CURB-65 scores can be used as indicators to evaluate the degree of inflammation. AECOPD infected with COVID-19 are more prone to coagulation disorders, hypoxemia, more severe illness, and easier progression, suggesting that COVID-19 infection is an independent influencing factor for the progression of AECOPD.

Graphene,as a new type of two-dimensional nanomaterial,has unique physical and chemical properties,which make it characterized by low resistivity,large specific surface area,good biocompatibility,and high chemical stability.It is expected to develop into an ideal material for highly sensitive electrochemical sensors.With the development of electrochemical sensor technology,graphene based electrochemical sensors with high sensitivity,specificity,speed,and convenience can be used to detect various infectious disease patho-gens.This article summarizes the application of electrochemical sensor detection technology based on graphene nanomaterials in the detection of respiratory pathogens,mosquito borne infectious disease pathogens,intestinal infectious disease pathogens,and other infectious disease pathogens,providing new ideas for achieving rapid detection technology and methods for early detection and diagnosis of infectious diseases.
5.Electroacupuncture alleviates orofacial allo-dynia and anxiety-like behaviors by regulating synaptic plasticity of the CA1 hippocampal region in a mouse model of trigeminal neuralgia

OBJECTIVE To investigate whether electroacupuncture(EA)ameliorates abnormal trigeminal neuralgia(TN)orofacial pain and anxiety-like behavior by altering synaptic plasticity in the hippocampus CA1.METHODS A mouse infraorbital nerve transection model(pT-ION)of neuropathic pain was established,and EA or sham EA was used to treat ipsilateral acu-puncture points(GV20-Baihui and ST7-Xia-guan).Golgi-Cox staining and transmission elec-tron microscopy(TEM)were administrated to observe the changes of synaptic plasticity in the hippocampus CA1.RESULTS Stable and per-sistent orofacial allodynia and anxiety-like behav-iors induced by pT-ION were related to changes in hippocampal synaptic plasticity.Golgi stain-ings showed a decrease in the density of dendritic spines,especially mushroom-type dendritic spines,in hippocampal CA1 neurons of pT-ION mice.TEM results showed that the density of synapses,membrane thickness of the postsynap-tic density,and length of the synaptic active zone were decreased,whereas the width of the synap-tic cleft was increased in pTION mice.EA attenu-ated pT-ION-induced orofacial allodynia and anx-iety-like behaviors and effectively reversed the abnormal changes in dendritic spines and syn-apse of the hippocampal CA1 region.CONCLU-SION EA modulates synaptic plasticity of hippo-campal CA1 neurons,and reduces abnormal oro-facial pain and anxiety-like behavior,providing evidence for a TN treatment strategy.
6.Literature Study and Suggestions on the Development Situation of Online Drugstores in China

OBJECTIVE:To p rovide policy suggestions for the development and supervision of online drugstores in China. METHODS:By literature review ,the relevant literatures and policy documents about the development of online drugstores were searched,screened and sorted out to summarize the development process of China ’s internet drug trading mode and policy ,analyze the development status of China ’s online drugstores ;on the basis of the development experience of some developed countries ,the suggestions were put forward for China ’s relevant policies. RESULTS & CONCLUSIONS :China’s medical e-commerce started relatively late ,and the policies related to online drug trading were basically released during last 20 years. The number of online drugstores showed a growing trend. The online drugstores were geographically allocated widely but unevenly. They sold many varieties,and policies on selling prescription medicines were gradually liberalized. These online drugstores provided diverse payment and delivery choices ,but their pharmaceutical service needed to be improved. The development models of online drugstores in the United Kingdom ,the United States and Germany could be referred. According to the current situation of online drugstores in China , combined with the development mode and operation strategy in foreign countries ,it is suggested to improve relevant laws and regulations,and strengthen legal support ;strengthen the government supervision to ensure drug safety ;use the industrial power to standardize medicine operation behavior ;set up the third part trading platform to promote medical insurance payment.
7.Effect of Cd on autophagy-related genes of celery.

Autophagy is one of the most common protective mechanisms during plant stress response. We studied the effect of exogenous Cd on autophagy in celery, by using transcriptome sequencing technique to analyze the differentially expressed genes under different Cd concentrations (0, 2, 4 and 8 mg/L). Eight differentially expressed autophagy-related genes were screened and identified by qRT-PCR. Cd had obvious toxic effect on celery, in a dose-dependent manner. Eight differentially expressed autophagy-related genes were screened, among which ATG8a, ATG8f, ATG13, AMPK-1 and AMPK-2 were up-regulated, whereas ATG12, VPS30 and VPS34 were first up-regulated and then decreased. The up-regulated expression of differential genes may resist Cd toxicity by increasing autophagosome structures; however, 8 mg/L Cd exceeded the autophagosome tolerance limit of celery, resulting in decreased expression of multiple autophagy-related genes. The above results can provide help for subsequent functional study of autophagy-related genes, and provide a reference for further exploring the tolerance mechanism of celery to Cd toxicity.
8.Comparison of effects of metaraminol and norepinephrine on elderly patients with septic shock

Objective:To compare the effect of metaraminol and norepinephrine (NA) on elderly patients with septic shock.Methods:In this multicenter prospective cohort study elderly patients with septic shock were divided into the metaraminol group ( n=100) and NA group ( n=100). The 28-day survival and average hospital stay, the hemodynamics and renal function of the patients were observed. Results:The average hospital stay in the metaraminol group was lower than that in the NA group ( P < 0.05). Blood pressure, heart rate, arterial lactate, B-type natriuretic peptide, left ventricular ejection fraction and 28-day survival had no significant differences between the two groups (all P > 0.05). The average urine output at day 3 and 7 after the administration of vasopressor in the metaraminol group were higher than those in the NA group (all P< 0.05), blood urea nitrogen at day 3, 7 and 14 after the administration of vasopressor in the metaraminol group were lower than those in the NA group (all P < 0.05), and the creatinine at day 3, 7 and 14 after the administration of vasopressor and the rate of renal replacement therapy in the metaraminol group were lower than those in the NA group (all P < 0.05). Conclusions:Metaraminol has similar effect to norepinephrine in improving 28-day survival and hemodynamics of elderly patients with septic shock, but is superior to norepinephrine in reducing the average hospital stay and protecting renal function.
9.Short-and long-term effects of olfactory ensheathing cells in the treatment of chronic spinal cord injury: a meta-analysis

BACKGROUND: A number of clinical trials addressing olfactory ensheathing cells for the treatment of chronic spinal cord injury have been conducted in the world, but the efficacy and safety are still controversial. OBJECTIVE: To evaluate the safety and efficacy of olfactory ensheathing cell transplantation for chronic spinal cord injury, and to further compare its short-and long-term efficacy. METHODS: PubMed, Cochrane Library, EMBASE, CNKI and WanFang databases were searched at July 23, 2018 for retrieval of clinical trials addressing olfactory ensheathing cells in the treatment of chronic spinal cord injury. Types and cases of adverse events during the safety trial should be recorded in detail. In the enrolled studies, American Spinal Injury Association scale was used to assess the motor, light touch, and pinprick scores of spinal cord injury patients before and after cell transplantation. The follow-up time was recorded. Systematic evaluation of efficacy data was performed using Review Manager 5.3. RESULTS AND CONCLUSION: Both short-and long-term follow-up data showed that the neurological function of patients was significantly improved after olfactory ensheathing cell transplantation (P < 0.05) , and the results were homogeneous (I2 < 50% and P> 0.1). However, the long-term efficacy was not as good as the short-term efficacy, which may be related to chronic rejection and olfactory ensheathing cell survival. The overall adverse event rate was 8.99%, and no complications associated with olfactory ensheathing cells occurred. These findings show that olfactory ensheathing cell transplantation is effective and safe in the treatment of chronic spinal cord injury, but it is still necessary to explore more minimally invasive approaches to reduce surgical complications. In addition, a large number of high-quality experiments and clinical trials are warranted to confirm factors affecting the long-term efficacy of olfactory ensheathing cell transplantation.
10.Neonatal Langerhans cell histiocytosis: a case report

We reported a case of neonatal Langerhans cell histiocytosis (LCH) with his clinical manifestations, examinations, diagnosis and treatment. This infant was hospitalized due to the rash and poor response. He was diagnosed as LCH after admission by imaging, skin biopsy and immunohistochemistry. The infant died because his parents refused all possible treatments. At the moment, no effective treatment is available for LCH patients, while the main managements are infection control and necessary supportive care. Pulmonary infections often contribute to the death of LCH patient and can aggravate the situation by stimulating histiocytic hyperplasia.
