1.Investigation on the utilization of parenteral nutrition preparations in 12 Hospitals of Jiangsu

Objective To investigate the use of parenteral nutrition preparations in Jiangsu Province,and to provide reference for the standardized management of parenteral nutrition preparations.Methods 720 cases using parenteral nutrition preparations from January 2017 to June 2017 in the department of general surgery of 12 hospitals in Jiangsu province were selected.The rate of nutritional risk screening,the indications of parenteral nutrition,the way of infusion,the rationality and economy of the prescriptions were retrospectively evaluated.The calorie,amino acid content,non-protein calorie/nitrogen ratio,glycolipid ratio and cation concentration of the patients received total parenteral nutrition were calculated.Results The total costs of parenteral nutrition preparations of 720 cases were 1.614 1 millions,and 346 cases did not have the indications for parenteral nutrition.The results of prescription comment showed that only 16 patients were screened for nutritional risk by Nutritional Risk Screening 2002 tool at admission.544 cases were intravenous dripped with amino acid and fat emulsion from peripheral vein.In the 176 total parenteral nutrition prescriptions,there were 39 non-protein calorie/nitrogen ratio cases,15 glycolipid ratio cases,69 cation concentration cases,61 calorie cases and 32 amino acid content cases failing to comply with the recommendation of the guidelines.Only 31 total parenteral nutrition prescriptions were completely reasonable.Conclusion The costs of parenteral nutrition preparations used in hospitals of Jiangsu are high but the rate of rationality is low.Nutrition support team should be established to regulate the use of parenteral nutrition preparations and save medical resources.
2.Comparison of the safety of docetaxel combined with carboplatin regimen and epirubicin combined with cyclophosphamide sequential docetaxel regimen in phase Ⅲ clinical study of negative breast cancer

Objective To compare the clinical efficacy and safety of docetaxel combined with carboplatin (TP) and epirubicin combined with cyclophosphamide sequential docetaxel (EC-T) adjuvant in the treatment of three negative breast cancer in phase III . Methods 62 cases of three negative breast cancer patients in phase III from May 2012 to October 2016 were selected and randomly divided into the control group and the experimental group, with 31 patients in each group. The control group was treated with epirubicin combined with cyclophosphamide and sequential docetaxel, and the experimental group was treated with docetaxel and carboplatin. The clinical indicators were compared and analyzed. Results There was no significant difference in the recent remission rate (77.42%) between the experimental group and the control group (74.19%). Two groups of patients with adverse reactions were restored within one month. There were 10 cases of WBC decrease in the experimental group, with the incidence rate of 32.25%. There were 18 cases of WBC decrease in the control group, the incidence rate was 58.06%, the difference was statistically significant (P<0.05). Conclusion Docetaxel combined with carboplatin and epirubicin adjuvant combined with cyclophosphamide than star sequential docetaxel in the treatment of three patients with negative breast cancer stage III were tolerated, TP occurred leukopenia and alopecia with low probability.
3.Clinical application of nasal-insertion type ileus-tube in treating adhesive small intestinal obstruction

Objective To investigate the clinical application of nasal-insertion type ileus-tube in the treatment of adhesive small intestinal obstruction. Methods A total of 221 patients with simple adhesive small intestinal obstruction, who were admitted to authors’ hospital during the period from January 2010 to Aug. 2014, were enrolled in this study. The patients were randomly divided into nasal-insertion type ileus-tube group (n=111) and nasogastric tube group (n=110). After the procedure, the patients were kept under close observation, focusing on the abdominal distention, gastrointestinal decompression amount, the recovery time of anal exhaustion and defecation, the vanishing time of intestinal air-liquid plane on erect abdominal X-ray film, etc. The cure rate, effective rate and transit-operation rate were calculated. The results were compared between the two groups. Results The tube placement operation was successfully performed in all patients. Compared with the nasogastric tube group, in the nasal-insertion type ileus-tube group the recovery time of abdominal distention, anal exhaustion and defecation and the vanishing time of intestinal air-liquid plane on erect abdominal X-ray film were obviously shorter, and the gastrointestinal decompression amount was larger. In the nasal-insertion type ileus-tube group the cure rate and effective rate were significantly increased, while the transit-operation rate was decreased; the differences between the two groups were statistically significant (P<0.05). Conclusion For the treatment of adhesive small intestinal obstruction, the placement of nasal-insertion type ileus-tube is effective and reliable. This technique can strikingly improve the clinical symptoms, therefore, it is worthy of promotion and application in clinical practice.
4.Speckle tracking echocardiography assessment of global and regional contraction dysfunction in the mice model of pressure overload.

Speckle tracking echocardiography (STE) has been applied to the evaluation of cardiac contraction dysfunction. However, there were few studies on alteration of global and regional STE parameters in the process of myocardial hypertrophy and heart failure. In this study, STE was applied to evaluate the global and regional cardiac function under heart failure and hypertrophy in the mice model of pressure overload. Adult mice were subjected to mild or severe aortic banding with a 25 Gauge (G) or 27 G needle. After surgery, STE and conventional echocardiography were used in the sham group (n=10), mild trans-aortic banding (TAB) group (n=14) and severe TAB group (n=10) for 8 weeks. The results showed that the mice subjected to severe TAB showed a significant change in fractional shortening (FS), left ventricular (LV) mass, and left ventricular end diastolic diameter (LVEDD) (P<0.05 for each). Meanwhile, there were no significant differences in FS and LVEDD between the sham group and mild TAB group during the experimental procedures (P>0.05 for both). STE analysis revealed that longitudinal strain (LS) was significantly decreased in the severe TAB group as compared with the sham and mild TAB groups (P<0.05 for both) from the postoperative week 1. LS in the mild TAB group was reduced as compared to the sham group (P<0.05). Radial strain (RS) and circumferential strain (CS) were significantly decreased in the severe TAB group as compared to the sham group and the mild TAB group (P<0.05 for both) from the postoperative week 1 (P<0.05 for both). Compared to the sham group, CS in the mild TAB group maintained unchanged during the test period, and RS was reduced only on the postoperative week 6 (P<0.05). Finally, regional contraction dysfunction was analyzed in both hypertrophic and failing myocardium in longitudinal and radial directions. It was found that LS was largest in the apex region and RS was smallest in the apex region in the healthy and hypertrophic myocardium. It was also found that compared to the sham group, only base longitudinal strain in the mild TAB group was decreased. Each of regional strain in the severe TAB group was uniformly depressed in radial and longitudinal directions. It is concluded that STE has provided a non-invasive and highly feasible way to explore the global and regional contraction dysfunction in hypertrophic and heart failure myocardium in the murine model of pressure overload.

5.Tolerance of soxifloxacin hydrochloride tablet after ascending single dose in Chinese healthy subjects

Objective To evaluate the safety and tolerance of single oral dose of soxifloxacin hydrochloride in Chinese healthy subjects.Methods Seventy-two eligible subjects were randomized into 7 escalation dose groups.Eight subjects in 50 mg group were randomized to receive either study drug or placebo in the ratio of 3 to 1;each ten subjects in 100 mg, 200 mg and 300 mg group received study drug to placebo was 4∶1, respectively.Twelve subjects in 400mg and 600mg group, the ratio of study drug to placebo was 2∶1 , and ten subjects in 800 mg group was 3∶2, respectively.Safety and tolerability were evaluated by monitoring adverse events, laboratory parameters, electrocardiography and electroen-cephalogram. Results A total of 72 subjects finished study. One subject in 400 mg group had bruise on the left ankle with creatine kinase elevated, considered not related to study drug, the other one in 600 mg group complained mild dizziness and two subjects in 800 mg group complained mild nausea.All of these symptoms were assessed as related to study drug.Conclusion Soxifloxacin hydrochloride was safe and well tolerated after single oral administration at dose range of 50 to 800 mg.
6.Ultrasound imaging features of hepatic lymphoma

Objective To explore the sonographic characteristics and to evaluate its diagnostic value of hepatic lymphoma (HL). Methods The clinical data and abdominal sonographic ifndings of 18 consecutive patients with histological proven diagnosis of lymphoma with liver involvement were reviewed, including contrast-enhanced ultrasound (CEUS) images. Results The liver sonographic features of 18 HL were differentiated as three types including nodular, diffuse and mixed type. Nodular type showed well-deifned hypoecho lesions in 6 cases with penetrating vascular lfow signals in 2 cases and no vascular lfow signals in 6 cases by color Doppler imaging. In 3 cases by CEUS, all the hepatic focal nodulars showed variable arterial enhancement but wash-out in the portal venous and late phases. For diffuse type cases, hepatomegaly without nodule was showed in 11 cases. Vasular in liver showed normal in 8 cases and abnormal in 3 cases. For mixed type cases, hepatomegaly and multiple ill-deifned hypoechoic lesions with normal vascular in liver were showed in 1 case. Hepatosplenomegaly and abdominal lymphoadenopathies were identiifed in 11 cases. Conclusions The characteristics and the types of sonographic ifndings may be helpful in differentiating HL from other liver lesions.
7.Effect of phospholipomannan of Candida albicans on the production of interleukin 6 and interleukin 8 in monocytes.

OBJECTIVETo investigate whether Candida albicans-native phospholipomannan (PLM) induce an inflammation response through Toll-like receptor(TLRé2 in human acute monocytic leukemia cell line (THP-1) cells.
METHODSHuman THP-1 monocytes were challenged with PLM in vitro. The mRNA expressions of TLR2, TLR4, proinflammatory cytokine [interleukin(IL)-6], and chemokine (IL-8) were assayed by real time reverse transcription polymerase chain reaction. The secretions of IL-6 and IL-8 were measured by enzyme-linked immunosorbent assay. The expression of TLR2 was analyzed with Western blot.
RESULTSPLM increased the mRNA expressions and secretions of proinflammatory cytokines (IL-6) and chemokines (IL-8) in THP-1 cells (all P=0.0000). PLM up-regulated the mRNA and protein levels of TLR2 (P=0.0000), whereas the mRNA level of TLR4 was not altered. PLM hydrolyzed with β-D-mannoside manno hydrolase failed to induce gene and protein expressions of TLR2, IL-6, and IL-8. Anti-TLRS-neutralizing antibody blocked the PLM-induced secretions of IL-6 and IL-8 in THP-1 cells (P = 0.0003, P = 0.0010).
CONCLUSIONCanidada albicans-native PLM may contribute to the inflammatory responses during Candida infection in a TLR2-dependent manner.

8.Effect of improved venous puncture method

Objective To observe the effect of improved venous puncture method and provide basis for popularizing the method which is rapid, painless and of less trauma. Methods According to single-blind random method, 2 132 patients undergone venous puncture treatment in the emergency department of our hospital from August 2008 to August 2009 were selected and divided into two groups: improved group ( 1 066 cases) and traditional group( 1 066 cases). Observed the effect of two groups. Results Improved method was superior to traditional method in pain easing, and there was statistical difference between the two groups (Z = 21.95 ,P <0.01 ). Improved method could increase puncture success ratio, with statistical significance as compared to traditional method ( x2 =36.61 ,P <0.01 ). Also, zhe former had more rapid venous return ( u = 196. 50,P <0. 01 ) and less incidence of bleeding of penetration points and subcutaneous ecchymosis after needle withdrawing ( x2 = 11.76 ,P < 0. 01 ) than the latter. Conclusions Improved puncture method is of less pain and higher puncture success ratio and is superior to the traditional method.
9.Familial aggregation of metabolic syndrome in adolescents with paternal metabolic syndrome

Objective To analyze the impact of parental Metabolic Syndrome(MS)on adolescents,and to explore the familial aggregation of MS with its components.Methods Using a 1:3 case-control familial study design to choose 26 MS male patients as proband and 78 healthy men as controls.Data regarding phenotype of their adolescence offspring were collected.Height,weight,waist circumference(WC),blood pressure,body mass index(BMI),waist-to-hip ratio(WHR)and waist-to-height ratio(WHtR)were measured or calculated.FPG,TC,TG,HDL-C and LDL-C were detected by automatic biochemical analyzer and hs-CRP was detected.Results WC,WHtR,WHR,DBP and hs-CRP of those adolescents with paternal MS were significantly higher than in controls(P<0.05).Rates of MS,Obesity depend WC,low level of HDL-C of adolescent with paternal MS were significantly higher than in controls(P<0.05).The rate of number on MS was significantly higher in case group than in control(r=0.231,P<0.05).Conclusion The phenotypes of MS were different between adolescents with or without parental MS,indicating that the familial aggregation of MS had been existed in their adolescent offspring,and mainly presented in central obesity,increased blood pressure and inflammation.
10.Pharmacokinetics of single-dose astromicin sulfate in Chinese healthy volunteers

Objective To evaluate the pharmacokinetics of single dose of astromicin injection in healthy Chinese volunteers.Methods A randomized,three period cross-over,latin-square design study was conducted.Twelve male volunteers were administrated intravenously 150,200 and 250 mg of astromicin within 0.5 h infusion.The concentrations of astromicin in serum and urine were determinated by precolumn derivatization-fluorescence HPLC method.Results The main pharmacokinetic parameters of three dose(150,200,250 mg)were as follows:C_(max) was(11.27±2.65),(15.83±3.14)and(21.97±3.67)mg·L~(-1);t_(1/2)β was(2.56±0.72),(2.97±1.30)and(2.66±1.39)h;AUC_(0-t)was(30.92±8.49),(39.58±11.57)and(47.31±11.59)mg·h·L~(-1);AUC_(0-∞) was(31.78±8.73),(40.50±11.56)and(48.20±11.95)mg·h·L~(-1);V_c was(11.49±2.61),(10.69±1.52)and (10.20±1.93)L;CL was(5.33±2.03),(5.49±1.51)and(5.68 ±1.35)L·h~(-1),respectively.Urinary recovery rates was(78.75±7.00)%,(78.95±12.57)%and(79.94±10.28)%,respectively.Conclusion The main pharmacokinetic parameters of astromicin in difereneut doses were nearly fit linear dynamic feature,and the drug maily excreted through kidney.
