Bimaxillary hyperhypodontia (BHH) is a very rare numeric anomaly with a prevalence of 0.002% to 3.1% described by the presence of a supernumerary tooth in the premaxilla region and a missing mandibular incisor tooth. This case highlights the multidisciplinary management of a child presenting with BHH who complies with the recommended protocol by surgically removing the supernumerary tooth and then proceeding with orthodontic treatment for function and aesthetics. A 9-year-old healthy Malay boy presented with a fully erupted tooth 21, a labially palpable bulge of unerupted tooth 11, a missing tooth 32, and a tendency for an anterior and posterior crossbite. The radiographs showed an inverted, unerupted, conical-shaped supernumerary tooth overlapping the unerupted tooth 11 and hypodontia of tooth 32. The management was surgical removal of the supernumerary tooth and the placement of an upper removable appliance with a palatal expansion screw followed by comprehensive fixed orthodontics.

OBJECTIVES: 1. To present a rare case of a true teratoma of the nasopharynx in a Filipino newborn. 2. To present a rational diagnostic and management approach to the case. DESIGN: Case report SETTING: Tertiary Government Hospital PATIENT: One female newborn RESULTS: A newborn presented with a congenital large naso-oropharyngeal mass. She did not present with signs of airway obstruction. The oral mass was found to be attached to the nasopharynx. The initial consideration was a teratoma. The patient also manifested with an incomplete cleft palate and dermoid cyst on the left eye. Contrast CT scan confirmed the attachment of the mass to the left nasopharyngeal wall. The mass was subsequently resected and delivered transorally. The final histopathology showed teratoma. Otorhinolaryngologic management of this case encompassed the following priorities: (1) ensuring vital & vegetative function (airway & feeding), (2) examination of the newborn for concomitant congenital anomalies, (3) resection of the mass after accurate delineation of its extent and dimensions and (4) planning for future repair of the palatal defect. CONCLUSION: True teratomas of the nasopharynx are rare lesions (originating from the 3 embryonic germ cell layers). Airway management is the first priority of the otorhinolaryngologist in these cases because of potential to cause upper airway obstruction. (Author)

OBJECTIVES: 1. To present an unusual case of neurofibroma of the neck that does not meet the criteria for neurofibromatosis. 2. To describe the role of surgery in the treatment and reconstruction of a cosmetically unacceptable, disfiguring lesion. 3. To emphasize the importance of early detection and possibility of malignant transformation in neurofibroma. 4. To present the criteria in the diagnosis of neurofibromatosis. DESIGN: Case Report SETTING: Tertiary Hospital PATIENT: A 36 years old male with a huge bleeding neck mass. RESULT: Repeated incision biopsy of the lateral neck mass revealed neurofibroma. CT scan of the mass showed that it was superficial to the sternocleidomastoid muscle and was not adherent to the major vessels of the neck. A wide excision of the mass was done and the defect was reconstructed with a pectoralis major flap. CONCLUSION: Acase of neurofibroma that does not meet the criteria for von Recklinghausen's disease is presented. This may represent a distinct form of neurofibromatosis. In this kind of tumor, surgery remains the treatment of choice for resectable lesions. Early detection of the tumor is imperative for its successful treatment. (Author)

As his symptoms progressed, further evaluation revealed a mass that was obstructing a large portion of the airway. Timely and methodical investigation and intervention proved to be life-saving. CONCLUSION: A working knowledge of the differential diagnoses of chronic cough is crucial in the diagnosis and management of the compromised airway. A high index of suspicion, systematic and thorough investigation and a multi-disciplinary approach are vital components of the treatment plan. The patients is 6 years old girl, who was diagnosied Chediac-Higashi syndrome phase acceleration according to the following criteria:(1) Photophobia, partial oculocutaneous albinism, intermittent high febrile fine light light hair, hepatosplenomegaly (2) family past history: Consanguineous marriage (3) Giant granules within the bone marrow and peripheral blood granule leukocyte. The patient was responded to well with antibiotic, high dose C vitamin and corticoid. The diagnosis, treatment and Etiology of this sydrome are also discussed.

Subjects: Patients without health insurance were discharged in Gia Luong district hospital, Bac Ninh province in 4/1998; 81,08% population of Gia Luong district have no health insurance 0,8% population received the human health insurance. 383 inpatients without health insurance (58,8%) and 41,2% inpatient with health insurance. The average duration of treatment was 7,4 days, less than this of inpatient with health insurance (8,5 days).Total cost/patient/round of treatment was 375.000 VND. This cost was higher 2,7 times than average income. 40,5% inpatients had to borrow. 1/4 (25%) patients want to early discharged during uncompleted free-disease. The rate of poor population received hospital bursar reduction or was very low because of the complicated procedure.

On 97 cases treated in the Military Hospital No108, the remarks are as follows: 1) Rectal cancers occurred mainly in the age ranged 40-60 years old, men to women ratio is 1.85. 2) Clinic symptoms include constipation, diarrhoea with blood and pus occurred alternatively, tenesmus… 3) Most of tumors located at the lower rectal ampulla; adenoepithelioma accounted for 93.2%. 4) Rectal resection with sphincter conservation performed on 4.1%; Mile's operation on 59.8%; and palliative operation on 38.1% of cases; bleeding complication happened on 4.1% of cases and the most common post operative complication was perineum infection 26.8%. 5) Mortality 6.2%. 6) More than 5 years survival 52.3%

The authors report a case of Von hippel Lindau disease in a 20 years old patients with presenting signs: diminution of visual acuity, intracramial hypertension with a left cerebollar syndrome and existence of an retinal angioma on the right side and papilla oedema on the left side. A scanning examination revealed a cystic tumor with a solid little mass. Angiography showed a capillary nodus coressponding to the mass lesion, with feeding arteries coming from the anterio superior cerebellar artery. Removal of the cyst and the tumor was successful. Pathological examination: Hemangi oblastoma. Negative ultrasound examination of the kidney, pancreas.

The analysis showed that 207 mothers (78% of the cases) had never received tetanus toxiod. 96% (254) of the cases did not meet the criteria set for "protection at birth", 73.8% of the mothers did not seek antenatal care. The rate of delivery at home was 71%. The rate of delivery by traditional birth attendants (TBA) was 50%.
