1.Is the Mixed Use of Magnetic Resonance Enterography and Computed Tomography Enterography Adequate for Routine Periodic Follow-Up of Bowel Inflammation in Patients with Crohn’s Disease?

Objective:
Computed tomography enterography (CTE) and magnetic resonance enterography (MRE) are considered substitutes for each other for evaluating Crohn’s disease (CD). However, the adequacy of mixing them for routine periodic follow-up for CD has not been established. This study aimed to compare MRE alone with the mixed use of CTE and MRE for the periodic follow-up of small bowel inflammation in patients with CD.
Materials and Methods:
We retrospectively compared two non-randomized groups, each comprising 96 patients with CD. One group underwent CTE and MRE (MRE followed by CTE or vice versa) for the follow-up of CD (interval, 13–27 months [median, 22 months]), and the other group underwent MRE alone (interval, 15–26 months [median, 21 months]). However, these two groups were similar in clinical characteristics. Three independent readers from three different institutions determined whether inflammation had decreased, remained unchanged, or increased within the entire small bowel and the terminal ileum based on sequential enterography of the patients after appropriate blinding. We compared the two groups for inter-reader agreement and accuracy (terminal ileum only) using endoscopy as the reference standard for enterographic interpretation.
Results:
The inter-reader agreement was greater in the MRE alone group for the entire small bowel (intraclass correlation coefficient [ICC]: 0.683 vs. 0.473; p = 0.005) and the terminal ileum (ICC: 0.656 vs. 0.490; p = 0.030). The interpretation accuracy was higher in the MRE alone group without statistical significance (70.9%–74.5% vs. 57.9%–64.9% in individual readers; adjusted odds ratio = 3.21; p = 0.077).
Conclusion
The mixed use of CTE and MRE was inferior to MRE alone in terms of inter-reader reliability and could probably be less accurate than MRE alone for routine monitoring of small bowel inflammation in patients with CD. Therefore, the consistent use of MRE is favored for this purpose.
2.Prevalence of Primary Immunodeficiency in Korea.

This study represents the first epidemiological study based on the national registry of primary immunodeficiencies (PID) in Korea. Patient data were collected from 23 major hospitals. A total of 152 patients with PID (under 19 yr of age), who were observed from 2001 to 2005, have been entered in this registry. The period prevalence of PID in Korea in 2005 is 11.25 per million children. The following frequencies were found: antibody deficiencies, 53.3% (n = 81), phagocytic disorders, 28.9% (n = 44); combined immunodeficiencies, 13.2% (n = 20); and T cell deficiencies, 4.6% (n = 7). Congenital agammaglobulinemia (n = 21) and selective IgA deficiency (n = 21) were the most frequently reported antibody deficiency. Other reported deficiencies were common variable immunodeficiencies (n = 16), X-linked agammaglobulinemia (n = 15), IgG subclass deficiency (n = 4). Phagocytic disorder was mostly chronic granulomatous disease. A small number of patients with Wiskott-Aldrich syndrome, hyper-IgE syndrome, and severe combined immunodeficiency were also registered. Overall, the most common first manifestation was pneumonia. This study provides data that permit a more accurate estimation PID patients in Korea.

3.A Case of a Human Tail Without Associated Anomalies.

Human tails or caudal cutaneous appendages are rare congenital anomalies with lesions usually arising from the midline of the lumbosacrococcygeal area. There have been only around 70 cases reported in literature. A 4-month-old male infant born as the 1st child of a dizygotic twin was presented with a penis-shaped mass of 4-cm in the coccygeal area. The mass was entirely covered by skin without any skin defect and was connected to the midline skin of the posterior coccygeal area by a stalk. Magnetic resonance images showed fat density of the mass without any demonstrable communication with the spinal canal nor any underlying central nervous system anomalies. The patient underwent a simple surgical excision of the mass without residual effects and complications. Histopathologic examination of the mass revealed lobules of mature adipose tissue, fibromuscular soft tissue, small vessels and nerves with normal hair follicles on the skin.

4.Reversible Pulmonary Hypertension in Adolescent with Left Atrial Myxoma.

We report a patient of left atrial huge myxoma presenting with severe pulmonary hypertension in adolescents. A patient was a 14-year-old boy presented with sudden onset dyspnea. Transthoracic echocardiographic study revealed the presence of a nodular, 4.34 x 8.11 cm sized, mobile, hyperechoic mass in the left atrium and severe pulmonary hypertension with tricuspid insufficiency. After surgical therapy, tricuspid regurgitation and pulmonary hypertension was decreased and the patient was stabilized and had an uneventful clinical course.

5.Development of a Questionnaire to Investigate the Level of Awareness of Allergic Rhinitis.

PURPOSE: Despite the increasing prevalence of allergic rhinitis, there are difficulties in early diagnosis and proper treatment due to lack of education and misunderstanding of the disease. Development of efficient education materials based on the level of awareness of allergic rhinitis is required. Therefore, this study was aimed to develop a questionnaire for investigating the level of awareness of allergic rhinitis. METHODS: Questions related to the level of awareness of allergic rhinitis were prepared on the basis of the literature, professional advice and inquiries from patients and caregivers. These items were categorized into 4 areas: (1) basic knowledge, (2) symptoms, (3) diagnosis and environmental management and (4) treatment. Through the eight preliminary survey on 38 adults, a questionnaire of 60 items was developed. The results were analyzed by using SPSS 12.0. RESULTS: A tool for investigating the level of awareness was completed, which was composed of 20 items with an internal reliability of 0.67. According to the results of the survey with 60 items, the areas of basic knowledge, symptoms and environmental management of allergic rhinitis showed high awareness. The items regarding the association between allergic rhinitis and asthma, necessity of regular checkup, awareness of maintenance treatment, importance of treatment with intranasal steroids, and the influence of treatment on immunity, growth and development of patient showed low awareness. CONCLUSION: A reliable tool was developed for investigating the level of awareness of allergic rhinitis. It may be useful in developing educational materials to optimize its educational effect on allergic rhinitis.

6.A Case of Intracranial Dermoid Cyst Associated with Scalp Abscess.

Intracranial dermoid cyst is a rare congenital disease originating in the embryo. Intracranial dermoid cysts are usually detected before five years of age; however, asymptomatic cysts have been discovered after the third decade of life through various complications. Sometimes, there are neurologic symptoms caused by a cystic mass effect and if there is a dermoid sinus, purulent infections can occur. Early diagnosis is important and as such, CT or MR imaging for recognition of the location, size, and characteristics of the cysts are recommended before surgical resection. Extradural dermoid cysts with an intact occipital dermoid sinus are a rare event. We report a case of intracranial dermoid cyst associated with scalp abscess. In this case, a three-year-old boy was presented with scalp abscess and we performed MR imaging, which revealed a cystic region. The prognosis after resection operation was good without any complication.

7.A Case of Atypical Miller-Fisher Syndrome with Internal Carotid Artery Stenosis.

Miller-Fisher syndrome is an acute poly-neuropathy typically characterized by the triad of ophthalmoplegia, ataxia, and areflexia. Other poly-neuropathy diseases include Guillain-Barre syndrome and Bickerstaff brainstem encephalitis. However, not all cases of Miller-Fisher syndrome have the typical three symptoms. These diseases have been differentiated by clinical manifestations and anti-GQ1b IgG antibody testing. In Miller-Fisher syndrome, anti-GQ1b IgG antibody is present in over 95% of patients; however, the frequency is not so high in normal subjects or in those with other poly-neuropathy diseases. Infectious illnesses, such as respiratory diseases and gastrointestinal diseases, usually precede Miller-Fisher syndrome. In general, Miller-Fisher syndrome is a self-limiting illness; however, a few cases of Miller-Fisher syndrome progressing to respiratory failure have been reported, particularly in children. In this case, we report abnormal angiographic findings. We report a case of atypical Miller-Fisher syndrome with internal carotid artery stenosis treated with prednisolone.

8.Clinical and Radiological Outcomes with the Lumbar Total Disc Replacements.

OBJECTIVE: To determine the clinical and radiographic results and incidence of complications in lumbar arthroplasty patients. METHODS: From January, 2003 to March, 2005, 46 patients were implanted with 30 cases of the Charite Artificial Disc and 16 cases of the Prodisc. There were 28 males and 18 females with a mean age 40.6 years(range, 21~70). Diagnoses included 19 patients with discogenic back pain, and 27 patients with FBSS. All patients were examined at a minimum 2-year follow-up(mean 3.5 years). Clinical outcomes were assessed by Oswestry Disability lndex(ODI), Visual Analogue Scale(VAS),and Macnab's Criteria. Dynamic lateral flexion and extension radiographs were used to radiologic assessment. RESULTS: The ODI was reduced from 67.6% to 8.9% at the latest follow up. The mean VAS baseline was 8.29 and 2.08 at the latest follow-up. Clinical outcome was excellent or good in 40 patients(mean 87, 93% FBSS, 78% Discogenic back pain). Mean flexion/ extesion range of motion at L4-5 was 9.1degrees Charite, 9.8degrees Prodisc and at L5-S1 it were 5.5degrees, 5degrees. A total of 7 complications(15%) were noted [subsidence (2), polyethylene core wire break down (1), vertebral body posterior inferior cortex breakage (1), anhydrosis (2), nerve root damage (1)]. CONCLUSIONS: This study demonstrates the safety and efficacy of the total disc replacements especially FBSS cases.

9.Iron Status in Korean Middle School Students and Possible Association with Obesity.

BACKGROUND: Iron deficiency (ID) in Korean adolescents still remains a problem. This study aimed to assess the prevalence of iron status and investigate the relationship between the iron status and obesity. METHODS: Hematological examinations were performed on apparently healthy 12~14 year old students (M:F=451:442) living in Incheon during September, 2004. ID was defined as a serum ferritin concentration <10ng/mL. The diagnosis of iron deficiency anemia (IDA) was established when anemia (male <12.5g/dL, female <12g/dL) was associated with a ferritin level <10ng/mL and/or transferrin saturation <16%. Using age- and gender-specific BMI percentiles, overweight was defined as a BMI=85th percentile. RESULTS: The prevalence of ID and IDA were 8.4 and 15.6% and 0.9 and 4.5% in males and females, respectively. Using an analysis based on the BMI, the prevalence of obesity were 21.8 and 16.2% in males and females, respectively. The prevalence of ID in male students was decreased in the obesity (0.0%) compared with the non-obesity group (11.4%). In female students, ID showed a higher frequency in the non-obesity (20.4%) compared with the obesity group (12.0%). CONCLUSIOM: Iron deficiency still remains a major nutritional problem in adolescent females, with the prevalence of obesity significantly increasing. Although no association between the prevalence of iron deficiency and obesity was shownin this study, it is important to screen for iron deficiency and obesity, and provide effective nutritional education. Furthermore, the association of obesity with iron deficiency in relation to eating behavior should be investigated.

10.Comparison of Two Internet Based Telepathology Systems: CORBA and ActiveX System.

Telepathology systems will be common systems in hospitals. The two systems were designed and implemented in web environments for test. One was implemented with the Common Object Request Broker Architecture (CORBA) technique. The other system was implemented in the form of ActiveX. The histopathological materials were stained by Hematoxylin and Eosin. By the Donpisha CCD camera attached to an Olympus BX-51 optical microscope 180 color images come to be acquired. For evaluation of the systems, transmission times and telediagnosis concordance rates were measured. Image processing ability was tested using two telepathology systems. For the local area test, system I using CORBA had measured image transmission times of 0.1 s, 0.2 s, and 0.4 s at the file sizes of 100 K byte, 900 K byte and 3.6 M byte respectively. Transmission times for system II using Component Object Model (COM) were slightly slower, ranging from 0.02 s to 0.05 s. In the long distance area test, system II transmission times were 0.5 s, 0.8 s, and 2.0 s. The overall concordance rate of telediagnosis for the 180 images was 78.3%. In this study, we compared our systems about image transmission, and processing for the further development of system configurations.
