It is well established that increased excitability of the presympathetic neurons in the hypothalamic paraventricular nucleus (PVN) during hypertension leads to heightened sympathetic outflow and hypertension. However, the mechanism underlying the overactivation of PVN presympathetic neurons remains unclear. This study aimed to investigate the role of endogenous corticotropin-releasing factor (CRF) on the excitability of presympathetic neurons in PVN using Western blot, arterial blood pressure (ABP) and renal sympathetic nerve activity (RSNA) recording, CRISPR/Cas9 technique and patch-clamp technique. The results showed that CRF protein expression in PVN was significantly upregulated in spontaneously hypertensive rats (SHRs) compared with normotensive Wistar-Kyoto (WKY) rats. Besides, PVN administration of exogenous CRF significantly increased RSNA, heart rate and ABP in WKY rats. In contrast, knockdown of upregulated CRF in PVN of SHRs inhibited CRF expression, led to membrane potential hyperpolarization, and decreased the frequency of current-evoked firings of PVN presympathetic neurons, which were reversed by incubation of exogenous CRF. Perfusion of rat brain slices with artificial cerebrospinal fluid containing CRF receptor 1 (CRFR1) blocker, NBI-35965, or CRF receptor 2 (CRFR2) blocker, Antisauvagine-30, showed that blocking CRFR1, but not CRFR2, hyperpolarized the membrane potential and inhibited the current-evoked firing of PVN presympathetic neurons in SHRs. However, blocking CRFR1 or CRFR2 did not affect the membrane potential and current-evoked firing of presympathetic neurons in WKY rats. Overall, these findings indicate that increased endogenous CRF release from PVN CRF neurons enhances the excitability of presympathetic neurons via activation of CRFR1 in SHRs.

Objective To summarize the clinical characteristics of Listeria monocytogenes meningitis (LMM) with complications, and analyze the outcomes of next generation sequencing. Methods Clinical characteristics, laboratory findings, imaging features, antibiotics treatment, and next generation sequencing of cerebrospinal fluid were analyzed in 3 LMM patients who were hospitalized in the Department of Infectious Diseases of Beijing Children′s Hospital Affiliated to Capital Medical University from July 2015 to November 2017. Results The three patients were 1‐year‐old girl, 2‐year‐old girl, and 9‐year‐old boy, with normal immune function. They had eaten refrigerated food, milk or dairy products before onset. Symptoms included fever, headache, abdominal pain, diarrhea, vomiting, and convulsions, etc. The complications of two cases (case 2 and 3) were appendicitis and Meckel′s diverticulitis. The other one (case 1) was with sepsis and pneumonia. Leukocyte counts in cerebrospinal fluid were elevated in all the three cases, and cranial magnetic resonance imaging showed meningeal or periventricular involvement. All the children were diagnosed with LMM by positive CSF culture. CSF for next generation sequencing was sent after carbapenem antibiotics using, yet all the results were positive. The positive results were returned 2, 9, and 9 days earlier than culture results, respectively. The gene coverage was 5.00%, 7.00%, 0.04%, and the reads was 2 561, 1 011 and 8, respectively. All the three children had recurrent fever despite using cephalosporin. Levels of leukocytes in the blood and CSF further elevated. After using carbapenem antibiotics, patients improved eventually and were discharged from hospital. Conclusions LMM can occur in children with normal immune function and is usually associated with digestive system symptoms. Listeria monocytogenes can be detected quickly and accurately by the next generation sequencing technology, without being limited to sampling time and antibiotics application.

Objective To evaluate the clinical safety and efficacy of percutaneous interventional therapy in pediatric patients with secundum atrial septal defect(ASD).Methods Clinical data of 40 patients(age≤2 years)with secundum atrial septal defect treated in our hospital from February 2014 to December 2017 were analyzed retrospectively. There were 13 males and 27 females in these patients.Ultrasound of heart showed that there were 37 patients with single ASD,3 patients with multiple ASDs.One associated with pulmonary stenosis(PS),and 1 associated with patent ductus arteriosus. There were 6 patients with pulmonary hypertension, and the diameter of ASD was (10.6 ± 2.0) mm. All patients were proved to have secundum atrial septal defect before intervention. In the intervention, the transport system was delivered along the femoral vein, inferior vena cava and right atrium through atrial septal defect to the left atrium, and the occluder was released there. Results Of the 40 patients, 38 cases were successfully implanted, and the other two patients were not satisfied with the location of occlusion. The diameter of the ASD occluder was(12.0±2.1)mm and the transport sheath 7-9 F.Plug2 occluder was implanted in the patient with patent ductus arteriosus. To the patient with PS,pulmonary valve balloon angioplasty was performed, and then the pressure gradient reduced obviously, after that ASD occlusion was performed. The total follow up period was from 2 months to 3 years. No residual shunt and unsatisfactory device position were found during the follow up period. The pulmonary pressure reduced to normal, and the right atrium and right ventricle were smaller in a different degree. All patients had no arrhythmia and other complications. Conclusion Transcatheter closure of ASD is safe, reliable, and has fewer complications. It is worthy of popularization and application.Appropriate occluder should be selected according to the size and edge of ASD to reduce complications, such as residual shunt and valve injury.

OBJECTIVETo explore the polymorphisms of the human platelet antigen (HPA) gene of Han and Menggu population of Heilongjiang province in China, to determine platelet antigen system with clinical significance by judging the rate of incompatibility of HPA, as well as to establish a database of donors HPA including the Han nationality and the minority nationality.
METHODSThe samples of unrelated healthy Menggu population (100 cases) people were selected from Heilongjiang province in China, and 123 cases in healthy Han population in Heilongjiang as control were collected. The technique of PCR-sequence specific primers (PCR-SSP) was used for genotyping of 34 alleles in HPA1-17 gene. The gene frequency and genotype frequency were separately calculated, and the allele frequency distribution in Menggu population was compared with the results from Han population.
RESULTSIn the Han population, monomorphic HPA-4,HPA-7-14,HPA-16, 17 were found in the samples, none of HPA-b was found in these samples. For HPA-1, 2, 4, 5 and 6, aa homozygosity was predominant. In the Menggu population, monomorphic HPA 1, HPA 5, 6, HPA 7-14, HPA 16,17 were found in the samples, none of HPA-b was found in these samples. For HPA 2, 4 aa homozygosity was predominant. HPA-3, 15 had the greatest heterozygosity in 2 population. HPA-1a and HPA-3a frequency of Menggu population were significantly different from that of Han population in Heilongjiang.
CONCLUSIONDistribution of the allele polymorphism of HPA 1-17 in Heilongjiang Han population is similar to that in Menggu population, and it shows its own characteristics. When the local HPA genotyped database of platelet donors in Heilongjiang is established, the count of Menggu donor should be increased.
