1.Application Progress of Virtual Reality Technology for the Treatment of Spinal Cord Injury

Spinal cord injury (SCI) is a prevalent neurological disorder that often results in motor, sensory, cognitive, balance, and walking impairments, and can even lead to pain, significantly affecting the physical and mental health and quality of life of patients. Conventional treatment methods, primarily conducted in hospital settings, tend to be monotonous and lack engagement, leading to low patients’ compliance, motivation and cooperation. In contrast, virtual reality (VR) technology can serve as an effective adjunctive therapeutic approach, immersing patients in simulated virtual environments for functional training in specific contexts and tasks, thereby providing a sense of real experience to achieve assessment and therapeutic outcomes. The interactive and engaging nature of VR technology markedly enhances patients’ interest and motivation in treatment, offering new possibilities for home-based rehabilitation and presenting patients with alternative treatment options. This review systematically introduces the application of VR technology in the treatment of patients with SCI, focusing on its roles in assessing functional impairments, improving motor function, enhancing balance and walking abilities, alleviating pain, and promoting recovery from cognitive and psychological disorders, as well as its impact on other functional impairments and its combined use with other technologies. Furthermore, this article delves into the current advantages and limitations of VR technology, aiming to deepen the understanding of VR among physicians and therapists, promote its widespread clinical application, advance the field of rehabilitation medicine, provide theoretical foundations and directions for future research, and offer new insights and references for clinical practice.
2.Risk factors for recurrent intussusception in children after ultrasound-guided saline enema reduction

Objective:To explore the risk factors for recurrence of intussusception in children after successful ultrasound-guided saline enema reduction.Methods:The clinical and follow up data of 355 hospitalized children with intussusception at the First Affiliated Hospital of Zhengzhou University from Feb 2018 to Feb 2023 were reviewed.Patients were divided into two groups by recurrence develped and the differences were compared, Data with significant differences were incorporated into multi-factor logistic analysis.Results:The overall recurrence rate was 15.8% (56/355). By univariate variable analysis model, there were statistically significant differences between the two groups in age, previous intussusception history, vomiting, maximum diameter of concentric circles shown by ultrasound, and concurrent bowel organic diseases (lead points) (all P<0.05). In multivariate Logistic regression model, age, previous intussusception history, maximum diameter of concentric circles, and lead points were independent risk factors for recurrent intussusception after saline enema.The optimal cut-off values for age and maximum diameter of concentric circles were 2 years and 33.5 mm, respectively, according to ROC curve analysis. Conclusion:Age older than 2 years, previous intussusception history, maximum diameter of concentric circles longer than 33.5 mm, and lead points are independent risk factors for recurrence after saline enema.
3.Effects of miR-155 on apoptosis of chronic myeloid leukemia cells and expression of heat shock proteins

Objective :
To explore the effects of microRNA-155 (miR-155) on apoptosis of chronic myeloid leukemia ( CML) cells,and the influence of miRNA-155 regulating the expression of heat shock proteins ( HSP) 27, HSP60,HSP70.
Methods :
Reverse transcription-quantitative real-time PCR ( RT-qPCR) was used to detect the expression levels of miR-155 in CML-resistant imatinib (IM) cell line K562-G and CML cell line K562 .K562-G cells were infected with the lentivirus carrying miR-155 or the negative control lentivirus ,and they were named miR-155 group and control group.The effect of miR-155 on the proliferation of drug-resistant cells was detected by cell counting kit-8 ( CCK-8) method. RT-qPCR and Western blot were used to detect the effect of miR-155 on the expression of heat shock proteins HSP27,HSP60,HSP70.Flow cytometry was used to detect the percentage of cell apoptosis in miR-155 group and control group.
Results :
ompared with K562 cells,miR-155 showed low expres- sion in K562-G cells (P <0. 05) .The proliferation of miR-155 group cells decreased significantly from the 36th hour (P<0. 05) .Compared with the control group,in the miR-155 group,HSP60 and HSP70 increased (P < 0. 05) ,while HSP27 decreased (P<0. 01) .The apoptosis rate of miR-155 group was higher than that of control group (P<0. 05) .
Conclusion
miR-155 promotes the apoptosis of chronic myeloid leukemia cells,increases the expression of HSP60 and HSP70,and decreases the expression of HSP27 .
4.The value of dynamic nomogram of multi spiral CT features combined with inflammatory indicators in predicting microvascular invasion of hepatocellular carcinoma before surgery

Objective To explore the value of dynamic nomogram constructed by multi spiral computed tomography(MSCT)features combined with inflammatory indicators in predicting the status of microvascular invasion(MVI)of hepatocellular carcinoma(HCC)before surgery.Methods The clinical and imaging data of 137 patients with postoperative pathologically confirmed HCC were analyzed retrospectively.According to the status of the MVI,they were divided into positive group(44 cases)and negative group(93 cases).Multivariate logistic regression analysis was used to screen independent risk factors for predicting the MVI status of HCC patients,and a joint prediction model was constructed,which was displayed in the form of a dynamic nomogram.The receiver operating characteristic(ROC)curve,calibration curve and Hosmer-Lemeshow test were used to evaluate the diagnostic efficiency,calibration and goodness of fit of the model,Akaike information criterion(AIC)and Bayesian information criterion(BIC)were used for comparison between the models,and a 5-fold cross-validation and decision curve analysis(DCA)were also used to evaluate the stability and clinical applicability of the model.Results Multivariate logistic regression analysis showed that necrosis and delayed-phase enhancement(DEd),and alkaline phosphatase to lymphocyte ratio(ALR)were independent risk factors for predicting MVI status in HCC patients.The area under the curve(AUC)of the dynamic nomogram was 0.721,with the sensitivity of 0.705 and the specificity of 0.656.The AIC and BIC values were 152.372 and 158.212,respectively.The calibration curve and the Hosmer-Lemeshow test showed that the model had a high degree of calibration and goodness of fit(χ2=2.372,P=0.967),the average AUC of the 5-fold cross-validation was 0.787,and the DCA showed that the nomogram model had a good clinical applicability.Conclusion The dynamic nomogram model constructed by MSCT features combined with inflammatory indicators is feasible to predict the MVI status of HCC patients before surgery,and the dynamic nomogram can directly generate the prediction results of different individuals.

The aim of this study was to explore the impact of chronic apical periodontitis(CAP)on atherosclerosis in apoE-/-mice fed high-fat diet(HFD).This investigation focused on the gut microbiota,metabolites,and intestinal barrier function to uncover potential links between oral health and cardiovascular disease(CVD).In this study,CAP was shown to exacerbate atherosclerosis in HFD-fed apoE-/-mice,as evidenced by the increase in plaque size and volume in the aortic walls observed via Oil Red O staining.16S rRNA sequencing revealed significant alterations in the gut microbiota,with harmful bacterial species thriving while beneficial species declining.Metabolomic profiling indicated disruptions in lipid metabolism and primary bile acid synthesis,leading to elevated levels of taurochenodeoxycholic acid(TCDCA),taurocholic acid(TCA),and tauroursodeoxycholic acid(TDCA).These metabolic shifts may contribute to atherosclerosis development.Furthermore,impaired intestinal barrier function,characterized by reduced mucin expression and disrupted tight junction proteins,was observed.The increased intestinal permeability observed was positively correlated with the severity of atherosclerotic lesions,highlighting the importance of the intestinal barrier in cardiovascular health.In conclusion,this research underscores the intricate interplay among oral health,gut microbiota composition,metabolite profiles,and CVD incidence.These findings emphasize the importance of maintaining good oral hygiene as a potential preventive measure against cardiovascular issues,as well as the need for further investigations into the intricate mechanisms linking oral health,gut microbiota,and metabolic pathways in CVD development.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
7.Molecular mechanisms of pancreatic cancer cells regulation by exosome derived from human bone marrow mesenchymal stem cells in vitro

Objective:To investigate the molecular mechanism by which exosomes secreted from human bone marrow mesenchymal stem cells (BMSCs) regulate pancreatic cancer (PC) cells.Methods:The exosomes secreted by BMSCs were extracted and identified. Human PC cell lines (PANC-1 and AsPC-1) were then divided into 3 groups: simple cell group, exosome group and inhibitor group. For inhibitor group, PI3K/AKT Signaling Pathway inhibitor LY294002 was added, followed by exosomes. Subsequently, the protein expression of phosphatidylinositol 3-kinase (PI3K) protein kinase B(AKT) and their phosphorylated types was detected by Western-blot, and the contents of PC cell markers B7-H4 and CA199 were detected by ELISA kit. The CCK-8 experiment was mainly conducted to detect the proliferation ability of cells, the flow cytometry to detect apoptosis, and the Transwell experiment to detect the migration and invasion ability of cells.Results:The exosomes secreted by BMSCs were extracted successfully. The expression of PC cell markers B7-H4 and CA199, the proliferation rate, and the invasion and migration ability of cancer cells were all decreased for both PANC-1 and AsPC-1, while the apoptosis rate increased, all of which had statistical significance ( P<0.001). There were no significant differences for the above parameter in inhibitor group compared with the simple cell group ( P>0.05). The p-PI3K and p-Akt of PANC-1 cells in simple cell group were (5.81±1.87), (5.30±1.21), while up-regulated to (12.74±3.28), (11.22±2.35) with statistically significant differences ( P<0.001) after exosomes were added. Meanwhile, the above 2 kinds of protein expression of AsPC-1 cells in simple cell group were (4.75±0.87), (4.15±1.32), while increased to (10.83±3.19), (9.28±2.33) in exosome group with statistically significant differences ( P<0.001). On the other hand, there was no significant differences for the protein expression of P-PI3K and P-AKT in inhibitor group ( P>0.05) compared with the simple cell group. Conclusion:Exosomes secreted by BMSCS can inhibit the proliferation of PC cells and its migration and invasion ability by activating PI3K/AKT signaling pathway, thus playing a role in antitumor effect.
8.Clinical effects of autologous fat transplantation combined with liposuction in buttock shaping

Objective:To observe the clinical effects of autologous fat transplantation combined with liposuction in buttock shaping.Methods:January 2020 to January 2021, fifty-eight female patients (aged 21-36 years) with hip depression, hip prolapse, obvious inferior plica and fat hypertrophy of buttocks were treated in the clinic of medical cosmetology of Hangzhou Tongrong Lige Medical Technology Co., Ltd., the average age is 29. There were 20 cases of waist-abdomen liposuction + hip shaping, 18 cases of thigh liposuction + hip shaping, 16 cases of thigh liposuction + hip shaping, and 4 cases of simple hip shaping. All patients were followed up for 3-12 months to observe the shape of buttocks and complications (hematoma, infection, fat liquefaction).Results:A total of 58 patients were followed up for 3-12 months. Among the 58 patiens, 45 patiens had significant effect, 11 patients had good effect, and 2 patients had poor effect. Of the 11 cases with good results, 6 cases had poor hip shape and 5 cases had slight symmetry of the lateral buttocks. The reason was that the patients′ buttocks were flat and needed 2-3 times of filling. The buttocks were filled again one year after operation, and better results were achieved. No complications such as infection, hemmatoma, fat liquefaction and uneven bump occurred.Conclusions:The patients are satisfied with the shape of the buttocks after autologous fat transplantation combined with liposuction in buttock shaping. The incidence of surgical complications is low.
9.Revision of childhood acute erythroid leukemia in the diagnosis by World Health Organization 2016 classification

Objective:To analyze the relationship between FAB morphological classification and World Health Organization (WHO) 2016 classification in children with acute erythroid leukemia(AEL), and to summarize the clinical features and prognosis.Methods:Clinical data of de nova childhood AEL patients from January 1, 2002 to December 31, 2019, in Pediatric Blood Disease Center, Institute of Hematology & Blood Disease Hospital were retrospectively analyzed.All of them were re-evaluated according to the WHO 2016 classification.Results:(1) A total of 20 patients were diagnosed as AEL by FAB classification.According to the criteria of WHO 2016, they were re-diagnosed as myelodysplastic syndromes (MDS)- refractory anemia with excess of blasts (11 cases), acute myeloid leukemia with MDS-related changes (3 cases), acute monocytic leukemia (1 case), and pure red leukemia (PEL, 5 cases). (2) Pathological hematopoiesis was frequently detected in bone marrow smears.Auer bodies were seen occasionally in some blasts.The most common antigen expressing were CD 117, CD 13, CD 33, CD 34, CD7, and CD 38.Karyotype analysis was performed in 18 cases successfully, involving 6 cases with abnormal karyotypes, including + 8, -7, 22p+ , t (3; 5: ? ), + 3q-, 15q-, and del (9)(q13). (3) Thirteen cases were treated by chemotherapy, and the one-course complete remission rate was 38.5%.By July 1, 2020, only 2 cases were alive without disease.The overall survival was 49 months and 11 months, respectively. Conclusions:Childhood AEL is susceptible to pathological hematopoiesis, poor response to early chemotherapy and poor prognosis.After re-evaluation according to WHO 2016 classification, most of them were diagnosed as MDS-related.Therefore, adjusting the suitable induction regimen with allogeneic hematopoietic stem cell transplantation may improve the prognosis.
10.The phenotypes and genotypes of four patients with Dubin-Johnson syndrome.

OBJECTIVE:
To explore the genetic etiology in four patients with hyperbilirubinemia, and discuss the correlation between clinical characteristics and molecular basis.
METHODS:
The data of clinical manifestation and auxiliary examinations were collected. Genomic DNA of the four patients was extracted and analyzed by next-generation sequencing using the panel including genes involved in hereditary metabolic liver diseases. Suspected variants were verified by Sanger sequencing.
RESULTS:
All of the four patients were males with normal liver enzymes. It was revealed that all the patients had heterozygous variants, among which c.3011C>T, c.2443C>T and c.2556del were the variants which have not been reported previously.
CONCLUSION
All of the patients were diagnosed as Dubin-Johnson syndrome (DJS) caused by ABCC2 gene variants. The novel variants add to the spectrum of genetic variants of the disease. Because of the favorite prognosis, precise diagnosis can greatly reduce the psychological pressure of patients and avoid excessive treatments. At the same time, it could provide pertinent genetic counseling for the families.
