1.Evaluation of Animal Models of Chronic Obstructive Pulmonary Disease Based on Clinical Characteristics in Traditional Chinese and Western Medicine

		                        			Chronic obstructive pulmonary disease (COPD) is a common clinical chronic airway disease with high morbidity, high mortality, a heavy disease burden, and complex mechanisms that have not been fully elucidated. Clinical problems promote the continuous progress of basic research. The establishment and evaluation of animal models is an important way to delve into the pathogenesis and treatment strategies of COPD. On the basis of the etiology, pathogenesis, diagnostic criteria, and syndrome differentiation of COPD in traditional Chinese medicine (TCM) and Western medicine, this paper summarizes the establishment methods and characteristics of existing animal models of COPD and analyzes the fitting degree of the models with the disease characteristics in Western medicine and syndrome characteristics in TCM. The results showed that the animal models of COPD in Western medicine were mainly induced by single factors and compound factors, and the model with the highest fitting degree was established by cigarette smoke combined with lipopolysaccharide and hormone. The model showed a fitting degree of 84% with the disease characteristics in Western medicine and 70% with the syndrome characteristics in TCM, being consistent with the clinical characteristics of COPD induced by multiple factors. Most of the animal models of COPD in TCM were established on the basis of disease models and combined with TCM etiology and pathogenesis characteristics, and prepared by syndrome differentiation. Among them, the model of accumulation of cold and water retention in lung had the highest fitting degree of 92% with the TCM diagnostic criteria. The models of phlegm-heat obstructing lung and phlegm-stasis obstructing lung had the fitting degree of 94% with clinical manifestations in Western medicine and the highest fitting degree with the diagnostic criteria in Western medicine. Different animal models of COPD have their own advantages and disadvantages, and most of them simply replicate the manifestations of COPD at a certain stage, failing to reflect the multiple causes and the dynamic changes of TCM etiology and pathogenesis. Moreover, the syndromes of these models fail to match the clinical syndromes in TCM. Therefore, establishing the animal models reflecting clinical characteristics of COPD in TCM and Western medicine and improving the model evaluation criteria are important contents to promote the overall development of integrated traditional Chinese and western medicine for COPD. 
		                        		
		                        		
		                        		
		                        	
2.Mechanism of Chaipo Decoction in Alleviating Pyroptosis in Asthmatic Rats via Regulation of NLRP3/Caspase-1/GSDMD Pathway

		                        			ObjectiveTo investigate the therapeutic effects of Chaipo decoction on bronchial asthma in rats and its regulatory effects on the nucleotide-binding oligomerization domain-like receptor pyrin domain-containing protein 3 (NLRP3)/cysteinyl aspartate-specific protease-1 (Caspase-1)/Gasdermin D (GSDMD) pathway, aiming to elucidate its mechanism in ameliorating pyroptosis. MethodsSixty male Sprague-Dawley (SD) rats were randomly divided into six groups (n=10 per group): normal control, asthma model, Chaipo decoction low-dose (5.0 g·kg-1), medium-dose (10.0 g·kg-1), high-dose (20.0 g·kg-1), and dexamethasone (1.0 mg·kg-1). The asthma model was established in all groups except the normal control group via ovalbumin (OVA) sensitization and challenge. Rats in the Chaipo decoction groups received intragastric administration of Chaipo decoction at the corresponding doses, while the dexamethasone group was treated with dexamethasone. The normal and model groups were administered equivalent volumes of saline. After 14 days of intervention, asthma symptom scores were assessed. Dynamic lung compliance (Cdyn), lung resistance (RL), and functional residual capacity (FRC) were measured using a small animal pulmonary function testing system. Lung tissue pathology was evaluated by hematoxylin-eosin (HE), Masson's trichrome, and periodic acid-Schiff (PAS) staining. Levels of interleukin (IL)-6, IL-1β, and IL-18 in bronchoalveolar lavage fluid (BALF) were determined by enzyme-linked immunosorbent assay (ELISA). Expression of NLRP3 and apoptosis-associated speck-like protein (ASC) in lung tissues was assessed by immunohistochemistry (IHC). Protein levels of NLRP3, Caspase-1, GSDMD, and other pyroptosis-related proteins were measured by Western blot. ResultsCompared with the normal group, the model group exhibited significantly increased asthma symptom scores, inflammatory scores, collagen deposition, PAS scores, RL, FRC, levels of IL-6, IL-1β, and IL-18 in BALF, and expression levels of NLRP3, ASC, and other pyroptosis-related proteins in lung tissue (P0.01), while Cdyn was significantly decreased (P0.01). Compared with the model group, all doses of Chaipo decoction markedly improved asthma symptoms, with significantly reduced symptom scores (P0.05, P0.01). Pulmonary function analysis showed that medium and high doses of Chaipo decoction significantly increased Cdyn (P0.05, P0.01) and decreased RL and FRC (P0.05, P0.01). Histopathological evaluation indicated that high-dose Chaipo decoction significantly reduced inflammatory scores, collagen deposition, and goblet cell hyperplasia in lung tissue (P0.05, P0.01). ELISA results showed that all doses of Chaipo decoction significantly decreased IL-6, IL-1β, and IL-18 levels in BALF (P0.05, P0.01). IHC and Western blot analyses demonstrated that medium and high doses of Chaipo decoction markedly downregulated NLRP3, ASC, and other pyroptosis-related proteins in lung tissue (P0.05, P0.01). ConclusionChaipo decoction effectively improves pulmonary function and pathological damage in asthmatic rats, potentially by inhibiting the NLRP3/Caspase-1/GSDMD pathway and reducing pyroptosis. 
		                        		
		                        		
		                        		
		                        	
3.Improvement effects of 3,5,6,7,8,3′,4′-heptamethoxyflavone of Fructus Aurantii on rats with damp blockage of the middle energizer

		                        			OBJECTIVE To investigate the improvement effects of 3,5,6,7,8,3′,4′-heptamethoxyflavone (HMF) of Fructus Aurantii on rats with damp blockage of the middle energizer. METHODS The rats were randomly divided into normal group, model group, positive control group (Raceanisodamine tablet, 1 mg/kg), HMF low-dose, medium-dose and high-dose groups (0.3, 0.6, 0.9 mg/kg), with 7 rats in each group. Except for the normal group, the other groups were modeled by internal and external composite factors. After successful modeling, the rats in each group were given the corresponding drug or normal saline, once a day, for 14 days. The general behavioral states such as dietary intake, water intake and mental state of the rats were observed, and the fecal water content rate and saliva flow rate were measured. Hematoxylin-eosin (HE) staining was used to observe the pathological and morphology in gastric and small intestinal tissues of rats. The plasma content of aldosterone was detected, and the expression of aquaporins (AQP3) in the gastric tissue of rats was determined. RESULTS Compared with the normal group, the dietary intake and water intake of the model group rats were significantly decreased (P<0.01), the fecal water content rate, salivary flow rate, plasma content of aldosterone and the expression of AQP3 in gastric tissue were increased significantly (P<0.01). Gastric tissue injury invaded the mucosal muscle layer, resulting in mucosal muscle layer rupture; pathological and morphological changes such as small intestinal villous erosion and glandular structure destruction were observed in the small intestine. Compared with the model group, the dietary intake and water intake of rats were increased in HMF groups; fecal water content rate, salivary flow rate, plasma content of aldosterone, the expression of AQP3 in gastric tissue were decreased, most of the above differences were statistically significant (P<0.05 or P<0.01). The pathological and morphological changes in the gastric and small intestine tissues of rats had been improved to varying degrees. CONCLUSIONS HMF of Fructus Aurantii with dry property HMF could improve the symptoms of rats with damp blockage of middle energizer, the mechanism of which may be associated with reducing the content of plasma aldosterone and down-regulating the expression of gastric AQP3.
		                        		
		                        		
		                        		
		                        	
4.The effect of NLRP3 on airway epithelial-mesenchymal transition by regulating Th17/Treg imbalance in asthmatic mice

		                        			This study was designed to explore the correlation between alterations in NLRP3 levels and Th17/Treg imbalance in asthmatic mice undergoing epithelial-mesenchymal transition(EMT).A murine model of asthma was established by intraperitoneal injection combined with nebulization of ovalbumin(OVA).Mice were randomly grouped into asthma model group and normal control group.The airway reactivity was detected with non-invasive lung function instrument.Hematoxylin and Eosin(HE)and Masson's trichrome staining were applied to evaluate the histopathological injury of lung tissue and the extent of lung fibrosis;RT-qPCR was applied to detect EMT-related biomarkers(Snail,E-Cadherin,N-Cadherin),the specific transcription factors of T cell subsets(RoRγt,Foxp3)and NLRP3 in lung tissue of mice;Western blot was used to detect the protein expression of E-cadherin,N-Cadherin and NLRP3 in lung tissue of mice.The Th17 and Treg cell populations in the spleen were enumerated via flow cytometry.Furthermore,the expression levels of NLRP3,IL-17 and IL-10 in bronchoalveolar lavage fluid(BALF)were analyzed by Giemsa staining.Compared with the control group,the asthma model group showed higher level of airway resistance,coupled with an obviously decrease in pulmonary ventilation compliance.Pathological alterations in lung tissue were evident,characterized by thickening of the airway epithelium,airway stenosis,infiltration of inflammatory cells,higher expression levels of N-Cadherin and NLRP3 proteins(P<0.05),lower expression level of E-Cadherin(P<0.001)and higher levels of marker genes(Snail and N-Cadherin)in lung tissue.Furthermore,model mice demonstrated higher level of NLRP3 in BALF(P<0.05),higher level of Th17 in spleen,and higher levels of retinoic acid orphan receptor(ROR)-γt mRNA(P<0.05)and Th17-related cytokines(IL-17)(P<0.01).Concurrently,model mice also showed an obviously decrease in the prevalence of Treg cells,Forkhead box Foxp3 mRNA(P<0.001),and Treg-related cytokine IL-10(P<0.05).The results of the Pearson correlation analysis indicated that the level of NLRP3 mRNA was positively correlated the ratio of RoR γt mRNA,but negatively correlated with Foxp3 mRNA in the lung tissue of asthmatic mice.Additionally,NLRP3 in BALF demonstrated a positive correlation with IL-17 and a negative correlation with IL-10.In conclusion,These findings suggest that NLRP3 may trigger bronchial EMT by exacerbating the immune imbalance of Th17/Treg cells.

Epilepsy serves as an indicator of CP severity and has a significant impact on the early survival and future quality of life of children with CP. Therefore, it is crucial to investigate the shared mechanisms underlying CP and epilepsy. 		                        			Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
		                        		
		                        		
		                        		
		                        	
7.Health benefits of home-based exercise rehabilitation for people with severe disabilities in Shenzhen,China

		                        			Objective To analyze the needs and current situation of home-based exercise rehabilitation for people with disabilities,develop service plans for different types of disabilities,and assess its health benefits for people with severe dis-abilities. Methods Based on the World Health Organization(WHO)Global Action Plan on Physical Activity 2018-2030:More Ac-tive People for a Healthier World and WHO Guidelines on Physical Activity and Sedentary Behavior,functional and individualized home-based exercise rehabilitation plans for people with severe disabilities were developed.Begining from May,2023,a six-month intervention was conducted for 37 people with severe disabilities.They were assessed with International Physical Activity Questionnaire(IPAQ),WHO Disability Assessment Schedule 2.0(WHODAS 2.0),WHO Quality of Life(WHOQOL-BREF)before and after intervention. Results The time of low-intensity leisure physical activity of IPAQ increased after intervention,while the scores of do-mains of cognition,mobility,self-care,getting along,life activities and participation,and total score of WHO-DAS 2.0 decreased,and the scores of physical health,psychological health,independence,and environment di-mensions,and total score of WHOQOL-BREF increased. Conclusion The functioning-oriented and individualized home exercise rehabilitation program can improve the level of physical activity,enhance the level of activity and boost the quality of life for people with severe disabilities.

		                        			Acute kidney injury (AKI) is a common serious complication after acute ischemic stroke (AIS), which is associated with an increased mortality and disability. However, the clinical prevalence is often underestimated or overlooked. 10.Development and validation of a predictive model for acute kidney injury in young, middle-aged and elderly patients with acute ischemic stroke

		                        			Objective:To develop a risk prediction model for acute kidney injury (AKI) in young, middle-aged and elderly patients with acute ischemic stroke (AIS), and verify the predictive ability of the model.Methods:Patients admitted to the Department of Neurology, Lianyungang Hospital Affiliated to Xuzhou Medical University from January 2018 to August 2022 were retrospectively included as a modeling cohort, and patients with AIS from September 2022 to September 2023 were prospectively included as a validation cohort. Independent risk factors for AKI were determined by multivariate logistic regression analysis, and risk prediction models for AKI in young AIS patients group and middle-aged and elderly AIS patients group were developed. The predictive power of the model was tested using the receiver operating characteristic (ROC) curve. Results:The young group included 760 patients with AIS, of which 584 (76.84%) were in the modeling cohort, and 146 (25.00%) had AKI. Multivariate logistic regression analysis showed that anemia, systolic blood pressure, homocysteine, alcohol consumption, blood urea nitrogen, and National Institutes of Health Stroke Scale (NIHSS) score were independent risk factors for AKI (all P<0.05). ROC analysis showed that the area under the curve of the predictive model was 0.938 (95% confidence interval 0.912-0.963), the Youden index was 0.747, and the optimal cut-off value was 0.249. The sensitivity and specificity of predicting AKI were 84.8% and 89.9%, respectively. A total of 1 417 patients with AIS were included in the middle-aged and elderly group, of which 833 patients (58.79%) were in the modeling cohort and 230 (27.61%) had AKI. Multivariate logistic regression analysis showed that hypertension, atrial fibrillation, previous stroke history, smoking, infection, triglycerides, NIHSS score, use of antihypertensive drugs, use of loop diuretics, serum creatinine, and blood urea nitrogen were the independent risk factors for AKI ( P<0.05). ROC analysis showed that the area under the curve of the predictive model was 0.838 (95% confidence interval 0.808-0.868), the Youden index was 0.539, the optimal cut-off value was 0.242, and the predictive sensitivity and specificity were 78.3% and 75.6%, respectively. The Hosmer-Lemeshow goodness of fit test showed the predictive accuracy of the model was in good agreement with the actual occurrence of risk (the young group: χ2=8.968, P=0.345; the middle-aged and elderly group: χ2=11.250, P=0.188). The validation cohort analysis shows that the model had high prediction accuracy and credibility in two groups.
