1.Dosimetric comparison between abdominal deep inspiration breath hold and free breath in intensity modulated radiotherapy for left breast cancer

Objective To explore the dosimetric differences between abdominal deep inspiration breath hold(ADIBH)mode and free breath(FB)mode in intensity modulated radiation therapy(IMRT)for left breast cancer.Methods From July 2022 to May 2023,a total of 22 patients who needed adjuvant radiation therapy after left breast cancer surgery in the hospital were selected as the research objects.The simulated computed tomography(CT)positioning images of ADIBH and FB modes were collected,the planned target volume(PTV)and endangered organs were outlined,the IMRT plan was designed,and the dosimetric param-eters of the two modes were compared.Results There was no significant difference in the mean dose(Dmean),homogeneity index(HI)and conformity index(CI)of PTV between the ADIBH and the FB modes(P>0.05).Compared with the FB mode,the heart Dmean,V5,V10,V20,V30 and V40 in the ADIBH mode decreased by 2.95 Gy,12.21%,8.26%,6.56%,5.41%and 3.48%,respectively,and the left anterior descending(LAD)coronary artery Dmean,maximum dose(Dmax),minimum dose(Dmin)and V40 decreased by 15.99 Gy,16.10 Gy,0.82 Gy and 13.73%,respectively,with statistical significance(P<0.05).Compared with the FB mode,the dose and volume of heart irradiation in the ADIBH mode at the same level were significantly reduced.Pearson correlation analysis showed that there was a positive correlation between heart Dmean and LAD Dmean in the ADIBH mode(r=0.72),and between heart Dmean and LAD Dmean in the FB mode(r=0.69).Compared with the FB mode,the left lung Dmean of the ADIBH mode decreased by 0.99 Gy,and the difference was statistically significant(P<0.05).However,there was no significant difference in left lung V5,right lung Dmean and right breast Dmean between the two breathing modes(P>0.05).Conclusion ADIBH mode can effectively reduce the dose to the heart and LAD,and play a good protective role.

Copper,a trace element,plays a crucial physiological role in the human body and is involved in many important reactions.In multicellular organisms,copper metabolism encompasses absorption,distribution,sequestration,and excretion.Influx of extracellular copper ions is primarily mediated by the high-affinity copper transporter protein 1.Copper utilization pathways include the Atox1-ATP7A/B-Cp pathway,the COX17-Sco1/2-CCO pathway,and the CCS-SOD1 pathway,with copper ions being released into bile by ATP7B for excretion.The copper transport system plays a critical role in maintaining copper homeostasis within the body,ensuring normal tissue function.Both copper deficiency and excess can be harmful,and an imbalance in copper homeostasis may lead to various diseases and is associated with the development of tumors.Recent research has introduced the concept of copper-dependent cell death,a novel mecha-nism of cell death distinct from other known forms.This article provides an overview of the copper transport system,diseases related to copper homeostasis imbalance,and copper-dependent cell death,aiming to provide a theoretical basis for the prevention and treatment of copper-related diseases.
3.Discussion on the current situation and prospect of compact county medical community in China based on typical cases

In order to further promote hierarchical medical system and enhance the capacity of primary healthcare services, China began to build compact county medical community. At present, the development of China′s compact county community still exists such problems as the construction of service system, the basic institutional mechanism, the quality of supply capacity and the core guarantee mechanism. In order to solve the existing problems, the authors took Shaxian District of Sanming City, Fujian Province, Dancheng County of Zhoukou City, Henan Province, and Yangqu County, Taiyuan City, Shanxi Province as examples to summarize the experience of the advanced pilot areas of the policy. In the future, the construction of compact county community in China should be based on the construction of " single-core multi-layer" system, starting from the five levels of collaborative management, practical operation, system construction, basic guarantee and multi-party supervision, to comprehensively enhance the healthcare services, and finally achieve the goal of the strategy of hierarchical medical and Healthy China.
4.Influencing factors of medication adherence in patients with severe mental disorders in Zhengzhou

ObjectiveTo investigate the status and influencing factors of medication adherence in patients with severe mental disorders in Zhengzhou, so as to provide references for the formulation of prevention and treatment measures for severe mental disorders. MethodsFrom March to June 2021, a stratified multistage cluster sampling method was applied to select 342 patients from the National Information System for Severe Mental Disorders in Zhengzhou. The general demographic data of patients were collected via self-designed questionnaire, and the medication status was investigated, then the influential factors were summarized. The differences in influential factors of medication adherence were compared between the medication adherence group and the medication non-adherence group. Thereafter, Logistic regression analysis was applied to explore the factors influencing medication adherence. ResultsA total of 320 patients were included in the final analysis, altogether 76.56% of patients (n=245) complied with medication. The differences between patients in the medication adherence group and those in the medication non-adherence group were statistically significant in terms of residence, occupation, and outpatient chronic disease reimbursement (χ2=14.015, 7.502, 13.106, P<0.05 or 0.01). In the questionnaire of influential factors on medication adherence, there were statistically significant differences in the scores of lack of insight, stigma and drug-related factors between the two groups (Z=7.588, 2.379, 2.893, P<0.05 or 0.01). Outpatient chronic disease reimbursement was a protective factor for medication adherence (OR=2.727, 95% CI: 1.320~5.634, P<0.01), while rural residence (OR=0.465, 95% CI: 0.221~0.977, P<0.05) and lack of insight (OR=0.398, 95% CI: 0.286~0.553, P<0.01) were risk factors for medication adherence. ConclusionPatients with severe mental disorders in Zhengzhou have a high rate of medication adherence, moreover, the outpatient chronic disease reimbursement, lack of insight and residence may be influencing factors for medication adherence in patients with severe mental disorders.
5.MRI features of solitary fibrous tumors in the spinal canal

Objective To investigate the MRI features of solitary fibrous tumors(SFT)in the spinal canal.Methods MRI images of 5 cases with pathologically proved STF in the spinal canal were analyzed retrospectively.Results Of 5 lesions,there were 2 in the cervical spine,3 in the thoracic spine;1 in the epidural space and 4 in the subdural extramedullary space.On MRI plain scan,3 lesions showed homogeneous iso-intense signal on T1WI and hypo-intense signal on T2WI,2 lesions showed heterogeneous signal,1 showed patchy hypo-intense signal on T1WI and T2WI at the upper edge of lesion,which had been confirmed as hemorrhage and the other lesion showed internal cystic variation.All of the 5 lesions enhanced on enhancement scan,with moderate enhancement in 2 lesions and significant enhancement in 3 lesions.Cystic and hemorrhagic area were not enhanced.The"dural tail sign"was showed in 3 cases.Conclusion The diagnosis of SFT should be considered when a lesion shows a localized solitary mass in the spinal canal with hypo-intense on T2WI and moderate to significant enhancement.
6.Expression of stem cell surface marker CD133 in hepatocellular carcinoma and its positive subset proliferation

BACKGROUND:CD133 has been used as a marker to separate and study different tumor stem cells by using its protein specific features. OBJECTIVE:To investigate the expression of stem cell surface marker CD133 in hepatocel ular carcinoma and the in vitro proliferation of its positive subsets. METHODS:The human hepatoma MHCC97-H cell lines were cultured and sorted, CD133+and CD133-MHCC97-H cells were obtained, and the expression of CD133 and cell migration and invasion were detected. After 7 days of culture, cell proliferation ability and Notch expression were detected. After 14 days of culture, cell cloning efficiency was detected. CD133+and CD133-MHCC97-H cells were implanted into nude mice subcutaneously, and 4 weeks later, tumor formation in mice was detected. RESULTS AND CONCLUSION:(1) CD133 expression and cell clone formation:The expression of CD133 and cell cloning efficiency of CD133+MHCC97-H cells were significantly higher than those of CD133-MHCC97-H cells (P<0.05). (2) Cell proliferation:After 3-7 days of culture, the absorbance value of CD133+MHCC97-H cells was significantly higher than that of CD133-MHCC97-H cells (P<0.05). (3) Cell migration and invasion:The number of CD133+MHCC97-H cells passing through the cell membrane was significantly more than that of CD133-MHCC97-H cells (P<0.05). (4) The Notch protein expression of CD133+MHCC97-H cells was significantly higher than that of CD133-MHCC97-H cells (P<0.05). (6) Tumor formation test:The tumor size of CD133 -MHCC97-H cells was larger than that of CD133-MHCC97-H cells (P<0.05). To conclude, CD133+hepatocel ular carcinoma cell subsets with strong proliferation ability have some characteristics of cancer stem cells, and strong invasion, metastasis and tumorigenic abilities.
7.Variety methods study on hemolytic effect of colored injection doxorubicin hydrochloride in vitro

Objective The visual inspection method were not appropriate to perform a hemolysis evaluation for colored injection like doxorubicin hydrochloride,this article adopted three methods to evaluate the hemolysis test of doxorubicin hydrochloride in vitro and provide reference for clinical drug safety.Methods Using rabbit erythrocytes as experimental object,the durg concentration 4.0 and 2.0 mg/mL was chosen which range of clinical concentration and preclinical safety evaluation concentration,to evaluate the hemolysis test of doxorubicin hydrochloride injection with blood analyzer test,direct colorimetric assay,and indirect colorimetric assay.Results The evaluation results of three different methods were very consistent.The tube's hemolysis rate of 4.0 mg/mL dose was far greater than 5%,which means serious hemolysis;Only 0.1 mL tube of 2.0 mg/mL dose (according to the drug concentration equal to 0.5 mL tube of 0.4 mg/mL drug concentration) without hemolysis occurring,the other tubes' hemolysis rates were far greater than 5%,which means serious hemolysis.Conclusion The hemolysis phenomenon may occur when 2.0 mg/mL dose of doxorubicin hydrochloride solution for iv injection is used in clinic and dilution (final concentration not more than 0.4 mg/mL) is recommended.
8.Micro-invasive embedding combined with montelukast sodium for children cough variant asthma:a randomized controlled trial.

OBJECTIVETo observe the effects of micro-invasive embedding combined with montelukast sodium and simple montelukast sodium for children cough variant asthma (CVA).
METHODSA total of 240 patients were randomly assigned into an observation group and a control group, 120 cases in each one. Considering of cases dropping, 101 patients in the observation group and 105 cases in the control group were included. Montelukast sodium chewable tablets were applied before sleep for 3 months in the control group, 5 mg a time, once a day. Based on the treatment as the control group, micro-invasive embedding was used for 3 months in the observation group, twice in the first month and once in the other two months. The acupoints were Feishu (BL 13), Danzhong (CV 17), Dingchuan (EX-B 1), and Zusanli (ST 36). Follow-up was conducted 9 months after treatment in the two groups. The cough score, serum immunoglobulin (IgE, IgG, IgA), platelet activating factor (PAF) were observed before and after treatment. The indices were compared before and after treatment and at follow-up, including pulmonary function indices[peak expiratory flow rate (PEF), forced expiratory volume at the 1st second (FEV1)], and small airway function indices[forced expiratory flow rate with remaining 25% vital capacity (MEF25%), forced expiratory flow rate with remaining 50% vital capacity (MEF50%), forced expiratory flow rate with remaining 75% vital capacity (MEF75%) and mid expiratory flow rate (MEF25%-75%)]. Also, the total effects were evaluated.
RESULTS①The total effective rate in the observation group was 93.1% (94/101), which was better than 87.6% (92/105) in the control group (<0.05). The cough disappearance time of the cured children in the observation group was (10.38±2.64) d, and it was shorter than (10.72 ±2.60) d of those in the control group (<0.05). After treatment, the cough score apparently decreased compared with those before treatment in the two groups (both<0.05), with better result in the observation group (<0.05). At follow-up, the recurrence frequency of the observation group was (1.43±1.20), and it was less than (1.91±1.71) in the control group (<0.05). ②The levels of serum IgA and IgG after treatment in the two groups increased, and those of serum IgE and PAF decreased, compared with those before treatment. There was statistically significance except IgG in the control group before and after treatment (all<0.05), with better Results in the observation group after treatment (all<0.05). ③ Compared with those before treatment, all the pulmonary function indices were improved obviously after treatment and at follow-up in the two groups (all<0.05), without statistically significance between the two groups (both>0.05). ④ There was no statistically significance before and after treatment on small airway function indices in the two groups (all>0.05). The indices at follow-up increased compared with those before treatment in the two groups (all<0.05), with better Results in the observation group (all<0.05).
CONCLUSIONSMicro-invasive embedding combined with montelukast sodium achieved de-finite effect for children CVA, which can improve the body's immune and microcirculation. The effect is better than that of simple montelukast sodium on improving small airway function, etc.
9.Effects of rapamycin on promoting oral squamous cancer cell apoptosis by inhibiting TLR4, IL-6 and PGE2 expression

Objective To investigate whether the effects of rapamycin on promoting oral squamous cancer cell apoptosis is related with inhibiting TLR4 expression and inflammatory factor IL-6 and PGE2 expression.Methods Human oral squamous carcinoma SCC-15 cell line was cultured and interfered by rapamycin.Then the activity of SCC-15 cells was detected by CCK 8,the SCC-15 cells apoptosis was detected by the Hochest33342/PI double staining,the invasion ability was determined by the transwell method.The TLR4 protein expression level was detected by Western blot and IL-6 and PGE2 expressions were detected by ELISA.Results Rapamycin could promote cell apoptosis,the invasion ability of SCC-15 cells was significantly decreased.After rapamycin intervention,the TLR4 protein expression was decreased and expression levels of IL-6 and PGE2 were reduced,showing statistical difference as compared with the negative group (P<0.05).Conclusion Rapamycin promotes oral squamous cancer SCC-15 cell apoptosis,which may be related with inhibiting TLR4,IL-6 and PGE2 expression.
10.Optimization of parameters of passive cutaneous anaphylaxis in rats

OBJECTIVE To optimize the parameters of passive cutaneous anaphylaxis(PCA)in rats immunized by ovalbumin(OVA). METHODS 1-2 month-old Sprague-Dawley rats were immu?nized by ip injection of OVA(0.2,1.0 and 5.0 mg per rat)mixed with complete Freund′s adjuvant once every other day 3 times. Serum was collected on the 12th-16th days after final immunization. Then the rats were intracutaneously injected with sensitized serum and then stimulated by iv injection of the same dose of OVA mixed with Evans blue after a latent period of 0.5,1.5,3,6,12,24,36,48 and 60 h. Finally,the diameters of blue spots in the skin were measured at stimulation. RESULTS Serum total-IgE(T-IgE)and OVA-specific IgE(sIgE)levels increased significantly and reached the peak on the 3rd-7th days and 12th-16th days after final immunization,respectively. There was no correlation between the serum T-IgE level and OVA-sIgE level when the rats were immunized with OVA at OVA 0.2-5.0 mg per rat. The rats experienced PCA after injection of OVA 1.0 and 5.0 mg per rat. Diameters of blue spots in the skin reached the maximum value after rats were sensitized for 0.5-3 h. Moreover,the shape,color and size of blue spots were better 30-60 min after stimulation. CONCLUSION Optimized PCA is as follows:1-2 month-old rats are immunized on the 1st,3rd and 5th days by ip injection of OVA 1.0-5.0 mg. The immunizing serum is collected at 12-16 d after final immunization. The rats are stimulated by OVA and Evans blue after a latent period of 0.5-3 h. Diameters of blue spots in rats′ skin are then measured 30-60 min after stimulation.
