1.Effect of Shenlong Dingji Formula (参龙定悸方) on the Quality of Life in Patients with Paroxysmal Atrial Fibrillation of Qi-Yin Deficiency and Phlegm-Stasis Obstructing Collaterals Syndrome

ObjectiveTo explore the effectiveness and safety of Shenlong Dingji Formula (参龙定悸方) on the quality of life in patients with paroxysmal atrial fibrillation (PAF) of qi-yin deficiency and phlegm-stasis obstructing collaterals syndrome. MethodsA total of 60 patients with PAF of qi-yin deficiency and phlegm-stasis obstructing collaterals syndrome were recruited and randomly divided into a treatment group and a control group, with 30 patients in each group. The control group received standard western medicine treatment, while the treatment group was additionally given Shenlong Dingji Formula orally, one dose per day. Both groups were treated for 4 weeks. The primary outcome measure is the Atrial Fibrillation Effect on Quality of Life (AFEQT) score including scores of four dimensions,i.e. atrial fibrillation-related symptoms, treatment concerns, daily activities, and treatment satisfaction. The secondary outcome measures included the frequency and duration of symptomatic atrial fibrillation episodes and traditional Chinese medicine (TCM) syndrome scores covering symptoms such as palpitations, chest tightness, fatigue, shortness of breath, reluctance to speak, spontaneous sweating, stabbing pain, and insomnia. These indicators were assessed at baseline (before treatment), after 2-week of treatment, after 4-week of treatment, and 4 weeks after the end of treatment (follow-up). Additionally, safety indicators before and after treatment and adverse events occurring during the trial were recorded to evaluate safety. ResultsA total of 56 patients completed the study, with 28 in each group. Primary outcome indicators: 1) the treatment group showed significant improvement in the total score of the AFEQT scale, with significantly higher total scores after 2-week treatment, 4-week treatment, and follow-up compared to the previous time point (P<0.05). In the control group, the AFEQT score significantly increased only after 4-week treatment compared to baseline (P<0.05). In the treatment group, the AFEQT scores after 2-week, 4-week treatment, and during follow-up were all higher than those of the control group at the corresponding time points (P<0.01). 2) In the treatment group, there was no statistically significant difference in the AFEQT treatment satisfaction dimension score during follow-up compared to that after 4-week treatment (P>0.05). However, the scores for all other dimensions at each time point were higher than those at the previous time point (P<0.05). In the control group, the scores for the atrial fibrillation-related symptom dimension were higher after 2-week and 4-week treatment than those of the previous time points (P<0.05). For the treatment satisfaction dimension, significant increases were observed only after 2-week and 4-week treatment compared to baseline (P<0.05). Secondary outcome indicators: 1) In the treatment group, the frequency and duration of symptomatic atrial fibrillation episodes decreased significantly at each time point compared to the previous time point (P<0.05), except for the duration of trial fibrillation at follow-up. In the control group, the frequency of episodes decreased significantly at all time points compared to baseline (P<0.05), while the duration of trial fibrillation showed a significant reduction at follow-up compared to those after 2-week treatment (P<0.05). 2) In the treatment group, TCM syndrome scores significantly reduced after 2-week treatment, 4-week treatment, and during follow-up compared to the previous time point and baseline (P<0.05). In the control group, significant reductions were observed only after 4-week after treatment and during follow-up (P<0.05). The TCM syndrome scores in the treatment group were lower than those in the control group at the same time points (P<0.01). No adverse events occurred during the trial in either group, and safety indicators showed no significant changes after treatment. ConclusionShenlong Dingji Formula effectively improves the quality of life, alleviates TCM syndromes, and reduces the frequency and duration of symptomatic atrial fibrillation in patients with PAF of qi-yin deficiency and phlegm-stasis obstructing collaterals syndrome, and demonstrates good safety.
2.CT-guided pulsed radiofrequency combined with continuous nerve block for the treatment of refractory postherpetic neuralgia:a clinical study

Objective To investigate the clinical efficacy of CT-guided pulsed radiofrequency combined with continuous nerve block in the treatment of refractory postherpetic neuralgia(PHN).Methods A total of 208 patients with refractory PHN,who were admitted to the Hengshui Municipal People's Hospital of China between January 2021 and January 2023,were selected as the subjects of study.Using random number table method,the patients were divided into combination group and control group,with 104 patients in each group.The patients of control group received CT-guided pulsed radiofrequency therapy,and the patients of combination group received additional continuous nerve block therapy on the basis of the treatment of control group.The pain degree at different time point,clinical effective rate,number of analgesia remedy times,quality of sleep,and the levels of serum high mobility group box 1(HMGB1),interleukin-1 β(IL-1β)and interleukin-10(IL-10)were compared between the two groups.Results During the follow-up period,4 patients were lost in touch.Finally,103 patients were included in the combination group and 101 patients were included in the control group.The total treatment response rate in the combination group was 89.32%,which was significantly higher than 78.22%in the control group(P<0.05).There were statistically significant differences in visual analogue scale(V AS)scores and Athens insomnia scale(AIS)scores including the time effect,inter-group effect and time-group interaction effect,between the two groups(P<0.05).The postoperative one-week,2-week,4-week VAS scores and AIS scores in the combination group were remarkably lower than those in the control group(P<0.05).The number of analgesia remedy times in the combination group was smaller than that in the control group,and the used dosage of tramadol in the combination group was lower than that in the control group(P<0.05).Four weeks after treatment,the serum levels of HMGB1,IL-1β and IL-10 in the combination group were lower than those in the control group(P<0.05).Conclusion For the treatment of refractory PHN,CT-guided pulsed radiofrequency combined with continuous nerve block can effectively alleviate neural inflammatory damage,and improve pain symptoms and sleep quality,besides,its analgesic effect and clinical efficacy are superior to CT-guided pulsed radiofrequency alone.(J Intervent Radiol,2024,33:264-268)
3.Curative effect of pulsed radiofrequency combined with nerve block for the treatment of refractory postherpetic neuralgia

Objective To discuss the curative effect of pulsed radiofrequency combined with nerve block in treating refractory postherpetic neuralgia(PHN).Methods A total of 201 patients with PHN,who were admitted to the hospital to receive treatment between March 2020 and March 2023,were enrolled in this study.Using random number table method,the patients were divided into pulsed radiofrequency group,nerve block group and combination group,with 67 patients in each group.On the basis of routine medication,the patients of pulsed radiofrequency group received nerve pulsed radiofrequency treatment,the patients of nerve block group received nerve block therapy,while the patients of combination group received nerve block followed by nerve root pulsed radiofrequency.At the preoperative(T0),postoperative 7-day(AT7),30-day(AT30)and 90-day(AT90)time point,the short-form of McGill pain questionnaire(SF-MPQ),Pittsburgh Sleep Quality Index(PSQI),levels of pain mediators including β-endorphin,substance P,calcitonin gene-related peptide(CGRP),and levels of inflammatory factors including tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were determined.The adverse reactions were recorded.Results The postoperative AT7,AT30 and AT90 scores of PRI,VAS,PPI and PSQI,and levels of substance P,CGRP,TNF-α and IL-6 in combination group were lower than those in pulsed radiofrequency group and in nerve block group(all P<0.05),while level of β-endorphin in combination group was higher than that in the other two groups(P<0.05).No obvious adverse reactions were observed in all three groups.Conclusion For the treatment of patients with intractable PHN,the combination therapy of pulsed radiofrequency and nerve block is superior to single therapy in improving pain and sleep quality,which may be related to the regulation of the levels of pain mediators and inflammatory factors.
4.Effect of pulsed radiofrequency combined with nerve block on SF-MPQ score,levels of serum substance P and IL-1β in patients with refractory postherpetic neuralgia

Objective To explore the effect of pulsed radiofrequency combined with nerve block on the score of short-form of McGill pain questionnaire(SF-MPQ)and the levels of serum substance P(SP)and interleukin(IL)-1β in patients with refractory postherpetic neuralgia(PHN).Methods A total of 219 patients with PHN,who were admitted to the Hengshui Municipal People's Hospital of China between April 2021 and April 2022,were collected as the objects of study.Using random number table method,the patients were divided into group A(n=73,receiving pulsed radiofrequency therapy),group B(n=73,receiving nerve block therapy),and group C(n=73,receiving pulsed radiofrequency combined with nerve block therapy).The SF-MPQ score,levels of serum SP and IL-1β,degree of anxiety and depression,and quality of life were compared between each other among the three groups.Results Among the 219 patients with PHN,6 patients were excluded from this study due to losing in touch or due to receiving other therapies,and finally group A had 71 patients,group B had 70 patients,and group C had 72 patients.At 7 d after treatment(AT7),30 d after treatment(AT30)and 90 d after treatment(AT90),in group C the scores of pain rating index(PRI),visual analogue scale(VAS)and present pain intensity(PPI)were obviously lower than those in group A and group B(P<0.05),the levels of serum SP and IL-1β were strikingly lower than those in group A and group B(P<0.05),and the scores of patient health questionnaire-9(PHQ9),generalized anxiety disorder 7-item(DAG7)and Generic Quality of Life Inventory 74(GQOL-74)were remarkably lower than those in group A and group B(P<0.05).Conclusion Pulsed radiofrequency combined with nerve block can effectively reduce the pain degree and the serum SP and IL-1β levels,relieve the degree of anxiety and depression in patients with refractory PHN,which can improve the quality of life.
5.Relationship between cerebral microbleeds and cognitive impairment in elderly PD patients based on MRI susceptibility weighted imaging

Objective To use MRI susceptibility weighted imaging(MRI-SWI)to examine cerebral microbleeds(CMB)in elderly Parkinson's disease(PD)patients,and analyze the relationship be-tween CMB and cognitive impairment in them.Methods A prospective cohort study was conduc-ted on 80 elderly PD patients admitted to Huai'an No.3 People's Hospital from June 2020 to April 2024.All patients underwent MRI-SWI examination and PD-Cognitive Rating Scale(PD-CRS).According to the results of MRI-SWI examination,they were divided into a CMB positive group(17 cases)and a CMB negative group(63 cases),and based on the results of PD-CRS,they were also assigned into a cognitive impairment group(34 cases)and a normal cognitive group(46 ca-ses).General clinical data were compared between the CMB positive and negative groups,and be-tween the cognitive impairment group and normal cognitive group.CMB type and severity were compared between the cognitive impairment group and the normal cognitive group.Multivariate logistic regression analysis was applied to investigate the relationship between CMB and cognitive impairment in elderly PD patients.Results The CMB positive group had longer PD course,ad-vanced Hoehn Yahr(H-Y)late stage,higher unified Parkinson's disease rating scale(UPDRS)Ⅰscore,and larger ratio of cognitive impairment than the CMB negative group(P<0.05,P<0.01).The proportion of cognitive impairment in patients with lobular CMB was higher than that in deep-subtentorial CMB and mixed patients,and the proportion of cognitive impairment in patients with severe CMB was higher than that in patients with mild and moderate CMB(P<0.05).Multivariate logistic regression analysis showed that the occurrence of cognitive impairment in elderly PD patients was related to the course of disease,H-Y mid stage,H-Y late stage,CMB,and UPDRS Ⅰ score,which may be risk factors for cognitive impairment(OR=1.356,95%CI:1.121-1.640;OR=16.585,95%CI:2.532-108.623;OR=5.786,95%CI:1.073-31.206;OR=6.553,95%CI:1.066-40.281;OR=2.102,95%CI:1.384-3.194).Conclusion The coexistence of CMB in elderly PD patients can increase the risk of cognitive impairment,with a higher risk of cognitive impairment in cerebral lobe CMB and severe CMB.Therefore,early detection is crucial.
6.Immune infiltration mechanism of lupus nephritis and prediction of potential traditional Chinese medicine targeting based on CIBERSORT deconvolution algorithm

Objective:Using bioinformatics methods to study the immune infiltration mechanism of lupus nephritis(LN)and to explore potential target Chinese medicines,which can provide new directions for clinical treatment of LN.Methods:Gene expres-sion profile microarray dataset of LN was downloaded from GEO database,differential expressed genes(DEGs)were screened using R software,and GO and KEGG enrichment analysis were performed on these DEGs.Protein interaction network analysis of DEGs was performed by applying STRING database,key genes were screened by using Cytoscape,core gene expression was validated by Nephro-seq database,and the infiltration and correlation of 22 kinds of immune cells in LN were calculated by CIBERSORT deconvolution algorithm.Finally,herbal prediction of significantly enriched immune-related biological processes and key target genes were performed by Coremine Medical database.Results:A total of 367 LN-related DEGs were obtained,of which 253 were up-regulated and 114 were down-regulated.GO was mainly enriched in response to viruses,defense response to viruses,regulation of viral life cycle,regulation of viral processes,etc;KEGG was mainly enriched in S.aureus infection,COVID-19,influenza A,complement and coagulation cascade,pertussis,etc.Ten key genes were screened:IFIT3,OAS2,MX1,OAS1,RSAD2,XAF1,ISG15,IFIT1,ITGAM and PTPRC.Immune infiltration by CIBERSORT deconvolution algorithm showed that monocytes and initial B cells were highly expressed in LN,while memory B cells,naive CD4+T cells,follicular helper T cells and resting natural killer cells were lowly expressed in LN.Immune cell correlation analysis showed a positive correlation between initial B cells and plasma cells,a positive correlation between resting natural killer cells and resting dendritic cells,a negative correlation between monocytes and regulatory T cells,and a negative correlation between resting CD4+memory T cells and monocytes.Coremine Medical predicted that Salvia miltiorrhiza,Panax notogin-seng,Scutellaria baicalensis,ginseng,honey,monkey mushroom and peony were found to be most closely related to immunity in LN.Conclusion:The development and progression of LN are results of the combined involvement of multiple genes and pathways.Mono-cytes,memory B cells,initial CD4+T cells and initial B cells are most closely associated with LN.The predicted Salvia miltiorrhiza,Panax notoginseng,Scutellaria baicalensis,ginseng,honey,monkey mushroom and peony may be used as target herbs for the poten-tial treatment of LN.
7.Transesophageal echocardiography for surgical therapy of pediatric atrioventricular valve diseases

Objective To explore the value of transesophageal echocardiography(TEE)for surgical therapy of pediatric atrioventricular valve diseases.Methods Data of 273 children with atrioventricular valve diseases who underwent surgical operation under extracorporeal circulation were retrospectively analyzed.Transthoracic echocardiography(TTE)was performed within 1 week before,while TEE was performed before and after surgical operation.Whether TTE diagnosis should be adjusted were evaluated according to findings of TEE,so were the effects of surgical treatments,and the ultrasonic diagnostic coincidence rate before operation were compared taken surgical findings as standards.Results TEE was successfully completed in all 273 children.The diagnostic coincidence rate of preoperative TTE was 83.88%(229/273).After adjustment or supplement according to TEE in 44 cases(44/273,16.12%),the ultrasonic diagnostic coincidence rate reached 100%(273/273),higher than that of preoperative TTE alone(P<0.001).Residual shunt was found in 5 cases,while no obvious improvement of valvular regurgitation was noticed in 3 cases with post operative TEE,and after corresponding treatments,re-examination of TEE showed satisfactory efficacy in all the above cases.Conclusion TEE could be used to accurately diagnose pediatric atrioventricular valve diseases before and evaluate the efficacy after surgical treatments,having important clinical application value.
8.Analysis of Key Genes and Immune Infiltration Mechanism of Primary Sj?gren's Syndrome and Prediction of Targeted Traditional Chinese Medicine Based on Bioinformatics

Objective Using bioinformatics methods to study the immune infiltration mechanism of Primary Sj?gren's syndrome(pSS)and to explore potential target Chinese medicines,which can provide new directions for the clinical treatment of pSS.Methods Gene expression profile microarray dataset of pSS was downloaded from the GEO database,differential genes were screened using R software,and gene ontology(GO)and gene pathway enrichment(KEGG)enrichment analysis was performed on these differential genes.Protein interaction network analysis of differential genes was performed by applying the STRING database,key genes were screened by using Cytoscape,and ELISA for the verification of key genes expression.Immune infiltration and correlation of immune cells in pSS were calculated by CIBERSORT inverse convolution method in 22.Finally,the herbal prediction of key target genes was performed by using the Coremine Medical database.Results A total of 232 differential genes were obtained,of which 207 were up-regulated and 25 were down-regulated.GO was mainly enriched in:leukocyte mediated immunity,lymphocyte mediated immunity,leukocyte cell-cell adhesion,etc;KEGG was mainly enriched in Hematopoietic cell lineage,Primary immunodeficiency,Intestinal immune network for IgA production,Phagosome,Leishmaniasis.Ten key genes were screened:PTPRC,CD19,LCP2,CCR5 and CD69 etc.The hub genes expression in the pSS is the same as that of GSE40611.Immune infiltration showed that memory B cells,T cells CD4 memory activated,and T cells CD4 na?ve were highly expressed in the pSS.Immune cell correlation analysis showed a positive correlation between initial Monocytes and T cells regulatory(Tregs),a positive correlation between Macrophages M1 and B cell na?ve,and a negative correlation between Plasma cells and T cells CD4 memory activated.COREMINE Medical predicted that Ginseng,Panax notoginseng,Tripterygium wilfordii,Burnet,Magnolia,and Strychni may treat pSS.Conclusion The development and progression of pSS are the results of the combined involvement of multiple genes and pathways.Memory B cells,T cells CD4 memory activated,and T cells CD4 na?ve may promote the development of pSS.The predicted Ginseng,Panax notoginseng,Tripterygium wilfordii,Burnet,Magnolia,Strychni may be used as target herbs for the potential treatment of pSS.
9.Temporal dependence of neuronal alpha-synuclein oligomerization and nuclear translocation induced by paraquat

Background Paraquat (PQ), one of the environmental poisons associated with sporadic Parkinson's disease (PD), can cause abnormal aggregation of alpha-synuclein (α-syn), but the research on its conformational changes and subcellular localization is limited. Objective To investigate the effect of PQ on α-syn conformation and subcellular localization in dopaminergic neurons. Methods Forty-eight SPF C57BL/6 male mice were selected and randomly divided into a control group and a model group. The model group was intraperitoneally injected with PQ (15 mg·kg−1), and the control group was intraperitoneally injected with 0.9% normal saline, twice a week for eight weeks to construct a PD-like mouse model. The changes of neurobehavior (by open field test and pole climbing test) were observed to evaluate motor ability of mice. Immunohistochemical staining (IHC) was used to detect the expression levels of tyrosine hydroxylase (TH) and α-syn in the midbrain. Western blotting (WB) was used to measure the protein expression levels of TH and α-syn in midbrain. Human neuroblastoma SH-SY5Y cells were used as dopaminergic neuron in vitro models. After the cells were treated with PQ (100 μmol·L−1) for 0, 12, 24, 36 and 48 h, the expressions of α-syn in whole cell, cytoplasm, and nucleus were detected by WB; the expression level of extracellular α-syn was detected by enzyme-linked immunosorbent assay (ELISA); the change of α-syn location was observed by immunofluorescence assay (IFA). Results The neurobehavioral tests' results showed that compared with the control group, the residence time in peripheral area of mice in the PQ model group increased with the increase of exposure time (P<0.05), the residence time and moving distance in the central region decreased (P<0.05), and the pole climbing time increased (P<0.05). The mouse IHC results showed that compared with the control group, the number of TH positive cells in the midbrain decreased in the model group at week 6 and 8 (P<0.05), while the expression level of α-syn increased at week 4, 6, and 8 (P<0.05). The WB results of mouse showed that the relative expression of TH decreased significantly after 6 and 8 weeks of PQ exposure (P<0.05), and the relative expression of oligomer α-syn increased after 4, 6, and 8 weeks of PQ exposure (P<0.05). The WB of in vitro models results showed that the relative expression of α-syn in cells increased with time (R2=0.7440, P<0.05); the relative expression of α-syn in cytoplasm increased firstly and then decreased with time (P<0.05); the relative expression of α-syn in nucleus increased with time (R2=0.7913, P<0.05). The IFA results of in vitro models showed that the expression of oligomerized α-syn increased and translocated to the nucleus (P<0.05). The ELISA results of in vitro models showed that α-syn increased with the increase of PQ exposure time (P<0.05). Conclusion PQ can increase the expression of α-syn in dopaminergic neurons, induce oligomerization and translocation to the nucleus.
10.The mechanism of HDAC6 in paraquat-induced autophagy dysfunction of dopaminergic neurons by mediating aggresome-autophagy-lysosomal pathway

Objective:To explore the mechanism of HDAC6 mediated aggresome-autophagy-lysosome pathway in paraquat-induced autophagy in dopaminergic neurons.Methods:Human neuroblastoma cell (SH-SY5Y cell) was used as model of dopaminergic neurons in vitro. The cells were treated with terminal concentrations of 0, 25, 50, 100, 200 and 400μmol/L PQ for 24 hours, and the cells were induced by 100 μmol/L PQ for different time (0, 12, 24, 36, 48, 60 and 72 h) . Cell viability was detected by CCK-8 assay. The expression levels of HDAC6, α-syn, Dynein IC1/2, LC3, Beclin1, p62 and Lamp-1 were detected by Western blot. Immunofluorescence double-labeling method was used to observe the expression and localization of HDAC6, α-syn, Dynein IC1/2, LC3, Lamp-1 and γ-tubulin in cells.Results:CCK-8 assay showed PQ induced cell survival rate decrease in a time and dose dependent manner ( R=-0.950、-0.960, P<0.05) .Western blot showed that compared with control group, the protein levels of HDAC6, α-syn, p62 in PQ-exposed group were significantly increased ( P<0.05) , but there was a significant decrease in expression level of the ratio of autophagy-related protein LC3 Ⅱ/LC3 Ⅰ, Beclin1, Dynein IC1/2, Lamp-1in PQ-exposed group ( P<0.05) . The results of immunofluorescence double-labeling showed that compared with the control group, the fluorescence signals of HDAC6 and α-syn in the PQ-exposed group increased, and the protein expression level increased, while the fluorescence signals of Dynein IC1/2, LC3, and Lamp-1 decreased. The protein expression level is reduced. HDAC6 gradually accumulates from the diffuse shape to the nucleus; Under normal circumstances, α-syn, Dynein IC1/2, γ-tubulin, LC3, and Lamp-1 are mainly distributed in the cytoplasm. After PQ is infected, they gather in the nucleus and co-localize with HDAC6 in the area around the nucleus. Conclusion:PQ may induce abnormal aggregation of α-syn by inducing HDAC6-mediated aggresome-autophagy-lysosomal pathway disorder.
