1.Corneal Collagen Cross-linking for Corneal Ulcer from Moraxella Group

PURPOSE: To report a case of corneal collagen cross-linking for corneal ulcer caused by the Moraxella group.CASE SUMMARY: A 77-year-old male had decreased visual acuity for several days in his right eye. The patient showed severe stromal ring infiltrates with a corneal epithelial defect measuring (5.0 × 7.0 mm), a corneal endothelial plaque, and a hypopyon measuring less than 1.0 mm in height in the anterior chamber of the right eye. There was no abnormal finding in the right eye using B-scan ultrasonography. Before starting treatment, a corneal culture was conducted. The culture tests showed the presence of the Moraxella group. Because the patient was diagnosed with a corneal ulcer caused by the Moraxella group, corneal collagen cross-linking (CXL) was performed. The antimicrobial susceptibility test confirmed that this Moraxella group was sensitive to ceftazidime, so the patient was treated with 5% ceftazidime eye drops and 0.5% moxifloxacin eye drops every 2 hours for 9 months after corneal collagen CXL. The uncorrected visual acuity was 0.1 in the right eye, and there was almost no corneal stromal melting on anterior segment optical coherence tomography.CONCLUSIONS: This is the first known case of a corneal ulcer, in the Republic of Korea, caused by the Moraxella group and treated with corneal collagen CXL. Corneal collagen CXL should be considered as a surgical treatment for patients who have an impending corneal perforation due to a corneal ulcer because it is a simple procedure and causes fewer serious complications than other treatments.

2.A Report of Two Case of Ocular Toxicity Resulting from Direct or Indirect Bee Venom

PURPOSE: To report a patient stung by a bee, who was diagnosed with sterile endopthalmitis and another patient diagnosed with optic neuritis, with decreasing visual acuity, after refined bee venom injection around the orbital tissue. CASE SUMMARY: A 82-year-old female visited our hospital for decreased visual acuity in the right eye and ocular pain due to a bee sting. The bee sting penetrated the sclera into the vitreous. In the anterior segment, severe cornea edema and anterior chamber cells were seen. Using ultrasonography, inflammation was seen around the intravitreal area. After 3 months, intravitreal inflammation regressed but the patient's visual acuity was light perception negative, and corneal opacity, neovascularization, and phthisis bulbi were detected. A 55-year-old male visited our hospital for ocular pain in the right eye and decreasing visual acuity after refined bee venom injection around the orbital tissue. The best-corrected visual acuity in the right eye was 15/100, there was moderate injection on the conjunctiva. A relative afferent pupillary defect, abnormal color vision test results, and a defect in the visual field test were observed. There was no pain during external ocular movement, and other general blood tests, and a brain MRI were normal. Based on these symptoms, methylprednisolone megatherapy was started for treatment of optic neuritis. After treatment, visual acuity of the right eye was 9/10 and all other clinical optic neuritis symptoms regressed. CONCLUSIONS: Based on these two cases, ocular toxicity from bee venom could result from both direct and indirect courses. Treatment using refined bee venom might be harmful, and caution is recommended in its use.

3.Polymicrobial Keratitis of Pseudomonas aeruginosa, Acinetobacter baumannii, and Ochrobactrum anthropi

PURPOSE: To report polymicrobial keratitis involving Pseudomonas aeruginosa, Acinetobacter baumannii, and Ochrobactrum anthropi. CASE SUMMARY: A 53-year-old female complained of pain and secretion in her right eye, which started 6 weeks before her visit. She applied steroid ointment, which was received from the dermatologist, to her eyelid 7 days prior to her visit but this treatment worsened her symptoms. At the initial visit, the visual acuity of the right eye was light perception, and purulent secretions were observed. Using a slit lamp, severe conjunctival hyperemia, hypopyon, and a ring-shaped central corneal ulcer were observed. The anterior chamber and fundus were not observed due to corneal lesions but ultrasonography showed no intraocular inflammation. Infectious keratitis was suspected and cultured by corneal scraping. During the incubation period, 0.5% moxifloxacin, 2% voriconazole, and 1% cyclopentolate were administered. A total of 400 mg of moxifloxacin and 100 mg of doxycycline were given orally. In the primary culture, Pseudomonas aeruginosa and Acinetobacter baumannii were identified so 5% ceftazidime, which was sensitive for the antibiotic susceptibility results was further instilled. Thereafter, the keratitis improved but the keratitis again worsened while maintaining the topical treatment. A secondary culture was positive for Ochrobactrum anthropi. Treatment with 1.4% gentamicin, which was sensitive for the antibiotic susceptibility test was added and the keratitis improved. A conjunctival flap was performed because of the increased risk of perforation. CONCLUSIONS: We report polymicrobial keratitis involving Pseudomonas aeruginosa, Acinetobacter baumannii, and Ochrobactrum anthropi for the first time in the Republic of Korea.

4.Bilateral Frosted Branch Angiitis in Kikuchi-Fujimoto Disease

PURPOSE: A case of frosted branch angiitis in Kikuchi-Fujimoto disease is reported. CASE SUMMARY: A 33-year-old male complained of a sudden decrease in visual acuity that developed in both eyes 5 days prior. He suffered from a headache, chills, myalgia, and flank pain 1 week before. The initial best-corrected visual acuity (BCVA) was 0.1 in the right eye and 0.2 in the left eye. On slit lamp examination, no inflammatory finding was observed in the anterior chamber and vitreous body of both eyes. On fundus examination, a diffuse vascular sheathing-like frosted branch was found in the retinal vessels, and retinal hemorrhage was observed. Fluorescein angiography showed staining and leakage of dye along the vascular sheathing. Serological findings were negative, showing no evidence of an autoimmune disease or viral infection. Neck ultrasonography revealed non-tender left cervical lymph node enlargement >1 cm in diameter. Ultrasound-guided fine needle aspiration cytology showed findings compatible with Kikuchi-Fujimoto disease, including necrotic changes and pronounced karyorrhexis, plus histiocyte and lymphocyte infiltration without neutrophils. We started systemic steroid therapy. One month after treatment, the BCVA of both eyes improved to 1.0. CONCLUSIONS: In patients with frosted branch angiitis, systemic disease such as Kikuchi-Fujimoto disease should be considered.

5.Comparison of Biometric Measurements and Refractive Results among Low-coherence Reflectometry, Partial Interferometry and Applanation Ultrasonography.

PURPOSE: To compare the measurement results and the accuracy of the predicted refractive error after cataract surgery among 3 ocular biometry devices; OA-2000®, IOL Master® and A-scan ultrasound in posterior subscapular cataracts. METHODS: Biometry measurements including axial length, anterior chamber depth and the keratometry of 80 cataractous eyes were measured using ultrasonography, OA-2000® and IOL Master®. To calculate the intraocular lens (IOL) power, the SRK/T formula was used and 3 months after cataract surgery, the refractive outcome was compared to the preoperatively predicted refractive error. RESULTS: The number of eyes measured by the 3 devices (A-scan, IOL Master® and OA-2000®) was 57 (group A) and the number of eyes measured by 2 devices (A-scan and OA-2000®) was 22 (group B). When cataract grading was performed based on the Lens Opacity Classification system III, the severity of posterior subscapular opacity was significantly different between the 2 groups (p = 0.001). Although no difference was observed in the measured biometry values including axial length, anterior chamber depth and keratometry in groups A and B, the predicted refractive error was significantly different in group B; OA-2000® showed a significantly higher accuracy in predicting IOL power than A-scan. CONCLUSIONS: In cataract patients whose posterior subscapular opacity is not severe, the accuracy for predicting refractive error after cataract surgery was not significantly different among the 3 devices included in our study (A-scan, IOL Master® and OA-2000®). However, in patients with severe posterior subscapular opacity, OA-2000®, that provides a Fourier domain light source-calculated predicted refractive error of IOL may be more accurate.

6.Comparison of Ocular Biometry and Refractive Outcomes Using IOL Master 700, IOL Master 500, and Ultrasound.

PURPOSE: To compare the new swept-source optical coherence tomography based IOL Master 700 to both the partial coherence interferometry based IOL Master 500 and ultrasound A-scan in terms of the ocular biometry and the prediction of postoperative refractive outcomes. METHODS: A total 67 eyes of 55 patients who received cataract surgery were included in our study. The axial length, anterior chamber depth, and keratometry were measured using IOL Master 700, IOL Master 500, and A-scan. The predictive errors, which are the differences between predictive refraction and post-operative refraction 1 month after surgery, were also compared. RESULTS: Axial length measurements were not successful in 5 eyes measured using IOL Master 700 and in 12 eyes measured using IOL Master 500. The mean absolute postoperative refraction predictive errors were 0.63 ± 0.50 diopters, 0.66 ± 0.51 diopters, and 0.62 ± 0.51 diopters for IOL Master 700, IOL Master 500, and A-scan, respectively, and these values exhibited no statistically significant differences. The mean axial lengths were 24.25 ± 2.41 mm, 24.24 ± 2.40 mm, and 24.22 ± 2.39 mm; the mean anterior chamber depths were 3.09 ± 0.39 mm, 3.17 ± 0.39 mm, and 3.15 ± 0.46 mm; and the mean keratometry values were 44.12 ± 1.82 diopters, 44.57 ± 2.10 diopters, and 43.98 ± 1.84 diopters for the IOL Master 700, IOL Master 500, and A-scan groups, respectively. None of these parameters showed statistically significant differences between the three groups. Regarding pair-wise comparison, there were significant differences between the IOL Master 700 and the other devices. CONCLUSIONS: The ocular biometric measurements measured using IOL Master 700, IOL Master 500, and A-scan showed no significant differences. However, IOL Master 700 demonstrated a superior ability to successfully take biometric measurements compared to IOL Master 500. Therefore, IOL Master 700 is capable of measuring ocular biometry for cataract surgery in clinical practice.

7.Ocular Findings in Mucolipidosis Type II.

PURPOSE: To report ocular findings of a mucolipidosis type II patient with novel mutation. CASE SUMMARY: A 10-year-old boy visited our pediatric genetic metabolic clinic for evaluation of his overall developmental delay and short stature. The boy was diagnosed with mucolipidosis type II (I-cell disease) using plasma enzyme assay and DNA sequencing of the GNPTAB gene mutation. An ophthalmologic investigation was then performed, and a depressed nasal bridge, broad nose, and swelling in the upper lid of both eyes were noted. The best corrected visual acuity was 0.32 and 0.1 and the intraocular pressure was 35 mmHg and 24 mmHg in the right and left eyes, respectively. The anterior chamber angles of both eyes were normal and mild cornea opacity in both eyes was observed. Fundus examination revealed retinal atrophy with folds in both eyes, as well as optic disc edema and optic atrophy in the right and left eyes, respectively. Atherosclerotic changes in the retinal vessels and cystoid macular edema in the left eye were observed, and ocular ultrasound revealed increased posterior sclera thickness in both eyes. CONCLUSIONS: Ocular manifestations of mucolipidosis type II are not currently well-known, and differentiation from other metabolic disorders may be difficult. An ophthalmic work-up can assist in diagnosis, and regular ophthalmic examinations should be used to maintain visual function in mucolipidosis patients.

8.An Unusual Case of Endogenous Klebsiella Pneumonia Endophthalmitis.

PURPOSE: To report an unusual case of endogenous endophthalmitis in a patient with Klebsiella pneumoniae primary liver abscess. CASE SUMMARY: A-54-year-old man with diabetes mellitus and liver abscess was referred to us for consultation of visual loss in his left eye for 2 days. On the first examination, the patient's left visual acuity was hand motion and the left intraocular pressure was 13 mmHg. Vitreous opacity and inflammatory membrane were detected with increased echogenicity using ultrasonography. Vitectomy and intravitreous antibiotic injection were performed under the impression of endogenous endophthalmitis caused by liver abscess on the day of the first visit. Culture revealed Klebsiella pneumoniae from blood and liver abscess. After vitrectomy, the patient showed improvement. However, on the 20th and 40th postoperative days, the patient complained of blurred vision, and inflammation and hypopyon were observed in the anterior chamber. An intracameral antibiotic injection and anterior chamber washing were performed. The patient has not complained of any other symptoms to date. CONCLUSIONS: In this patient with endophthalmitis, inflammation and hypopyon in the anterior chamber were evident three times after vitrectomy. Ultimately, the inflammation was effectively controlled by intracameral antibiotic injection and anterior chamber washes.

9.A Prospective Study of Anterior Segment Ocular Parameters in Anisometropia.

PURPOSE: The aim of this study was to investigate the differences in anterior segment ocular parameters in anisometropia >1 D. METHODS: This study included 202 eyes of 101 subjects ranging from 10 to 40 years of age with anisometropia of 1 D or more. The subjects were divided into groups according to anisomyopia, anisoastigmatism, and anisohypermetropia. After providing informed consent, each patient underwent a detailed ophthalmological examination including cycloplegic refraction, best-corrected visual acuity, cover test, axial length (AL) measurement using A-scan ultrasound biometer, keratometry, anterior chamber depth, and central corneal thickness measurement. For each participant, the eye with greater refractive error was compared to the fellow eye via paired t-tests. Correlations between parameters were studied using the Pearson correlation coefficient. RESULTS: The average age of subjects was 21.7 ± 9.3 years. Of 101 subjects, 31 had anisomyopia; 42 had anisohypermetropia; and 28 had anisoastigmatism. A predisposition toward greater myopia in right eyes was noted in anisomyopia (24 of 31 subjects, 77%). The inter-ocular acuity difference was significant in all three groups (p < 0.01). As the degree of anisometropia increased, there was significant positive correlation in the difference in AL in myopes (r = 0.863, p < 0.01) and hypermetropes (r = 0.669, p < 0.01) and the difference in corneal curvature in anisoastigmatism (r = 0.564, p = 0.002) and hypermetropes (r = 0.376, p = 0.014). A significant positive correlation was also present between the anterior chamber depth difference and refractive difference in hypermetropes (r = 0.359, p = 0.020). CONCLUSIONS: This study showed that anisomyopia is correlated only with anterior chamber differences. Anisohypermetropia is correlated with AL differences as well as corneal curvature difference and anterior chamber depth difference. The amount of anisoastigmatism correlates only with corneal curvature difference.

10.Comparison of Anterior Chamber Depth and Central Corneal Thickness Measured Using Different Devices.

PURPOSE: To compare the accuracy and clinical usefulness of different devices by measuring anterior chamber depth (ACD) with three devices and central corneal thickness (CCT) with four devices. METHODS: In 180 eyes of 90 healthy subjects, ACD was measured using A-scan, Lenstar LS900®, Pentacam®, and CCT was measured using ultrasound pachymetry (USP), Lenstar LS900®, Pentacam®, and anterior segment optical coherence tomography (OCT). RESULTS: The average ACT measurements using Lenstar LS900®, A-scan, and Pentacam® were 3.27 ± 0.35 mm, 3.26 ± 0.36 mm, and 3.25 ± 0.36 mm, respectively. The measurements were significantly correlated (p < 0.001) but without statistically significant difference (p = 0.017). The Bland-Altman plots showed a low degree of agreement. The average CCT measurements using Pentacam®, USP, Lenstar LS900®, and OCT were 553.31 ± 25.23 µm, 547.26 ± 23.83 µm, 541.38 ± 24.49 µm, and 531.40 ± 22.33 µm, respectively. The measurements were significantly correlated (p < 0.001) and statistically significantly different (p < 0.05). The Bland-Altman plots showed a low degree of agreement. CONCLUSIONS: ACD and CCT measured using different devices were highly correlated, but the ACD measurements were not statistically different; however, the CCT measurements were statistically different, and agreement was low between both measurements.
