1.Analysis of three-dimensional visualization imaging of severe portal vein stenosis after liver transplantation and clinical efficacy of portal vein stent implantation

Objective To analyze three-dimensional imaging characteristics and advantages for severe portal vein stenosis after liver transplantation, and to evaluate clinical efficacy of portal vein stent implantation. Methods Clinical data of 10 patients who received portal vein stent implantation for severe portal vein stenosis after liver transplantation were retrospectively analyzed. Imaging characteristics of severe portal vein stenosis, and advantages of three-dimensional reconstruction imaging and interventional treatment efficacy for severe portal vein stenosis were analyzed. Results Among 10 patients, 3 cases were diagnosed with centripetal stenosis, tortuosity angulation-induced stenosis in 2 cases, compression-induced stenosis in 2 cases, long-segment stenosis and/or vascular occlusion in 3 cases. Three-dimensional reconstruction images possessed advantages in accurate identification of stenosis, identification of stenosis types and measurement of stenosis length. All patients were successfully implanted with portal vein stents. After stent implantation, the diameter of the minimum diameter of portal vein was increased [(6.2±0.9) mm vs. (2.6±1.7) mm, P<0.05], the flow velocity at anastomotic site was decreased [(57±19) cm/s vs. (128±27) cm/s, P<0.05], and the flow velocity at the portal vein adjacent to the liver was increased [(41±6) cm/s vs. (18±6) cm/s, P<0.05]. One patient suffered from intrahepatic hematoma caused by interventional puncture, which was mitigated after conservative observation and treatment. The remaining patients did not experience relevant complications. Conclusions Three-dimensional visualization technique may visually display the location, characteristics and severity of stenosis, which is beneficial for clinicians to make treatment decisions and assist interventional procedures. Timely implantation of portal vein stent may effectively reverse pathological process and improve portal vein blood flow.
2.Effect of nalbuphine hydrochloride combined with dexmedetomidine on post-operative recovery quality and pain in patients undergoing laparoscopic bariatric surgery

Objective To investigate the effects of nalbuphine combined with dexmedetomidine on postop-erative recovery quality and pain in patients who undergoing laparoscopic bariatric surgery.Methods A total of 169 patients who underwent laparoscopic bariatric surgery at our hospital were included and divided into control group(group C),nalbuphine group(group N),dexmedetomidine group(group D),and nalbuphine combined with dexme-detomidine group(group ND)using randomised numerical table method.Group C received intravenous injection of saline,group N and group ND received intravenous injection of nalbuphine before the end of the surgery,and group D and group ND received pumping of dexmedetomidine before anesthesia induction and during surgery.Compare the postoperative recovery quality score(QoR-40),hemodynamics at different time points,visual analogue scale score(VAS),sedation-agitation scale(SAS),first time out of bed activity and exhaust time,and incidence of nausea and vomiting among four groups.Results The postoperative QoR-40 scores of patients in group ND were better than those in group C and group N(P<0.05),and the QoR-40 scores in group D were better than those in group C(P<0.05).MAP and HR were more stable during the awakening period in group ND and group D(P<0.05).Compared with group C,patients in all three groups had lower VAS scores and SAS scores(P<0.05)and consumed less remedial analgesic medication(P<0.05).In terms of adverse reactions,the incidence of postoperative nausea,vomiting and coughing in the group ND was lower than that in the group C(P<0.05).Conclusion The combination of nalbuphine and dexmedetomidine could improve the quality of postoperative recovery and pain in patients under-going laparoscopic bariatric surgery,reduce hemodynamic fluctuations during the patients′ recovery period,reduce the incidence of nausea and vomiting,and improve the patients′ prognosis.
3.Melatonin alleviates CoCrMo particle-induced osteolysis by inhibiting NLRP3 inflammasome activation

BACKGROUND:Periprosthetic osteolysis is the most common long-term complication of total joint arthroplasty.Many studies suggest that the inflammasome may play an important role during the osteolysis.Melatonin is a rhythm-regulated hormone secreted by the pineal gland with many functions including anti-inflammatory,anti-oxidation,and antitumor,but its effects on osteolysis and inflammasome have yet to be explored. OBJECTIVE:To explore the effect of melatonin on the osteolysis induced by wear particles and the role of melatonin on the activation of NLRP3 inflammasome. METHODS:(1)In vivo test:Fifteen C57BL/6 mice were randomly divided into sham operation group,osteolysis group and melatonin group by random number table method,with 5 mice in each group.The osteolysis model of the osteolysis group and the melatonin group was established by injecting cobalt-chromium-molybdenum(CoCrMo)particles into the sagittal suture of the skull.After injection,the melatonin group was intraperitoneally injected with 50 mg/(kg·d)of melatonin for 14 consecutive days.After drug intervention,the mouse calvarium was collected for micro-CT analysis to observe the micro-structural changes around the sagittal suture.(2)In vitro test:Mouse bone marrow-derived macrophages and THP-1 cells(which had been induced to differentiate into macrophages)were taken and divided into seven groups:normal group,lipopolysaccharide group,lipopolysaccharide+CoCrMo group and melatonin 0.5,1,1.5,2 mmol/L groups(lipopolysaccharide and CoCrMo were added to the melatonin intervention groups).After the intervention for 6 hours,the expression of related proteins(NLRP3,Caspase-1,interleukin-1β,and gasdermin D,gasdermin D-N terminal)in the inflammasome of cell lysate or cell culture supernatant was detected by western blot assay.Cytotoxicity and cell death were observed through lactate dehydrogenase release and live-dead fluorescence staining. RESULTS AND CONCLUSION:(1)In vivo test:Micro-CT scanning 3D reconstruction images showed that the bone mass around the sagittal suture of the skull of mice in the osteolysis group was significantly reduced,and the bone tissue structure was severely damaged.Compared with the osteolysis group,the bone mass around the sagittal suture of the skull in the melatonin group was significantly increased,and the destruction of tissue structure was reduced.(2)In vitro test:For mouse bone marrow-derived macrophages,lipopolysaccharide significantly up-regulated NLRP3 protein expression in cell lysate,and melatonin intervention could reduce NLRP3 protein expression in a dose-dependent manner.CoCrMo particles significantly up-regulated the protein expressions of the gasdermin D-N terminal in cell lysate and Caspase-1 and interleukin-1β in the supernatant of cell culture,while melatonin intervention could reduce the expression of these proteins in a dose-dependent manner.For THP-1 cells,the protein expressions of Caspase-1 and interleukin-1β in the supernatant of cell culture were significantly up-regulated by CoCrMo particles,and the expression of these proteins was decreased dose-dependent by melatonin intervention.Lactate dehydrogenase release and live-dead fluorescence staining showed that CoCrMo particles significantly increased the release of lactate dehydrogenase and cell death in the supernatant of mouse bone marrow-derived macrophage culture,and melatonin intervention could reduce the release of lactate dehydrogenase and cell death.(3)The results show that melatonin can inhibit particle-induced inflammasome activation and pyroptosis to suppress periprosthetic osteolysis.
4.A nomogram model for predicting malnutrition after a tracheotomy

Objective:To explore the risk factors for malnutrition after a tracheotomy and to construct a predictive model useful for its prevention through early intervention.Methods:Clinical data describing 440 tracheotomy patients were subjected to a retrospective analysis. The variables examined were age, sex, etiology, Glasgow Coma Score (GCS), activities of daily living (ADL) score, age-corrected Charlson comorbidity index (aCCI), food intake, swallowing function, incidence of infections, as well as any history of diabetes mellitus, hypertension, smoking or alcohol consumption. Patients identified as being at risk of malnutrition (NRS-2002≥3) were screened using the Nutritional Risk Screening tool (NRS-2002) and the European Society of Clinical Nutrition and Metabolism′s ESPEN2015 criteria. The subjects were thus categorized into a malnutrition group of 343 and a control group of 97. Unifactorial and multifactorial logistic regression analyses were performed, and stepwise regression was applied to include the factors found significant in the unifactorial analysis into the multifactorial logistic regression analysis, and to construct a column-line graph prediction model. The clinical utility of the model was assessed by applying the receiver operator characteristics (ROC) curves, calibration plots and decision curve analysis (DCA).Results:Of the 440 persons studied, 343 (78%) were malnourished. The multivariate logistic regression analysis showed that pulmonary infection, dysphagia, low GCS score and high aCCI score were significant risk factors for malnutrition after a tracheotomy. A prediction nomograph was constructed. After fitting and correcting, the area under the curve (AUC) of the prediction model′s ROC curve was 0.911, the specificity was 80.4%, and the sensitivity was 91.3%. That was significantly higher than the AUCs for pulmonary infection (0.809), dysphagia (0.697), aCCI (0.721) and GCS (0.802). Bootstrap self-sampling was used to verify the model internally. After 1000 samples the average absolute error between the predicted risk and the actual risk was 0.013, indicating good prediction ability. The DCA results demonstrated that the model has substantial clinical applicability across a range of nutritional interventions, particularly for threshold probability values ranging from 0 to 0.96.Conclusion:Pulmonary infection, dysphagia, low GCS score, and high aCCI score are risk factors for malnutrition among tracheotomy patients. The nomogram model constructed in this study has good predictive value for the occurrence of malnutrition among such patients.
5.Effect of Acupuncture on Learning and Memory Ability and Intestinal Flora of SAMP8 Mice

OBJECTIVE To explore the effect of acupuncture on the learning and memory ability and intestinal flora structure of senescence-accelerated mouse prone 8(SAMP8)mice.METHODS Eighteen 6-month-old SAMP8 mice were randomly divided in-to model group and acupuncture group,with 9 mice in each group,and 9 senescence-accelerated mouse resistant strain 1(SAMR1)mice of the same month as the normal group.The acupuncture group selected Baihui and Yongquan points for acupuncture,5 times a week,20 min each time,for 8 consecutive weeks.Morris water maze was used to detect the learning and memory ability of mice in each group,immunofluorescence method was used to detect β-amyloid protein(Aβ)expression in the cerebral cortex,and 16S rDNA technology was used to detect changes in intestinal flora.RESULTS The results of the water maze experiment showed that compared with the normal group,the learning and memory abilities of the mice in the model group were significantly reduced(P<0.05),and the learning and memory abilities of the mice in the acupuncture group were significantly improved compared with the model group(P<0.05).Immunofluorescence results showed that compared with the normal group,the model group had increased positive expression of Aβ in the cerebral cortex,and obvious Aβ plaque deposition could be seen;the acupuncture group had fewer Aβ plaques than the model group.Alpha diversity index showed that compared with the normal group,the Ace and Simpson values of the model group de-creased(P<0.05)and the Shannon value increased(P<0.05);compared with the model group,the Ace and Chao values of the acu-puncture group increased(P<0.05,P<0.01).In the beta diversity analysis,the results of principal component analysis(PCA)and principal coordinate analysis(PCoA)showed that the samples in each group were relatively clustered,and the distance between sam-ples in each group was far away.The Venn diagram showed that there were 664,689,and 649 OTUs in the normal group,model group,and acupuncture group,respectively.Results of intestinal flora structure analysis showed that compared with the normal group,the abundance of Bacteroidetes,Ruminococcaceae,and Eubacterium xylanophilum in the model group increased(P<0.05),and the abundance of Fusobacterium,unclassified Fusobacterium,and Porphyromonas decreased(P<0.05);compared with the model group,the abundance of Bacteroidetes and Eubacterium xylanophilum decreased in the acupuncture group(P<0.05),and the abundance of Fusobacterium,unclassified Fusobacterium,and Porphyromonas increased(P<0.05).CONCLUSION Acupuncture can reduce cortical Aβ deposition and improve learning and memory abilities in SAMP8 mice,which may be related to regulating the structure of intestinal flora and increasing species diversity.
6.Comparison of the effects of different probiotic preparations on serum TMAO,coagulation function,and intestinal mucosal barrier function in patients with ulcerative colitis

Objective To explore the differences in the effects of different probiotic preparations on serum trimethylamine oxide(TMAO),coagulation function,and intestinal mucosal barrier function in patients with ulcerative colitis,in order to provide reference for the application and selection of probiotic preparations in the treatment of ulcerative colitis.Methods 120 patients with active,mild to moderate ulcerative colitis admitted Into the General Hospital of the Southern Theater Command of the People's Liberation Army of China from January 2023 to February 2024 were selected and randomly divided into group A(n=30),Group B(n=30),Group C(n=30)and Group D(n=30)by random number table method.All four groups were treated with oral administration of mesalazine enteric coated tablets(1 g/time,3 times/day).In addition,Group A,Group B and Group C were treated with different probiotic preparations such as Peifeikang(0.42 g/time,2 times/day),Jinshuangqi(2 g/time,2 times/day),and Meichangan(0.5 g/time,3 times/day).All four groups were continuously treated and observed for 2 months.After treatment,modified Mayo score and modified Baron endoscopic score of the four groups were evaluated and compared,the treatment efficacy of the four groups were evaluated and the incidence of adverse reactions among the four groups was statistically compared.The before and after treatment coagulation function indicators(prothrombin time(PT),activated partial thromboplastin time(APTT),plasma fibrinogen(Fig)),serum TMAO,inflammatory factors(C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6))and intestinal mucosal barrier function(diamine oxidase(DAO),D-lactate and lipopolysaccharide(LPS))levels among the four groups were compared.Results Compared with the other three groups,the treatment effective rate of in Group D was lower,while the modified Mayo score and modified Baron endoscopic score after treatment in Group D were higher(P<0.05);The treatment effective rate in Group C was lower than those of Group A and Group B,while the modified Mayo score and modified Baron endoscopic score after treatment in Group C were higher than those in Group A and Group B(P<0.05);The after treatment modified Mayo score and modified Baron endoscopic score in Group B were higher than those in Group A(P<0.05);However,there was no statistically significant difference in the treatment effectiveness between Group A and Group B(P>0.05).Compared with those of before treatment in the same group,the after treatment PT and APTT in all four groups were shortened,while their plasma FIB levels and serum TMAO,IL-6,TNF-α,CRP,DAO,D-lactate,LPS levels were also all decreased(P<0.05).Comparison results of after treatment APTT,PT,plasma FIB levels and serum TMAO,IL-6,TNF-α,CRP,DAO,D-lactate,LPS level of four groups showed that Group A<Group B<Group C<Group D(P<0.05).The incidence of adverse reactions in Group D were lower than that in Group A,Group B and Group C(P<0.05);While there was no statistically significant difference among that of Group A,Group B and Group C(P>0.05).Conclusion The application of probiotic preparations in patients with ulcerative colitis can help improve clinical treatment efficiency,improve clinical symptoms,coagulation function,inflammatory response,and intestinal mucosal barrier function,and reduce the occurrence of adverse reactions.Its application is safe and reliable,among which the application effect of multi probiotic preparations is better than that of single probiotic preparations,and the application effect of Peifeikang is the best.
7.Clinical application value of contrast-enhanced chest CT in selective arterial embolization in patients with hemoptysis

Objective To investigate the clinical application value of contrast-enhanced chest CT in the detection of responsible vessels for hemoptysis before selective arterial embolization(SAE).Methods The clinical data of 74 patients with hemoptysis trea-ted with interventional therapy and preoperative contrast-enhanced chest CT and digital subtraction angiography(DSA)were ana-lyzed retrospectively.The responsible vessels were identified and then embolized via angiography.The detection of the responsible vessels via preoperative contrast-enhanced chest CT was analyzed.The patients were followed up to observe the efficacy and compli-cations,and the influencing factors of interventional efficacy and recurrence were analyzed.Results A total of 245 responsible ves-sels were detected by preoperative contrast-enhanced chest CT,including bronchial arteries(n=178),ectopic bronchial arteries(n=10)and non-bronchial systemic artery(NBSA)(n=57),which could accurately show the anatomical information of responsible vessels.A total of 4 posterior intercostal arteries were missed.The diagnostic accuracy was 98.4%(245/249).All patients were followed up for 12 to 25.6 months.The immediate hemostasis rate was 93.2%(69/74)and the effective rate was 79.7%(59/74),respectively.The factors affecting the efficacy were bronchial artery to pulmonary circulation fistula,pleural thickening at the bleeding site,and underly-ing lung disease.Among the 59 patients with effective treatment,underlying lung disease was the influencing factor for postoperative recurrence.Conclusion Contrast-enhanced chest CT can provide anatomical information about the responsible vessels for interven-tional therapy of hemoptysis,improving surgical efficiency and reducing the recurrence rate of hemoptysis.
8.Analysis of expression and prognostic value of SGOL1 in lung adenocarcinoma based on bioinformatics

Objective To study expression and prognostic value of Shugoshin-1(SGOL1)in lung adenocarcinoma by bioinformatics method.Methods Expression profile data and clinical data of lung adenocarcinoma and normal tissues were downloaded from The Cancer Genome Atlas database,and expression difference and clinical correlation analysis of SGOL1 were performed.R package"pROC"was used to plot receiver operator characteristic(ROC)curves to evaluate accuracy of SGOL1 expression in predicting clinical diagnosis in lung adenocarcinoma patients.Effects of SGOL1 expression on prognosis of lung adenocarcinoma patients were evaluated by R package"survival","survminer"and univariate and multivariate Cox regression analysis.By searching Tumor Immune Single-Cell Hub and TIMER2.0 databases,expression distribution of SGOL1 in lung adenocarcinoma and its relationship with immune cell infiltration were analyzed,functional enrichment analysis of SGOL1 and its co-expression was performed by using LinkedOmics database.Search tool for the retrieval of interaction gene/proteins was used to construct a protein-protein interaction network for SGOL1.Results Compared with normal tissues,expression level of SGOL1 in tumor tissues was significantly upregulated(P<0.001).Compared with paracancer tissues,expression level of SGOL1 in tumor tissues was significantly upregulated(P<0.001).In different clinical and pathological stages of lung adenocarcinoma,compared with stage Ⅰ,expression levels of SGOL1 in stages Ⅱ,Ⅲ and Ⅳ were significantly higher(P<0.05).ROC curve showed that SGOL1 had a good diagnostic efficiency in lung adenocarcinom patients,with area under the curve of 0.959(95%CI:0.942-0.975).Overall survival,disease specific survival,disease-free survival and progression free interval of high expression group of SGOL1 were significantly shorter than those of low expression group of SGOL1(P<0.05).Univariate and multivariate Cox regression analysis showed that clinical stage(HR=1.629,P<0.001)and SGOL1 expression level(HR=1.447,P=0.002)were associated with poor prognosis in lung adenocarcinoma patients.It can be used as an independent risk factor for the prognosis of lung adenocarcinoma patients.Expression level of SGOL1 was negatively correlated with infiltration level of B cells,CD4+T cells and dendritic cells(P<0.05).Expression level of SGOL1 was positively correlated with infiltration level of macrophages,CD8+T cells and neutrophils(P<0.05).Enrichment analysis showed that SGOL1 may play role in mitosis,cell cycle,p53 signaling pathway and amino acid metabolism pathways.Analysis of protein-protein interaction network suggests that SGOL1 was closely related to multiple molecules such as CBX1,PPP2CA,PPP2R5C,CDCA8,ESPL1,PPP2R1A,BUB1,PPP2R5A,SGO2,CDC20,etc.Conclusion SGOL1 is highly expressed in lung adenocarcinoma tissues,and it is associated with poorer prognosis in lung adenocarcinoma patients.SGOL1 can be used as one of prognostic biomarkers for lung adenocarcinoma patients.
9.High tibial osteotomy on varus knee osteoarthritis with medial meniscus posterior root injury

Objective To explore clinical effect of distal tibial tubercle-high tibial osteotomy(DTT-HTO)in treating knee osteoarthritis(KO A)with medial meniscus posterior root tear(MMPRT).Methods A retrospective analysis was performed on 21 patients with varus KOA with MMPRT from May 2020 to December 2021,including 3 males and 18 females,aged from 49 to 75 years old with an average of(63.81±6.56)years old,the courses of disease ranged from 0.5 to 18.0 years with an average of(5.9±4.2)years,and 4 patients with grade Ⅱ,14 patients with grade Ⅲ,and 3 patients with grade Ⅳ according to Kellgren-Lawrence;14 patients with type 1 and 7 patients with type 2 according to MMPRT damage classification.The distance of medi-al meniscusextrusion(MME)and weight-bearing line ratio(WBLR)of lower extremity were compared before and 12 months after operation.Visual analogue scale(V AS),Western Ontarioand and McMaster Universities(WOMAC)osteoarthritis index,and Lysholm knee score were used to evaluate knee pain and functional improvement before operation,1,6 and 12 months after operation,respectively.Results Twenty-one patients were followed up for 12 to 18 months with an average of(13.52±1.72)months.MME distance was improved from(4.99±1.05)mm before operation to(1.87±0.76)mm at 12 months after operation(P<0.05).WBLR was increased from(15.49±7.04)%before operation to(62.71±2.27)%at 12 months after operation(P<0.05).VAS was decreased from(7.00±1.14)before operation to(2.04±0.80),(0.90±0.62)and(0.61±0.50)at 1,6 and 12 months after operation.WOMAC were decreased from preoperative(147.90±9.88)to postoperative(103.43±8.52),(74.00±9.54)and(47.62±9.53)at 1,6 and 12 months,and the difference were statistically significant(P<0.05).Lysholm scores were increased from(46.04±7.34)before oepration to(63.19±8.93),(81.10±6.41)and(89.29±3.04)at 1,6 and 12 months after operation(P<0.05).Conclusion For the treatment of varus KOA with MMPRT,DTT-HTO could reduce medial meniscus pro-trusion distance,improve the ratio of lower limb force line,and effectively reduce knee pain and improve knee joint function.
10.Lipidomics driven biomarker discovery in chronic lung disease

Chronic lung diseases(CLD)include chronic obstructive pulmonary disease,asthma,and idiopathic pulmonary fibrosis.Studies have shown that CLD are closely related to disorders in lipid metabolism.Therefore,lipids,as biomarkers of CLD,may be of great value in the diagnosis,prevention,and monitoring of disease treatment.This review discusses lipidomics from four major aspects:the technical methods of lipidomics,the selection of clinical samples for lipidomics,the discovery of biomarkers for CLD,and the differentiation of traditional Chinese medicine syndromes from CLD.
