1.Complete genome sequencing and traceability analysis of SARS-CoV-2 in Binhai New Area, Tianjin

		                        			Objective:To analyze the whole genome traceability and variation analysis of SARS-CoV-2 in local COVID-19 outbreaks in Binhai New Area, Tianjin.Methods:The whole-genome high-throughput sequencing was performed on throat swab samples collected from one local asymptomatic infected person and five confirmed cases of COVID-19 in Binhai New Area of Tianjin from November 7 to December 5, 2020. The sequencing data were assembled by De novo. MAFFT v7.0 multiple sequence alignment program and MEGA X software were used to compare the above data and construct phylogenetic tree (Neighbor-joining method).Results:The genetic similarity between the sequences of 6 SARS-CoV-2 strains and Wuhan reference sequence (Wuhan-Hu-1) was greater than 99.9%. Two of six strains were genetically identical, conform to the L-Lineage European Branch Ⅱ.1(America Branch)/B.1; The other four strains had the same genes and were in line with the characteristics of L-Lineage European Branch Ⅰ/B.1.1.These six strains belonged to different evolutionary branches and two different transmission chains. There were 18 nucleotide mutation sites in sequences of six SARS-CoV-2 strains, eight of which were synonymous mutation sites, nine of which were missense mutation sites, resulting in nine amino acid mutation sites, and important mutation sites of RDRP-P323L and S-D614G were found in all of the six samples.Conclusions:In this study, there were two COVID-19 outbreaks in Binhai New Area of Tianjin, and the sequences of six SARS-CoV-2 strains belonged to different evolutionary branches and two different transmission chains. It might come from porters′ contact with imported cold chain items contaminated with SARS-CoV-2 from different sources. All the sequences of six SARS-CoV-2 strains had P323L and D614G mutations, which indicated that the virus mutation and transmission ability were stronger. 2.Efficacy and Safety of Eplerenone in the Treatment of Essential Hypertension :Reevaluation of Systematic Review/Meta-analysis

		                        			OBJECTIVE:To reeval uate the systematic review/Meta-analy sis of efficacy and safety of eplerenone in the treatment of essential hypertension. METHODS :Retrieved from PubMed ,Embase,Cochrane Library ,Web of Science ,Wanfang database,CNKI,VIP,systematic review/Meta-analysis about eplerenone in the treatment of essential hypertension were collected from the inception to June 24th,2021. After literature screening and data extraction ,the quality of included literatures were evaluated with PRISMA statement ;methodology quality of included literatures were evaluated with AMSTAR 2 scale;GRADE method was adopted to evaluate the evidence quality of outcome measures. Efficacy and safety index evaluation of included literatures were summeried. RESULTS :A total of 8 systematic reviews/Meta-analyses were included ,involving 5 systematic reviews and 3 Meta-analysis,including 73 outcome indicators. PRISMA scores ranged from 7.5 to 23.5,including 6 literatures (75.0%)with≤15 points,1(12.5%)with >15-<21 points and 1(12.5%)with ≥21 points. The results of AMSTAR 2 evaluation indicated that the methodological quality of 2 studies was low ,and that of 6 studies was very low . GRADE quality evaluation results showed that there were 3 high quality indicators ,24 medium quality indicators and 46 low or very low quality indicators;the factors contributed to downgrading evidence quality were limitation ,inconsistency,imprecision and publication bias. In terms of efficacy ,compared with placebo ,eplerenone could significantly reduce clinical blood pressure (CBP)and 24-hour ambulatory blood pressure (ABP). Its effect in reducing CBP was significantly better than other antihypertensive drugs or equivalent to other antihypertensive drugs. The effects of eplerenone on reducing clinical systolic blood pressure was not as good as spironolactone and enalapril ,or bett er than calcium channel blocker ,enalapril and angiotensin receptor antag onist,or equivalent to  calcium channel blocker and enalapril ;the effect of eplerenone  on reducing clinical diastolic blood pressure was not as good com as spironolactone ,calcium c hannel blocker and enalapril ,or as good as enalapril ,but better than angiotensin receptor antagonist. In terms of safety ,there was no significant difference in the incidence of ADR ,serious ADR or hyperkalemia caused by eplerenone ,compared with placebo ,or the incidence of ADR was higher than that of placebo. There was no statistical significance in the incidence of ADR or serious ADR ,compared with other antihypertensive drugs. CONCLUSIONS :Efficacy and safety of eplerenone in the treatment of essential hypertension was good ,but in view of the poor methodological quality of systematic reviews or Meta-analysis and the low or very low level of outcome indicator evidence ,the authenticity and effectiveness of the conclusion will be reduced ,so that those indcaters should be interpreted carefully.
		                        		
		                        		
		                        		
		                        	
3. Relation of perceived social support, job insecurity and psychological distress among clinical nurses

		                        			A total of 462 clinical nurses in Shandong Province were selected by convenience sampling method. They were assessed with job insecurity scale, perceived social support and 10-item Kessler Psychological Distress Scale (K10) for the evaluation of job insecurity, self-esteem and psychological distress.

4.Surveillance analysis of Yersinia eterocolitica in Gaomi City, Shandong Province

		                        			Objective To investigate the distribution of host animals and the serological distribution and virulence gene of Yersinia eterocolitica in Gaomi City.Methods Sample stools of poultry and domestic and diarrhea patients,as well as flies and chilled (frozen) poultry and domestic animal meats in Gaomi City were preliminarily identified and the suspected strains isolated were sent to Shandong Provincial Center for Disease Control and Prevention for further identification from 2012 to 2015.The bacteria were analyzed by the systematic biochemistry to determine the serotypes,bio-types and virulence genes [Yersinia enterocolitica adhesion aggression site gene (ail),heat-tolerance enterotoxin gene (ystA),organism type 1A enterotoxin gene (ystB),adhesin (yadA),yop regulator transcription activation action factor (virF)].Results Eighty-five stains of Yersinia enterocolitica were detected in 2 860 specimens and the detection rate was 3.0% (85/2 860).The chicken strains accounted for 50.6% (43/85),the swine strains accounted for 20.0% (17/85),and the sheep strains accounted for 10.6% (9/85).In all the serotypes,the highest was O ∶ 8 (24.7%,21/85),followed by O ∶ 5 (12.9%,11/85),the bacteria were detected in the stools of chickens,duck,and swine etc.,as well as in diarrhea patients and chilled (frozen) poultry and domestic animal meats,biotype is 1A.One strain O ∶ 3/4 was detected from swine stool with ail,ystA,yadA and virF genes.Conclusions The Yersinia enterocolitica is widely distributed and both pathogenic and non-pathogenic strains coexist in Gaomi City;the main serological types are O ∶ 5/1A and O ∶ 8/1A;the pathogenic serotype is O ∶ 3/4,and chicken,swine and sheep are the main host animals.
		                        		
		                        		
		                        		
		                        	
5.Evaluation of the clinical application efficacy of cryopreserved apheresis platelets and fresh apheresis platelets

		                        			Objective To evaluate the clinical value of apheresis platelets throught heanalysisof case control on the clinical efficacy and safety of cryopreserved apheresis platelets and fresh apheresis platelets.Methods 2 035 clinical cases of platelet transfusion in August 2014 to December 2016 by Using the closed loop intelligent path management and evaluation information system,456 cases were selected as control cases.Platelets were divided into the cryopreserved apheresis platelets group (group A,n=199) and fresh apheresis platelets group (group B,n=257) according to the transfused platelet type.The clinical application value of cryopreserved single platelets was evaluated by comparing the basic data,the effective indexes and safety indexes of the two groups.Results 1) The cases were 43.6% (199/456) in A groups,and 56.4% (257/456) in B groups,there were no significant difference in gender,age and medical and surgical cases between A and B group (P>0.05);2) 199 cases in group A were cryopreserved platelets of 2 275 U,including 121 medicine cases,the total amount of transfusion was about 60.9% (1 385/2 275),78 surgical cases accounted for 39.1% (890/2 275);In the distribution of diseases,the blood system diseases accounted for 49.2% (1 120/2 275),the total amount of obstetrics and gynecology disease infusion accounted for 10.6% (240/2 275),and the amount of tumor radiotherapy and chemotherapy accounted for 6.2% (140/2 275);The proportion of ABO blood type distribution was O type 25.9%,A type 22.9%,Btype 20.7%,ABtype 30.5%,respectively;3) The Plt counts of group A and B were significantly different before and after transfusion (P <0.05).But there was no significant difference between the two groups of cases before transfusion and 24h Plt count after transfusion,the Plt counts difference,and 24 h CCI (P>0.05);4) The effective rates of platelet transfusion in group A and B were 76.9% and 76.7%,respectively.Which has no significant difference between the two groups (P>0.05)).There was no significant difference between the two groups in medical and surgical cases (P>0.05),but the effective rate of surgical cases in group A (84.6%) was higher than that in B group (75.3%).The difference effect of medicine and surgery cases in B group were not statistically significant (P>0.05),but the difference effect of medicine and surgery cases in A group was statistically significant (P<0.05),platelet transfusion inefficient in surgical cases (15.4%) was significantly lower than that of cases (28.1%);5) The incidence of adverse reactions of blood transfusion was 3.5%,4.7% in group A and B,and the blood transfusion mortality rate was zero,the difference was not statistically significant (P> 0.05).Conclusion The clinical effectiveness and safety of cryopreserved apheresis platelets are similar to those of fresh apheresis platelets,and the former can be widely Used in clinic,in particular,it has certain advantages in the surgical hemostatic effect.but for repeated infusion cases or platelet transfusion ineffective cases should be given priority to fresh apheresis platelets.
		                        		
		                        		
		                        		
		                        	
6.Analysis of influencing factors of platelet transfusion during bone marrow empty window period on patients with allogeneic peripheral blood stem cell transplantation

		                        			Objective To analyze the influence of different factors and their relating correlation results on platelet transfusion during the bone marrow empty window period on the patients who have undergone allogeneic peripheral blood stem cell transplantation (allo-HSCT) with retrospective analysis of case-control data.Methods Clinical data of 153 cases were collected by the clinical blood management and evaluation information system with discharge diagnosis of allo-HSCT in the hematology department of The First Affiliated Hospital of Nanchang University within a time frame from January 2014 to December 2016.A total of 90 cases were considered valid for retrospective analysis according to the case exclusion criteria.The average transfusion dose for patients with allo-HSCT during the bone marrow empty window period was defined as the threshold value which divided the 90 cases into the observation group of 38 cases receiving more than 6 Units of platelet transfusion and the control group of 52 cases with less than 6Units of platelet transfusion.The amount of platelets transfused during the bone marrow empty window period,clinical indexes include Hb,ANC,Plt,SF before pretreatment,platelet engraftment time and the number of mononuclear cells implanted were compared and analyzed by Logistic regression.Results (1) There was no significant difference between the two groups in gender,age,primary diagnosis,HLA matching,Hb before pretreatment and the number of mononuclear cells implanted (P>0.05).The ANC(×109/L) (1.24±0.57 vs 3.36±1.33) and Plt(×109/L) (43.55±68.29 vs 126.62±84.73) counts before pretreatment in the observation group were significantly lower than those in the control group(P<0.05).SF(μg/L) (2351.05 ± 1 587.96 vs 1 000.96± 362.97)before pretreatment and P LT recovery time (d) (16.84± 2.47 vs 12.73 ± 1.65)was significantly higher than that in the control group(P<0.05).Donor-recipient ABO blood group typing incompatibility (15 vs 10) was significantly higher than the control group (P<0.05);(2) Single factor Logistic regression analysis showed that ABO blood group matching,clinical indexes include ANC,Plt,SF before pretreatment,PLT recovery time were statistically significant,Only ANC before pretreatment and PLT recovery time had significant effect on the platelet transfusion during bone marrow empty window period in patients with allo-HSCT in multivariate Logistic regression analysis(P<0.05).Condusion The ANC before pretreatment and PLT recovery time are independent factors for platelet transfusion of the bone marrow empty window period in patients with allo-HSCT.The PLT recovery time is an independent risk factor,which indicates that the longer the duration of PLT implantation,the greater the amount of platelet transfusion will be needed.Besides,the ANC before pretreatment is the independent protective factor,which indicates that the greater the ANC,the smaller the amount of platelet transfusion is required.

		                        			Gastric carcinoma is a heterogeneous malignant disease involving genetic factors.To identify predictive markers for gastric cancer treatment in Chinese patients,we evaluated the association between polymorphisms of the gene encoding cytochrome P450 2A6 (CYP2A6) and outcomes of S-1 plus oxaliplatin (SOX) chemotherapy treatment.Clinical data on 60 consecutive gastric cancer patients receiving SOX regimen were collected prospectively.We sequenced all exons of CYP2A6 and a total of 22 different polymorphisms were detected in the present study.Comprehensive analyses of these genetic polymorphisms were performed to determine their association with both safety and efficacy of SOX regimen.Our results showed that polymorphisms of CYP2A6 were associated with the safety and efficacy of SOX treatment.Among them,missense mutations CYP2A6 rs60823196 and rs138978736 could be possible risk factors (P < 0.05) for severe diarrhea induced by SOX,whereas CYP2A6 rs138978736 could be a conceivable predictor for overall survival of patients treated with SOX adjuvant chemotherapy.Further large-scale randomized prospective studies are warranted to confirm these findings.
		                        		
		                        		
		                        		
		                        	
8.Anti-helicobacter pylori effect of total alkaloids of sophora alopecuroides in vivo.

BACKGROUNDHelicobacter pylori (H. pylori) infection could lead to most gastroduodenal diseases and is even identified as a carcinogen of gastric cancer. Total alkaloids of sophora alopecuroides (TASA) is widely used in herbal remedies to treat various infectious diseases, including stomach-associated diseases. This study is aimed at evaluating the antimicrobial activity of TASA on H. pylori-infected BALB/c mice mouse gastritis.
METHODSTotally 120 BALB/c mice were orally inoculated with H. pylori Bacterial liquid to construct BALB/c mice H. pylori infection gastritis animal model, after the model was successfully created. We randomly assigned 100 infected mice into 10 treatment groups, the first group (normal saline); the second group (bismuth pectin); the third group (omeprazole); the fourth group (TASA 2 mg/d); the fifth group (TASA 4 mg/d); the sixth group (TASA 5 mg/d); the seventh group (TASA + bismuth pectin); the eighth group (TASA + omeprazole); the ninth group (bismuth pectin + clarithromycin + metronidazole); the tenth group (omeprazole + clarithromycin + metronidazole), 5 other non-infected mice as negative control. Mice were orally inoculated twice a day and 7 days continuously. Then the mice were killed 4 weeks after treatment, we used realtime PCR to detect 16sDNA of H. pylori to test both the colonization and the clearance mice of bacteria of each treatment. We applied hematoxylin and eosin (HE) staining and immunostaining of mice gastric mucosa to observe the general inflammation and related factors interleukin 8 (IL-8), cyclooxygenase 2 (COX-2), and nuclear factor-kappa B (NF-κB) expression change after treatments.
RESULTSFirstly, we ensured that after 6-week intragastric administration, the bacteria colonization reached an exceed peak which is far higher than positive threshold (P < 0.001); secondly, after treatments, it is revealed that TASA combined with omeprazole or bismuth pectin showed promising antimicrobial activity against H. pylori as well as conventional triple therapy (P < 0.001); thirdly, HE staining showed that the inflammation on mice gastric mucosal membrane were also relieved obviously in TASA combined treatments and conventional triple therapy compared with normal saline treated mice, moreover, from immunohistochemistry results, H. pylori-induced IL-8, COX-2, and NF-κB were consistently suppressed in seventh, eighth, ninth, and tenth group to a certain extent.
CONCLUSIONThese results open the possibility of taking TASA as an anti-inflammatory agent for H. pylori gastritis.

9.Identification and virulence gene detection of non-O1 and non-O139 Vibrio chol-erae isolates causing septicemia

		                        			Objective To identify an suspected Vibrio cholerae isolated from Xiangyang Central Hospital and characterize the strain in terms of antibiotic resistance and relevant virulence genes.Methods Pathogen identification and susceptibility testing were completed with MicroScan WalkAway 40 Automated Microbiology System.Slide agglutination was used for serotyping. PCR and sequencing technology were employed for 16s RNA gene analysis.PCR technique was used to detect six major viru-lence genes.Results This suspectedVibrio cholerae isolate was confirmed as non-O1 and non-O139 Vibrio cholerae .Suscep-tibility testing results indicated that the strain was sensitive to ampicillin,chloramphenicol,trimethoprim-sulfamethoxazole, and tetracycline.16s RNA gene sequence analysis showed 100% homologous with the registered sequence in National Center for Biotechnology Information database.Virulence genes rtxC and toxR were identified.The other virulence genes such as tcpAET,ctxA,hlyA,and tcpACL were negative.Conclusions This suspected Vibrio cholerae isolate is confirmed as non-O1 and non-O139 Vibrio cholerae .The pathogenic factors may be related to the virulence genes rtxC and toxR.
		                        		
		                        		
		                        		
		                        	
10.Observations on the Efficacy of Electroacupuncture plus Acupuncture Point Injection in Treating Carpal Tunnel Syndrome

		                        			Objective To assess the clinical efficacy of electroacupuncture plus acupuncture point injection in treating carpal tunnel syndrome. Method Sixty-three patients with carpal tunnel syndrome were allocated, using a random number table, to a treatment group (32 cases) and a control group (31 cases). The treatment group received electroacupuncture plus acupuncture point injection and the control group, conventional acupuncture. One course of treatment was given. After treatment, the clinical therapeutic effects and the recurrence rates based on the symptoms and signs were compared between the two groups. Result The total efficacy rates in the treatment and control groups were 93.8% and 77.4%, respectively. There was a statistically significant difference in clinical therapeutic effect between the two groups (P<0.05). There was a statistically significant posttreatment difference in the recurrence rate between the two groups of patients (P<0.05).
