1.Research on content and psychometric properties of evaluations of functioning and adaptive behaviors in children with intellectual and developmental disabilities in educational settings using ICF

Objective To explore the content and the psychometric properties of assessment tools used for evaluating functioning and adaptive behavior in school-age children with intellectual and developmental disabilities within educational settings. Methods The most used assessment tools included Vineland Adaptive Behavior Scales(VABS),Supports Intensity Scale for Children(SIS-C),Strengths and Difficulties Questionnaire(SDQ)and Repetitive Behavior Scale-Revised(RBS-R),for assessing functioning and adaptive behavior children with intellectual and developmental disabili-ties.Employing the framework and methods of the International Classification of Functioning,Disability,and Health(ICF),this research encoded and categorized the assessment dimensions and items of the four tools,and explored their psychometric properties. Results VABS's assessment content was solely focused on activities and participation,including speaking(d330),con-versation(d350),toileting(d530),eating(d550),drinking(d560),basic interpersonal interactions(d710),com-plex interpersonal interactions(d720),changing and maintaining body positions(d410-d429),carrying,moving and handing objects(d430-d449),and walking and moving(d450-d469).SIS-C assessed activities and participa-tion,and environmental factors,including washing oneself(d510),caring for body parts(d520),community life(d910),using transportation(d470),school education(d820),basic learning(d130-d159),looking after one's health(d570),basic interpersonal interactions(d710),and products and technology for education(e130).SDQ fo-cused on body functions,and activities and participation,including emotional functions(b152),global psychoso-cial functions(b122),attention functions(b140),and basic interpersonal interactions(d710).RBS-R focused on body functions,and activities and participation,including involuntary movement reaction functions(b755),invol-untary movement functions(b765),looking after one's health(d570),energy and drive functions(b130),under-taking a single task(d210),carrying out daily routine(d230),attention functions(b140),and handling stress and other psychological demands(d240).VABS was characterized by good specificity and excellent sensitivity.SIS-C demonstrated very good internal consistency,reliability and validity.SDQ was good in internal consistency,with excellent credibility and validity.RBS-R showed good internal consistency,reliability and validity. Conclusion SDQ and RBS-R cover both body functions,and activities and participation,SIS-C covers activity and par-ticipation,and environmental factors,while VABS solely assesses activities and participation.In terms of body functions,the assessment items primarily focus on mental functions(b130-b189)and movement functions(b750-b799).For activities and participation,the tools assess content across eight domains of functioning.Regarding en-vironmental factors,the assessment content mainly includes products and technology for education(e130),as well as design,construction and building products and technology of buildings for public use(e150).VABS,SIS-C,SDQ,and RBS-R are all norm-referenced measures,with moderate to excellent internal consistency,and good to excellent reliability and validity.
2.Effects of comprehensive moxibustion with Huolong cupping based on meridian theory in patients with lung-spleen qi deficiency type allergic rhinitis

Objective:To explore the effect of comprehensive moxibustion with Huolong cupping in patients with lung-spleen qi deficiency type allergic rhinitis.Methods:This study was a randomized controlled trial. From October 2022 to April 2023, convenience sampling was used to select 66 patients with lung-spleen qi deficiency type allergic rhinitis who visited the Otolaryngology Head and Neck Clinic of Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine as the research subject. The patients were randomly divided into an experimental group ( n=33) and a control group ( n=33). Both groups received routine oral Chinese medicine treatment. On the basis of routine treatment, the experimental group performed comprehensive moxibustion with Huolong cupping on the meridians of the Du meridian and bladder meridian. This study compared the scores of Nasal Airway Resistance (NAR), Total Nasal Symptom Score (TNSS), Visual Analogue Scale (VAS), and Chinese version of Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ) between two groups of patients before and after intervention. Results:Eventually 30 patients in each of the experimental group and control group completed the study. After intervention, the NAR score of the experimental group was lower than that of the control group, and the difference was statistically significant ( P<0.05). Repeated measures analysis of variance showed that with the increase of treatment time, the TNSS, VAS, and RQLQ scores of the experimental group were lower than those of the control group with statistical differences ( P<0.05), and the time effect, inter group effect, and interaction effect were statistically significant ( P<0.05) . Conclusions:Huolong cupping comprehensive moxibustion can reduce nasal resistance, meliorate nasal symptoms and accompanying nasal symptoms, and improve the quality of life of patients.
3.Association between taste disorders and novel coronavirus infection in patients with type 2 diabetes mellitus

Objective To investigate the current status of taste disorders in type 2 diabetes mellitus(T2DM)and to explore whether the taste disorders persists after 3 months of novel corona virus(COVID-19)infection.Methods 95 T2DM out patients(23 without COVID-19 infection history,72 infected with COVID-19 3~4 months ago)visiting the Endocrine Department of the Strategic Support Force Medical Center from February 20 to March 10,2023 were collected.Taste test box was used to test the taste function.General information,biochemical indicators,taste disorders,etc.were compared between the two groups.Results The average age of T2DM patients in this group was(58.3±9.6)years old,61 patients were male(64.2%),the median duration of DM was 11 years,and the median HbA1c was 7.3%.In taste testing,the proportion of sour,sweet,bitter,salty taste perception disorders was 60.0%,45.3%,57.9%,41.1%,84.2%.The average number of days from infection to enrollment into COVID-19 group was 102.4 days.The proportion of acid,sweet,bitter and salty sensory disorders was 61.1%,44.4%,55.6%and 41.7%in COVID-19 group and 56.5%,47.8%,65.2%and 39.1%in non-COVID-19 group.The prevalence of taste disorders in COVID-19 group was higher than that in non-COVID-19 group(86.1%vs 78.3%).Conclusions Taste disorders are common in T2DM patients.Compared with uninfected T2DM patients,there is no significant difference in the prevalence of taste disorders 3 months after COVID-19 infection.

This article aims to comprehensively review the concept, etiology, classification, classical cortical mapping, assessment, a proposed flowchart for the diagnosis and differential diagnosis, treatment, rehabilitation, mechanisms of development and recovery, prognosis, and influencing factors for post-stroke aphasia(PSA)types in the Chinese language.We emphasize the necessity and significance of neuroimaging assessment of the brain and blood vessels and neuropsychological assessment in the diagnosis and differential diagnosis of PSA in Chinese.We also recommend and encourage the use of the dichotomies of internal vs.external and anterior vs.posterior as a starting point, based on the association of anatomical locations of the brain and blood vessels with brain language areas and language disorders.A classification system of PSA in Chinese developed from this approach in the form of a flowchart is well-suited for guiding the clinical treatment of cerebral stroke.Incorporating the "four elements" , the flowchart enables convenient diagnosis, classification and differential diagnosis of PSA in Chinese and facilitates targeted and personalized rehabilitation planning to benefit the patient.This article introduces the use of memantine, piracetam, donepezil and other drugs for PSA treatment, evaluates clinical trials on memantine conducted in China and abroad and its mechanisms of action for the treatment of PSA, and discusses how rehabilitation therapy achieves therapeutic effects.For the treatment of PSA, clinical research and practice using drugs such as memantine, piracetam and donepezil in combination with non-pharmacotherapy and rehabilitation training should be promoted.
5.Application of individualized and staged psychological intervention in liver cancer patients during perioperative period

Objective:To explore the application of individualized and staged psychological intervention in liver cancer patients during perioperative period, to provide reference for perioperative nursing of patients with liver cancer.Methods:A total of 80 liver cancer patients who were hospitalized in Hefei Binhu Hospital and Hefei First People's Hospital from January to December 2019 were divided into observation group and control group by random number table method, with 40 cases in each group. The control group was given routine perioperative care, and the observation group was treated with routine perioperative care and individualized phased psychological intervention. All were intervented for 7 days. The mental state, self-efficacy, quality of life, and psychosocial adaptation levels before and after the intervention were evaluated by Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), and General Self-Efficacy Scale (GSES), the Quality of Life Scale, etc.Results:There was no significant difference in HAMA, HAMD, GSES, quality of life, and psychosocial adaptation level between the two groups before intervention ( P>0.05). The HAMA and HAMD scores in the observation group after intervention were (16.01 ± 2.25) and (15.75 ± 2.06) points, which were lower than those in the control group (18.46 ± 2.49) and (17.29 ± 2.33) points, and the differences were statistically significant ( t values were 4.617, 3.312, P<0.05). The overall function scores of GSES and quality of life in the observation group after intervention were (28.49 ± 3.82) and (46.41 ± 4.63) points, which were higher than those in the control group (25.35 ± 3.64) and (43.74 ± 4.95) points, the differences were statistically significant ( t values were 3.764, 2.491, P<0.05); all aspects of psychosocial adaptation scores in the observation group after intervention were higher than those in the control group, the differences were statistically significant ( t values were 2.278 - 4.578, P<0.05). Conclusions:The application of individualized and staged psychological intervention in liver cancer patients during perioperative period can significantly alleviate negative emotion, enhance their self-efficacy, improve level of social-psychological adjustment and quality of life.
6.Analysis of the Effect of Intelligent Regulation of Infusion Sequence on Promoting Intravenous Rational Drug Use

OBJECTIVE:To evaluate th e eff ects of infusion sequence regulation on the promotion of intravenous rational drug use. METHODS :The intelligent control of infusion sequence was realized with Markov chain machine learning method on the basis of intelligent reminder manual labeling infusion sequence. Selected from Binhu Hospital of Hefei during Jan. to Sept. 2017, 164 800 inpatients’medication orders were collected as data before intelligent control of infusion sequence ,and 264 600 inpatients’ medication orders were collected as data after intelligent control of infusion sequence. The correct rate of incompatibility ,storage time limit ,administration frequency ,chronopharmacology,preventive drugs ,drug interaction ,auxiliary drugs and infusion sequence for intravenous irritant drugs were compared before and after regulation ;the times of flushing tube ,the cost of flushing tube infusion and the change of liquid property in the infusion tube were compared before and after regulation. RESULTS :The correct rate of incompatibility ,storage time limit ,administration frequency ,chronopharmacology,drug interaction and infusion sequence for auxiliary drugs after regulation were significantly higher than before regulation ;the correct rate of infusion sequence for preventive drugs and intravenous irritant drugs ,the times of flushing tube ,the cost of flushing tube infusion and the case number of the liquid change (proportion) in infusion tube were significantly lower than before regulation (P<0.001). CONCLUSIONS:Intelligent regulation of infusion sequence can improve the rationality of intravenous medication sequence to a certain extend.
7.Design and Upgrading of Finished Infusion Label in PIVAS of Our Hospital

OBJECTIVE:To design and upgrade the finished infusion label in P IVAS of Hefei Binhu hospital ,so as to improve the safety and effectiveness of intravenous medication. METHODS :By investigating the experience and suggestions about the use of infusion labels by pharmacists and clinical nurses in PIVAS ,taking clear ,concise,focused,rational layout ,comprehensive information as improving principle ,the infusion label was designed and upgraded. The effect of upgrading were evaluated by the recognition rate of label scanning ,scanning time and rate on label (94 701,113 759 groups,respectively),and the correct rate , time and rate of drug delivery checking in 30 days before and after upgrading ,as well as satisfaction degree ,which made among PIVAS pharmacists (30),nurses(50)and patients (49). RESULTS :The upgraded label simplified part of the content and optimized the layout structure ,removed redundant content ,focused on the patient safety information that nurses needed to pay attention to when checking ,and added the marking of infusion sequence and precautions. By changing the barcode into two-dimensional code and adding hidden display function ,more information about drugs and rational drug use related to the infusion of patients was provided. Compared with original label ,after upgrading ,the recognition rate of new label scanning increased from 99.27% to 99.96%,the scanning time reduced from 3 518.75 s/d to 2 110.10 s/d,and the scanning rate increased from 0.57 group/s to 0.95 group/s;the correct rate of drug delivery checking increased from 99.73% to 99.91%,the time of drug delivery checking decreased from 5 423.55 s/d to 4 818.85 s/d,and the speed of drug delivery checking increased from 0.36 group/s to 0.41 group/s. The satisfaction degree of pharmacists ,nurses and patients were increased from 70.00% to 93.33%,from 62.00% to 90.00%,from 20.40% to 89.80%,respectively. CONCLUSIONS:The design and upgrading of infusion labels can improve the working efficiency of staff ,and improve the quality of pharmaceutical care and nursing care , and satisfaction, promote the improvement on the safety and effectiveness of intravenous medication for patients.
8.Exploration on application of multidiscipline team in public hospital′s administration

Multidiscipline team ( MDT ) provides comprehensive, cross-discipline treatment solutions and effective diagnosis for major, complicated and rare diseases, featuring significantly clinical outcomes. It deserves efforts on how to introduce MDT from clinical work to hospital administration. To this end, this article discussed the application of MDT model at large-scale public hospital administration. MDT hospital administration teams, and such management methods as project management and PDCA, as well as information technology systems were called into play. These measures have effectively resolved long-standing roadblocks commonly encountered, and removed horizontal barriers, sharply raising work efficiency.
9.Report of eight cases of familial fatal insomnia

Objective To investigate the clinical features,polysomnography,imaging examination,genetic analysis and laboratory examination of eight patients with familial fatal insomnia (FFI).Methods The clinical data,neuropsychological examination,results of cerebrospinal fluid analysis,imaging examination and polysomnography of eight patients with FFI in Xuanwu Hospital,Capital Medical University from 2009 to 2018 were retrospectively analyzed and summarized.Results Among the eight FFI patients,there were 3 males and 5 females,the onset age being (49.8+14.3) years (19 to 64 years) and the course of disease being eight to 18 months.D178N mutation in the PRNP gene of chromosome 20 and 129 amino acid polymorphisms of M/M were found in genetic examination in all the eight patients,of which five patients had family history.All the patients had sleep disorders,sleep-related involuntary movement,sleep-related dyspnea,laryngeal stridor.All the patients showed rapid progressive dementia with or without symptoms or signs of psychosis,ataxia,pyramidal and extrapyramidal.All the eight patients had progressive sympathetic symptoms,including hypertension,sweating,tachycardia,irregular breathing,and dysarthria.Cerebrospinal fluid 14-3-3 protein was found positive in one patient,and negative in seven patients.Electroencephalograph showed diffuse slow wave and non periodic synchronous discharge.Single-photon emission computed tomography or 18F fluorodeoxyglucose positron emission tomography showed decreased thalamic glucose metabolism in three patients.Seven patients showed decreased total sleep time,sleep awakening cycle disorder,especially the reduction or loss of rapid eye movement,laryngeal stridor and involuntary movement in polysomnography.Conclusions FFI is characterized by sleep disorder,sleep-related involuntary movement,dyspnea,laryngosis,rapid progressive dementia and sympathetic symptoms.The family history,polysomnography and positron emission tomography are helpful for the diagnosis of FFI.PRNP gene detection can confirm the diagnosis of FFI.
10.Summary of professor's experience for intractable facial paralysis.

Professor's experience of diagnosis and treatment for intractable facial paralysis is introduced. Professorfocuses on the thinking model that combines TCM, western medicine and acupuncture, and adopts the differentiation system that combines disease differentiation, syndrome differentiation and meridian differentiation; he adopts the treatment integrates etiological treatment, overall regulation, symptomatic treatment as well as acupuncture, moxibustion, medication and flash cupping. The acupoints ofmeridians are mostly selected, and acupoints of governor vessel such as Dazhui (GV 14) and Jinsuo (GV 8) are highly valued. The multiple-needles shallow-penetration-insertion twirling lifting and thrusting technique are mostly adopted to achieve slow and mild acupuncture sensation; in addition, the facial muscles are pulled up with mechanics action. The intensive stimulation with electroacupuncture is recommended at Qianzheng (Extra), Yifeng (TE 17) and Yangbai (GB 14), which is given two or three treatments per week.
