1.Effectiveness of Gatekeeper Training for Families of People With Mental Disorders in Korea: A Randomized Controlled Trial

		                        			 Objective:
		                        			Given the high suicide risk among people with schizophrenia and other mental disorders and the significance of intervention in the daily life setting, tailored gatekeeper training is necessary for families of people with mental disorders. This study evaluated the effectiveness of a suicide prevention education program for families of people with mental disorders (SPEM-F), developed by the Korea Foundation for Suicide Prevention. 
		                        		
		                        			Methods:
		                        			Fifty-nine family members of people with mental disorders were recruited from eight community psychiatric rehabilitation centers in Korea and enrolled in a cluster randomized controlled trial. Four facilities were randomized to the experimental group (n=30) and four to the control group (n=29). The study parameters were measured at baseline, post-intervention, and 1-month follow-up; the effects of SPEM-F (90 minutes, conducted offline) were analyzed using repeated-measures analysis of variance. The dependent variables were suicide-related knowledge, attitude, self-efficacy, preventive behavioral intention, and family problem-solving communication. 
		                        		
		                        			Results:
		                        			The experimental group showed significant increases in suicide-related knowledge (p<0.001), self-efficacy (p<0.05), and preventive behavioral intention (p<0.01) compared with the control group until the 1-month follow-up, including an upward trend in improved family problem-solving communication (p=0.069). 
		                        		
		                        			Conclusion
		                        			The results confirm that SPEM-F is an effective evidence-based gatekeeper training program for families of people with mental disorders, as it focuses on knowledge, self-efficacy, preventive behavioral intention, and communication improvement. As families of people with mental disorders including schizophrenia, are both highly motivated and close to people at high risk for suicide, SPEM-F can effectively promote suicide prevention and gatekeeper activities. 
		                        		
		                        		
		                        		
		                        	
2.Prerequisites and Factors for Efficient Organ Procurement Interview

		                        			 Purpose:
		                        			: In order to successfully conduct an organ procurement interview between the coordinator and the family of the brain dead, it is important to understand the conditions and factors of an efficient organ procurement interview. 
		                        		
		                        			Methods:
		                        			: This research uses a literature research. At first I explored the related literature found in the Google Scholar search engine. Subsequently, additional related literature was found in the reference of the literature. The contents related to the subject of this study were identified through close reading of 20 selected literature on organ procurement interviews. Then, appropriate keywords were assigned to the found contents, and the keywords were classified into categories according to the contents. 
		                        		
		                        			Results:
		                        			: 14 conditions and factors affecting the effective organ procurement interview were identified: Detailed informing on treatment and diagnostic procedures for the brain dead, effectively delivering bad news of the physicians; understanding and acceptance of brain death, emotional stability, dispositions and values, recognition of the will to donate expressed by the brain dead, positive perception of medical staff, sufficient knowledge about brain death and organ donation of the family of the brain dead; creating an appropriate environment for the interview, listening and empathy, providing accurate and detailed information on organ donation, disposition and qualification, communication and relationship building skills, preparation for the interview of the coordinators. 
		                        		
		                        			Conclusion
		                        			: The results of this study can be used as a guideline to efficiently conduct the organ procurement interview and for organ procurement interview education of coordinators. 
		                        		
		                        		
		                        		
		                        	
3.The principle and methodology of close reading to improve attentive listening and empathy of physicians

		                        			 Background:
		                        			Close reading is used in medical education to improve doctors’ listening, empathy and relationship-building ability, but no method for close reading has been suggested yet. In order to efficiently use close reading in medical education, it is essential to present concrete methods of close reading. 
		                        		
		                        			Methods:
		                        			The development of the Close Reading methods is based on the basic principles of text interpretation, resources of knowledge in the evaluation process and the theory of variation analysis presented by Lucius-Hoene and Deppermann. 
		                        		
		                        			Results:
		                        			The basic principles of text interpretation include data-orientedness, reconstructive attitude, prerequisites for meaningfulness, multilevel consideration, sequence analysis and contextuality, circularity and coherence, explicativity and argumentativity. As knowledge resources there is everyday knowledge, ethographic-historical knowledge, knowledge about linguistic-communicative phenomena, theoretical knowledge. Variation analysis includes substitution, deletion, permutation, insertion. 
		                        		
		                        			Conclusion
		                        			The basic principle of text interpretation is worth using as guidelines for text interpretation that readers of close reading should pay attention to. Knowledge resources can be used as the resources and argument for text interpretation. And the Variation analysis can be used to verify the validity of the close reading results. 4.Clinical Evaluation of Unilateral Open-Angle Glaucoma: A Two-Year Follow-Up Study

		                        			To evaluate the clinical characteristics of unilateral open-angle glaucoma, patients diagnosed with unilateral open-angle glaucoma from January 2017 to October 2018 were divided into primary open-angle glaucoma and normal-tension glaucoma groups according to the type of glaucoma diagnosed. The glaucoma and the contralateral eyes were compared, and the contralateral eye was analyzed for conversion to glaucoma and its risk factors were assessed during the 2-year follow-up period. Among 99 patients, 36 were diagnosed with primary open-angle glaucoma and 63 with normal-tension glaucoma. When comparing the glaucoma eye with the contralateral eye, the visual field mean deviation value (all p<0.001), peripapillary retinal nerve fiber layer thickness (all p<0.001), macular ganglion cell layer-inner plexiform layer thickness (p< 0.001, p=0.003), and optic nerve cup-disc ratio (p=0.005, p<0.001) were significantly different in both the primary open-angle glaucoma and normal-tension glaucoma groups. In normal-tension glaucoma, peripapillary retinal nerve fiber layer thickness was significantly thinner in the glaucoma conversion group than in the glaucoma non-conversion group (p=0.008). It was significantly associated with glaucoma conversion (odds ratio=0.97, p=0.023). In conclusion, in patients with unilateral open-angle glaucoma, the contralateral eye may develop glaucoma. In particular, if the peripapillary retinal nerve fiber layer thickness is decreased in normal-tension glaucoma, the possibility of glaucoma conversion is high; hence, careful examination is required.

6.Study on the Establishment of Fixed Night Shifts and Flexible Work Arrangements for Nurses

		                        			PURPOSE: The purpose of this study was to investigate the welfare status of nurses and the status of fixed night shifts and flexible work arrangements in both general hospitals and advanced general hospitals in Korea. METHODS: A total of 167 hospitals participated in this study, done from April to October 2018. RESULTS: There were 103 hospitals utilizing the fixed night shift system, 22 in the two-shift system, 3 in the fixed holiday shift, 79 in the fixed shift system, and 39 in the short-time working system. The average welfare benefits for nurses increased to 33.02 million won on average. However, welfare services for the nurses were poor: 41.8% of the respondents had a nurses' lounge, 81.7% were given dormitory space at their place of employment, 41.8% had access to welfare facilities, and 56.4% were provided with welfare support countermeasures. CONCLUSION: The results show an increasing trend of hospitals to utilize fixed night shift and flexible work arrangements. An institutional strategy at the national policy level should be established to implement flexible work arrangements including the fixed night shift system, which has a poor level of welfare.

7.Efficacy of parenteral glutamine supplementation in adult hematopoietic stem cell transplantation patients

		                        			BACKGROUND: Hematopoietic stem cell transplantation (HSCT) patients need parenteral nutrition because of nausea, vomiting, and mucositis caused by conditioning regimens. The demand for glutamine increases during the HSCT period. We evaluated the effects of glutamine-containing parenteral nutrition on the clinical outcomes of HSCT patients. METHODS: In this retrospective analysis, we reviewed HSCT patients from Seoul National University from August 2013 to July 2017. Depending on their glutamine supplementation status, 91 patients were divided into 2 groups: glutamine group (N=44) and non-glutamine group (N=47). We analyzed the rate of weight change, infection (clinically/microbiologically documented), complications (duration of mucositis and neutropenia, acute graft versus host disease), and 100-days mortality in each group. RESULTS: Regarding the clinical characteristics of the patients, there were no significant differences between the 2 groups except that there was a larger proportion of myeloablative conditioning regimen in the glutamine group (P=0.005). In the glutamine group, the average number of days of glutamine use, parenteral nutrition, and mucositis was 7.6±1.4, 14.6±9.9, and 13.3±9.5, respectively. Furthermore, multivariate analysis revealed odds ratios of 0.37 (95% CI, 0.14–0.96; P=0.042) and 0.08 (95% CI, 0.01–0.98; P=0.048) for clinically documented infection and 100-days mortality, respectively, in the glutamine group. CONCLUSION: Results showed that the glutamine group had less clinically documented infection and 100-days mortality than the non-glutamine group, but the other outcomes did not show significant differences. The extended duration of glutamine supplementation according to the period of total parenteral nutrition and mucositis should be considered.

10.Comparative in vivo biodistributions of nanoparticles and polymers of ¹⁷⁷lutetium-labeled hyaluronic acids in mice during 28 days.

		                        			Hyaluronic acid (HA) has been investigated for biomedical and pharmaceutical applications. This study was conducted to determine the distributions of HA nanoparticles (NPs; size 350–400 nm) and larger HA polymers in mice at intervals after application. ¹⁷⁷Lutetium (Lu)-labeled HA-NPs or HA polymers were intravenously injected (5 mg/kg) into male ICR mice, and radioactivity levels in blood and target organs were measured from 0.25 h to 28 days post-injection. In blood, the radioactivities of HA-NPs and HA polymer peaked at 0.5 h after injection but were remarkably decreased at 2 h; subsequently, they maintained a constant level until 6 days post-injection. HA-NPs and HA polymers were observed in the liver, spleen, lung, kidney, and heart (in ascending order) but were seldom observed in other organs. After 3 days, both the HA-NP and HA polymer levels showed similar steady decreases in lung, kidney, and heart. However, in liver and spleen, the HA-NP levels tended to decrease gradually after 1 day and both were very low after 14 days, whereas the HA polymer level accumulated for 28 days. The results indicate that HA-NPs, with their faster clearance pattern, may act as a better drug delivery system than HA polymers, especially in the liver and spleen.
