1.Professional self-concept of nurses: the influence of work stressors and related factors

BackgroundThe professional self-concept of nurses is a crucial indicator for their personal growth, development and career planning. Previous studies have shown that work stressors during the internship period may lead to a decrease in the level of professional self-concept among nursing students. Given the existing differences in social roles between nursing students and clinical nurses, the influencing factors of professional self-concept in clinical nurses and its relationship with work stressors require further research. ObjectiveTo explore the relationship between work stressors and professional self-concept among nurses, so as to provide references for improving the level of professional self-concept of clinical nurses. MethodsFrom December 2022 to February 2023, a stratified random sampling method was employed to select 260 in-service nurses from Mianyang Hospital of Traditional Chinese Medicine as the study subjects. Chinese Nurses Stressor Scale (CNSS) and Professional Self-concept of Nurses Instrument (PSCNI) were used for assessment. Pearson correlation analysis was conducted to examine the correlation between CNSS scores and PSCNI scores of nurses. Multiple linear regression analysis was utilized to explore the factors influencing the professional self-concept of nurses. ResultsA total of 238 nurses (91.54%) completed valid questionnaires. PSCNI total score yielded a statistical difference among nurses with different marital statuses (F=8.947, P<0.05). PSCNI total scores were significantly higher in nurses with emergency medical service experience than those without such experience (t=2.208, P<0.05), and were significantly lower in nurses with abnormal physical examination findings in the past year than those without abnormal findings (t=-2.584, P<0.05). Correlation analysis revealed that CNSS total score and subscale scores were negatively correlated with PSCNI total score (r=-0.275~-0.169, P<0.01). Multiple linear regression analysis indicated that work expectation-related stressors, marital status and emergency medical service experience might be influencing factors of their professional self-concept (β=-0.350, 0.345, 0.183, P<0.01). ConclusionNurses' work stressors are closely correlated with their professional self-concept, and high levels of stressors related to nurses' expectations may lead to a decrease in their professional self-concept levels. 2.Finite element analysis of anterograde and retrograde intramedullary nail for different areas of femoral shaft fractures

BACKGROUND:Intramedullary nail has achieved a good clinical result in the treatment of femoral shaft fractures,but some patients still have aseptic nonunion due to mechanical instability.The femur is the longest and largest bone in the human body,but there are few studies on whether the fracture of the femur has different biomechanical results in different areas and the influence of different inserting methods on the stability of fracture fragments in different areas. OBJECTIVE:To analyze the biomechanical characteristics of anterograde and retrograde intramedullary nails in the treatment of different areas of femoral shaft fractures,and to evaluate the best way of insertion to reduce the incidence of nonunion. METHODS:CT data of a healthy volunteer were selected to import into the software of Mimics 19.0 and Geomagic studio 2017 to extract and optimize the three-dimensional model of the right femur.The anterograde and retrograde intramedullary nail models were built with Solidworks 2017 software and assembled with femoral shaft fracture models at different fracture areas according to standard surgical techniques.The models were imported into Abaqus 2017 software in STEP format to set material attribute parameters,boundary conditions,load and submit calculation,and the results were viewed in the visualization module.Among them,the antegrade and retrograde intramedullary nails of the upper femoral shaft fracture were A1 and A2 models,B1 and B2 models in the middle segment,and C1 and C2 models in the lower segment. RESULTS AND CONCLUSION:(1)In models A1,B1 and C2,the overall stress distribution of the femur was more uniform,and the placement,the displacement and angle of the fracture site,and inversion angle of the proximal femoral bone fragment were smaller.(2)For the upper and middle femoral shaft fractures,the anterograde intramedullary nail has a better biomechanical effect.For lower femoral shaft fractures,a retrograde intramedullary nail is preferable.
3.Effect of vessel number on computational fluid dynamics in vascular networks

BACKGROUND:Mechanical factors can affect the angiogenic ability of vascular endothelial cells.How the vessel number affects the hydrodynamic properties of microvessels remains to be clarified. OBJECTIVE:To investigate the influence of vessel number on the hydrodynamics of vascular networks based on computational fluid dynamics. METHODS:Three three-dimensional models of vascular network with different vessel numbers were constructed using the Geometry module of ANSYS 19.0 software,and then the vascular network was meshed to tetrahedral elements in Mesh module.The vascular network was assumed to rigid wall without slip,and the blood was assumed to laminar,viscous,and incompressible Newtonian fluid.Blood density,velocity,and a series of blood viscosity coefficients were also established.The Navier-Stokes equation was used for calculation.Hydrodynamic properties of different parts of vascular network with different vessel numbers were analyzed and compared. RESULTS AND CONCLUSION:The streamline,velocity,and mass flow all had the same trend in the vascular network,that is,the outlet and inlet were higher and the middle junction of vascular network was lower.The more the number of vessels,the thinner the blood flow lines in each part of the vascular network.Also,the velocity,mass flow,and wall shear decreased with the increase of the number of blood vessels.Therefore,the changes in vessel number could influence the hydrodynamic environment in the vascular network.Computational fluid dynamics indicates that the changes in vessel numbers can influence the hydrodynamic properties of blood,and provides a new idea for treating bone hypoperfusion-induced diseases(fracture nonunion,bone defect,osteoporosis,etc.)through tonifying kidney and activating blood circulation based on the coupling between angiogenesis and osteogenesis.
4.A Prediction Model for Colorectal Adenoma and Colorectal Cancer Based on Routine Test

Objective To analyze the routine test parameter levels of patients with colorectal adenoma and colorectal cancer,and develop a prediction model.Methods A total of 580 patients diagnosed with colorectal adenoma(117 patients)and colorectal cancer(463 patients)were included in the retrospective study.The patients were randomly divided into two groups according to a 7:3 ratio:a training set with 406 cases and a validation set with 174 cases.Logistic regression analysis was used to establish a prediction model,and a nomogram was drawn.The model′s discrimination,calibration,and clinical applicability were evaluated using receiver operating characteristic curve(ROC),calibration plot,and decision curve analysis(DCA).Results Univariate logistic regression analysis identified 13 potential predictors:age,fecal occult blood test(FOBT),fibrinogen(FIB),thrombin time(TT),albumin(ALB),white blood cell value(WBC),neutrophil count(NEUT#),hematocrit value(HCT),mean corpuscular hemoglobin(MCH),red cell distribution width(RDW),platelet count(PLT),mean platelet volume(MPV),and activated partial thromboplastin time(APTT).Multivariate logistic regression analysis showed MPV,FIB,ALB,FOBT,TT,and HCT were risk factors for colorectal cancer in patients with colorectal adenoma(P<0.05).A nomogram was constructed based on these predictors to build a prediction model.The AUC of the ROC curve was 0.915 for colorectal cancer in the training set and 0.836 in the validation set.Calibration plots demonstrated high prediction accuracy and good model calibration.DCA results indicated the prediction model provided greater net benefit compared with the extreme models at threshold probabilities of approximately 55%-95%.Conclusion The developed prediction model exhibits satisfactory discrimination,calibration,and clinical applicability.The model can serve as an auxiliary tool in distinguishing between colorectal adenoma and colorectal cancer in patients.
5.Knowledge Graph Construction and Visualization Analysis of Shen Nong Ben Cao Jing Based on Named Entity Recognition

Objective To construct the knowledge map of Shen Nong Ben Cao Jing;To analyze basic knowledge of materia medica,explore implicit knowledge,and conduct visualization display;To provide methodological references for the study of ancient books.Methods The types of knowledge entities and relationships between entities involved in the Shen Nong Ben Cao Jing were organized and expressed.A training corpus dataset was produced using the BIO sequence labeling method;a self-developed CNLP text labeling system was used for text labeling;the BERT model was used to recognize named entities;the relationships between entities were set based on rules and semantic associations;the data were imported into the Neo4j-community 4.4.9 graph database using Cypher language for storage and visualization display after knowledge fusion;finally a knowledge graph was constructed.Results The knowledge map of Shen Nong Ben Cao Jing included 5 273 nodes and 11 064 relationships.The pattern layer contained 14 entity classes and 16 relationship types.Through Cypher language query,knowledge was visualized from the aspects of TCM classification,medicinal property theory,compatibility of seven emotions and application of TCM.Conclusion The knowledge graph constructed in this study intuitively reflects the knowledge recorded in Shen Nong Ben Cao Jing and the recessive relationship,which is suitable for knowledge mining and intuitive multi-dimensional display of ancient TCM books.
6.Clinical pathological characteristics and immune microenvironment significance of EGFR T790M mutation in non-small cell lung cancer patients and its prognostic implications

Background and purpose:Epidermal growth factor receptor exon 20 T790M(EGFR T790M)mutation is one of the acquired resistance mechanisms in non-small cell lung cancer(NSCLC)against first-/second-generation EGFR tyrosine kinase inhibitors(EGFR TKIs).Additionally,EGFR T790M mutation can also be observed in NSCLC patients who have not undergone EGFR TKIs treatment.This study aimed to compare the clinical pathological characteristics and prognostic differences between NSCLC patients with de novo and acquired EGFR T790M mutation,and further explore the immune microenvironment features of acquired T790M mutation in NSCLC.Methods:This study retrospectively included 3 762 cases of NSCLC diagnosed at Fudan University Shanghai Cancer Center from April 2020 to September 2022.Among them,2 070 cases(55.02%)exhibited EGFR mutations,and 556 cases(14.77%)received EGFR TKIs treatment.Specifically,there were 119 cases(3.16%)of NSCLC with EGFR T790M mutation,including 51 cases(1.35%)of de novo T790M mutation and 68 cases(1.81%)of acquired EGFR T790M mutation.Clinical data of the patients were collected for comparative analysis between NSCLC patients with de novo and acquired T790M mutation.Multiple immunofluorescence histochemistry(mIHC)was employed to explore the immune microenvironment characteristics of NSCLC patients with acquired T790M mutation.Results:The proportion of de novo and acquired T790M mutations was higher in female patients compared to males.Patients with de novo T790M mutation tended to be younger.Both de novo and acquired T790M mutations were more commonly found in poorly differentiated carcinomas.Among NSCLC patients with de novo T790M mutation,there was a higher rate of programmed death ligand-1(PD-L1)expression(60.00%).In contrast,among NSCLC patients with acquired T790M mutation,the rate of PD-L1 expression was lower(22.39%).Acquired T790M mutation in NSCLC was often accompanied by TP53 alterations(39.7%).Cox regression analysis results indicated that mesenchymal to epithelial transition(MET)factor alteration was a risk factor for the occurrence of acquired T790M mutation(P=0.000 5).The average overall survival(OS)showed no significant difference between de novo and acquired T790M mutations(35.4 and 37.3 months respectively).However,patients with acquired T790M mutation exhibited a higher proportion of recurrence and metastasis.In acquired T790M mutation,there was a higher presence of immune cell infiltration within the stromal compartment,such as CD20+B cells,CD23+B cells,CD8+T cells,CD8+PD-1-/+cells,CD20+PD-1-/+cells and CD23+PD-1-/+cells.Additionally,the study found that when EGFR was accompanied by tumor suppressor gene(TSG)alterations,the average distance between tumor cells and CD8+T cells,CD20+B cells,CD8+PD-1+cells,CD20+PD-1+cells and CD23+PD-1+cells was closer compared to cases with only EGFR mutations.Conclusion:In comparison to patients with de novo T790M mutation,patients with acquired T790M mutation exhibit a lower rate of PD-L1 positivity.Acquired T790M mutation often accompanies TP53 alterations,and MET alteration is identified as a risk factor triggering acquired T790M mutation.Although patients with acquired T790M mutation face higher risk of recurrence and metastasis,their average OS does not significantly differ from those with de novo T790M mutation.In cases of acquired T790M mutation,the presence of TSG mutations can alter the spatial distribution of immune cells,potentially leading to benefits from immunotherapy.
7.Preliminary Discussion on Post-marketing Comprehensive Evaluation Indicator System of Ancient Famous Classical Formula Preparations

The construction of the comprehensive evaluation index system of the famous classical formula preparations after the marketing has both theoretical and practical significance. In this study, literature related to the post-marketing comprehensive evaluation of traditional Chinese medicine(TCM) compound preparations was retrieved from China National Knowledge Infrastructure(CNKI), China Science and Technology Journal Database(VIP), Wanfang Data Knowledge Service Platform(WanFang) and SinoMed from January 1, 2000 to April 30, 2022. CiteSpace 6.1.R2, a scientometrics software, was used to visualize the keywords involved, and to analyze the dynamic evolution trend and research hotspots in this field. Then, the existing comprehensive post-marketing evaluation index system of TCM compound preparations was screened and extracted, and the research status was systematically analyzed by mathematical statistics. It was found that there were problems such as the generalized boundaries between assessment dimensions and assessment elements, the lack of data sources for individual evaluation indexes, unset weight of some index system and insufficient application degree. In addition, according to the characteristics of famous classical formulas, the authors discuss the importance of evidence evaluation based on combination of disease and syndrome, pharmacovigilance of famous classical formulas preparations, and whole-process quality control of famous classical formulas, and put forward the construction strategy of comprehensive post-marketing evaluation of the famous classical formula preparations, which is oriented by clinical value, centered on evidence evaluation, and guaranteed by the whole-process quality control.
8.Clinical Application Analysis of Da Qinjiaotang Based on Ancient and Modern Literature

Da Qinjiaotang is a common classical prescription for the treatment of stroke. It originates from Collection of Writings on the Mechanism of Disease, Suitability of Qi, and the Safeguarding of Life as Discussed in the Basic Questions (《素问病机气宜保命集》) by physician LIU Wansu, and is composed of Gentianae Macrophyllae Radix, Glycyrrhizae Radix et Rhizoma, Chuanxiong Rhizoma, Angelicae Sinensis Radix, Paeoniae Radix Alba, Asari Radix et Rhizoma, Notopterygii Rhizoma et Radix, Saposhnikoviae Radix, Scutellariae Radix, Gypsum Fibrosum, Angelicae Dahuricae Radix, Atractylodis Macrocephalae Rhizoma, Rehmanniae Radix, Rehmanniae Radix Praeparata, Poria, and Angelicae Pubescentis Radix. Doctors of all dynasties have disputed the composition principle of the prescription and argued whether its treatment of stroke belongs to the theory of "internal wind" or "external wind". Through collating and analyzing ancient and modern literature related to the indications of Da Qinjiaotang, this paper was dedicated to the origin of syndrome differentiation and treatment of Da Qinjiaotang. According to LIU Wansu's original works, Da Qinjiaotang is a prescription for the treatment of "internal wind", and in the prescription, wind medicinal herbs such as Gentianae Macrophyllae Radix, Notopterygii Rhizoma et Radix and Angelicae Pubescentis Radix removes stagnation, clears sweat pore, and makes qi and blood channels flow smoothly. However, later generations, affected by the idea of "external wind", believe that this prescription is used for the treatment of "external wind". Ancient physicians gradually supplemented the symptoms of stroke, such as wry eye and mouth, hemibody pain and limb numbness, which were treated by Da Qinjiaotang, and Da Qinjiaotang was also applied to the treatment of other diseases, such as tendon dryness, convulsion and arthralgia. Modern doctors still explain the disease pathogenesis from the theory of "external wind" as deficiency in channels and collaterals and the entry of pathogenic wind, and the prescription has the effect of dispersing wind, clearing heat and nourishing and activating blood. In clinical practice, Da Qinjiaotang is mainly used to treat cerebrovascular diseases and peripheral facial paralysis in nervous system diseases, gouty arthritis and rheumatic arthritis in the rheumatic immune system and skin diseases. The above findings facilitate the research and development of Da Qinjiaotang.
9.Research on the rule of ancient prescriptions of yin deficiency syndrome

Objective:The data mining method was used to analyze the medication rules of the ancient prescriptions for yin deficiency syndrome, so as to provide reference for clinical treatment of yin deficiency syndrome.Methods:We searched the database of ancient prescriptions, selected the prescriptions for yin deficiency syndrome, used the Ancient and Modern Medical Case Cloud Platform V2.2.3 to count the frequency and attribute of drug use, and carried out hierarchical cluster analysis to find the core prescription. MATLAB was used to analyze the association rules, and the law of drug compatibility was found. The Pajek64 5.13 was used for complex network analysis to find out the core traditional Chinese medicine.Results:There were 467 prescriptions that meet the requirements, 302 kinds of drugs were used in total. The four properties of the drugs were mainly cold, the five flavors mainly sweet, the meridian of the drugs mainly kidney meridian. The core drugs were Rehmanniae Radix Praeparata, Ophiopogonis Radix, Rehmanniae Radix, and Poria. The most commonly used drug pair was Dioscoreae Rhizoma- Rehmanniae Radix Praeparata, the second was Corni Fructus- Rehmanniae Radix Praeparata, Poria-Rehmanniae Radix Praeparata. The commonly used drugs can be clustered into two categories. Conclusion:We can preliminarily reveal that the ancient medication law of yin deficiency syndrome is mainly nourishing Yin liquid and reducing fire, paying attention to nourishing kidney yin, using Yin nourishing medicine often combined with medicine of tonifying lung, liver, spleen yin and diverging to reduce fever. It can provide certain reference for clinical treatment of yin deficiency syndrome.
10. Children′s non-carcinogenic health risk assessment of heavy metals exposure to residential indoor dust around an e-waste dismantling area in South China

Objective:
To evaluate the non-carcinogenic health risk of heavy metals (As, Cd, Cr, Cu, Mn, Pb and Zn) in residential indoor dust for young children around an e-waste dismantling area in South China.
Methods:
A village around an e-waste dismantling area in South China was selected as a research site in October 2016. Convenience sampling method was used to select 36 houses in the village and 36 dust samples were collected by vacuum cleaner. The concentrations of heavy metals (Cd, Cr, Cu, Mn, Pb and Zn) in each sample were determined and expressed by the average value. Non-carcinogenic health risk assessment was conducted using the US Environmental Protection Agency (EPA) Health Risk Assessment (HRA) model, the American Toxicology and Disease Registry (ATSDR) Target-organ Toxicity Dose (TTD) approach and the ATSDR Binary Weight-of-Evidence (BINWOE) model.
