1.Hand-sewn expanded polytetrafluoroethylene valved conduit for right ventricular outflow tract reconstruction.

OBJECTIVES:
Due to the lack of large-sized pulmonary valved conduit products in clinical practice, hand-sewn expanded polytetrafluoroethylene (ePTFE) valved conduit has been used for right ventricular outflow tract (RVOT) reconstruction in many heart centers around the world. This study aims to summarize the early results of the ePTFE valved conduit and the sewing technology of the conduit in combination with the latest progress, and to provide a reference for the application of ePTFE valved conduit.
METHODS:
A total of 21 patients using ePTFE valved conduit for RVOT reconstruction in the Second Xiangya Hospital, Central South University from October 2018 to October 2020 were prospectively enrolled in this study. The age at the implantation of the conduit was 4.3 to 43.8 (median 15.1) years old, with weight of (38.9±4.1) kg. In this cohort, 14 patients underwent re-reconstruction of RVOT, including 12 patients with pulmonary regurgitation at 6.3 to 31.0 (median 13.8) years after tetralogy of Fallot (TOF) repair, and 2 patients with failed bovine jugular vein conduit (BJVC). Seven patients underwent Ross operations. Among them, 3 were for aortic valve stenosis, 2 were for aortic regurgitation, and 2 were for both stenosis and regurgitation. The ePTFE valved conduits were standard hand-sewn during the surgery. The 3 leaflets were equal in size with arc-shaped lower edge of the valve sinus. The free edge of the valve leaflets was straight with the length of about 1 mm longer than the diameter. The height of the valve sinus was 4/5 of the diameter. The junction of the valve leaflet was 3/4 of the height of the sinus. The designed leaflets were then continuous non-penetrating sutured into the inner surface of Gore-Tex vessel to make a valved conduit. Valved conduits with diameter of 18, 20, and 22 mm were used in 2, 9, and 10 cases, respectively. The surgical results, postoperative recovery time, and serious complications were summarized, and the changes of postoperative cardiac function status and hemodynamic status of the conduits were investigated.
RESULTS:
During the implantation of ePTFE valved conduit for RVOT reconstruction, 2 patients underwent mechanical mitral valve replacement with Ross operation, 2 patients with pulmonary regurgitation with repaired TOF underwent left and right pulmonary artery angioplasty, and 1 patient with failed BJVC underwent tricuspid valvuloplasty. The cardiopulmonary bypassing time for patients underwent re-reconstruction of RVOT was (130.9±16.9) min, with aorta clamping for 1 patient to repair the residual defect of the ventricular septum. The cardiopulmonary bypassing and aorta clamping time for Ross operation were (242.7±20.6) min and (145.6±10.5) min, respectively. The duration of postoperative ventilator assistance, intensive care unit stay, and hospital stay were 3.5 h to 7.7 d (median 17.1 h),11.2 h to 29.5 d (median 1.9 d), and 6.0 to 56.0 (median 13.0) d, respectively. All patients survived after discharge from hospital. The follow-up rate after discharge was 100% with median time at 15.0 (13.0 to 39.0) months. No death happened during the follow-up. One patient underwent stent implantation due to right coronary stenosis 2 months after Ross operation. One patient underwent balloon dilation due to right pulmonary artery ostium stenosis 1 year after re-reconstruction of RVOT. The cardiac function of all patients recovered to NYHA class I 6 months after operation. The peak pressure gradient across the valve measured by transthoracic echocardiography before discharge was (9.4±2.6) mmHg (1 mmHg=0.133 kPa), and (18.3±6.1) mmHg at the last follow-up. There was no significant increase in the gradient during the follow-up (P=0.134). No patient suffered from mild or more pulmonary regurgitation.
CONCLUSIONS
Hand-sewn ePTFE valved conduit is feasible for RVOT reconstruction. It is a promising material for RVOT reconstruction which can effectively meet clinical need. In our experience, the ePTFE valved conduit is simple to manufacture with satisfactory early outcomes.In the application of ePTFE valved conduit, attention should be paid to implantation indications and postoperative anticoagulation management, especially to the preparation details of the valved conduit, to obtain better function and durability of the conduit after implantation.

2.Diagnosis and treatment of duodenal foreign bodies

Most foreign bodies are often discharged without obvious symptoms after swallowing, but some may be misdiagnosed and stay in the body, resulting in serious complications. Different diagnostic methods should be integrated to diagnose such patients. Once diagnosed, they should be treated as soon as possible if necessary. The authors reported a case of retention in the body for 6 years after swallowing a wooden chopstick, resulting in the insertion into the psoas muscle after duodenal perforation, complicated by abdominal abscess. During the operation, endoscopic assisted laparoscopic foreign body removal was performed, and the patient showed good prognsis, which provided references for the diagnosis and treatment of the disease.

Objective To investigate the predictive value of extra pulmonary multiple factors including creatine kinase-isoenzyme MB (CK-MB) for the prognosis of patients with acute paraquat poisoning.Methods A retrospectively analysis were conducted on 641 patients who were treated at the First Affiliated Hospital of Zhengzhou University due to oral paraquat poisoning from October 2002 to April 2017.The observation end point was that the patients died from paraquat poisoning within 3 months after admission or were still alive within 3 months after paraquat poisoning.The patients' data were retrieved,including general information,the dose of poison,urinary paraquat concentration,arterial blood gas analysis,alanine transaminase (ALT),total bilirubin (TBIL),uric acid (UA),aspartate transaminase (AST),creatine kinase (CK),CK-MB,B type natriuretic peptide (BNP),lactic dehydrogenase (LDH),high sensitivity troponin T (hsTnT),C-reaction protein (CRP) and procaicitonin (PCT).According to the patient's prognosis within 3 months,the patients were divided into a survival group and a non-survival group.The above indicators were compared between the two groups and the diagnostic value of CK-MB for acute paraquat poisoning was analyzed according to the receiver operating characteristics (ROC) curve.Collect the last arterial blood gas analysis,and laboratory test results were analyzed by binary logistic regression analysis to determine the risk factors for death in patients with acute paraquat poisoning.Results Among the 641 patients with acute paraquat poisoning,315 (49.1%) patients survived and 326 (50.9%) died.Compared with the survival group,patients in the non-survival groupthere were older,had a shorter hospital stay,and had a higher oral paraquat dose and urinary paraquat concentration;Lac,TBIL,UA,AST,CK,CK-MB,BNP,LDH,CRP and PCT were higher,while blood gas analysis index were lower in the non-survival group (P<0.05).Binary logistic regression analysis showed that the dose of paraquat,CK-MB and AST were closely related to the prognosis of patients with acute paraquat poisoning.The optimal cut-off value of ingestion dose,the first urinary paraquat concentration on admission and CK-MB in predicting the prognosis of patients with acute paraquat poisoning were 7 g (AUC=0.918,sensitivity 80.6%,specificity 87.5%,Yoden index 0.681,P<0.01),5.16 μg/mL (AUC=0.879,sensitivity 93.8%,specificity 70.1%,Yoden index 0.639,P<0.01),and 18.2 U/L (AUC=0.846,sensitivity 83.9%,specificity 71.9%,Yoden index 0.558,P<0.01),respectively.Binary logistic regression analysis of the last biochemical indicators of paraquat poisoning showed that the dose of poison,the last CK-MB,the last SCr,urinary paraquat concentration,and the last blood Na+ were closely related to the prognosis of patients with acute paraquat poisoning.Among them,the last CK-MB>18.05 U/L often indicated poor prognosis (AUC=0.808,sensitivity 79.7%,specificity 65.8%,Yoden index 0.455,P<0.01).Conclusions In the treatment of patients with acute paraquat poisoning,there are significant differences in extra pulmonary factors such as heart,liver,kidney,electrolytes and inflammatory markers in patients with different prognosis,so the monitoring and follow-up should be improved,in addition to focusing on the presence and treatment of pulmonary fibrosis.In particular,CK-MB is an independent risk factor for the prognosis of acute paraquat poisoning.In the late stage of poisoning,CK-MB,SCr,and blood Na+ have a strong predictive value for the prognosis of the patients,and we should pay attention to the regular follow-up of the above mentioned laboratory items.

According to the pathological characteristics of symptomatic chronic thoracic and lumbar osteoporotic vertebral fracture (SCOVF), the different clinical treatment methods are selected, including vertebral augmentation, anterior-posterior fixation and fusion, posterior decompression fixation and fusion, and posterior correction osteotomy. However, there is still a lack of a unified understanding on how to choose appropriate treatment method for SCOVF. In order to reflect the new treatment concept and the evidence-based medicine progress of SCOVF in a timely manner and standardize its treatment, the clinical guideline for surgical treatment of SCOVF is formulated in compliance with the principle of scientificity, practicability and advancement and based on the level of evidence-based medicine.
5.Regulatory Effects of Stilbene Glucoside on JNK and PP 2B in APP/PS1/Tau Transgenic Dementia Mice

OBJECTIVE:To study the regulatory effects of stilbene glucosid e(TSG)on c-Jun N-terminal kinase (JNK)and protein phosphortase 2B(PP2B)in APP/PS1/Tau transgenic dementia (3×Tg-AD)mice,and to explore its potential mechanism of anti-Alzheimer’s disease (AD). METHODS :Totally 45 male 3×Tg-AD mice were randomly divided into model group ,positive control group (huperzine A ,0.15 mg/kg),TSG low-dose ,medium-dose and high-dose groups (0.033,0.1,0.3 g/kg),with 9 mice in each group. Another 9 normal male C 57BL/6J mice were included into normal control group. Administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 60 d. Normal control group and model group were given constant volume of normal saline intragastrically. After medication ,Morris water maze experiment was used to test the spatial learning and memory ability of mice in each group ;Nissl staining was used to observe the changes of Nissl bodies in cerebral cortex and hippocampus ;mRNA and protein expressions of JNK and PP 2B were detected by qRT-PCR and Western blotting assay. RESULTS:Compared with normal control group ,the escape latency was significantly prolonged (P<0.01),the retention time of the original platform quadrant was significantly shortened (P< and the times of crossing the platform was significantly reduced in model group (P<0.01);the number of Nissl bodies in cerebral cortex and hippocampus was significantly 729011126@qq.com reduced,the staining was slight ;the relative expressions of JNK mRNA and protein were significantly increased (P< 0.01),and the relative expressi ons of PP 2B mRNA and protein were significantly decreased (P<0.01). The mechanism may be related to down-regulating the transcription and expression of protein kinase JNK ,up-regulating the transcription and expression of protein phosphatase PP 2B.
6.Effects of Stilbene Glycoside on the Phosphorylation of Thr 205 and Ser 404 Sites of Tau Protein in AD Model Mice

OBJECTIVE:To study the e ffects of stilbene glycoside c(TSG)on phosphorylation of Thr 205,Ser404 sites of Tau protein in Aizheimer ’s disease (AD)model mice ,and to investigate the potential anti-AD mechanism of TSG. METHODS :APP/ PS1/Tau three transgenes (3×Tg-AD)mice were randomly divided into model group ,positive control group (huperzine,0.15 mg/kg),TSG low-dose ,medium-dose and high-dose groups (0.033,0.1,0.3 g/kg),with 6 mice in each group. In addition ,6 C57BL/6J mice were chosen as normal control group. Administration groups were given relevant medicine intragastrically. Model group and normal control group were given equal volume of normal saline intragastrically ,once a day ,for consecutive 60 days. After last medication ,immunofluorescence staining was used to detect Tau protein and phosphorylated Tau protein (Thr205, Ser404 sites) distribution and expression in brain tissue of mice in each group. Western blotting assay was used to detect phosphorylated Tau protein (Thr205,Ser404 sites)expression level in brain tissue of mice in each group. RESULTS :Compared with normal control group ,the expression of Tau protein,phosphorylated Tau protein (Thr205,Ser404 sites)in 729011126@qq.com the brain tissue of mice were increased in model group ,which were easy to aggregate and distributed more widely ;theirrelative expression were increased significantly (P<0.01). Results of Western blotting assay showed that the expression levels of phosphorylat ed Tau protein (Thr205,Ser404 sites)were increased significantly (P<0.01). Compared with model group ,the expression of Tau protein ,phosphorylated Tau protein (Thr205,Ser404 sites) in the brain tissue of mice were decreased in positive control group and TSG groups ;aggregation decreased,distribution narrowed and their relative expression were decreased significantly (P<0.01). Results of Western blotting assay showed that the expression levels of phosphorylated Tau protein (Thr205,Ser404 sites)were decreased significantly (P< 0.01). Compared with positive control group ,There was no significant difference in the distribution of Tau protein ,phosphorylated Tau protein (Thr205,Ser404 sites)in the brain tissue of mice in TSG groups ;the relative expression were not statistically significant(P>0.05);but Western blotting assay showed the expression levels of phosphorylated Tau protein (Thr205 site)in TSG medium-dose and high-dose groups as well as the expression levels of phosphorylated Tau protein (Ser404 site)in TSG groups were decreased significantly (P<0.05 or P<0.01). CONCLUSIONS :TSG can play an anti-AD effect on AD model mice by down-regulating the expression of phosphorylated Tau protein (Thr205,Ser404 sites)in brain tissue.
7.The effect of noninvasive ventilation with the helmet compared with facial mask in patients with acute respiratory failure: a randomized controlled study

Objective To explore the effect of noninvasive ventilation (NIV) with helmet or facial mask on clinical efficacy, tolerability, and prognosis in patients with acute respiratory failure. Methods Fifty patients with acute respiratory failure according to the inclusion criteria were recruited from January 2018 to July 2018 in Emergency Intensive Care Unit of the First Affiliated Hospital of Zhengzhou University. Included patients were randomly allocated into the helmet group or facial mask group. Based on conventional drug therapy, pressure support mode was performed with the interface of the helmet or facial mask. Oxygenation index, arterial carbon dioxide partial pressure, and respiratory rates were measured before and after the treatment, and the data were compared and analyzed by the repeated measures ANOVA. Tolerance score, complication rate, tracheal intubation rate, and mortality rate were recorded at each observation time point of the two groups. Results The oxygenation index before NIV, at 4 h and at the end of NIV treatment of the helmet group were significantly increased from (160.29±50.32) mmHg to (249.29±83.47) mmHg and (259.24±87.09) mmHg; the oxygenation index of the facial mask group were increased from (168.63±38.63) mmHg to (225.00±74.96) mmHg and (217.69±77.80) mmHg, and there was no significant difference within the two groups (P <0.05). The respiratory rates before NIV, at 4 h and at the end of NIV treatment of the helmet group were obviously decreased from (27.60±7.64) breaths/min to (17.92±4.55) breaths/min and (16.88±3.90) breaths/min; the respiratory rates of the facial mask group were decreased from (24.68±6.14) breaths/min to (20.36±4.25) breaths/min and (19.68±3.34) breaths/min, and the differences within the two groups were statistically significant (P <0.05). However, there were no significant differences on oxygenation index and respiratory rates between the helmet group and facial mask group (P >0.05). Patients in the helmet was better tolerated than those in the facial mask group [ratio of good tolerance 96% (24/25) vs 56% (14/25) (P = 0.001) and fully tolerance 80% (20/25) vs 36% (9/25) (P =0.002)] and had less complications (1/25 vs 10/25, P = 0.002). 84% patients in the helmet group and 76% patients in the facial mask group were successfully weaned and discharged after NIV treatment (P =0.480). Conclusions Similar clinical efficacy in improving blood gas exchange and relieving dyspnea were observed in the helmet group and the facial mask group in patients with acute respiratory failure. However, the helmet is better tolerant, and had lower complication rate, which is especially suitable for patients with chest trauma combined with facial injuries.
8.Effects of Stilbene Glycoside on Okadaic Acid-induced Tau Protein Phosphorylation in NG 108-15 Cells

OBJECTIVE: To observe the effects of stilbene glycosidec (TSG) on okadaic acid (OA)-induced Tau protein phosphorylation in NG108-15 cells, and to investigate the potential anti-Alzheimer’s disease (AD) mechanism of this compound. METHODS: AD model of NG108-15 cells was induced by OA. The survival rate of NG108-15 cells was observed by MTT assay after pretreated with low-dose, medium-dose and high-dose of TSG (50, 100, 200 μmol/L). The apoptosis of NG108-15 cells was detected by AO/EB double fluorescence staining. The protein and mRNA expression of CDK5 and GSK3β, and the protein expression of Tau and p-Tau were detected by Western blotting assay and RT-PCR. The distribution of CDK5, GSK3β and Tau protein were detected by immunofluorescence. RESULTS: The normal morphology of NG108-15 cells was observed in normal control group, but CDK5, GSK3β and Tau protein were not found or few was found. Contracted or globular early apoptotic cells were observed in model gorup; the distribution of CDK5, GSK3β and Tau protein was increased, while survival rate of the cells was decreased; protein and mRNA expression of CDK5 and GSK3β as well as ratio of the relative expression of p-Tau to that of Tau (p-Tau/Tau) were all increased significantly (P<0.05 or P<0.01). After pretreatment of TSG, the distribution of early apoptotic cells as well as CDK5, GSK3β and Tau protein were all decreased to some extent in administration groups, while survival rates of the cells were increased significantly. Protein expression of CDK5 and p-Tau/Tau in medium-dose group and high-dose group as well as mRNA expression of CDK5, protein and mRNA expression of GSK3β in administration group were decreased significantly (P<0.05). CONCLUSIONS: TSG can protect against AD model cells, the effects of which may be associated with improving survival rate of the cells, down-regulating the protein expression and gene transcription level of phosphokinase CDK5 and GSK3β, inhibiting Tau protein phosphorylation.
9.Effect of hemoperfusion combined with continuous veno-venous hemofiltration on acute paraquat poisoning:a Meta-analysis

Objective To comprehensively evaluate the clinical efficacy of hemoperfusion (HP) combined with continuous veno-venous hemofiltration (CVVH) on acute paraquat poisoning (APP).Methods Literatures of Chinese and English randomized controlled trials (RCTs), case control and cohort study on HP combined with CVVH in the treatment of APP from the PubMed, Embase, Wanfang, and CNKI up to November 2017 were enrolled (the subjects were > 16 years old). The obtained literatures were strictly screened and evaluated in quality, and data such as mortality, the life time of dead patients, inefficiency rate, incidence of multiple organ dysfunction syndrome (MODS) and acute respiratory distress syndrome (ARDS) were extracted. Meta-analysis was performed by RevMan 5.3. Results Twenty-one studies were included with 2222 subjects, among whom 976 subjects were in HP combined with CVVH group (experimental group) and 1246 subjects in HP group (control group). Compared with control group, the mortality in experimental group were significantly decreased [43.77% (362/827) vs. 55.26% (604/1093), odds ratio (OR) =0.68, 95% confidential interval (95%CI) = 0.56-0.82,P = 0.0001], the life time of death patients was significantly prolonged [mean difference (MD) = 4.63, 95%CI = 2.60-6.66,P < 0.00001], incidence of MODS [25.93% (70/270) vs. 55.36% (155/280),OR = 0.26, 95%CI = 0.14-0.49,P< 0.0001], and incidence of ARDS [30.37% (82/270) vs. 51.07%(143/280),OR = 0.42, 95%CI = 0.30-0.61,P < 0.00001], and inefficiency rate [8.72% (13/149) vs. 34.64% (53/153),OR=0.17, 95%CI = 0.09-0.34, P < 0.00001] were significantly reduced. Funnel chart showed that except the publication bias of mortality rate, there were less publication bias about other indicators among studies.Conclusion HP combined with CVVH can significantly reduce the mortality of patients with APP compared with HP alone on the whole, prolong the life time and reduce the occurrence of MODS and ARDS, thus improving the treatment efficiency.

Objective To investigate the learning curve of contrast-enhanced ultrasonography ( CEUS) in sentinel lymph node( SLN ) of breast cancer and provide a theoretical basis for leaners to learn SLN CEUS . Methods The multi-center study of SLN CEUS in breast cancer" was planned by Sichuan Cancer Hospital . According to the uniform inclusion and exclusion criteria , 511 patients with complete clinical data and follow-up results from 9 hospitals in Multi-center were included in this study . According to the inspection time ,the patients were divided into 3 groups named as group A ( 170 patients) ,group B ( 170 patients) and group C ( 171 patients ) ,respectively . The basic clinical data ,ultrasound imaging data , intraoperative and postoperative pathological findings of all patients were recorded . With the accumulation of cases examined ,analysis was performed to find the learning curve of the SLN CEUS examination time , SLN CEUS detection rate ,SLN CEUS surface marking accuracy rate and SLN CEUS diagnosis rate ,the learning curve was analyzed . Results ① There was no statistical significant difference in patients ages , tumors sizes ,tumors locations ,SLNs numbers and LCs numbers among the three groups( all P > 0 .05) . ②As the number of cases examined increases ,the examination time was reduced gradually ,but SLN detection rate ,surface marking accuracy and SLN diagnostic coincidence rate were increased gradually( F = 151 .75 , 1 .96 ,7 .49 ,5 .50 ; P = 0 .000 ,0 .143 ,0 .001 ,0 .005 ) . Conclusions The skill of the doctor is improved gradually when learning SLN CEUS . With the number of the cases increase ,the operating time of SLN CEUS is shorted ,and the SLN detection rate ,surface marking accuracy and SLN diagnostic coincidence rate of SLN-CEUS are gradually increased . It has an important clinical significance for beginners to learn the SLN CEUS technology .
