Objective To compare the application of four domestic and foreign qualitative occupational health risk assessment methods for cement dust hazard assessment and explore their applicability, and to find out a method suitable for qualitative occupational health risk assessment of cement dust. Methods The Risk Assessment Method for Occupational Accidents and Diseases of Romania (Romania method),the Australian Occupational Health and Safety Risk Assessment Method (Australia method),MES method, and the qualitative method of International Council on Mining & Metals (ICMM) were used to assess the occupational health risk of cement dust exposure posts in seven enterprises of Chongqing. The assessment results were analyzed and compared with Spearman correlation analysis, Mann-Whitney U test and weighted Kappa consistency test after standardizing by risk ratio (RR). Results The RRs of the four methods were all positively correlated with cement dust exposure concentration (the correlation coefficients were all greater than 0.6). The Romania method, the Australia method and the qualitative method of ICMM could identify a risk difference between the key posts and non-key posts. The qualitative method of ICMM was difficult to identify high-risk posts that require priority intervention. The Romania method and Australia method had strong consistency (Kappa=0.608, P<0.01), but only the Australia method could identify high-risk posts of cement dust. Conclusion In general, the Australia method is relatively better at identifying the risk differences of cement dust hazard in different posts and is more suitable for occupational health risk assessment of cement dust with more accurate assessment results.

Background::Homoharringtonine (HHT) is an effective anti-inflammatory, anti-viral, and anti-tumor protein synthesis inhibitor that has been applied clinically. Here, we explored the therapeutic effects of HHT in a mouse heart transplant model.Methods::Healthy C57BL/6 mice were used to observe the toxicity of HHT in the liver, kidney, and hematology. A mouse heart transplantation model was constructed, and the potential mechanism of HHT prolonging allograft survival was evaluated using Kaplan–Meier analysis, immunostaining, and bulk RNA sequencing analysis. The HHT-T cell crosstalk was modeled ex vivo to further verify the molecular mechanism of HHT-induced regulatory T cells (Tregs) differentiation. Results::HHT inhibited the activation and proliferation of T cells and promoted their apoptosis ex vivo. Treatment of 0.5 mg/kg HHT for 10 days significantly prolonged the mean graft survival time of the allografts from 7 days to 48 days ( P <0.001) without non-immune toxicity. The allografts had long-term survival after continuous HHT treatment for 28 days. HHT significantly reduced lymphocyte infiltration in the graft, and interferon-γ-secreting CD4 + and CD8 + T cells in the spleen ( P <0.01). HHT significantly increased the number of peripheral Tregs (about 20%, P <0.001) and serum interleukin (IL)-10 levels. HHT downregulated the expression of T cell receptor (TCR) signaling pathway-related genes ( CD4, H2-Eb1, TRAT1, and CD74) and upregulated the expression of IL-10 and transforming growth factor (TGF) -β pathway-related genes and Treg signature genes ( CTLA4, Foxp3, CD74, and ICOS). HHT increased CD4 + Foxp3 + cells and Foxp3 expression ex vivo, and it enhanced the inhibitory function of inducible Tregs. Conclusions::HHT promotes Treg cell differentiation and enhances Treg suppressive function by attenuating the TCR signaling pathway and upregulating the expression of Treg signature genes and IL-10 levels, thereby promoting mouse heart allograft acceptance. These findings may have therapeutic implications for organ transplant recipients, particularly those with viral infections and malignancies, which require a more suitable anti-rejection medication.

Multivariate logistic regression analysis was used to identify related factors of psychoactive substances use. Results:A total of 2 919 YMSM were included in the study, in whom 47.1% (1 374/2 919) had ever used psychoactive substances. The rate of psychoactive substance use in YMSM who were not students was 51.6% (853/1 653), which was higher than that (41.2%, 521/1 266) in YMSM who were students ( χ2=31.42, P<0.001). In the YMSM who were not students, multivariate analysis showed that the OR of psychoactive substance use was 1.49 times higher in smokers than in non-smokers (95% CI: 1.17-1.84), 4.14 times higher in those who had anal sex in the past six months than in those who had no anal sex (95% CI: 1.71-9.98), 1.65 times higher in those who had ≥3 anal intercourses in the last week than in those who had <3 anal intercourses (95% CI: 1.27-2.14), 1.99 times higher in those who had commercial sex in the past six months than in those who had no homosexual commercial sex (95% CI: 1.14-3.45), 2.37 times higher in those who had HIV test in the last year than in those who had no HIV test (95% CI: 1.90-2.94), 1.89 times higher in those who had STD diagnosis in the last year than in those who had no STD diagnosis (95% CI: 1.11-3.21), 0.66 times higher in those who had the first homosexual sex at age ≥20 years than in those who had the first homosexual sex at age <20 years (95% CI: 0.52-0.83), 0.48 times higher in those who were heterosexual or bisexual than in those who were homosexual (95% CI: 0.30-0.74) and 0.70 times higher in those who mainly searched sexual partners offline than in those who mainly searched sexual partners online (95% CI: 0.53-0.94). Meanwhile, in the YMSM who were students, multivariate analysis showed that the OR of psychoactive substance use was 1.31 times higher in local residents than in non-local residents (95% CI: 1.02-1.69), 1.61 times higher in smokers than in non-smokers (95% CI: 1.14-2.27), 1.52 times higher in those who had ≥3 anal intercourses in the last week than in those who had <3 anal intercourses (95% CI: 1.05-2.19), 3.31 times higher in those who had STD diagnosis in the last year than in those who had no STD diagnosis (95% CI: 1.68-6.50) and 0.53 times higher in those in Han ethnic group than in those in minor ethnic groups (95% CI: 0.30-0.94). Conclusions:The prevalence of psychoactive substance use was high in YMSM in Tianjin, and the influencing factors varied in different groups (students or not). It is necessary to conduct targeted interventions for different groups.

Journal of Southern Medical University 2023;43(4):537-543
OBJECTIVE:
To investigate the expression of microRNA miR-431-5p in gastric cancer (GC) tissues and its effects on apoptosis and mitochondrial function in GC cells.
METHODS:
The expression level of miR-431-5p in 50 clinical samples of GC tissues and paired adjacent tissues was detected using real-time fluorescence quantitative PCR, and its correlation with the clinicopathological features of the patients was analyzed. A cultured human GC cell line (MKN-45 cells) were transfected with a miR-431-5p mimic or a negative control sequence, and the cell proliferation, apoptosis, mitochondrial number, mitochondrial potential, mitochondrial permeability transition pore (mPTP), reactive oxygen species (ROS) production and adenosine triphosphate (ATP) content were detected using CCK-8 assay, flow cytometry, fluorescent probe label, or ATP detection kit. The changes in the expression levels of the apoptotic proteins in the cells were detected with Western blotting.
RESULTS:
The expression level of miR-431-5p was significantly lower in GC tissues than in the adjacent tissues (P < 0.001) and was significantly correlated with tumor differentiation (P=0.0227), T stage (P=0.0184), N stage (P=0.0005), TNM stage (P=0.0414) and vascular invasion (P=0.0107). In MKN-45 cells, overexpression of miR-431-5p obviously inhibited cell proliferation and induced cell apoptosis, causing also mitochondrial function impairment as shown by reduced mitochondrial number, lowered mitochondrial potential, increased mPTP opening, increased ROS production and reduced ATP content. Overexpression of miR-431-5p significantly downregulated the expression of Bcl-2 and increased the expressions of pro-apoptotic proteins p53, Bcl-2 and cleaved caspase-3 protein.
CONCLUSION
The expression of miR-431-5p is down-regulated in GC, which results in mitochondrial function impairment and promotes cell apoptosis by activating the Bax/Bcl-2/caspase3 signaling pathway, suggesting the potential role of miR-431-5p in targeted therapy for GC.

Objective:To investigate the clinical effect of long-stem hemiarthroplasty in the treatment of unstable intertrochanteric fracture in elderly patients with severe osteoporosis. Methods:A retrospective analysis was performed on 48 elderly patients with unstable intertrochanteric fractures of the femur with severe osteoporosis in Liaocheng People′s Hospital from April 2017 to April 2019.Twenty three patients received long-stem hemiarthroplasty (LHA group). Twenty five patients were treated with proximal femoral nail anti-rotation (PFNA) (PFNA group). PFNA group was used as the control group. The operative time, intraoperative blood loss, perioperative blood transfusion volume, number of intraoperative fluoroscopy, weight-bearing time after operation, the incidence of postoperative complications, hospitalization time, and Harris hip score of 1, 3, 6, 12 months after surgery, to investigate the efficacy of the application of long-stem hemiarthroplasty. Results:In LHA group, 23 patients were followed up for (18.6±3.9) (range from 12.0 to 26.0) months, and 25 patients in the PFNA group were followed up for (17.8±3.3)(range from 12.0 to 24.0) months. There was no significant difference in follow-up time between the two groups ( Z=-0.552, P=0.581). The operation time of LHA Group (60 (55, 73) h) was longer than that of PFNA Group (55 (50, 60) h). The intraoperative blood loss in LHA Group ((179.35±63.47) mL) was more than that in PFNA Group ((122.80±49.03) mL). The number of fluoroscopy in LHA Group (2 (2, 2) times)was less than that in PFNA Group (16 (14.5, 19.5) times). The time of weight bearing in LHA Group (4 (3, 5) d) was earlier than that in PFNA Group (33 (30, 36) d), and the differences were statistically significant ( Z=2.459, t=3.470, Z=6.216, Z=5.959; all P<0.05). There were no significant differences in perioperative blood transfusion, hospital stay and postoperative complications between the two groups (all P>0.05). Harris hip function score was significantly higher in LHA Group ((76.70±5.96), (82.13±6.38), (85.96±7.16), (88.78±7.67) points) and PFNA Group ((63.80±3.46), (71.56±2.55), (81.60±3.38), (88.08±4.83) points) increased gradually with the increase of follow-up time ( Fintra-group=432.557, Pintra-group<0.001), and the score reached the highest 12 months after operation.Harris hip function score of LHA group was higher than that of PFNA group( Finter-group=25.437, Pinter-group<0.001). There was interaction effect between follow-up time point and operation mode( Finteraction=53.464, Pinteraction<0.001). Conclusion:For the elderly patients with unstable intertrochanteric fracture with severe osteoporosis, the application of lengthened stem hemiarthroplasty can get out of bed early, reduce the complications of bed rest, reduce the number of intraoperative fluoroscopy, and recover the function of hip joint earlier and better with satisfactory results.

Objective To investigate the clinical and epidemiological characteristics of 135 confirmed cases of COVID-19 in Tianjin. Methods The clinical and epidemiological data of 135 cases of COVID-19 in Tianjin were collected, and the data were analyzed with descriptive method. The factors influencing the severity of the illness were analyzed. Results Among the 135 COVID-19 cases, 72 were males and 63 were females, the age of the cases was (48.62±16.83) years, and the case fatality rate was 2.22%. Local transmission caused 74.81% of the cases. A total of 33 clusters occurred, involving 85.92% of all COVID-19 cases. The median of the incubation period of COVID-19 was 6.50 days, the average generation interval was 5 days, and the household secondary transmission rate was 20.46%. Fever was the main symptom (78.63%), followed by cough (56.48%). Multivariate regression analysis indicated that age ( OR =1.038, 95% CI : 1.010-1.167) and the number of chronic underlying diseases ( OR =1.709, 95% CI : 1.052-2.777) were the risk factors of severe illness. Conclusions Fever was the main symptom at the early phase of COVID-19 in Tianjin, and the local cluster cases accounted for high proportion in confirmed COVID-19 cases reported in Tianjin. Severe illness was prone to occur in people with old age and multi underlying diseases. Strict isolation of close contacts and intensive care of high-risk groups are the main measures to reduce the morbidity and case fatality of COVID-19.

Long-term spaceflight affects numerous organ systems in the body, including metabolic dysfunction. Recently, ample evidence has demonstrated that the liver is a vulnerable organ during spaceflight. However, the changes in hepatocyte proliferation and cell cycle control under microgravity remain largely unexplored. In the present study, we first confirmed that the serum levels of aspartate aminotransferase, alanine aminotransferase and alkaline phosphatase, biochemical markers of liver function, were altered in rats under tail suspension (TS) conditions to simulate microgravity, as shown in previous reports. Next, we demonstrated that the cell proliferation activity, determined by Ki67, PCNA and PH3, was significantly decreased at the different TS time points (TS for 14, 28 and 42 days) compared with that in the control group. Consistently, the positive cell cycle regulators Ccna2, Ccnd1, Cdk1, Cdk2 and cyclin D3 were also significantly decreased in the TS groups as shown by quantitative real-time PCR and western blotting analysis. Subsequent analysis revealed that the aberrant hepatocyte proliferation inhibition under simulated microgravity was associated with the upregulation of miR-223 in the liver. We further found that miR-223 inhibited the proliferation of Hepa1-6 cells and identified CDK2 and CUL1 as its direct targets. In addition, the decreased expression of CDK2 and CUL1 was negatively correlated with the level of p27 in vitro and in vivo, which may have been responsible for retarding hepatocyte proliferation. Collectively, these data indicate that upregulation of miR-223 was associated with the inhibition of liver cell growth and reveal the role of miR-223 in rat hepatocyte proliferation disorders and the pathophysiological process under simulated microgravity.
